The effect of Horse riding exercise, McKenzie Exercise on back pain and muscle activity in patients with low back pain (승마 운동과 McKenzie 운동이 만성허리통증환자의 허리통증과 근활성도에 미치는 영향)

  • The aim of this study was to compare the effects of a horseback riding simulator, McKenzie, and gym ball exercises on the Korean-Oswestry Disability Index (K-ODI) and muscle activation in people suffering from chronic low back pain. The study was conducted on 30 adults with a K-ODI of 5 or more and chronic low back pain. This experiment was conducted from May to June 2020. They were randomly divided into the horse-riding exercise group (HEG), McKenzie exercise group (MEG), and the gym ball exercise group (GEG). Each group performed the relevant exercises for six weeks, three times a week. The low back pain was measured with K-ODI, and muscle activation was measured with surface electromyography (SEMG). A paired t-test was conducted to verify the change before and after the experiment in the groups, and a one-way ANOVA was conducted to verify the difference between the three groups. The results of the study showed significant differences before and after the experiment. K-ODI and muscle activity improved (p>.05), and significant differences were also found between the groups (p<.05). This study concluded that horse riding and McKenzie exercises could be effective methods for the treatment of pain and to stabilize the lumbar region in patients with chronic low back pain.

  • Objectives : To report the clinical application and safety of Traditional Korean Medical treatment for patients with low back pain during pregnancy. Methods : Three patients who suffered from low back pain during pregnancy were treated with acupuncture, pharmacopuncture, moxibustion, and herbal medicine for 12 to 21 days. Numeric rating scale (NRS) of low back pain and Oswestry Disability Index (ODI) were checked to measure the outcome. A liver function and renal function test was carried out to identify any drug-induced liver or renal injury, and any adverse events were monitored thoroughly until delivery. Results : Low back pain reduced on NRS in all cases, and no serious adverse effects were found in the patients or infants. Conclusion : This study suggests that Traditional Korean Medical treatment is a candidate therapy for relieving low back pain during pregnancy, and also is safe for both patients and infants.

  • This study is designed to evaluate the effect of conservative oriental medical treatment with Kyungkuen Chuna for Herniated intervertebral lumber disc(HIVD) patient. The improvement of the patient was evaluated by Range of Motion(ROM), Straight Leg Rasing Test(SLRT), Visual Analogue Scale(VAS), Oswestry Low-back Pain Disability Index(ODI). After conservative oriental medical treatment with Kyungkuen Chuna, ROM, SLRT, VAS and ODI were significantly improved. It is suggested that Conservative oriental medical treatment with Kyungkuen Chuna could be effective methods to treat the HIVD patient.

  • 본 연구는 복식호흡을 병행한 척추 안정화 운동이 요통환자의 균형능력과 통증에 미치는 영향을 알아보기 위해 시행되었다. 20대 요통환자 18명을 대상으로 하였으며 실험군은 척추안정화 운동을 실시하는 동안 복식호흡을 함께 적용하였으며 대조군은 척추안정화 운동만을 시행하였다. 운동은 8주간 적용하였으며 운동 시작 전과 후에 오스웨스트리 요통장애지수 설문지를 사용하여 통증을 비교하고 BioRescue를 사용하여 신체중심이 이동한 길이와 면적을 측정하여 균형능력을 비교하였다. 실험결과, 두 그룹 모두 운동 후, 요통장애지수가 유의하게 감소하였으나 그룹간의 유의한 차이는 없었다. 신체중심이동면적 비교에서 실험군은 운동 후 유의한 감소가 있었으나 그룹간의 유의한 차이는 없었다. 신체중심이동길이의 비교에서 눈을 감은 상태에서 실험군은 유의한 차이가 있었으나 대조군은 유의한 차이가 없었으며 그룹간 비교에서도 유의한 차이가 있었다. 이러한 결과를 통해 요통환자에게 안정화운동과 복식호흡을 함께 병행하여 실시한다면 더욱 효과적일 것이라고 생각된다.

  • Objectives: The purpose of this study is to report clinical effects of oriental medicine w~h conservative treatments including acupuncture, Chuna treatment and herbal medicine on a patient with low back pain and lower limb numbness caused by HIVD and schwannoma. Methods: The patient was diagnosed with HIVD and schwannoma through the MRI scan. This case was treated with acupuncture, Chuna treatment and herbal medicine during the whole admission period. We evaluated the progress of the symptoms with objective criteria such as NRS(Numerical Rating Scale), SLRT(Straight Leg Raising Test), ODI(Oswestry Disability Index) score and MMT(Manual Muscle Testing). Results and Conclusions: After weeks of conducting conservative treatment on patient with low back pain and lower limb numbness caused by HIVD and schwannoma occurred at the lumbar spine, significant improvements of the symptoms were seen. These results suggest that in the case of low back pain and lower limb numbness caused by HIVD and schwannoma, conservative treatments can be considered as one of the options of treating the symptoms beside surgical way.

