Estimation of Lower Jaw Density using CT data

  • Bone density is one of the factors in the early failure of dental implants and doctors should make a preoperative assessment of jaw bone density using patient's CT data before dental implant surgery in order to find out whether the patient has osteoporosis and osteopenia. The main goal of this study was to propose a method that based on image processing techniques in order to provide accurate information about where to drill and place an abutment screw of implants in the jaw bone for doctors and reduce human activity for the estimation of the local cancellous bone density of mandible using CT data. The experiment was performed on a computed tomography data of the jaw bone of two different individuals. We assumed that the result of the estimation of jaw bone density depends on the angle of drilling and average HU (Hounsfield Unit) values were used to evaluate the quality of local cancellous bone density of mandible. As a result of this study, we have been developed a toolbox that can be used to estimate jaw bone density automatically and found a positive correlation between the angle of the drill and time complexity but a negative correlation between the diameter of the drill and time complexity.

High concentration of calcium represses osteoblast differentiation in C2C12 cells

  • Calcium is the most abundant stored mineral in the human body and is especially vital for bone health; thus, calcium deficiency can cause bone-related diseases, such as osteopenia and osteoporosis. However, a high concentration of serum calcium, which is commonly known as hypercalcemia, can also lead to weakened bones and, in severe cases, osteosarcoma. Therefore, it is necessary to maintain the concentration of calcium that is appropriate for bone biology. In the present study, we aimed to elucidate the effects of high concentration of calcium, approximately 2 folds the normal calcium level, on osteoblast differentiation. The CaCl2 treatment showed dose-dependent suppression of the alkaline phosphatase activity and mineralized nodule formation. Calcium showed cytotoxicity at an extremely high concentration, but a moderately high concentration of calcium that results in inhibitory effects to osteoblast differentiation showed no signs of cytotoxicity. We also confirmed that the CaCl2 treatment repressed the mRNA expression and protein abundance of various osteogenic genes and transcriptional factors. Considered together, these results indicate that a high concentration of calcium negatively regulates the osteoblast differentiation of C2C12 cells.

Sex- and age group-specific associations between intakes of dairy foods and pulses and bone health in Koreans aged 50 years and older: Based on 2008~2011 Korea National Health and Nutrition Examination Survey (50세 이상 한국인의 성·연령군별 우유류와 두류 섭취량과 골 건강과의 관련성 : 2008~2011 국민건강영양조사 자료를 이용하여)

  • Purpose: This study was performed to examine associations of intakes of milk and dairy products, pulses, and soy foods with bone health in Koreans aged 50 yr and older. Methods: A total of 3,201 men and 3,581 women aged 50 yr and older who participated in the 2008~2011 Korea National Health and Nutrition Examination Survey were grouped by sex and age groups of 50~64 yr and 65 yr and older. Subjects within each sex and age group were divided into three bone health groups: normal, osteopenia, and osteoporosis groups based on bone mineral density. Intakes of nutrients and foods derived from 24-hour recall data were compared among three bone health groups. Associations between intake frequencies of foods, including milk, yogurt, tofu, or soy milk, and osteoporosis risk were evaluated based on confounding risk factor-adjusted logistic regression. Results: Calcium intake was in the order of normal, osteopenia, and osteoporosis in men (p < 0.01) and women (p < 0.05) aged 50~64 yr as well as in men aged 65 yr and older (p < 0.001). In women aged 50~64 yr, intake of milk and dairy products was lower in the osteoporosis group (p < 0.01) as compared with the osteopenia group. Intake of pulses or tofu was not significantly different among bone health groups. Odds ratio (OR) for milk intake frequency (${\geq}2$ times/week) compared to intake frequency less than 1 time/month was 0.45 (95% CI 0.24~0.85, p for trend = 0.022) in men aged 65 yr and older. The OR for yogurt intake frequency (1 time/month~1 time/week) was 0.47 (95% CI 0.30~0.73, p for trend = 0.019) in women aged 50~64 yr. Intake frequency of tofu or soy milk was not associated with reduced risk of osteoporosis in all groups. Conclusion: Dairy food intake was significantly associated with bone health, and its effect was sex- and age group-specific, whereas soy food intake was not. Dietary intervention to prevent osteoporosis would be effective for women aged 50~64 yr old and for men aged 65 yr and older.

