Facial esthetics is one of the most important goal of the orthodontic treatment and main concern of many patients. Facial esthetics should be considered in orthodontic diagnosis and treatment planning. Prediction of soft tissue profile changes after orthodontic tooth movement should be considered as well. The purpose of this study was to find out the effect of orthodontic treatment on lip profile in adult patient. The pre and post treatment cephalometric roentgenograms of 87 female adult with bimaxillary protrusion were used to analyze lip profile change. All subjects were treated with four bicuspids extraction. Obtained results were as follows . 1. Lip thickness changes after incisor retraction showed different patterns according to areas of the lip. The thickness of the red lip area showed 2.78 mm increase in average. In contrast the thickness of the cutaneous area showed 0.65 - 0.7 mm decrease according to the different cutaneous areas. 2. The length of the red lip area decreased(1.3mm) after incisor retraction. 3. The length of the cutaneous lip area increased(2.9mm) after incisor retraction.
The purpose of the present study is to evaluate changes of the soft tissue relative to underlying skeletal elements during orthodontic treatment, and the influence of orthodontic treatment quantitatively on various regions of the facial profile. 59 Korean young women were selected, whose Hellman dental age was IV A, IV C and V A. Lateral cephalometric head films were taken before and after orthodontic treatment. From tracings, landmarks on skeletal and soft tissue profile were located, and then their linear and angular measurements were made directly. The results were obtained as follow: 1) Soft tissues of the facial profile were closely related and dependent on the underlying dentoskeletal frameworks. Orthodontic treament resulted in the reduction of dentofacial protrusion with both upper and lower lips becoming less procumbent during treament. 2) Thickness of the upper lip increased considerably during orthodontic treatment, and this change was related to maxillary incisor retraction. The ratio between the amount of maxillary incisor retraction and that of increment of upper lip thickness was approximately 5:3. 3) Soft tissue thickness overlying Downs' point A, point B and pogonion was not modified by orthodontic treatment. 4) Holdaway's H line, relating facial profile to the underlying dentoskeletal framework, seemed to be the most practical approach to soft tissue analysis.
This article reports the orthodontic treatment of a patient with skeletal mandibular retrusion and an anterior open bite due to temporomandibular joint osteoarthritis (TMJ-OA) using miniscrew anchorage. A 46-year-old woman had a Class II malocclusion with a retropositioned mandible. Her overjet and overbite were 7.0 mm and -1.6 mm, respectively. She had limited mouth opening, TMJ sounds, and pain. Condylar resorption was observed in both TMJs. Her TMJ pain was reduced by splint therapy, and then orthodontic treatment was initiated. Titanium miniscrews were placed at the posterior maxilla to intrude the molars. After 2 years and 7 months of orthodontic treatment, an acceptable occlusion was achieved without any recurrence of TMJ symptoms. The retropositioned mandible was considerably improved, and the lips showed less tension upon lip closure. The maxillary molars were intruded by 1.5 mm, and the mandible was subsequently rotated counterclockwise. Magnetic resonance imaging of both condyles after treatment showed avascular necrosis-like structures. During a 2-year retention period, an acceptable occlusion was maintained without recurrence of the open bite. In conclusion, correction of open bite and clockwise-rotated mandible through molar intrusion using titanium miniscrews is effective for the management of TMJ-OA with jaw deformity.
A clear overlay appliance is a type of a removable appliance made from transparent thermoplastic plastic film that covers the entire dentition to move the teeth. It is one of the most favored orthodontic methods opted for by adult patients; this treatment is esthetic, does not cause discomfort and allows oral hygiene to be easily managed when compared to other conventional fixed treatment methods. However, the use of clear overlay appliances, such as invisalign or clear aligner, is associated with various clinical challenges. In particular, the appliances require longer treatment periods compared to fixed treatment, and due to the structural characteristics of the appliances, it is difficult to make proper posterior occlusion and certain type of tooth movement, including extrusion, rotation and tip. Thus, the clear overlay appliances are regarded as supplementary appliances by most orthodontists and have been used for simple orthodontic treatments, such as partial anterior alignments or orthodontic relapse cases. Owing to the remarkable advancement in the field of 3D digital technology over a period of 15 years, the accuracy and convenience of modern clear overlay appliances have continuously improved. Moreover, orthodontic outcomes have also been greatly improved by the introduction of new materials and successful application of various biomechanical methods from conventional orthodontic treatments in the design of clear overlay appliances. This study investigates the clinical limitations that should be considered during the application of clear overlay appliances and also examines the efforts and methods used to overcome these challenges.
Objectives: The purpose of this study was to identify factors affecting choice of treatment hospital (i.e., determinants) and satisfaction with the treatment hospital, and to provide a more satisfactory and high-quality medical service for orthodontic patients. Methods: A questionnaire survey was conducted for approximately 1 month beginning in August 2017, involving patients who visited orthodontic dentistry clinics in Seoul and Gyeonggi-do. Multiple regression analysis was performed on the results to examine factors that affected satisfaction with orthodontic treatment. Results: The determinant of dental clinics was 3.90 points overall; hospital environment and facilities were highest at 4.05, followed by dental hygienist at 3.99, and dentist at 3.97. Factors influencing satisfaction with orthodontic treatment were positively influenced by dentists (p<0.01), medical procedures (p<0.01) and medical expenses (p<0.01). Conclusions: It is important to provide comfort and convenience to patients by simplifying hospital facility management, reception, and reservation procedures.

