The purpose of this study was to investigate the change in blood velocity(mm/sec) when compression and/or heat were applied to the knee joint for the elderly who has both normal and painful legs with osteoarthritis(OA). Experimental compression knee band was prepared from the 3D knee data of the average women in 60's. 3D replica of knee was reduced by 7, 10, and 13% from the nude pattern in course direction. Clothing pressure was measured at the front and back of each healthy and painful knee of elderly women for one minute while standing and sitting on the chair. Blood velocity was measured at 13 cm upper from the mid-patella for 15 minutes. Results are as follows: first, compression or heating treatment itself did not change blood velocity of both legs; second, combination treatment with heating and compression was effective to increase blood velocity. In details, for healthy legs, combination treatments with compression by 10% reduced pattern(about 1.3kPa) and heating($43^{\circ}C$) induced the maximal blood velocity, however, for knees with OA, 7% reduced pattern(about 1.0kPa) with simultaneous heating($43^{\circ}C$) was more effective than other cases. These results indicated that pain and spasticity of knee joint with OA could be reduced by applying heat and compression therapy, where the compression level of painful knee should be slightly lower than of healthy leg.

Three cases who had medial compartment osteoarthritis of the knee (Kellgrene-Laurence grade 3 and Outerbridge grade 4) and anterior instability of the knee due to rupture of the anterior cruciate ligament in relative young ages underwent staged anterior cruciate ligament reconstruction followed by medial unicondylar arthroplasty in 2 cases and simultaneous anterior cruciate ligament reconstruction and unicondylar arthroplasty. We evaluated clinical results some kinds of preoperative and postoperative International Knee Documentation Committee (IKDC), Lysholm score and last follow-up hospital for special surgery (HSS), knee society score (KSS). We consider that medial unicondylar arthroplasty with staged or simultaneous anterior cruciate ligament reconstruction is very good option of the treatment for the anterior instability and pain from advanced arthritis.
Objectives: In order to investigate the efficacy and safety of the newly developed herbal medicine Jetongdan, a placebocontrolled, randomized clinical trial of patients with osteoarthritis of the knee was undertaken. Methods: Data were obtained from 80 patients with OA of the knee. After enrollment, they were asked to answer a disease-specific questionnaire (Western Ontario and McMaster Universities (WOMAC) OA index) and analyzed with the erythrocyte sedimentation rate (ESR) in order to evaluate the efficacy of Jetongdan, and analyzed for aspartate transaminase (AST) level, alanine transaminase (ALT) level, blood mea nitrogen (BUN) level, and creatinine (Cr) level in order to evaluate the safety of Jetongdan. Results: The liver function and renal function did not deteriorate after treatment with Jetongdan. Composite WOMAC score and physical function subscale was improved, but pain subscale, stiffness subscale, and ESR were not improved by. This was possibly because the baseline characteristics of the two groups were not homogenized after randomization. Conclusions: Jetongdan could be a promising treatment option for osteoarthritis of the knee. Further study in a larger population with appropriate severity grades is recommended.

PURPOSE: This study aimed to assess the relationship between the severity of radiographic features and pain and function in patients with knee osteoarthritis (KOA). METHODS: Seventy-eight subjects (14 men, 64 women) with KOA, between the ages of 41 and 83 years (mean age, 61.29 years), were included. All the subjects diagnosed with KOA were scored for severity of radiographic KOA according to the Kellgren-Lawrence (K/L) grade, visual analogue scale (VAS), knee joint range of motion (ROM), Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC), maximum muscle power (MMP), and sit-to-stand (STS) and one-leg standing (OLS) tests. Associations among the K/L grade, diagnosis, pain, and function were examined by correlation analysis. RESULTS: There were no significant differences between the K/L grade, and the VAS, STS test time, and WOMAC scores (p>.05). There were no significant differences between the K/L grade, bilateral ROM, MMP, and left OLS test time (p>.05). However, there was a significant difference between the K/L grade and right OLS test time (p<.05). The K/L grade was negatively correlated with the left OLS test time(r=-.24, p<.05) and with the right OLS test time (r=-.307, p<.01). CONCLUSION: These results suggest that radiographic KOA was not associated with pain, knee MMP, ROM, and STS test time, but had a weak negative correlation with OLS test time.

