Osseous metaplasia of the palate: a case report

  • Osseous metaplasia is defined as the formation of lamellar bone inside soft tissue structures where bone normally does not exist. It results from the transformation of non-osseous connective tissue into mature bone. This condition is rare in the oral and maxillofacial region. We report a case of osseous metaplasia of the maxilla, a rare benign tumor in an uncommon region. A 60-year-old male patient visited our clinic complaining of foreign body sensation and asymptomatic swelling on the right palatal side. However, he did not experience pain and reported no local trauma that he could remember. Intra-oral examination revealed an exophytic lesion on the right palatal portion. On computed tomography, there was a round hard-tissue mass approximately 2 cm in diameter on the right palate area. The mass was biopsied and diagnosed as an osseous metaplasia. We review the clinical, radiographic, and histologic features and common causes of osseous metaplasia and report a rare case of osseous metaplasia of the palate.

악골에 발생한 Florid Osseous Dysplasia의 치험례 (FLORID OSSEOUS DYSPLASIA : A CASE REPORT)

  • This is case report of florid osseous dysplasia occurred in the entire maxilla and mandible of 47-year-old female. Florid osseous dysplasia is an expansile, exuberant asymptomatic form of benign fibro-osseous disease of the periodontal ligament, often associated with jaw cysts and to be though as an abnormal reaction of bone to irritation or stimulation. The treatment was performed with removal of the necrotic bone, bone curretage and hyperbaric oxygen therapy. The patient did not well postoperatively and has shown sign of persistant infection with pus discharge.

혀에서 골성 이소종(osseous choristoma)에 대한 증례 보고 및 문헌 고찰 (A case report and literature review of osseous choristoma on the tongue.)

  • Choristoma is a growth of normal tissue that occurs at an abnormal region. In the oral cavity, osseous choristoma that has a normal bone feature mainly has been appeared. It is mainly occurred on the tongue. So, It can be found in dental practice or in discomfort in the mouth. Osseous choristoma in the oral cavity is extremely rare. But It can be treated excisional biopsy under local anesthesia. The prognosis is very good. This case is a report of osseous choristoma on the dorsal surface of the tongue. A 17-year-old female patient presented with a foreign body sensation of the tongue. A diameter of the mass is 0.7cm, and there is no pain. Panoramic radiographs showed a radiopaque image in the form of a double image. An excisional biopsy was performed and diagnosed as an osseous choristoma. 12 months after the resection, there were no signs of recurrence. Intraoral osseous choristoma is very rare, but it is clinically very simple and can be found in dental radiographs. We present this case because of Dentists who examine the oral cavity of the patient can easily diagnose the lesion and treat it by simple resection.

  • Cemento-osseous dysplasias are a group of disorders known to originate from periodontal ligament tissues and involve, essentially, the same pathological process. They are usually classified, depending on their extent and radiographic appearances, into three main groups. Periapical cemental dysplasia predominantly involves the periapical region of the anterior mandible. Florid cemento-osseous dysplasia presents with multifocal lesions in the tooth bearing or edentulous areas of the maxilla and mandible, often occurring bilaterally with symmetric involvement. Focal cemento-osseous dysplasia exhibits a single site of involvement in any tooth bearing or edentulous area of the mandible or maxilla, with the posterior mandible representing the most common site. We report the clinical, radiographic and histological findings of 3 types of cemento-osseous dysplasia with a review of literatures.

자기공명영상을 이용한 악관절내장증환자와 악관절증환자의 골변화에 관한 연구 (Evaluation of osseous changes of TMJ in internal derangement and osteoarthritis patients using MRI)

  • Purpose: To evaluate the osseous changes of TMJ in internal derangement and osteoarthritis patients using MRI. Materials and Methods: MR images of 111 TMJs in 64 patients were analyzed to evaluate the osseous changes, 111 TMJs were divided into 6 groups according to the radiologic Stages by Schellhas and Wilkes. On MR images, we evaluate the osseous changes of articular eminence and condylar head. Results: The most frequent Stage in internal derangement of TMJ was Stage I. And 28 joints (25.2%) revealed osteoarthritis with internal derangement. When osseous change of articular eminence and condylar head occur, flattening was the most common osseous change. Sclerosis was observed in all Stages and osteophytosis of condylar head was observed in Stage II (1.8%) and III (0.9%). Out of 28 joints with osteoarthritis, 6 joints (21.4%) showed joint effusion. Conclusion: MR image revealed abnormal configuration of disk, but the detection of minimal osseous change was subtle.