Glossectomy combined with radiotherapy causes different levels of tongue function disorders and leads to severe malocclusion, with poor periodontal status in cancer survivors. Although affected patients require regular access to orthodontic care, special considerations are crucial for treatment planning. This case report describes the satisfactory orthodontic management for the correction of severe dental crowding in a 43-year-old female 6 years after treatment for tongue cancer with total glossectomy combined with radiotherapy, to envision the possibility of orthodontic care for oral cancer survivors. Extraction was performed to correct dental crowding and establish proper occlusion following alignment, after considering the possibility of osteoradionecrosis. Orthodontic mini-implants were used to provide skeletal anchorage required for closure of the extraction space and intrusion of the anterior teeth. The dental crowding was corrected, and Class I occlusal relationship was established after 36 months of treatment. The treatment outcome was sustained after 15 months of retention, and long-term follow-up was recommended.
Objective: Body dysmorphic disorder (BDD) is a form of obsessive-compulsive disorder that may be negatively associated with the self-image. It might be associated with orthodontic treatment demand and outcome, and therefore is important. Thus, this study was conducted. Methods: The Yale-Brown Obsessive-Compulsive Scale modified for Body Dysmorphic Disorder (BDD-YBOCS) questionnaire was used in 699 orthodontic patients above 12 years of age (222 males, 477 females), at seven clinics in two cities (2020-2021). BDD diagnosis and severity were calculated based on the first 3 items and all 12 items of the questionnaire. The dental health component of the index of orthodontic treatment need (IOTN-DHC) was assessed by orthodontists. Multivariable and bivariable statistical analyses were performed on ordinal and dichotomized BDD diagnoses to assess potentially associated factors (IOTN-DHC, age, sex, marital status, education level, and previous orthodontic consultation) (α = 0.05). Results: IOTN-DHC scores 1-5 were seen in 13.0%, 39.9%, 29.8%, 12.4%, and 4.9% of patients. Age/sex/marital status/education were not associated with IOTN-DHC (p > 0.05). Based on 3-item questionnaire, 17.02% of patients had BDD (14.02% mild). Based on 12-item questionnaire, 2.86% had BDD. BDD was more prevalent or severer in females, married patients, patients with a previous history of orthodontic consultation, and patients with milder IOTN-DHCs (p < 0.05). Conclusions: IOTNDHC was negatively/slightly associated with BDD in orthodontic patients. Being female and married may increase BDD risk.
Odontogenic keratocysts (OKCs) are one of the most aggressive cysts in the oral and maxillofacial area because of their high recurrence rate and infiltrative behavior. In growing patients with OKCs, a radical treatment approach might cause numerous complications, including the disturbance of jaw growth and loss of the involved tooth. This case report describes successful comprehensive orthodontic treatment combined with marsupialization of the cyst in a young girl who exhibited an OKC with impacted teeth. The 10-year-old girl presented with an OKC extending from the mandibular symphysis through the left mandibular body, with ectopic impaction of the mandibular left canine and first premolar, as well as congenitally missing bilateral mandibular second premolars. Interestingly, spontaneous improvement of the positions of the ectopic impacted teeth, along with a reduction in the size of the cyst, occurred during marsupialization. The sequential use of removable and fixed appliances enabled orthodontic traction of the impacted teeth. The treatment outcome was stable at 2.5 years after the end of the treatment. We speculate that comprehensive orthodontic treatment combined with marsupialization can be an effective treatment strategy for patients with OKCs, especially when they are encountered in young, growing patients with impacted teeth.

This study was carried out to examine the main cause of influencing oral health impact profile (OHIP) and self-esteem of patients on orthodontic treatment. Questionnaire was used for research based on 200 patients on orthodontic treatment in four dental clinics specialized orthodontic treatment and located in Daegu. General characteristics, orthodontic treatment characteristics, OHIP, and self-esteem were examined to analyze the cause of influencing OHIP and self-esteem of patients on orthodontic treatment. The results, Influential factors in OHIP, The younger age (P<0.01), showed that healthier oral condition (P<0.001), and treatment done by family encouragement (P<0.05) were the influential factors of OHIP. Influential factors in self esteem showed that male (P<0.05), unmarried (P<0.01), and higher income (P<0.05) were influential factors of self esteem.
Adjunctive orthodontic therapy may be required to create appropriate space, to address misalignment and tooth size discrepancy for better function and esthetics. The desired interproximal alveolar contour and gingival embrasure form can be developed during treatment. Various applications of temporary anchorage devices for pre-prosthetic tooth movement allowed clinicians to achieve high efficiency, shorter treatment time, and less discomfort of patient. Biomechanical considerations for the periodontal status of the affected teeth are required to successfully control the vertical and horizontal space. Hence, the interdisciplinary approaches have an essential role in maximizing the favorable treatment outcome. In particular, pivotal Clinical decisions such as whether to open or close the space should be made by consensus of the involved dentists. This article presents the orthodontic treatment approaches for prosthodontic works including mesiodistal and vertical space regaining specially for cases of unrestored teeth over an extended period of time.
