정신건강의학과 안정병동 내 병원학교교실 참여자의 학교복귀율과 만족도 (Successful Schooling Rate and Satisfaction of the Inpatient Hospital School Participants among the Child and Adolescent Psychiatric Inpatients in the Closed Ward)

  • Objectives : The objective of this report is to identify the utilization of hospital school service during hospitalization among patients in their childhood and adolescence with psychiatric disorders. Methods : We retrospectively reviewed the medical record of child and adolescent psychiatric who were hospitalized during March 2009 through October 2012. We compared the one-year successful schooling and outpatient follow up rate between users and nonusers of the inpatient hospital school service. The hospital schooling experiences of the users were investigated upon follow-up visits to the outpatient clinic. Results : Sixty-three students received hospital school service during hospitalization among total 122 child and adolescent inpatients. Hospital school participants showed a significantly higher school reentry rate (61.9%) than non-participants (40.7%). However, there was no difference on follow up rate between the two groups. More than 60% of the 22 interviewed participants expressed an above-average level of satisfaction about hospital school service. Conclusion : Many patients with mental illness experience difficulty in receiving school education during treatment. That induces deterioration in disease, academic failure, poor social skills, low self-esteem, economic difficulties, and future job opportunities. The results of this study emphasize the importance of hospital school service and offer useful guidance for hospital school operation.

산후 우울 수준과 분만전후 관련 요인에 관한 연구 (Postpartum Depressive Score and Related Factors Pre- and Post-delivery)

  • Purpose: This study aimed to identify the scores of postpartum depression(PPD) on the first day, 1st week, and 6th week after the delivery and to explore their related factors before and after delivery in postpartum women. Methods: With a survey design, 293 postpartum women were recruited from a postpartum unit, Ilsin Christian hospital in Pusan via convenience sampling and were followed at 1st week and 6th week in the outpatient clinic. Results: Results showed that the scores of PPD(EPDS score) were low at postpartum 1st day, 1st week and 6th week but prevalence of PPD(EPDS ${\geq}13$)was 3.1%at 1st day, 8.2%at 1st week and 7.5%at 6th week, respectively. The pre-delivery factors were experience of depression, and the post-delivery factors were baby's sex(1st day), no caregiver for baby(1st week), and no help and concern for taking care of baby from husband and family(1st day and 6th week). The greater satisfaction with becoming a mother and her life, and greater maternal attachment were related to lower level of PPD at the three time points. Conclusion: Regular screening for postpartum depression and supportive and informative education is needed for postpartum women visiting the outpatient clinic for follow-up.

술 후 후두협착을 예방하기 위한 외래에서의 Mitomycin-C의 반복 국소적용 (Office-Based Mitomycin-C Application to Prevent Postoperative Laryngeal Stenosis)

  • Background and Objectives: Topical administration of mitomycin-C (MMC) has been reported to reduce or delay scar formation in airway surgery. However, it is not infrequent to experience recurrent stenosis or adhesion of operative wound even after a meticulous MMC application during the laryngeal surgery. Therefore we aimed to evaluate the effectiveness of repeated postoperative MMC applications and the technical feasibility of MMC applications to the laryngeal wound at an outpatient clinic. Methods: We reviewed medical records of 13 consecutive patients who received office-based MMC applications after laryngeal airway surgery at Samsung Medical Center, Seoul, Korea. The patients were grouped into 3 categories according to the site of surgical wound and the purpose of MMC application; group I : supraglottic stenosis (n=5), group II : cordectomy and arytenoidectomy site granulation prevention (n=3), Group III : laryngeal web prevention (n=5). Outcomes in each group and adverse effects of repeated MMC applications were evaluated. Results: Office-based MMC application was successfully performed one to four times with a week interval for each patient. No significant complications were observed except slightly decreased mucosal wave in one female patient who received 4 times of MMC application at the anterior commissure of vocal fold. Repeated MMC applications at the outpatient clinic resulted in wide or acceptable supraglottic airway in group I, clean wound healing without granulation formation in group II, and negligible or no web formation at the anterior commissure in group III. Conclusion : Office-based topical administration of MMC to the larynx was technically feasible. Postoperative repeated MMC applications were effective to reduce recurrent stenosis or adhesion of supraglottic structures, to prevent granuloma formation after laser arytenoidectomy and glottic web formation after anterior commissure resection.

종합병원 외래진료부 진로인지계획 모형에 관한 연구 (A Study on the Wayfinding Model of Outpatient Department in General Hospital)

  • Recently, hospital patients experience anxiety, confusion, and stress about wayfinding as the spacial layout and treatment circulatory system of hospitals have become complicated due to their oversized and complex structure. As part of finding a solution to the problem, this study seeks to examine what are the essential elements of the wayfinding planning of O.P.D. in general hospitals, to develop the model of wayfinding, and to suggest the methods of improving the wayfinding system. The research methods of this study adopted were literature review in wayfinding cognition, plan analysis of ten general hospitals, space analysis of these hospitals through space syntax, analysis of the system of visual-perceptual information through a field study, and analysis of surveys and follow-up surveys conducted to support the results. Based on these results, the proposals for finding decision points, providing the information, and developing a model planning are listed as follows. 1) The comprehensive understanding of O.P.D. spacial layout and the visual-perceptual information system is necessary to find the essential elements of wayfinding. 2) The decision points are found through the full understanding of spacial functions, circulation systems, and facility configuration, considering the spacial layout, the bound of the visual-perceptual information system, and the circulatory system. Furthermore, the information decision points could be confined by space syntax. 3) The checklist and color compound & color codes, developed through the planning of signage system and color system could be applied to the methods of providing the information. 4) The planning of wayfinding system according to the whole process of practices for outpatients was mentioned above. The system of visual-perceptual information developed through the process of this study should be integrated in the spacial layout of the whole O.P.D.

