The Usability Study for Gross Motor Function Classification System as Motor Development Prognosis in Children With Cerebral Palsy (뇌성마비 아동 운동발달 예후 지표로 대동작 기능 분류법 활용에 관한 연구)

  • Purpose: Lack of a valid prognosis of gross motor development in children with cerebral palsy (CP) and the absence of longitudinal data on which to base an opinion in Korea have made it difficult to plan treatment and counsel prognosis issues accurately. The purposes of this study were to examine whether the Gross Motor Function Classification System (GMFCS) is valuable to prognostication about gross motor progress in children with CP in Korea. Methods: Medical records of 61 patients were retrospectively reviewed that visited outpatient department and were diagnosed as CP. Various information was surveyed including CP type, visual acuity, cognitive function, motor acquisition age, ambulatory status, development curves of Gross Motor Function Measure (GMFM) according to each of the 5 level of GMFCS. All of them were compared with other studies. Also the gross motor development curves and the maximum GMFM score derived from this study were compared with the Palisano's report and the Rosenbaum's report. Results: Based on a total of 494 GMFM assessments provided by this study, the 5 distinct motor development curves and the maximum GMFM score were created. These observations is corresponding with the Palisano's and the Rosenbaum`s Development curves. Conclusion: The 5 distinct motor development curves (GMFCS) that were created by Palisano's and Rosenbaum's study is useful in Korea, providing parents and clinicians with a means to plan interventions and to judge progress over time.

Factors Influencing Suicidal Ideation in People with Mental Disorder (정신장애인의 자살사고에 미치는 영향요인)

  • Purpose: This study was conducted to investigate factors influencing suicidal ideation in people with mental disorder. Methods: The subjects were 176 people with mental disorder from community mental health centers, social rehabilitation facilities, and university hospital psychiatric outpatient departments. The instruments used included a suicidal ideation questionnaire, a self-esteem scale, a spiritual well-being scale, a family support scale, and a professional support scale. Data were analyzed with t-test, one-way ANOVA, Pearson's correlation coefficient, and hierarchical regression analysis. Results: Suicidal ideation was influenced by age, history of suicidal attempts, and existential spiritual well-being. This study shows that suicidal ideation has negative correlation with existential spiritual well-being, self-esteem, family support, and age. Existential spiritual well-being, history of suicidal attempts, and age accounted for 30% regarding suicidal ideation in people with mental disorder. Conclusion: The lower existential spiritual well-being and age, the higher suicidal ideation. And also suicidal ideation was higher in people who had attempted suicide than in those who had not. Based on the outcomes of this study, it is necessary to design an intervention program that emphasizes existential spiritual well-being in order to decrease suicidal ideation in young people with mental disorder.

An Explanatory Model of Dyspnea in Patients with Chronic Lung Disease (만성폐질환 환자의 호흡곤란 설명모형)

  • Purpose: The purpose of this study was to develop and test an explanatory model of dyspnea in patients with chronic lung disease (CLD). Methods: Participants were 181 patients with CLD, recruited from the outpatient pulmonary clinic of one university hospital in Korea. Data were collected using questionnaires, as well as measurement of 6-minute walking distance (6MWD), oxygen saturation ($SpO_2$), FEV1% predicted, and Body Mass Index (BMI). Results: The results indicated a good fit between the proposed dyspnea model and the collected data [$x^2$=91.27, p= .13, $x^2$/d.f.=1.17, Normal Fit Index= .934]. Oxygenation ($SpO_2$, = -.530), self-efficacy (= -.429), anxiety (= .253), depression (= .224), exercise endurance (6MWD, = -.211), and pulmonary function (FEV1% predicted, = -.178) had a direct effect on dyspnea (all p< .05) and these variables explained 74% of variance in dyspnea. BMI, smoking history, and social support had an indirect effect on dyspnea. Conclusion: The findings of this study suggest that comprehensive nursing interventions should focus on recovery of respiratory health and improvement of emotions, exercise ability, and nutritional status. From this perspective, pulmonary rehabilitation would be an effective strategy for managing dyspnea in patients with CLD.