Gender Difference in Osteoporosis Prevalence, Awareness and Treatment: Based on the Korea National Health and Nutrition Examination Survey 2008~2011 (골다공증 유병률, 인지율, 치료율 및 영향요인의 성별 비교: 국민건강영양조사 자료(2008~2011년) 활용)

  • Purpose: The aim of the study was to assess and identify gender differences in factors associated with prevalence, awareness, and treatment of osteoporosis. Methods: Data for 3,071 men and 3,635 women ($age{\qeq}50$) from the Korea National Health and Nutrition Examination Survey 2008~2011 were included. Osteoporosis was defined by World Health Organization T-score criteria. Impact factors and odds ratios were analysed by gender using multivariate logistic regression. Results: Osteoporosis prevalence rates were 7.0% in men and 40.1% in women. Osteopenia rates were 45.5% and 46.0% respectively. Among respondents with osteoporosis, 7.6% men and 37.8% women were aware of their diagnosis. Also 5.7% men with osteoporosis and 22.8% women were treated. Higher prevalence was found among respondents who were older, at lower socioeconomic levels, with lower body mass index and shorter height in both genders, and among women with fracture history, and non-hormonal replacement therapy. Awareness and treatment rates for the risk groups were similar compared to the low risk controls for both genders. Fracture history increased awareness and treatment rates independently for both genders. Women with perceived poor health status and health screening had increased awareness and treatment rates, but not men. Conclusion: Results indicate that postmenopausal women have a higher prevalence of osteoporosis than men and awareness and treatment rates were higher than for men. Despite gender difference in prevalence, osteoporosis was underdiagnosed and undertreated for both genders. Specialized public education and routine health screenings according to gender could be effective strategies to increase osteoporosis awareness and treatment.

The Relationship Between Body Composition and Bone Mineral Density in College Women (여대생의 신체조성과 골밀도의 관계)

  • Purpose: To investigate body compositons and bone mineral density(BMD) in college women and to find the relationship between them. Method: From January to March of 2001, BMD at four parts(forearm, lumbar, femur and whole body), body mass index(BMI), body fat mass(BFM), lean body mass(LBM) and body fat percentage(%Fat) were measured with the Dual Energy X-ray Absorptiometry. Other physical characteristics were measured with a scale, a height measurer, and questionnaires. Result: Grouping by the BMI, 43.2% showed low weight, and 5% over weight. When applying the percent Fat, 43.8 % was diagnosed as obesity group. The fact indicate that a majority of college women have unbalanced body composition with high percent Fat, compared to their body weight. Assessing the BMD with the WHO standards, 91.4~95.7% of the BMD of forearm and whole body was normal. But, 40.3% and 33.1~43.9% showed osteopenia at lumbar and femur, and 104 %, 0.7~7.2% showed osteoporosis. The BMD at all parts showed significant correlation each other(r=.29~.89, p=.001~.000). Body weight and BMI showed correlations to with BMDs at all parts of the body(r=.19~46, p=.025~.000; r=18~.45, p=.039~.000). But the percent Fat had a correlation with only femur neck BMD(r=.19, p=.024). Conclusion: This study showed a majority of healthy college women were exposed to the risk for osteoporosis. Additional study is required to develop nursing interventions to remove the risk factors of osteoporosis. In particular, the acquisition of balanced body composition is necessary, increasing body weight and BMI through the increase of LBM, not through the quantitative increase of BFM.

EXPERIMENTAL STUDY OF PERI-IMPLANT TISSUE REACTION IN OVARIECTOMIZED OSTEOPOROTIC RATS (난소절제로 유도된 골다공증 흰쥐에서 implant 주위 조직 반응에 관한 실험적 연구)

  • This study was designed to investigate the peri-implant tissue reaction in ovariectomized osteoporotic female rats, and to evaluate effects of estrogen, calcitonin, parathyroid hormone on the bone - implant interface in osteoporotic rats. 120 Sprague - Dawley rats were used in this experiments. Osteoporosis was induced by bilateral ovariectomy. They were divided 5 groups : sham-operated control group(Sham), ovariectomized group (OVX), OVX and estrogen treated group (OVX+E), OVX and PTH treated group (OVX+PTH), and OVX and calcitonin treated group (OVX+CT). Eight weeks after ovariectomy, two titanium screw implants were inserted into the left tibia of each rat. Eight weeks after the insertion of the implants, the periotest values (PTV) of implant were examined, and the rats were sacrificed, and examined the reaction of bone tissue surrounding the implant both histologically and histomorphometrically. The bone density and ash weight of opposite right tibia were examined. Over 40 rats were fractured on left tibia that was implant inserted. On histologically finding, all groups were osseointegrated well, especially in OVX+PTH group. In OVX group, tibial cortical bone showed many large harversian canal and microfracture lines. The OVX+PTH group showed the lowest mean PTV (-2.33) (p<0.05), and the hightest mean bone - implant contact percentage (89%) (p>0.05). But the OVX+CT group showed the highest mean bone density ($5.45mg/cm^3$) and ash weight (56.12%) (p<0.05). The results indicate that PTH treatment enhances osseointegration of implant in OVX rats, and CT treatment depresses bone turnover and prevent the development of osteopenia in OVX rats.

