임플란트 hexagon 높이에 따른 임플란트와 주위 조직의 응력분포 평가 (Evaluation of the stress distribution in the external hexagon implant system with different hexagon height by FEM-3D)

  • 연구 목적: 본 연구는 hexagon 높이에 따른 임플란트 각 부위와 주위 지지조직의 응력분포를 3차원 유한요소 해석을 통해 평가하여 hexagon 높이가 기계적 안정성에 미치는 영향을 평가하고자 시행되었다. 연구 재료 및 방법: 외측 연결 형태의 ${\phi}4.0mm{\times}11.5mm$ USII (Osstem Co., Pusan, Korea) 임플란트 시스템을 이용하여 하악 제 1대구치 부위에 임플란트를 식립하여 보철 수복한 경우를 연구 모델로 가정하고 임플란트 고정체의 외측 연결부인 hexagon의 높이를 각각 0.0 mm, 0.7 mm, 1.2 mm, 1.5 mm로 적용한 CAD data를 유한요소 모형화하였다. ABAQUS 6.4 (ABAQUS Inc., Providence, RI, USA)를 이용하여 산출된 응력 값 중에서 등가응력을 기준으로 각 요소(상부 치관, 지대주 나사, 고정체, 치밀골, 해면골)에서 나타나는 최대 응력 값을 비교 하였다. 결과: 외측 연결을 갖는 임플란트의 hexagon의 높이는 고정체, 지대주 나사, 상부 보철물 그리고 주위 지지골에 대해 응력 분산에 영향을 주었다. Hexagon의 높이가 증가할수록 임플란트의 응력 분산은 더 잘 이루어졌으며, 최대 응력 값의 감소를 보였다. Hexagon의 높이가 1.2 mm 이상이 되면 응력 분포에 더 이상 크게 기여하지 않았다. 결론: 외측연결을 갖는 임플란트에서 hexagon은 응력 분산에 필수적인 요소이며 그 높이가 증가할수록 더욱 효과적인 응력의 분산이 나타났다.

임플란트 드라이버의 호환성에 대한 연구 (A study on the compatibility of implant drivers)

  • 연구 목적: 임플란트의 종류가 다양해짐에 따라 임플란트 드라이버의 호환성에 대한 연구가 필요하여 임플란트 제품별 드라이버의 형태를 분류하고 직경을 측정하여 비교 분석하였다. 연구 재료 및 방법: 단국대학교 부속치과병원에서 사용 중인 12개회사[Nobel Biocare (Nobel), Institute Straumann (Straumann), Zimmer Dental (TSV), Shinhung (Luna), Astra Tech Dental (Astra), Dentium (Dentium), Osstem Implant (Osstem), DIO Implant (DIO), BIOMET 3i (3i), NeoBiotech (Neo), Megagen Implant (Megagen), SNUCONE (SNUCONE)] 임플란트 제품 드라이버를 연구대상으로 하였으며 임플란트 드라이버의 형태를 분류하고 이 중 호환이 가능한 Hexagon과 torx에 해당하는 드라이버들의 유효길이, 유효길이별 상단, 중단, 하단의 직경을 각각 10개씩 측정하였다. 각 직경의 측정값을 호환성 분석공식에 대입하여 호환성을 산출하였다. 결과: 분석결과, 유효길이 상단에서는 Neo, 3i, Megagen, DIO, SNUCONE, Luna들은 같은 직경(1.20 mm)을 가졌으며 Osstem (1.17 mm)은 호환 가능한 범위에 있었다. Dentium, Astra는 같은 직경(1.25 mm)을 가지며 TSV (1.23 mm) 가 이들과 호환 가능하였다. 유효길이 중단에서는 Dentium과 Astra가 같은 직경(1.35 mm)을 가졌고 3i, DIO, Osstem, TSV도 1.25 mm의 동일한 직경을 가졌다. Neo와 Megagen은 가장 작은 직경(1.22 mm)로 나머지와 호환되었다. 유효길이 하단에서는 일부 드라이버에 호환성이 확인되었으나 대부분 나사의 연결부 깊이가 2 mm 이내인 점을 감안하면 유효하지 않은 결과로 판단되었다. Nobel은 Straumann에 사용할 수 있는 것으로 나타났으며 대부분의 hexagon driver를 trox에 사용할 수 있었다. 결론: 임플란트 드라이버의 각 부위별 직경을 측정한 결과 임플란트 제품간의 호환성이 존재한다는 것을 알 수 있었다. 그러나 호환성이 있는 드라이버의 반복적인 사용이 임플란트 나사와 드라이버에 미치는 영향에 대한 연구가 아직 부족하므로 응급상황 시에만 제한적으로 사용할 것을 권장하며 이에 대한 추가적인 연구가 필요할 것으로 사료된다.