Psychological Intervention Effects of Group Music Therapy on Patients with Parkinson's Disease (파킨슨병 환자에 대한 집단 음악요법의 심리적 중재 효과)

  • Purpose: Music therapy is a non-pharmacological treatment of the behavioral and psychological symptoms. This study was conducted to contribute to the rehabilitation of patients with idiopathic Parkinson's disease in the areas of functionality, emotionality and sociability. Methods: The study included 35 participants, aged between 52 and 77 years, who were diagnosed with idiopathic Parkinson's disease (IPD) and reported suffering from depression, chronic illness anticipated stigma, and worrying about quality of life. The patients were recruited from the department of outpatient neurology at School of Medicine, D University in B Metropolitan City. Group music therapy was performed for nine weeks. The results were analyzed by repeated measures ANOVA. Results: In terms of stigma, there were significant differences between two groups and time (p<.05). Analysis of the interaction between time and service method revealed significant differences (p<.05). Between time, and the interaction between time and service method revealed significant differences in terms of quality of life (p<.05). Conclusion: The results suggest that group music therapy will be effective on the psychosocial well-being of patients with Parkinson's disease living in assisted and independent living communities.

The Effect of integrated health and welfare services on the control of medical use and medical expenses in Korean Veteran's Hospitals (보훈병원 통합의료복지서비스가 의료이용과 의료비 절감에 미치는 효과 분석)

  • This study aims to examine the effect of integrated health and welfare(IHW) services on medical use and medical expenses in Korean Veteran Hospitals. Data on the volume of medical use and medical expenses were collected from two patient groups of 5 Veterans' Hospitals: the beneficiary group who has received IHW services and the non-beneficiary group who has not. Each group was composed of 265 patients respectively. The results of the study revealed that the volume of home nursing services, home rehabilitation services, ambulatory services have increased significantly, whereas the average length of stay, number of using emergency services, and average medical expenses have decreased after receiving integrated health services in the beneficiary group. In contrast, the non-beneficiary group showed much more increase both in the volume of inpatient and outpatient services, as well as the average medical expenses, compared to those of the beneficiary group. This results imply that the provision of IHW services have positive impacts on the enhancing the effectiveness of the medical resources utilization for the veteran patients.

Relationships of Activities of Daily Living and Body Image with Quality of Life in Stroke Patients: Mediating Effects of Interpersonal Relations (뇌졸중 환자의 일상생활 수행능력, 신체상이 삶의 질에 미치는 영향: 대인관계 매개효과 중심)

  • Purpose: This study was conducted to identify factors associated with stroke patients' quality of life (QOL) and examine the mediating effects of interpersonal relations in the relationships of activities of daily living (ADL) and body image with QOL. Methods: In this study, 160 stroke patients were recruited from an outpatient clinic of a university hospital and rehabilitation clinic of a long-term care hospital. Participants completed a questionnaire which included sociodemographic characteristics, ADL, body image, interpersonal relation, and QOL. Descriptive statistics, t-tests, ANOVA, Pearson's correlation coefficients, multiple regression analysis, and process macro mediation analysis were conducted using SPSS. Results: The mean scores were 91.01 (range 6~100) for ADL, 61.19 (range 40~79) for body image, 87.53 (range 29~123) for interpersonal relations, and 186.67 (range 71~243) for QOL. Multiple regression analyses showed that ADL, body image, interpersonal relations, and participation in group activities after stroke were significantly associated with QOL among stroke patients (p<.05). There were no mediating effects of interpersonal relations in the relationships of ADL and body image with QOL (p>.05). Conclusion: This study showed that there is a need for physical, psychological, and social recovery to improve the QOL of stroke patients.

Nuss Procedure for Surgical Stabilization of Anterior Flail Chest with Mechanical Ventilation Weaning Failure: A Case Report

  • Flail chest is a critical medical condition in which multiple segmentally fractured adjacent ribs cause paradoxical movement of the thoracic cage in patients with severe blunt trauma injury. Surgical stabilization is considered essential in patients who require mechanical ventilation. However, there is no consensus on which surgical procedure to choose among the various available techniques or when to perform surgery. We report the case of a patient with traumatic anterior flail chest due to bilateral multiple fractures of the ribs requiring surgical stabilization in whom weaning from mechanical ventilation had failed. The Nuss procedure using double bars with the bridge technique was performed for chest wall stabilization. The patient was weaned from mechanical ventilation on postoperative day 44 and she underwent bar removal on postoperative day 71. After extensive rehabilitation for multiple trauma, she was discharged successfully. The patient currently shows no recurrence of chest wall depression in outpatient follow-up.

