Objective: The purpose of this study was to evaluate the effects of malocclusion and orthodontic treatment on the self-esteem of adolescents. Methods: The subjects were composed of 3509 female middle school students. Each subject was evaluated with Rosenberg's Self-esteem Scale to measure the level of self-esteem and also evaluated the degree of crowding and soft tissue profile. Results: Results showed that protrusion of lip area had no effects on self-esteem but crowding of upper anterior teeth had significant effects. The fixed orthodontic treatment group and the removable orthodontic treatment group reported no significant difference in self-esteem from the no orthodontic treatment experience group, but the debonding group showed significantly higher Self-esteem index. Conclusion: The results of this study suggest that anterior teeth alignment has influence on the self-esteem of female adolescents.
Objectives : The purpose of the study is to investigate the knowledge and self care behaviors of orthodontic patients using clear aligner. Methods : A total of 181 orthodontic patients using clear aligner in Daegu filled out the self-reported questionnaire from September 6 to October 18, 2010. and 162 subjects completed orthodontic treatment. Results : There existed a significant difference in knowledge and self care behaviors between gender and wearing duration of clear aligner(p<0.01). Treatment duration influenced significantly on the score of self care behaviors(p<0.01) and the high score of self care behavior showed in short duration of treatment. Conclusions : Self care behaviors were closely correlated with treatment duration. An information for the clear aligner will be helpful to improve the knowledge and self care behaviors.
The choice of surgical technique in orthognathic surgery is based primarily on the surgical treatment objectives (STO), which is a fundamental component of the orthognathic treatment process. In the conventional orthodontics-first approach, presurgical planning can be performed twice, during the preorthodontic (initial STO) and presurgical phases (final STO). Recently, a surgery-first orthognathic approach (SFA) without presurgical orthodontic treatment has been introduced and combined initial and final STO at the same time. In contrast to the conventional surgical-orthodontic treatment protocol that includes preoperative orthodontics for dental decompensations to maximize stable postoperative occlusion, the SFA potentially shortens the treatment period and minimizes esthetic concerns during the decompensation period because skeletal problems are corrected from the beginning. The indications for the SFA have been proposed in the literature, but no consensus exists. Moreover, because dental occlusion of the pre-orthodontic arches cannot be used as a guide for establishing the surgical treatment plan, there are fundamental limitations in accurate prediction of postsurgical results in the SFA. Recently, the concepts of postsurgical orthodontic treatment are continuously changing and evolving to overcome this inherent limitation of the SFA. The elimination of presurgical orthodontics can change the paradigm of orthognathic surgery but still requires cautious case selection and thorough discussion and collaboration between orthodontists and surgeons regarding the goals and postoperative management of the orthognathic procedure.
This study was undertaken to investigate soft tissue profile changes by orthodontic treatment in female patients. Traditional cephalometric appraisal yields data of dubious scientific value, the soft tissue profile forms were evaluated by finite element method. The subject was divided into three groups according to Angle's classification and each group was composed of 25 female patients averaged aged 12-14 years at the start of treatment. The changes in soft tissue form were evaluated by computing the degree of distortion in each triangle after treatment compared with the triangle before treatment. The conclusions were as follows; 1. The soft tissue profile forms were evaluated by finite element method and independent evaluation of each element by local changes was possible. 2. Maximum and minimum principal strains showed marked variability depending on the particular finite element and each group and Class II, III sample was greater than Class I sample. 3. Soft tissue size changes as a result of orthodontic treatment was not related to those of shape. 4. Soft tissue changes by orthodontic treatment were variable in individual patient, and were not related to Angle's classification.
Orthognathic surgery is the primary treatment option for severe skeletal discrepancy. However, orthodontic camouflage should be considered as an alternative treatment option, considering the risks of surgery. A 19.5-yearold man presented with a severe prognathic mandible with a Class III molar relationship and an anterior crossbite. Orthognathic surgery could be considered because of his severe skeletal discrepancy and mandibular prognathism. However, the anesthetist for orthognathic surgery did not recommend surgery under general anesthesia because of risk factors associated with the patient's aplastic anemia, including bleeding and infections. Thus, a camouflage treatment to promote backward rotation of the mandible via orthodontic extrusion of the posterior teeth was planned. An anterior bite plate, intermaxillary elastics, and fixed orthodontic appliances were used to extrude the posterior teeth and to align the dentition. After 17 months of nonsurgical orthodontic treatment, normal occlusion was achieved, and the facial profile was dramatically improved. This case report describes the dentoskeletal and soft-tissue effects of mandibular rotation and its long-term stability.
Orthodontic treatment for middle-aged patients has become more commonplace with various reasons including improved socioeconomic status. Understanding of oral status and treatment modalities of middle-aged patients is mandatory for accurate diagnosis and proper treatment planning. This study investigated 100 consecutive patients aged 40s and 50s and 100 aged 20s who had been examined and diagnosed at the Department of Orthodontics, Chonnam National University Dental Hospital. The results were obtained as follows; 1. Gender distribution showed female outnumbered male patients in young-aged adult patients, but middle-aged patients showed similar male and female distribution. 2. The major concern seeking orthodontic treatment was esthetics not only in young-aged but also in middle-aged adult patients, and a number of middle-aged patients were concerned about oral health as well. 3. Considerable number of middle-aged patients were referred by other dental specialties while young-aged adult patients were more self-motivated for orthodontic treatment. 4. Middle-aged adult patients had more missing teeth and periodontal disease than young-aged adults. 5. The most frequently-observed problem was dental spacing in middle-aged patients while dental crowding in young-aged adult patients. Middle-aged patients showed higher prevalence of deep overbite and overjet while most of young-aged adults presented opposite direction of problem in overbite and overjet. 6. Limited orthodontic treatment was required rather than comprehensive treatment in middle-aged patients, and the most common tooth moving area was anterior part of dentition in case of limited treatment. Need of interdisciplinary therapy with other dental specialties was more common in middle-aged patients. 7. Intervention of specific technique such as invisible TP, passive bracketing, passive wire bonding, and lingual orthodontics was more required in middle-aged patients. Considering that middle-aged patients have different characteristics than young-aged adults, the results of the present study suggest that different treatment modalities are required in middle-aged orthodontic patients in order to manage them properly and efficiently.