하악 임플란트 오버덴쳐에서 어태치먼트 종류에 따른 응력분포 (The Influence of Attachment Type on the Distribution of Occlusal Force in Implant Supported Overdentures)

  • 하악 완전 무치악 환자에서 이공 사이에 2개의 임플란트를 식립한 후 4종류의 어태치먼트를 사용한 하악 오버덴쳐를 제작하여 유한요소 모델을 만들고 여기에 3가지 하중 조건을 가하여 유한요소 해석 프로그램인 ANSYS 10.0을 이용하여 어태치먼트 종류에 따른 임플란트 주위골과 보철물 및 임플란트와 골 사이 계면에 발생하는 최대 응력의 크기와 응력 분포양상을 분석하였다. 하악골체의 전체적인 골격구조에서 치아 부분은 오버덴쳐로 모델을 형성하였고 오버덴쳐와 하악골 사이에 2mm 두께의 점막을 삽입하였다. 임플란트 오버덴쳐의 유한요소 모델은 ball and socket, Locator, magnet, bar의 각각 4가지 형태를 제작하였다. 고정체의 식립 위치는 양측 이공 전방이고, 경계 조건 중 고정점은 하악각 부위와 모형의 최후방 절점들을 고정점으로 설정하였다. 하중조건은 오버덴쳐의 좌측 제 2소구치에서 제 2대구치 편측 부위에 170N의 하중을, 좌 우측 제 2소구치에서 제 2대구치 양측 부위에 170N의 하중을, $90^{\circ}$수직 방향, $45^{\circ}$경사, 그리고 $0^{\circ}$ 수평 방향으로 가하였다. 결과 분석시 모든 하중방향에서 응력의 크기는 bar에서 임플란트나 오버덴쳐에서 가장 크게 나타났으며, 계면에서의 응력은 모든 어태치먼트에서 비슷한 응력분포가 나타났다. Ball and socket, Locator, magnet에서는 유의성을 보이지 않았다. 응력의 분포도에서는 4종류의 어태치먼트 모두 임플란트 치경부에서 응력이 집중된 양상을 보였으며 bar타입에서는 bar의 중앙에 응력이 크게나타났다. 이상의 결과로 볼 때 어태치먼트의 종류에 따라 응력분포의 차이가 있었음을 확인할 수 있었다. 어태치먼트의 선정 시 응력집중을 완화시킬 수 있는 바람직한 설계와 선택이 필요하리라 사료된다.

FIT OF FIXTURE/ABUTMENT/SCREW INTERFACES OF INTERNAL CONNECTION IMPLANT SYSTEM

  • Statement of problem. Accurate fit between the implant components is important because the misfit of the implant components results in frequent screw loosening, irreversible screw fracture, plaque accumulation, poor soft tissue reaction, and destruction of osseointegration. Purpose. This study is to evaluate the machining accuracy and consistency of the implant fixture/ abutment/screw interfaces of the internal connection system by using a Stereoscopic Zoom microscope and FE-SEM(field emission scanning electron microscope) Materials and methods. The implant systems selected in this study were internal connection type implants from AVANA(Osstem^{\circledR}), Bioplant(Cowell-Medi^{\circledR}), Dio(DIO^{\circledR}), Neoplant(Neobiotech 􀋓), Implantium(Dentium􀋓)systems. Each group was acquired 2 fixtures at random. Two piece type abutment and one piece type abutment for use with each implant system were acquired. Screw were respectively used to hold a two piece type abutment to a implant fixture. The implant fixtures were perpendiculary mounted in acrylic resin block. Each two piece abutment was secured to the implant fixture by screw and one piece abutment also secured to the implant fixture. Abutment/fixture assembly were mounted in liquid unsaturated polyester. All samples were cross-sectioned with grinder-polisher unit. Finally all specimens were analysed the fit between implant fixture/abutment/screw interfaces Results and conclusions. 1. Implant fixture/abutment/screw connection interfaces of internal connection systems made in Korea were in good condition. 2. The results of the above study showed that materials and mechanical properties and quality of milling differed depending on their manufacturing companies.

