  • Pancoast syndrome (PS) is characterized by a malignant neoplasm of the superior sulcus of the lung with destructive lesions of the thoracic inlet and involvement of the brachial plexus and cervical sympathetic nerves. The most common initial symptom of PS is shoulder pain; however, cough, dyspnea, and hemoptysis, signs often associated with lung cancer, are not as common. Investigation of PS can be difficult even with plain radiographs of the chest because it is surrounded by osseous structures such as the ribs, vertebral bodies, and manubrium. Due to these characteristics, orthopedic surgeons tend to make a misdiagnosis resulting in delay of appropriate treatment. Here we report on a patient who was supposed to undergo rotator cuff repair for his shoulder pain and weakness, and was eventually diagnosed with PS.

  • The author observed a case of the malignant fibrous histiocytoma of the mandible in 23 aged male patient, who had come to the Infirmary of Dental College, Yonsei University. With the help of careful analysis of radioraphic and clinicopathologic findings, the author had obtained the results as follows. 1. Lesion occured at 22 years of age. 2. Traumatic history or other special predisposing factors were not seen. 3. Lesion appeared as a intra osseous primary tumor not showing any metastatic change. 4. The author have concluded this disease as a malignant fibrous histiocytoma of the mandible according to serial findings.

  • Clinical assessment of bone-graft healing in the maxillofacial region is generally limited to clinical evaluation, radiographs, and biopsy. Sequential interpretation of osseous repair, more sensitive than with conventional radiography is possible with a non-invasive, non-destructive radionuclide method. Technetium-99m radionuclide bone scan was used in the evaluation of the progress of osteogenic activity in autologous iliac bone graft and freeze-dried bone allograft of dog's mandible. Bone scan was performed at 1wk, 2wk, 4wk, 6wk, and 8wk after grafting. In autologous graft the activity ratio for the graft bone remained greater than that of the host since 2자 after grafting; however, in lyophilized allograft the activity ratio for graft bone was greater than that of the host at 6자 after grafting.

  • Three-dimensional reconstruction of computed tomographic image(3D CT) is a well-established imaging modality which has been investigated in various clinical settings. It is commonly performed in case of congenital or developmental abnormalities, and traumatic fracture of skull and face that requires reconstruction of osseous structure. However reporting the 3D CT in laryngeal or tracheal stenosis is rare and its results are obscure. The authors performed 3D CT in six cases of tracheal stenosis and found diagnostic value of 3D CT. A Comparision of diagnostic information obtained from plain X-ray, 2D CT and 3D CT has performed in total six cases of tracheal stenosis. Surgical treatment of the tracheal stenosis was following in these cases : tracheal end to end anastomosis In 1 case, laryngotracheal end to end anastomosis in 2 cases. 3D CT information was compared with operative finding. In two of six cases, satisfactory information was not obtained from 3D CT in evaluating an exact stenosis of trachea. Future, it will be helped in evaluating of tracheal stenosis by 3D CT.

  • Tarsal tunnel syndrome is an entrapment neuropathy of the posterior tibial nerve or its branches in the fibro-osseous tunnel beneath the flexor retinaculum. This pathology is associated with multiple etiologies, including trauma, space-occupying lesions, and impaired biomechanics. We report a case of tarsal tunnel syndrome associated with gout tophi in a patient with untreated gout along with a review of the relevant literature on tarsal tunnel syndrome.

  • 본 연구는 1987년 1월부터 1991년 12월까지 연세대학교치과병원 구강악안면외과에 과두 골절이 의심되어 내원한 환자중 임상 및 방사선학적 소견에서 하악과두 골절로 진단되어 비외과적 치료를 받은 12세 미만의 13명의 소아환자를 연구 대상으로 하여 임상 및 방사선학적 소견을 분석하여 다음과 같은 결론을 얻었다. 비외과적인 보존적 또는 기능적 치료를 받은 후의 결과는 양호하였으며, 골의 재생과 개조는 신속하여 골 구조는 기능 중에 가해진 힘에 반응한다는 Wolff의 법칙과 일치하는 듯이 보였다. 두 예의 골절탈구에서 과두의 단축 및 과두가 골절된 악관절에서 clicking음을 보였으나 저작하거나 개구시에 큰 어려움은 없었다. 술후 관찰기간이 불충분하였으나 본 예의 대부분에서 비외과적인 술식에도 만족스러운 치료결과를 보였다.

Osseous metaplasia showing heterotopic ossification in the maxillary sinus

  • Radiopacity in the maxillary sinus can be observed in various conditions, such as in the presence of lesions in the maxillary sinus or as a sequela of maxillary sinus surgery. This report describes the case of a 57-year-old female patient who had no previous history of surgical treatment or traumatic injury of the nose or maxillary sinus. Both maxillary sinuses were indistinguishable on panoramic radiography and showed signs of radiopacity. Computed tomography images revealed that the maxillary sinuses were filled with bony tissue and exhibited signs of sinus mucosal thickening. Biopsy results showed fragments of trabecular bone with fibrous tissue.

A swelling of the maxilla: a case report and differential diagnosis

  • Ossifying fibromas are benign fibro-osseous tumors of mesenchymal origin. Although ossifying fibromas have principally been found in the jaw, they have also been reported in the frontal, ethmoid, sphenoid, and temporal bones, as well as the orbit and anterior cranial fossa. Ossifying fibromas affecting the jaw exhibit variable behaviors ranging from slow growth to occasionally aggressive local destruction. In the present article, we discuss a differential diagnosis considered for maxillary swellings and report a rare case of ossifying fibroma occurring in the maxilla.

  • For the successful guided bone regeneration(GBR) of maxillary bony defect, proper soft tissue coverage is one of the most important things. Soft tissue dehiscence can be most common reason of osseous reconstruction failure. If a vascular supply to the graft should not develop from the host tissue, then the graft may also foil. Both of these prerequisites can be aided by judicious use of the buccal fat pad(BFP). Many methods for adequate soft tissue coverage have been proposed and the use of the BFP is one of them. BFP is useful in posterior maxillary area, can cover larger area and have higher blood flow than other methods. so the use of the BFP may offer protection and early blood supply to maxillary bone graft. This report describes the history, anatomy, blood flow, and clinical usefulness with two clinical cases.

AN EXPERIMENTAL STUDY OF UNDEMINERALIZED FREEZE-DRIED HUMAN BONE (냉동건조 은행골의 개발 및 이의 실험적 연구)

  • In order to develop the allogeneic bone implants instead of autogenous bone grafts for maxillofacial reconstruction, undemineralized freeze-dried human bone was processed. The freeze-dried human bone was implanted into the cranial and mandibular defects of the rabbits. The implants were evaluated clinically, roentgenographically and histomophometrically. And immunohistochemical evaluation of the implants was performed on the rat. The results were as follows : 1. When compared with control defects of $0.8{\times}0.8\;cm$, the implants on the rabbit defects displayed complete osseous bridging clinically and roentgenographically. Histomophometrically a minimal inflammatory cell infiltrate was present but the defects healed well clinically. 2. When compared with control grafts, the freeze-dried implants on the rat muscle displayed decreased antigenicity by immunohistochemical evaluation, due to freeze-drying process. 3. Undemineralized freeze-dried human bone in this study can be preserved as a bank bone in this study and seems to be applicable for clinical allogeneic bone grafts.