Statement of problem: An orthodontic miniscrew implant has been used as a skeletal anchorage for orthodontic treatment. However, any relation among the influence of the cortical bone, morphologic differences of orthodontic miniscrew implants and new bone formation hasn't been made clear yet. Purpose: The purpose of this study was to evaluate whether the orthodontic miniscrew implant could work as an intraoral skeletal anchorage immediately and stably for orthodontic treatment after insertion of it. Material and methods: Two types of orthodontic miniscrew implants were used in this experiment; tapered type and straight type. One hundred and sixty eight orthodontic miniscrew implants were inserted into the tibiae of 21 rabbits and sacrificed on 3, 7, 11, 14, 21 and 28days later after insertion of them to study removal torque values and histologic and histomorphometric analyses. Results: The results were as follows. 1. The removal torque values of the tapered type were higher than those of the straight type in all groups(p<0.05). 2. There wasn't any distinguishing differences between the tapered type and the straight type about the new bone formation percentage. 3. The removal torque values for both the tapered type and the straight type were gradually decreased at early stages of the test but started to increase at the 7 days group of the straight type and the 11 days group of the tapered type. 4. New bone formation percentage was increased gradually for both the tapered and the straight types as time passed(p<0.05). 5. It was found that the tapered type showed lower values in the cortical bone about both the maximum equilibratory stress distribution and the maximum principal stress distribution than the straight type in linear finite elements analysis. Conclusion: According to the research, the removal torque values were decreased at 7 days group of the tapered type and 11 days group of the straight type after the insertion of the orthodontic miniscrew implants in tibiae of rabbits. Considering the human bone activity, it is better to apply the orthodontic force $3{\sim}4$ weeks later than to apply it immediately after the insertion of orthodontic miniscrew implants. Considering that general orthodontic force is about $250{\sim}500$ grams, the tapered type can be worked as a stable skeletal anchor age in an orthodontic treatment even if the orthodontic force is applied on it immediately after the insertion of it.

Skeletal anterior open bite is a difficult problem to correct in orthodontic treatment. To treat adult patients who have skeletal anterior open bite, we considered two methods. Combination treatment of orthodontics & surgery and camouflage orthodontic treatment. In adults, treatment of severe skeletal anterior open bite consists mainly of surgically repositioning the maxilla or the mandible. However, camouflage therapy is often the treatment of choice for skeletal open bite patients who have mild to moderate skeletal discrepancies when growth modification is no longer possible. But excellent results generally require careful coordination of the orthodontic and surgical phases of treatment. This is a case report of a skeletal anterior open bite patients who were treated with orthodontic treatment and orthognathic surgery. First case was diagnosed as skeletal class I malocclusion & bimaxillary protrusion with anterior open bite, and finally treatment ended for removal of open bite with orthodontic procedure and bimaxillary anterior segmental osteotomy surgery. Second case was diagnosed as skeletal class II malocclusion with open bite & mandibular retrusion, and was treated with only camouflage orthodontics because she feared to have a surgery. In a regular follow up visit after debonding we proposed to the patient advanced genioplasty, and in her agreement her facial esthetics was improved through the surgery.
The effect of orthodontic force on the collagenase and phosphatase activities of the adjacent alveolar bone was evaluated. Maxillary canines of male cats were treated orthodontically with closed coil spring so as to exert about 80g force. Sixteen cats were equally divided into one control group and seven experimental groups (12 hrs, 24 hrs, 36 hrs, 2 days, 3 days, 5 days and 7 days after orthodontic treatment). After sacrificing all animals on experimental intervals, collagenase, acid phosphatase (ACP) and alkaline phosphatase (ALP) activities were determined in the pressure and tension sides of alveolar bones. ACP activities increased in both the pressure and tension sides, but significantly increased in the pressure side continuously. ALP activities increased in the tension side at early stage (1-2 days after treatment), but changed small amount in the pressure side. Collagenase activities increased in the pressure side, especially at late stage (5-7 days after treatment). These results suggest that (1) orthodontic fore force increases the ACP, ALP and collagenase activities generally and (2) activities of ACP and collagenase increase in the pressure side, but that of ALP in the tension side and (3) activities of ACP and ALP increase at early stage, but that of collagenase at late stage after orthodontic treatment. Therefore it is shown that there are time differences in the formation and destruction of organic and inorganic components in the bone metabolism of alveolus with application of the orthodontic forces.
Outcome of esthetic ceramic restorations are affected by tooth size, gingival contour, occlusal relationship, etc. For this reason, demand of orthodontic treatment before esthetic ceramic restoration is increasing. If a Bolton ratio discrepancy, a problem of the maxillary incisor's vertical position, a problem of inclination of anterior teeth, a pathogenic occlusion is existed, a pre-prosthodontic orthodontics should be accomplished. These problems can be satisfactory only after the prosthetic treatment is performed after orthodontic treatment. When orthodontic treatment is given, it should be treated with the following principle. 1. Treat it in the direction of functioning occlusion. 2. Keep the patient's stable occlusal scheme. 3. Treat the teeth by considering the average tooth size and Bolton ratio. Ortho-Prostho combined treatment with optimal treatment plan can lead a patient's function, esthetics, and long-term stability.