Objective: The purpose of this case study was to investigate selective region high-frequency diathermy at trigger points with therapeutic exercises on pain, function, balance and gait in older patients with degenerative knee osteoarthritis (DKO). Design: A case report. Methods: The patient who participated in this study was a 71-year-old woman, who had been diagnosed with moderate osteoarthritis with grade II Kellgren & Lawrence grading scale. The intervention consisted of selective region high-frequency diathermy at trigger points, with hip and knee stretching and strengthening exercises. The participant was given assessments before and after every intervention session using the Visual Analogue Scale (VAS), Western Ontario and McMaster Universities Arthritis Index (WOMAC), the Timed Up and Go test (TUG) and the 10 Meter Walk Test (10MWT). The participant performed the intervention 18 times for a total of 30 minutes each. Results: As a result of this study, the patient VAS decreased to 3 points, and the WOMAC decreased to 53 points. In addition, the TUG decreased to 3.25 s and the 10MWT decreased to 1.14 s. Conclusions: The results of this study suggest that selective region high-frequency diathermy at trigger points with therapeutic exercises may be an effective intervention to decrease pain, improve knee function, balance and gait in patients with DKO. The selective region high-frequency diathermy with therapeutic exercises may be feasible and provide potential benefits for rehabilitation of DKO.

PURPOSE: This study examined the effects of motion taping during combined lower extremity exercise application on pain, ROM, muscle strength, and WOMAC index in middle-aged women with osteoarthritis of the knee joint METHODS: The subjects were 26 middle-aged women with osteoarthritis in the knee joint. Twenty-six subjects were divided into two groups. Each group contained 13 subjects. Three subjects dropped out in the middle of the experiment. The intervention was six weeks, three times a week. The control group performed traditional physical therapy and lower extremity exercise. The experimental group was motion taping applied to the lower extremity exercise. The measurement factors were knee joint pain, quadriceps muscle strength, and WOMAC index. The pain was measured using the numeric pain scale instrument (NPSI). The quadriceps strength was measured using the 1RM method. RESULTS: In both groups, pain, muscle strength, and WOMAC index showed significant improvement according to the intervention (p < .05). In comparison between groups of the change values pre-post intervention, pain and WOMAC index showed statistically significant differences between the two groups. On the other hand, there was no difference in strength between the two groups. CONCLUSION: When performing a rehabilitation exercise program for middle-aged women with osteoarthritis of the knee, motion taping has a positive effect on pain and the WOMAC index.

PURPOSE: The Western Ontario and McMaster Universities Arthritis (WOMAC) index has been used to measure the outcome of total knee replacement (TKR), but studies on its reliability and validity are limited. The present study examined the reliability and validity of this index for patients with knee osteoarthritis who underwent TKR. METHODS: Seventy-one inpatients and outpatients who underwent bilateral TKR for knee osteoarthritis were included in this study. The pain assessment scale and WOMAC index were used to evaluate the participants every two weeks to examine the test-retest reliability, internal consistency, and construct validity. RESULTS: The test-retest reliability scores for pain, stiffness, and physical function were .75-.92, .85-.90, and .75-.95, respectively. The corresponding intraclass correlation coefficients were .75-.88, .76-.88, and .71-.95, respectively. The internal consistency score in the first and second examinations was .92. Furthermore, the construct validity scores for pain, stiffness, and physical function were .83, .41, and .58, respectively. CONCLUSION: The application of the WOMAC index in patients who underwent TKR showed high test-retest reliability and internal consistency with the use of the WOMAC index and good validity with the use of the pain assessment scale.