Korean Medicine Treatment for Dyspepsia and Constipation in a Patient with HIV: A Case Report

  • Functional dyspepsia (FD) is a chronic and recurrent upper gastrointestinal symptom that has no organic cause. A 49-year-old male patient positive for human immunodeficiency virus (HIV) visited the clinic because of FD and constipation. He received complex Korean medicine treatment, including acupuncture and herbal medicines, from April 14 to July 18, 2022 (27 times) in the outpatient department. Gastrointestinal Symptom Rating Scale (GSRS), Nepean Dyspepsia Index (NDI), FD-related quality of life (FD-QoL), Euro QoL visual analog scale (EQ-VAS), and Numerical Rating Scale (NRS) were used as evaluation tools in this study. Symptoms were relieved after 3 months of treatment, and his QoL improved (GSRS, 15-3; NDI, 110-21; FD-QoL, 52-20 [eating status, 10-3; liveliness status, 12-8; psychological, 1-0; role-functioning status, 18-9]; EQ-VAS, 40-65; NRS, 8-4). The results revealed that complex Korean medicine treatment could alleviate FD and constipation in patients with HIV.

The Effects of High-Intensity Aerobic Interval Training In The Elderly With Myocardial Infarction (노인 심근경색 환자에서 고강도 간헐적 유산소운동의 효과)

  • To investigate the effect of high-intensity interval training (HIIT) on the outcomes of cardiac rehabilitation(CR) in elderly and younger patients with myocardial infarction(MI). Of the MI patients who received hospital-based CR from 2015 to 2017, we retrospectively reviewed the medical records of the patients who continued follow-up through the outpatient clinic. A total of 39 patients (20 elderly patients (${\geq}60year$($64.1{\pm}3.6$)) and 19 younger patients (< 60 year($55.6{\pm}2.3$)) was included in this study. The HIIT program was composed of 10 minutes of warm-up, 30 minutes of interval aerobic exercise, and 10 minutes of cool down. The aerobic exercise based on their initial exercise tolerance test outcome consisted of 3-minute usual intensity phase (60-70% of heart rate/VO2 reserve) and 4-minute of high-intensity phase (70-90% of heart rate/VO2 reserve). Exercise capacity was measured by exercise tests before and after hospital- based CR. Before CR, elderly group had a significantly lower exercise capacity in Exercise time, peak VO2, METs, Anaerobic threshold, VEmax, 02pulse than younger group. Both group showed similar improvement of cardiopulmonary exercise capacity after CR. And HIIT is safe and effective in elderly patients. It is considered necessary to further expansion for the participation of high intensity interval training in elderly patients.

Factors Influencing the Health Care Utilization of Disabled Workers Who Returned to Their Original Workplace after Occupational Accident (원직복귀 산업재해 근로자의 의료이용에 영향을 미치는 요인)

  • The aim of this study is to explore the factors that influence the health care utilization of disabled workers who returned to their original workplace after an occupational accident. The study sample consisted of 457 workers who suffered from occupational accidents and were rated as disabled based on the data from the PSWCI in 2021. Data analysis was performed using a hierarchical multiple regression analysis with SPSS WIN 26.0. The study findings revealed that the number of outpatient visits was affected by factors such as daily wage payment, economic status, physical activity limitations, subjective health status, and chronic diseases. Moreover, health care expenditures were determined by factors such as having a caregiver for daily living, burn injuries, and physical activity limitations. Therefore, to increase the accessibility of health care utilization for disabled workers, the role of nurses in the workplace should be expanded, employers should include a health care utilization plan when developing a return-to-work plan, and research on the introduction of a paid sick leave system that allows workers to rest when they are sick is needed. It is also necessary to alleviate the economic burden by increasing the coverage of benefits for burn and rehabilitation treatments.