APPLICATION OF FINITE ELEMENT ANALYSIS TO EVALUATE PLATFORM SWITCHING

  • Statement of problem. Platform switching in implant prosthesis has been used for esthetic and biological purpose. But there are few reports for this concept. Purpose. The purpose of this study is evaluation of platform switching in wide implant by three dimensional finite element analysis. Materials and Methods. The single implant and prosthesis was modeled in accordance with the geometric designs for Osstem implant system. Three-dimensional finite element models were developed for (1) a wide diameter 3i type titanium implant 5 mm in diameter, 13 mm in length with wide cemented abutment, titanium alloy abutment screw, and prosthesis (2) a wide diameter 3i type titanium implant 5 mm in diameter, 13 mm in length with regular cemented abutment, titanium alloy abutment screw and prosthesis(platform switching) was made for finite element analysis. The abutment screws were subjected to a tightening torque of 30 Ncm. The amount of preload was hypothesized to 650N, and round and flat type prostheses were loaded to 200 N. Four loading offset point (0, 2, 4, 6 mm from the center of the implants) were evaluated. Models were processed by the software programs HyperMesh and ANSA. The PAM-CRASH 2G simulation software was used for analysis of stress. The PAM-VIEW and HyperView were used for post processing. Results. The results from experiment were as follows; 1. von Mises stress value is increased in order of bone, abutment, implant and abutment screw. 2. von Mises stress of abutment screw is lower when platform switching. 3. von Mises stress of implant is lower when platform switching until loading offset 4 mm. 4. von Mises stress of abutment is similar between each other. 5. von Mises stress of bone is slightly higher when platform switching. Conclusion. The von Mises stress pattern of implant components is favor when platform switch ing but slightly higher in bone stress distribution than use of wide abutment. The research about stress distribution is essential for investigation of the cortical bone loss.

골밀도 평가방법과 임플란트 안정성 변화의 관찰 (Observation of the change of the dental implant stability and bone density evaluation methods)

  • Purpose: The aim of this study was to examine the correlation of the subjective and the objective evaluation of edentulous ridge bone quality, and to evaluate the change of the dental implant stability in each bone density group for early healing period after implant installation. Methods: Sixty-seven implants(Osstem implant$^{(R)}$, Seoul, Korea) were included in this study. We evaluated the bone density by 2 methods. The one was the subjective method which was determined by practitioner s tactile sense, the other was the objective bone type was based on Hounsfield units. The implant stability in each bone type group was assessed by resonance frequency analyzer(Osstell mentor$^{(R)}$). Data were analyzed for the change of the implant stability, and they were compared to verify the difference of groups at the time of installation, 2, 6, 10, 14 weeks postoperatively. Spearman's correlation was used to demonstrate the correlation between the subjective and the objective evaluation of the bone density, and analysis of variance(ANOVA) was used to analyze the differences of implant stability at each time point. Results: There was no close relation between the subjective and the objective evaluation of the bone density(r=0.57). In the subjective groups, there was statistically significant difference between the type 1 and 3 at 10 weeks and between the type 2 and 3 at 14 weeks. In the objective groups, there was no statistically significant difference between the D 1, 2, 3, 4, and 5 group with regard to RFA from baseline to 14 weeks(P>0.1). Conclusions: The implant stability increased over time during the study, and it was improved with bone density proportionally after 2weeks postoperatively. It is recommended that the decision of bone density is base on Hounsfield unit for implant loading time.

Transfer learning in a deep convolutional neural network for implant fixture classification: A pilot study

  • Purpose: This study aimed to evaluate the performance of transfer learning in a deep convolutional neural network for classifying implant fixtures. Materials and Methods: Periapical radiographs of implant fixtures obtained using the Superline (Dentium Co. Ltd., Seoul, Korea), TS III(Osstem Implant Co. Ltd., Seoul, Korea), and Bone Level Implant(Institut Straumann AG, Basel, Switzerland) systems were selected from patients who underwent dental implant treatment. All 355 implant fixtures comprised the total dataset and were annotated with the name of the system. The total dataset was split into a training dataset and a test dataset at a ratio of 8 to 2, respectively. YOLOv3 (You Only Look Once version 3, available at https://pjreddie.com/darknet/yolo/), a deep convolutional neural network that has been pretrained with a large image dataset of objects, was used to train the model to classify fixtures in periapical images, in a process called transfer learning. This network was trained with the training dataset for 100, 200, and 300 epochs. Using the test dataset, the performance of the network was evaluated in terms of sensitivity, specificity, and accuracy. Results: When YOLOv3 was trained for 200 epochs, the sensitivity, specificity, accuracy, and confidence score were the highest for all systems, with overall results of 94.4%, 97.9%, 96.7%, and 0.75, respectively. The network showed the best performance in classifying Bone Level Implant fixtures, with 100.0% sensitivity, specificity, and accuracy. Conclusion: Through transfer learning, high performance could be achieved with YOLOv3, even using a small amount of data.