정신과 환자의 낮병원 이용 경험에 대한 내러티브 탐구 (Narrative Inquiry on Experience of Mentally Ill Patients in Psychiatric Day Hospital Care)

  • 본 연구는 내러티브 탐구를 활용하여 정신과 낮병원 이용 환자들의 경험을 연구하였다. 연구자들은 낮병원 이용경험이 있는 정신과 환자 4명을 대상으로 3회 이상 심층면담을 실시하였으며 낮병원 퇴원 후 외래 진료 과정에서 추가 면담을 실시하여 자료를 수집했다. 연구 참여자들의 낮병원 이용 경험의 의미는 '세상과 끊어진 줄을 다시 묶다.', '힘이 되는 마음의 안식처.', '삶에 대한 방향성을 찾다.' 세 가지 주제로 나타났다. 본 연구는 정신과 낮병원 이용 당사자들의 이야기를 깊이 있게 다루었다는 점에서 의의가 있다. 연구 결과를 바탕으로 정신과 환자의 고립 해소를 위한 제도적 지원, 다양한 지지체계 활성화 노력, 프로그램 개발 활성화의 정책적 제안을 한다.

여성전문병원의 고객관계관리(CRM)가 고객충성도에 미치는 영향 (Impact of Customer Relationship Management on Customer Loyalty among Patients Visiting a Woman's Hospital)

  • Although a variety of customer relationship management (CRM) activities have been provided by many hospitals in Korea, there lacks empirical evidence on the effect of CRM. The present study was conducted to examine the effect of CRM in terms of the customer response to CRM in a woman's hospital setting. A total of 380 patients receiving inpatient or outpatient care from woman's hospital between October 25 and November 4, 2005 were surveyed for the degree of their experience of and preference for CRM activities of the hospital by 5-point Likert-type scale. Patients were also asked about the level of customer loyalty to the hospital. Eighteen CRM activities offered by the hospital was classified into 4 types of CRM strategies according to Berry and Parasuranman: price, social, structural, and relationship recovery strategy. There's a significant positive correlation between the degree of experience of CRM and preference for CRM(r=0.49, p<0.001). Regression analysis results showed the significant positive relationship between the degree of experience of CRM and customer loyalty(${\beta}$=0.448, p<0.05). Among the 4 CRM strategies, only social(${\beta}$=0.127, p<0.05) and structural strategy(${\beta}$=0.266, p<0.05) showed signifiant positive relationship with customer loyalty. Overall, the favorable customer response to CRM in terms of preference for CRM and customer loyalty indicates that there's a positive effect of CRM on the continuity of the relationship between patients and health care providers.

정신과 환자의 낮병원 직업재활 프로그램 참여 경험에 관한 질적 연구 (Qualitative Study on Vocational Rehabilitation Program Participation Experience of Mentally Ill Patients in Psychiatric Day Hospital Care)

  • 본 연구는 일반적 질적 연구와 집단 포토보이스(G-Photovoice) 방법을 활용하여 정신과 환자의 낮병원 직업재활 프로그램 참여 경험을 연구하였다. 연구자는 낮병원 직업재활 프로그램 참여 경험이 있는 정신과 환자 5명을 대상으로 2회 이상 면담을 실시하였으며 낮병원 이용과 외래 진료 이후 추가 면담을 실시하였다. 더불어 사진을 활용한 반 구조화된 집단 면담을 진행하였다. 연구결과 연구 참여자들의 낮병원 직업재활 프로그램 참여 경험은 '직업 활동의 어려움을 경험함', '함께 일하는 것에 대한 즐거움', '안정감을 얻음', '자신이 성장했음을 느낌'으로 나타났다. 이러한 연구결과에 기반하여 본 연구에서는 낮병원 직업재활을 활성화하고 정신과 환자의 성장을 촉진시킬 수 있는 함의를 논의하였다.