An Investigation on the Effects of Wet Cupping on Wisu (BL21) for Non-acute Low Back Pain: A Pilot Randomized Controlled Trial (비급성 요통에 대한 위수혈 자락관법의 효과 탐색: 예비 무작위 대조군 시험)

  • Objectives This study was designed to investigate the effects of wet cupping on Wisu (BL12) in non-acute low back pain patients. Methods We recruited 30 participants for this study. Fifteen patients were randomly assigned to the Wisu (BL21) treatment group (WT group) and 15 were assigned to the non-acupoint treatment group (NT group). Both groups were treated with the pricking-cupping bloodletting method three times. Values at baseline and follow-up were analyzed by Wilcoxon signed rank test and the differences between the two groups were determined by Wilcoxon rank sum test. p-values less than 0.05 were considered significant. The primary outcome was the visual analogue scale (VAS), and secondary outcomes were the Oswestry disability index (ODI), Rolland-Morris disability questionnaire (RMDQ), Euroqol-5 dimension questionnaire (EQ-5D) and finger-to-ground distance (FTGD). These outcomes were measured on the day of first treatment before the procedure and on follow-up 7 days after the last treatment. Results Significant changes were identified in the VAS for pain and ODI in each group after wet cupping treatment on Wisu (p<0.05). However, no significant changes were found between groups. Meanwhile, RMDQ and EQ-5D were significantly decreased only in the NT group (p<0.05) without any differences between groups. FTGD was decreased in both groups, but not significantly. Conclusions Wet cupping with both Wisu treatment and non-acupoint had significant effects on non-acute low back pain, although there were no differences between the two groups. A large-scale study is needed to identify the effect of wet cupping on Wisu.

A Clinical Case of Oriental Medical Treatment on Failed Back Surgery Syndrome (요추 추간판 탈출증으로 3차례 수술적 처치를 시행한 수술 실패 증후군 환자에 대한 보존적 치료 치험 1례)

  • Objectives : This study was performed to report the effect of oriental medical treatment for the failed back surgery syndrome(FBSS) patient after three times surgical operation treatment on herniated nuclues pulposus(HNP) L5-S1. Methods : A 70-year old patient after three times surgical operation treatment was admitted with low back pain and left leg radiation pain. We treated her by oriental medical treatment and measured McGill Pain Questionnaire-Short Form(SF-MPQ), visual analogue scale(VAS) and Oswestry disability index(ODI). Results : After treatment, most symptoms decreased, SF-MPQ changed 36 to 17, VAS score changed 9 to 4, ODI changed 39 to 27. Conclusions : In this study, oriental medical treatment was effective in failed back surgery syndrome patient. But the rigorous studies will be needed to define clearly that oriental medical treatment is effective in failed back surgery syndrome.

  • Objective: Herniation of Nucleus Pulpous(HNP) of L-spine is the most important reason that causes low back pain. The aim of this study is to investigate the effectiveness of Bee venom acua-acupuncture therapy for HNP. Methods: We divided HNP patient into 2 groups: Control group with acupuncture therapy only, and Experimental group with both acupuncture and bee venom acua-acupuncture therapy. To estimate the efficacy of treatment that applied for two groups, we used visual analog scale(VAS) and Oswestry disability index(ODI). We compared the VAS and ODI score of two groups statistically. Results & Conclusions: 1. In VAS, experimental group shows statitically significant improvement rate, but control group does not show statistical significance. 2. In ODI, both the experimental and control group shows statistically significant improvement. 3. By comparing the experimental and control group, there was more significant improvement in experimental group than control group of VAS and ODI. As a result, bee venom acua-acupuncture therapy can be used with acupuncture therapy for highly effective treatment for HNP.

  • Objectives : The purpose of this study was to evaluate the effect of electro-acupuncture treatment for low back pain and radicular pain in patients diagnosed with Lumbar HIVD. Methods : This clinical study was carried out on 92 patients with low back pain and radicular pain diagnosed with HIVD, who had been admitted from Mar. 2011 to Dec. 2011. 60 patients were divided into two groups, group A and group B, of 46 patients each. Group A was treated with electro-acupuncture treatment and general acupuncture treatment twice per day for more than 2 weeks. Group B was treated with general acupuncture treatment twice per day for more than 2 weeks. Verbal numerical rating scale(VNRS) and Oswestry disability index(ODI) were used to evaluated the effectiveness of the Oriental medical treatment. Results : In both two groups, VNRS, ODI, patient's condition grade decreased significantly in ststistics as treatment was perfomed. Between the two groups there was no significant difference in the VNRS, patient's condition grade. But there was significant difference in the ODI. Conclusions : These results suggest that both electro-acupuncture and general acupuncture were effective in reducing low back pain and radicular pain caused by HIVD. More systematic clinical stuty on electro-acupuncture is needed.

  • Objectives : This is a case series reporting of the improvement in patients with a lumbar herniated intervertebral disc by means of acupotomy combined with oriental medicine. Methods : We treated seven patients with a lumbar herniated intervertebral disc with acupotomy combined with oriental medical treatment. We checked the numeric rating scale(NRS), oswestry low back pain disability index(ODI) and range of motion(ROM). Results : NRS and ODI decreased, and ROM improved in all cases. Conclusions : This study shows acupotomy has a meaningful effect when applied on a lumbar herniated intervertebral disc.