디지털 수술용 가이드의 지지타입에 따른 정확도 평가 (The accuracy evaluation of digital surgical stents according to supported type)

  • 목적: 본 연구의 목적은 디지털 소프트웨어를 이용하여 제작된 수술용 스텐트를 이용하여 지지 형태에 따른 수술용 스텐트의 정확성을 분석하는 것이다. 재료 및 방법: 총 5세트의 치아모형에 대하여 양측에 지대치가 있는 치아지지형 무치악 모델과 근심에만 지대치가 있는 치아-조직 지지형 무치악 모델을 제작하였다. 모델을 스캐닝을 시행하고, 전산화 단층 촬영을 실시하여 In2Guide 소프트웨어를 이용하여 전산화 단층 촬영술의 정보와 모델 스캐닝 정보를 중첩하여 임플란트 고정체(USII, $4{\times}10mm$, Osstem, Seoul, Korea)에 대한 가상적인 위치배열을 실시하고 수술용 스텐트 제작하였다. DMAX 수술 키트을 사용하여 임플란트 고정체를 식립하였다. 식립후 전산화 단층 촬영을 실시하여 찍어 술전에 계획된 임플란트와 실제 식립된 임플란트 사이에 오차(angle difference, coronal difference, apical difference)를 측정하여 통계분석을 시행하였다. 결과: 악궁에 따른 정확도 결과에서 하악이 전반적으로 각도, 길이오차의 값이 작게 나타났으나 이는 통계적 유의성이 없었다. 스텐트의 지지 형태에 따른 정확도 결과에서 치아지지형 스텐트가 치아-조직지지형 스텐트보다 길이오차와 각도오차의 값이 통계적으로 유의하게 작은 값을 보였다. 결론: 악궁(상악, 하악)은 수술용 스텐트의 정확도에 영향을 미치지 않았으며, 치아 지지형 스텐트는 치아-조직지지형 스텐트보다 더 적은 오차를 보여 주었다.

컴퓨터보조 기반 점막지지 서지컬템프레이트를 이용한 하악전치부 임플란트 식립 (Implant Fixture Installation in the Anterior Mandible by Use of a Mucosa Supported Surgical Template Based on Computer Assisted Treatment Planning)

  • A 73-year-old Korean female patient with a fully edentulous mandible was planned to have five implant fixtures installed in the anterior mandible for the fixed prosthesis. After 3-dimensional (3D) computed tomographic scanning was transferred to OnDemand3D$^{(R)}$ (Cybermed Co., Seoul, Korea) software program for the virtual planning, five fixtures of MK III Groovy RP implants of Branemark System$^{(R)}$ (Nobel Biocare AB Co., Goteborg, Sweden) were installed in the anterior mandible between both mental foramens using In2Guide$^{(R)}$ (CyberMed Co., Seoul, Korea) mucosa-supported surgical template with Quick Guide Kit$^{(R)}$ (Osstem Implant Co., Seoul, Korea) systems. Fixture installations were completed successfully without any complications, such as mental nerve injury, bony bleedings, fenestrations and other unexpected events. Postoperative computed tomographic scans were aligned and fused to the planned implant, then angular and linear deviations were compared with the planned virtual implants. The mean angular deviation between the planned and actual implant axes was $3.42{\pm}1.336^{\circ}$. The mean distance between the planned and actual implant at the neck area was $0.544{\pm}0.290$ mm horizontally and $0.118{\pm}0.079$ mm vertically. The average distance between the planned and actual implant at the apex area was $1.166{\pm}0.566$ mm horizontally and $0.14{\pm}0.091$ mm vertically. These results could be considered more precise and accurate than previous reports, and even our recent results. The entire procedures of this case are reported and reviewed.

Bioactive characteristics of an implant surface coated with a pH buffering agent: an in vitro study

  • Purpose: The purpose of this study was to evaluate the effectiveness of conventional sandblasted, large-grit, acid-etched (SLA) surface coated with a pH buffering solution based on surface wettability, blood protein adhesion, osteoblast affinity, and platelet adhesion and activation. Methods: Titanium discs and implants with conventional SLA surface (SA), SLA surface in an aqueous calcium chloride solution (CA), and SLA surface with a pH buffering agent (SOI) were prepared. The wetting velocity was measured by the number of threads wetted by blood over an interval of time. Serum albumin adsorption was tested using the bicinchoninic acid assay and by measuring fluorescence intensity. Osteoblast activity assays (osteoblast adhesion, proliferation, differentiation, mineralization, and migration) were also performed, and platelet adhesion and activation assays were conducted. Results: In both the wetting velocity test and the serum albumin adsorption assay, the SOI surface displayed a significantly higher wetting velocity than the SA surface (P=0.000 and P=0.000, respectively). In the osteoblast adhesion, proliferation, differentiation, and mineralization tests, the mean values for SOI were all higher than those for SA and CA. On the osteoblast migration, platelet adhesion, and activation tests, SOI also showed significantly higher values than SA (P=0.040, P=0.000, and P=0.000, respectively). Conclusions: SOI exhibited higher hydrophilicity and affinity for proteins, cells, and platelets than SA. Within the limits of this study, it may be concluded that coating an implant with a pH buffering agent can induce the attachment of platelets, proteins, and cells to the implant surface. Further studies should be conducted to directly compare SOI with other conventional surfaces with regard to its safety and effectiveness in clinical settings.