Comparative study on the osseous changes of the TMJ and mandibular asymmetry after conservative or operative treatment in condylar fracture patients

  • Purpose: To compare the effects of the conservative treatment and operative treatment by observation of osseous changes of the TMJ and mandibular asymmetry in condylar fracture patients. Materials and Methods: 33 condylar fracture patients (17 with conservative and 16 with operative treatment) were included in this study. After a minimum of 6 months after the surgical procedure, patients were given a follow up examination of the osseous changes using a transcranial view. Differences in the osseous changes of both groups were compared and the asymmetry indices were calculated on a postero-anterior skull view. Results: The TMJ of the operative treatment group showed more significant osseous changes than the conservative treatment group. The affected TMJ showed more significant osseous changes than the unaffected TMJ in the both groups. The unaffected TMJ of the conservative group and the affected TMJ of the operative group showed significant osseous changes. The mandibular asymmetry indices in the conservative and operative group were 5.12 and 7.30 respectively at the time of treatment, and 2.39 and 3.41 respectively at the follow-up. But the mandibular asymmetry between the both groups showed no statistical differences. Conclusion: The TMJ of the operative group showed more significant osseous changes than the conservative group, but the mandibular asymmetry between the both groups showed no statistical difference.

개화성 백악질-골 이형성증: 증례보고 (Florid cemento-osseous dysplasia: a report of two cases)

  • Cemento-osseous dysplasia occurs in the tooth bearing areas of the jaws and is probably the most common fibro-osseous manifestation. They are usually classified into three main groups according to their extent and radiographic appearance: periapical (surrounds the periapical region of teeth and are bilateral), focal (single lesion) and florid (scleroticsymmetrical masses) cemental-osseous dysplasias. Florid cemento-osseous dysplasia clearly appears to be a form of bone and cemental dysplasia that is limited to the jaws. Patients do not have laboratory or radiologic evidence of bone disease in other parts of the skeleton. For asymptomatic patients, the best management consists of regular recall examinations with prophylaxis and the reinforcement of good home hygiene care to control periodontal disease and prevent tooth loss. The treatment of symptomatic patients is more difficult. At this stage, there is an inflammatory component caused by the disease and the process is basically a chronic osteomyelitis involving dysplastic bone and cementum. Antibiotics might be suggested, but are not always effective. Two cases of florid cemento-osseous dysplasia diagnosed in two Korean females are reported with a review of the relevant literature.

  • florid osseous dysplasia(FOD)는 악골에 전반적인 방사선 불투과성을 보이는 질환으로 환자는 대개 무증상이나 만성 골수염에 이환 될 가능성이 상당히 높다. 치료로는 환자가 무증상인 경우 아무런 처치가 필요하지 않으나 구강위생을 청결히 하여 치아를 보존하여야 하며 치과 보철물 등의 장착에 장애가 될 경우나 만성 골수염에 이환 된 경우 주의 깊은 외과적 처치가 요구된다고 할 수 있다.

악안면부의 섬유골성 병소 명칭에 대한 고찰 (Review of nomenclature revision of fibro-ossous lesions in the maxillofacial region)

  • Fibro-osseous lesions are composed of connective tissue and varying amount of mineralized substances, which may be bony or cementum-like structures. It is necessary for oral radiologist to differentiate due to the tendency of these fibro-osseous lesions to show similar histopathologic appearances, while the management of each lesion is different. However we often encounter a little difficulty in judgement because there are some overlaps between concept of each lesions. So recently I suggest, we face a need to review basic concept and classification of several fibro-osseous jaw lesions. In this article, several fibre-osseous lesions, such as fibrous dysplasia, cemento-ossifying fibroma and cemento-osseous dysplasia, will be discussed basing on the review of literature. particular emphasis will be made on the nomenclature revision of WHO's classification in 1992.

Recurrent symptomatic cemento-osseous dysplasia: A case report

  • Cemento-osseous dysplasia (COD) is a benign fibro-osseous lesion of bone, in which normal bone is replaced by fibrous tissue, followed by calcification with osseous and cementum-like tissue. COD is classified into 3 categories according to its location: periapical, focal, and florid COD (FCOD). On radiography, FCOD appears radiolucent in its early stages. As it matures, radiopacities appear within the lesion, causing them to show a mixed appearance of radiolucency and radiopacity. Because FCOD is usually asymptomatic and grows in a self-limited manner, it does not require treatment. Secondary infection is the most frequent cause of symptomatic cases. We report a case of FCOD with symptoms that appeared after a dental restoration procedure and persisted after repeated operations. The purpose of this report is to emphasize the importance of thorough radiological evaluations of patients with FCOD before treatment.
