상악결손부의 2차적 재건에 있어 유리 혈관화 피판의 적용 (Late reconstruction of oncological maxillary defect with microvascular free flap)

  • Microvascular reconstruction of maxillary composite defect after oncologic resection has improved both esthetic and functional aspect of quality of life of the cancer patients. However, a lot of patients had prior surgery with radiation and/or chemotherapy as a part of comprehensive cancer treatment. Sometimes it is nearly impossible to find out adequate recipient vessel for maxillary reconstruction with microvascular anastomosis. Therefore long pedicle of the flap is needed to use distant neck vessels located far from the reconstruction site such as ipsilateral transverse cervical artery or a branch of contralateral external carotid artery. For this reason, although we know the treatment of the choice is osteocutaneous flap, it is difficult to use this flap when we need long pedicle with complex three dimensional osseous defect. Vascular option for these vessel-depleted neck patients can be managed by a soft tissue reconstruction with long vascular pedicle and additional free non-vascularized flap that is rigidly fixed to remaining skeletal structures. For this reason, maxillofacial reconstruction by vascularized soft tissue flap with or without the secondary restoration of maxillary bone with non-vascularized iliac bone can be regarded as one of options for reconstruction of profound maxillofacial composite defect resulted from previous oncological resection with chemo-radiotherapy.

MRONJ 예방과 치료를 위한 최신지견 (Contemporary Concept for Prevention and treatment of MRONJ (Medication Related Osteonecrosis of Jaw))

  • Bisphosphonates are widely used mainly for the treatment of osteoporosis and bone metastasis of malignancy. Since the first report of MRONJ, there have been many studies associated, however the pathogenesis of MRONJ is not yet clear. Medication-related osteonecrosis of the jaws (MRONJ) is a serious complication associated with long-term medication therapy. It is characterized by exposed necrotic bonein the jaw, which has persisted for more than 8weeks despite continuous treatment by dentist. The mechanism of development of MRONJ is still unclear and there is no definitive standard treatment for MRONJ. The purpose of this study is to investigate the jaw bone destruction mechanism of accumulated bisphosphonates, so that we can develop therapeutic method to repair the defect and stop the destruction process. The authors performed simultaneous application of PRF(Platelet rich fibrin) and BMP-2(Bone morphogenetic protein-2) to stimulate not only soft tissue healing but also osseous regeneration. Our case series demonstrate that simultaneous application of platelet rich fibrin and bone morphogenetic protein-2 can be a treatment of choice for MRONJ.

  • The osteosarcoma is the most co1t1.mon primary malignant lesion of bone, even so it is relatively rare tumor in the jaw bones. It is derived from undifferentiated mesenchymal elements of bone, which from neoplastic osteoid and osseous tissue. It may affected primarily young adult males and more frequently mandible than maxilla. Mass, swelling and pain are the most presenting symptoms. Radiographic appearance will be sclerotic, in which bone formation is excessive: osteolytic, in which bone destruction predominates: and mixed, in which sclerotic and osteolytic changes are intermingled. We report a case of osteosarcoma on the mandibular ramus area in a 20 years old male with a brief review of the concerned literatures.

족근 관절 경비 인대 결합 손상 (Ankle Syndesmotic Injury)

  • Ankle injuries may involve the distal tibiofibular syndesmosis and can be associated with a variable degree of trauma to the soft tissue and osseous structures that play an important role in ankle joint stability. Ankle syndesmotic injury may occur solely as a soft tissue injury or in association with variable ankle fractures. Ankle syndesmotic injury does not necessarily lead to ankle instability; however, the coexistence of deltoid ligament injury critically destabilizes the ankle joint. The prevalence of these injuries may be higher than previously reported. The diagnosis of syndesmotic injury as not always easy because isolated ankle sprains may be missed in the absence of a frank diastasis and syndesmotic instability may be unnoticed in the presence of bimalleolar ankle fractures. Controversies arise at almost every phase of treatment includings : type of fixation(screw size, type of implant), number of cortices required for fixation and of need for hardware removal. Regardless of controversies, the most important goal should be restore and maintain the normal tibiofibular relationship to allow for healing of the ligamentous structures of the syndesmosis.

청소년기에서 발생한 슬개골 상단부 소매형 골절 (Sleeve Fracture of the Superior Pole of Patella in an Adolescent)

  • A patella sleeve fracture is a type of avulsion fracture in which a small osseous fragment gets, along with a sleeve of periosteum and cartilage, is pulled off from the patella. The avulsed sleeve of cartilage and periosteum is not apparent on plain radiographs, which can result in a missed diagnosis or an underestimate of the extent of injury. An avulsion or sleeve fracture of the patella in a child can occur at the superior or the inferior pole of the patella. However, most reported cases of sleeve fractures involve the inferior patellar pole, with fractures involving the superior patellar pole being very rare. The authors report a case of a sleeve fracture of the superior pole of the patella in an adolescent; the fracture was diagnosed with magnetic resonance imaging (MRI) and was treated with surgery after early diagnosis. The course was uneventful, and the outcome was excellent.

