Atypically Large Calcific Tendinitis of the Shoulder: A Case Report

  • Calcific tendinitis of the shoulder joint is common disease causing acute pain, mainly involving the supraspinatus or infraspinatus muscle, and less frequently the teres minor or subscapularis muscle. This study reports on the satisfactory arthroscopic removal of calcium deposits as well as infraspinatus and supraspinatus muscle repair without relapse via minimal incision using suture anchors. This was a case of atypically extensive calcific tendinitis involving the infraspinatus muscle, with a bursal side partial rupture of the supraspinatus muscle in a 61-year-old female whose chief complaint was chronic pain of the right shoulder exacerbated by limited movement.

  • Bony lesions of the glenoid and Hill-Sachs lesions are the most common injuries after a first-time traumatic shoulder dislocation. However, fracture of the coracoid process after traumatic shoulder dislocation is rare. A single, open surgical procedure could be performed by a Latarjet procedure using a fractured fragment of the coracoid process. If a fracture of the coracoid process is associated with a traumatic anterior shoulder dislocation, the Latarjet procedure may be the most appropriate surgical option.

  • Purpose: To evaluate the efficacy of distraction osteogenesis for fourth brachymetatarsia. Materials and Methods: Seven patients (10 cases) who were treated by distraction osteogenesis for fourth brachymetatarsia from March 2000 to December 2003 were reviewed retrospectively. Results: The average length gain of fourth metatarsus was 16.6 mm (37%) and the average healing index was 50 days/cm. The final results according to AOFAS functional scale were excellent in 8 cases and good in 2 cases. Conclusion: Distraction osteogenesis is an effective treatment for fourth brachymetatarsia in spite of some minor complications.

  • Traumatic dislocation of the peroneal tendon is a rare injury. It should be distinguished from other conditions that can cause pain and disability of lateral aspect of the ankle joint. We experienced a case of chronic recurrent dislocation of the peroneal tendon, which was treated by deepening of the peroneal groove of the lateral malleolus and reattachment of the superior peroneal retinaculum. The clinical result was satisfactory.

  • We have performed the operations for the restoration of shoulder abduction and elbow flexion in 17 patients(20 cases of neurotization) of brachial plexus injury at the Department of Orthopaedic Surgery, Korea University Hospital from October 1991 to May 1997. The mean follow-up period was 42 months. After neurotization for brachial plexus injury, functional recovery of shoulder abduction and elbow flexion were good and fair in 65%. In the functional evaluation, the clinical results of whole arm type were poorer than those of upper arm type, and the operations performed within 6 months since the injury were better than those of other cases.

  • Clavicle medial end fracture is rare, and it has not been studied extensively. Although there is debate regarding its treatment methods, because of the complications of conservative treatment, surgical treatment has been considered more than conservative treatment. This study describes a surgical method using double-plate fixation for treatment of clavicle medial end fractures in which plates were used on each anterior and superior border according to the anatomical structure of the clavicle. In addition, we report operative results of three patients treated by double-plate fixation.

  • Humeral head chondrolysis has been widely reported as a devastating complication after arthroscopic shoulder surgery; however little is known about post-arthroscopic humeral head osteonecrosis. We experienced a 66-year-old female patient with rapidly progressive osteonecrosis of the humeral head only seven months after arthroscopic Bankart and rotator cuff repair. The patient had no systemic risk factors for osteonecrosis. A satisfactory result was achieved with reverse total shoulder arthroplasty for severe humeral head destruction and an irreparable massive rotator cuff tear. Shoulder surgeons should be aware of such severe complication, perform routine radiographs, and pay close attention to the presence of constant pain or loss of motion after arthroscopic shoulder surgery.

  • Although the lateral discoid meniscus of the knee has been observed frequently, the medial discoid meniscus has very rarely been reported in the literature. A 22-year-old female patient was diagnosed as having a medial discoid meniscus with horizontal tear by means of magnetic resonance image and arthroscopy. The patient was treated by arthroscopic partial meniscectomy.

  • Primary osteoarthritis of the ankle without history of trauma is rare. We are reporting the use of a low tibial osteotomy on 2 ankles in one patient. The osteotomy is designed to correct the varus tilt and anterior opening of the distal tibial joint surface where it has been shown by weight-bearing radiographs. Follow-up at 8 months the result showed to be excellent, by Takakura's scoring system. We experienced that slight overcorrection of deformity by low tibial osteotomy is effective in treating intermediate-stage primary osteoarthritis of the ankle.