Purpose: We defined osteoarthritis of the knee as knee pain and crepitus in over 40 years old. The usual clinical manifestation include pain, stiffness, crepitus and loss of function. Methods: We studied 40 cases of the degenerative arthritis of knee clinically. The body mass index(BMI, weight(kg)/height($m^2$) was used as a measure of obesity. Pain self assessment scale by Million Index in according to age, occupation, BMI. Results: Gender by pain self assessment scale was 6.5 in male and 6.6 in female(P<0.05). The prevalence rates obese was 40%. Occupation by pain self assessment scale was 7.0 in Sitting and 5.7 in Standing(P<0.05). Prevalence was increased with age by pain self assessment scale in aged 40-69 years. Conclusion: It has been known that the obesity is one of the predispsing factors of the primary degenerative arthritis of knee. A flexion weight bearing view of the knee obtained at $30^{\circ}$ to $40^{\circ}$ of the joint flexion may be more sensitive in assessing damages to hyaline cartilage because the knee flexion is an important component of the stance phase.

PURPOSE: To determine the effect of four weeks of exercise training on knee circumference and range of motion (ROM), pain perception, wellness, and VO2max in patients at Osun State University Teaching Hospital, Osogbo, Nigeria. METHODS: Forty subjects who met the inclusion criteria were randomized into control and experimental groups. patients were subjected to four weeks of flexibility and strengthening exercise, which included squatting exercise, ROM exercise, both groups were given some cardiovascular exercise. VO2max), Wellness score and circumference. RESULTS: Compared to the baseline values (week 1), there was increase in the knee ROM, and decrease in the pain perception in both groups after four weeks. However, the improvements seen in the experimental group, were significantly better relative to the control group after four weeks of intervention. owever, knee ROM and wellness scores of the experimental group, and the VO2max of both significantly increase, but significantly decrease in the knee circumference and pain perception of both groups following 4 weeks of exercise was recorded. CONCLUSION: he exercise modality in the study promote cardiovascular endurance, knee joint integrity, reduce pain perception in subjects. xercise intervention s beneficia to both groups since both reduction in the pain felt and also in knee swelling.
Objectives : In cases of osteoarthritis, the hypofunction of the cartilage and joint leads to a limited range of joint motion, swelling, and pain, which is generally treated using pharmaceutical drugs (e.g., anti-inflammatory agents, cartilage protectants, and nonsteroidal anti-inflammatory drugs) or replacement arthroplasty. However, long-term drug treatment is associated with adverse effects on the gastrointestinal systems. The present study aimed to evaluate the ability of Giheolsotong-hwan to treat of osteoarthritis symptoms in the MIA-induced rat model based on histological analysis, and factors that are associated with inflammation and bone mineral metabolism. Methods : Giheolsotong-hwan was administered orally at doses of 200 mg/kg/day or 400 mg/kg/day for 2 weeks before direct injection of monosodium iodoacetate ($3mg/50{\mu}{\ell}$ of 0.9% saline) into the intra-articular space of the rats' right knee. The rats subsequently received the same doses of oral Giheolsotong-hwan for another 4 weeks. We evaluated the treatment effects based on serum biomarkers and histopathological analysis of the knee joints. Results : Compared to those in control rats, the Giheolsotong-hwan treatments significantly decreased the serum concentration of inflammation factors (i.e., $IL-1{\beta}$, IL-6, $TNF-{\alpha}$, $PGE_2$, and $LTB_4$), and bone degrade factors (i.e., MMP-9, CTX-II, and COMP). In addition, the Giheolsotong-hwan treatments significantly increased the concentration of glycosaminoglycans of bone defence factors, but no chage the TIMP-1. Furthermore, the Giheolsotong-hwan treatments effectively preserved the knee cartilage and proteoglycan. Conclusion : The results indicate that Giheolsotong-hwan treated osteoarthritis symptoms. Thus, Giheolsotong-hwan may be a novel oriental therapeutic option for the management of osteoarthritis.