  • A 6-year-old intact male Doberman pinscher presented with a thin soft stool and urinary signs. On radiography, three masses were observed in the caudal abdomen. One mass was described as a large, round, mineralized mass, with an "eggshell" appearance. The second mass was located caudal to the mineralized mass, and the third mass was located between the two masses. The second and the third masses had a soft tissue density. Ultrasonography was performed to identify the features and the origins of the masses. The first mass contained a large amount of anechoic fluid and had a thin wall; it was determined to be a cyst. Strong acoustic shadowing artifact was created by the mineralized cystic wall. Caudal to the cyst, the second mass was shown to be the prostate. A hypoechoic stalk connected the prostate and the cyst, which represented the cyst as a paraprostatic cyst with mineralization. The third mass was the normal urinary bladder. The cyst was removed surgically and confirmed by histopathologic examination. This report described typical clinical findings, diagnostic imaging, and treatment of a paraprostatic cyst.

  • For the study of the temporomandibular joint in rheumatoid arthritis 30 patients were selected who were diagnosed as rheumatoid arthritis through the clinical, radiographic examination and laboratory findings. Temporomandibular joint involvement was evaluated through the clinical, radiographic examination. The results were as follows; 1. TMJ was involved in 15 patients of 30 patients with rheumatoid arthritis. (50% involvement). 2. Duration of rheumatoid arthritis was more longer in patients with TMJ involvement than in patients without TMJ involvement. 3. Osseous changes in TMJ were in order of frequency erosion, flattening, osteophyte, sclerosis, deformity, and most common involved site was mandibular condyle. 4. Most common positional change of condyle was forward position in centric occlusion, and restricted movement of condyle in 1inch mouth opening. 5. TMJ involvement of rheumatoid arthritis was almost bilateral. 6. Main symptoms of TMJ were pain, stiffness, tenderness, limitation of mouth opening, crepitation 7. There was not the case of ankylosis. 8. There was statistically insignificant correlation between mandibular deviation and TMJ involvement, but some cases showed severe deviation on mouth opening.

하악에 발생된 결체조직성 섬유종 (Desmoplastic Fibroma of the Mandible)

  • Desmoplastic fibroma is a rare. benign intraosseous fibroblastic tumor. which is locally aggressive. It is osseous counterpart of soft tissue fibromatosis. The authors experienced the patient who complained persistent mouth opening limitation with mild swelling on the left mandibular angle area. After careful analysis of clinical. radiological and histopathological findings. we diagnosed as desmoplastic fibroma of the mandible. The results were as follows: 1. Main clinical symptoms were mouth opening limitation which had been persistent for 9 months and mild swelling on the left mandibular angle area. 2. Radiographs showed the radiolucent lesion and expansion of lingual cortex. CT finding is homogeneous soft tissue mass with expansion of left mandibular ramus. Destruction of medial wall of ramus and invasion to adjacent soft tissue is also seen. 3. Histopathologically, plump spindle shaped fibroblasts arranged in bundles or fascicles are observed. The cells of tumor are infiltrating into muscle fiber with destruction of bony trabeculae and merged with surrounding salivary gland.

이종 탈회 건조골 및 Hydroxyapatite 제재의 골 이식이 성견 치조골 결손부 치유에 미치는 영향에 관한 비교연구 (A COMPARISON OF DEMINERALIZED FREEZE-DRIED BONE AND HYDROXYAPATITE IN THE TREATMENT OF PERIODONTAL OSSEOUS DEFECTS IN DOGS)

  • This study was conducted to evaluate the healing potential of hydroxyapatite and demineralized freeze dried bone in 5 dogs. Chronic periodontitis was induced by ligating elastic wire randomized as follows. The group in which only flap operation was performed was used as control. The group in which flap operation using nonresorbable nonporous hydroxyapatite (Orthomatrix)was performed was used as experimental I. The group which flap operation using resorbable porous hydroxyapatite (Biocoral) was performed as experimental II. The group in which flap operation using demineralized freeze-dried bone was performed was used as experimental III. Thereafter dogs serially sacrificed at the 1,2,4 and 8 weeks and the specimens were prepared, and stained with Hematoxilin-Eosin stain for the light microscopic evaluation. The results of the this study were as follows : 1. Control group : progressive inflammatory cell infiltration till 4 weeks and epithelial undergrowth. 2. Group I. : epithelial undergrowth and new bone formed with fibrous margin around HA granule. 3. Group II. : no epithelial undergrowth and direct bone formation at the porous granule 4. Group III. : could not see epithelial undergrowth but obviously new cementum formation.

Interdisciplinary rehabilitation of a root-fractured maxillary central incisor: A 12-year follow-up case report

  • Single-tooth implantation has become a common treatment solution for replacement of a root-fractured maxillary incisor in adults, but the long-term esthetic results can be unfavorable due to progressive marginal bone loss, resulting in gingival recession. In this case report, a maxillary central incisor with a root fracture in its apical one-third was orthodontically extruded and extracted in a 21-year-old female. Implant surgery was performed after a 3-month healing period, and the final crown was placed about 12 months after extraction. After 12 years, favorable osseous and gingival architectures were visible with adequate bone height and thickness at the buccal cortical plate, and no gingival recession was seen around the implant-supported crown. Although modern dentistry has been shifting toward simplified, clinical procedures and shorter treatment times, both general dentists and orthodontists should be aware of the possible long-term esthetic advantages of orthodontic extrusion of hopelessly fractured teeth for highly esthetically demanding areas and should educate and motivate patients regarding the choice of this treatment solution, if necessary.