A Study of Prevalence and Related Risk Factors of Osteoporosis for Employees (직장 근로자의 골다공증 유병실태와 관련요인 연구)

  • Background and Objectives: The study was performed to investigate the status of osteoporosis for S electronics workers in H hospital of G city form April 21 to May 31 2008. Material and Method: The result of this research is following. Results: As results of osteoporosis analysis based on general characteristic, for men, something of 21 was the most frequent and osteoporosis of 13 was the second most frequent, and for women, good of 12 was the most frequent and care was the second most. The results concludes that men have more problems with osteoporosis that women have. In age of 40 years or more, care level of 24(88.9%) was the most frequent and good level of 15 people was the second most. This result concludes that age of 40 year is target subjects for care. In position, senior has 14 people who have care level(42.4%) and 8 people(24.2%)of osteoporosis level. The result concludes than the senior has more osteoporosis than the employ has. Conclusion: In conclusion, the age, position, weight are related to osteoporosis.

A Study on BMD of Some Women Suffering from General Pain after Childbirth (산후신통(産後身痛)을 호소하는 일부 산모의 골밀도 분석)

  • Purpose: The purpose of this study was to examine Bone Marrow Density (BMD) of women suffering from general pain after childbirth. Methods: We studied 18 patients visiting $\bigcirc\bigcirc$hospital from March 2007 to May 2010. The subjects were categorized in two groups, symptom group(n=12) and no symptom group(n=6). We measured BMD using Dual Energy X-ray Absorptiometry (DEXA) and studied the difference of BMD between two groups by Student T-test using SPSS for windows(version 13.0). Results: The general characteristics such as age, days of postpartum, primiparity, cesarean section, breast-feeding of two groups were not different statistically. BMD, T-score and Z-score of symptom group were statistically lower than no symptom group. Conclusion: The results suggest that women suffering from general pain after childbirth shows low BMD, T-score and Z-score. It seems to be due to the kidney deficiency after childbirth.

A case of Rothmund-Thomson syndrome (Rothmund-Thomson 증후군 1례)

  • Rothmund-Thomson syndrome (RTS), is a rare autosomal recessive disorder, characterized by : skin photosensitivity, poikiloderma, sparse hair, sparse eyebrows/lashes, short stature, skeletal abnormalities, cataracts, and an increased risk of malignancy. Skeletal abnormalities include : dysplasia, absent or malformed bones, such as absent radii, osteopenia, and delayed bone formation. RTS is thought to result from chromosomal instability, and children with RTS are at risk of cancer. Reported cancers in children with RTS include : basal cell carcinoma, squamous cell carcinoma of the skin and osteosarcoma of bone. We report an 11 year-old boy, who presented to our institution with poikilodermatous skin change with telangiectasia and hyperpigmentation, absence of radius and thumb, and the development of osteosarcoma of the left tibia. The patient is now receiving supportive care and is receiving maintenance chemotherapy after surgery for osteosarcoma.

A Case of Neonatal Bartter Syndrome (신생아 Bartter 증후군 1례)

  • Battler and Bartter-like syndromes, which include classic Bartter syndrome(type III), neonatat Bartter syndrome(type I, II or IV) and Gitelman syndrome, are autosomal - recessively inherited renal tubular disorders characterized b)r hypokalemic metabolic alkalosis, salt wasting and normal to low blood pressure. Neonatal Bartter syndrome is characterized by intrauterine polyhydramnios, premature delivery, life-threatening episodes of fever and dehydration, subsequent failure to thrive, and severe hypercalciuria with nephrocalcinosis and osteopenia. It is caused by mutations in NKCC2(type I), ROMK(type II) or BSND(type IV) genes. If diagnosed and treated early, the progression to renal failure can be prevented and catch-up growth and normal development are achieved. We report here a 6 month-old infant with neonatal Bartter syndrome who presented with hypokalemic metabolic alkalosis, polyhydramnios and premature delivery, persistent high fever and dehydration, failure to thrive, hypercalciuria, and nephrocalcinosis. He received indomethacin and potassium chloride per os and show ed catch-up growth and normal developmental profile at 19 months of age. (J Korean Soc Pediatr Nephrol 2005;9:255-262)