3차진료기관 외래약국 투약대기시간에 영향을 주는 요인 (Factors Affecting Patient Waiting Times at the Outpatient Pharmacy Department in a Tertiary Care Hospital)

  • Background: The number of outpatients visiting large university teaching hospitals has increased drastically with the introduction of a nationwide health care insurance in 1989 and the improvement of the socio-economic status of the population. This resulted in long waiting times for services, particularly prescribed drugs, which have been patients' chief complaints. Hospitals have tried to solve the problem with limited success because their approach lacked comprehensive research. The objective of this study is to investigate associations between waiting times and variables defining a total work system. Methods: Data for the outpatient pharmacy department in a tertiary care university teaching hospital located in Seoul was analyzed to achieve the study objective. Associations of pharmacy system variables -- work load, work force, pharmacist work schedule, machine problems, and inventory control -- with mean and 99th percentile of waiting times were examined by the hierarchical stepwise regression method. Day was a unit of the analyses. Results: The regression models explained 65.8% of variance in the mean waiting time and 61.34% in the 99th percentile of waiting times. The break-down of the printer for drug envelops, Automatic Tablet Counters (ATCs), and main computer system lasted longer than 30 minutes increased the mean for 7.7 minutes, 4.5 minutes, and 7.0 minutes, respectively, and the 99th percentile for 14.8 minutes, 9.0 minutes, and 15.7 minutes, respectively. Concerning the work force, study results showed that there were significant differences in the productivity of pharmacists with work experience more than three years, one to three years, and less than one year, and showed that peak time aid work by pharmacists at job assignments other than the outpatient pharmacy, part-time pharmacists, and the installation of ATCs were effective in reducing waiting times, Finally, study findings indicated that the operational policy of work assignment and rotation schedule, supply and inventory of drugs at work tables, and readiness for undisrupted work during the work hours could have a significant effect on waiting times. Conclusion: The study results indicated that efforts to reduce waiting times for prescribed drugs should be geared toward every components of the pharmacy work system ranging from work schedule of pharmacists and supply of dugs at work tables. These findings should provide hospital managers with right directions in battling the problem.

소아청소년과 외래를 방문한 부모들의 발열공포와 관련 요인 (Fever Phobia: A Survey of Children's Parents in a Pediatric Outpatient Clinic)

  • 목적 일 지역 소아청소년과 외래를 방문한 부모들의 자녀의 발열에 대한 발열공포의 수준 및 부모의 발열염려 정도와 대상자의 특성 간의 관련성을 파악하고자 함이다. 방법 소아청소년과 외래를 방문한 부모 151명을 대상으로 구조화된 자기기입식 설문지를 이용하여 자료를 수집하였다. 수집된 자료는 기술통계 및 카이제곱 검정을 통해 분석하였다. 결과 약 50%의 대상자들이 $37.8^{\circ}C$를 발열의 최저기준 체온으로 그리고 $38.9^{\circ}C$를 고열의 최저기준 체온으로 정의했다. 약 3/4의 대상자들이 발열의 해로운 효과로 경련과 뇌손상을 언급했으며, 발열에 대해 '매우 염려'를 하였고, 자녀의 발열 시 1시간 이내에 체온을 다시 측정하였고, 미온수 목욕을 제공했으며, 열성질환이 있는 경우 잠자는 아동을 해열제를 주기 위해 깨웠다. 부모의 자녀 발열에 대한 염려는 이전 열성 경련 경험 여부와 한 자녀 가정의 부모와 통계적으로 유의한 관련성이 있었다. 결론 소아청소년과 외래를 방문한 부모들에게도 발열공포가 광범위하게 존재하고 있었다. 부모들을 위한 발열 및 발열 관리 교육 프로그램의 개발과 평가가 요구된다. 의사와 간호사가 부모들의 발열관련 정보의 1차적 근원임을 고려할 때 건강관리제공자들은 부모들의 발열에 대한 비현실적인 공포를 경감시키는 데 중요한 역할을 하여야 할 것이다.

일부농촌지역 재가 낙상경험노인의 낙상두려움 관련요인 (Factors Influencing the Fear of Falling in Elderly in Rural Communities)

  • 본 연구는 농촌지역 재가 낙상경험 노인의 낙상 두려움을 살펴보고, 그 관련요인을 확인하고자 시도하였다. 본 연구는 2010년 2월 1일부터 3월 31일까지 경상남도 하동군의 1개 면과 산청군의 1개 읍에 거주하는 노인 중 지난 1년간 낙상 경험이 있는 노인을 대상으로 일대일 면접을 통해 수집한 735명의 자료를 분석하였다. 낙상두려움간의 차이를 보인 것은 성별, 연령별, 학력, 동거 여부, 이동수단, 음주, 경제수준, 낙상횟수, 의료 이용여부, 건강식품구입여부, 보조용품구입여부, 낙상계절, 낙상장소, 낙상이유, 신발종류, 손상정도, 활동제한, 일상생활수행능력 이었다. 농촌 지역 노인의 낙상두려움의 예측요인으로서 위계적 중 다회귀분석을 실시한 결과 낙상횟수가 가장 영향력이 높았으며, 성별, 입원여부, 건강식품구입, 일상생활수행능력 등이 낙상에 대한 두려움의 관련요인으로 나타났다. 이상의 결과를 종합해 볼 때 낙상을 자주 경험하는 노인일수록 낙상에 대한 두려움이 증가할 수 있으므로 낙상이 빈번한 노인을 대상으로 한 자신감 강화 교육 프로그램의 개발과 일상수행능력을 향상시킬 수 있는 방안을 모색하여 보급함으로써 낙상에 대한 두려움을 감소시킬 수 있을 것으로 사료된다.
