  • Objectives: The present study aimed to provide clinical evidence on the effect of herbal medicine on the treatment of hepatitis C using a systematic review and meta-analysis of randomized controlled trials (RCTs). Methods: This study included RCTs that verified the effects of herbal medicine for the treatment of hepatitis C. A literature search of English, Chinese, and Korean databases was performed, and the selected literature was assessed to investigate the risk of bias. Results: The analysis included 14 RCTs. The total effective rate was the most commonly used outcome measure. The meta-analysis revealed that the total effective rate and total bilirubin difference were higher and that the recurrent rate was lower in the herbal medicine group than in the western medicine group (RR: 1.20, 95% CI: 1.06-1.35, p=0.003, I2=52; MD: 0.52, 95% CI: -0.62 to 1.66, p=0.37, I2=0; RR: 0.35, 95% CI: 0.11-1.09, p=0.07, I2=0). The included RCTs had relatively poor quality and a small sample size. Conclusion: Treatment with herbal medicine was found to be partially effective in treating hepatitis C. The included RCTs had relatively poor quality and a small sample size. This study can serve as a foundation for further clinical studies on the development of treatment methods for hepatitis C based on herbal medicine.

  • Objectives : To review the recent trend of randomized controlled clinical trials on insomnia and to provide information for future clinical trials. Methods : A total of 667 pieces of literature were searched using the key words 'insomnia' and 'randomized controlled trial' and using the title 'insomnia' with the topic 'trial or trials', published from 2008 to 2012 through Web of Science. Studies including randomized controlled clinical trials were sorted from the search result and finally 104 pieces of the literature were selected and examined. Results : Besides 104 clinical trials, 14 trials related to CAM (Complementary and Alternative Medicine) were also reviewed. On average, 20 trials were annually conducted and they showed a growing trend. Participants were between 31 and 90 (34.6%), and were observed for less than 30 days (28.8%) in most trials. As intervention methods for clinical trials, non-pharmaceutical methods were used in 59 studies (56.7%), pharmaceutical drug in 43 studies (41.3%) and combinations in 2 studies (1.9%). In 60 studies, only insomnia without any underlying diseases was examined and other 44 studies involved other diseases. As diagnosis assessment tools, Sleep diary and Polysomnography were used. Conclusions : Randomized controlled trials relevant to insomnia were on the increase, but only a small number of clinical trials on Oriental Medicine have been performed. Larger scientific and well-founded randomized controlled trials are required for developing Oriental Medicine and establishing high-quality guideline going forward.

  • Objectives : The aim of this study is to summarise pattern-identification of shoulder pain based on the classics of oriental medicine, current literature and domestic papers. Methods : The materials selected were sourced from the classics of oriental medicine, current literature and domestic papers which contained data related to pattern-identification of shoulder pain. The pattern-identifications were compared to determine the similarities, and these were classified. Results : Thirty-five studies were reviewed, and thirty-seven pattern-identifications were collated. These were classified into the following groups: wind-cold-dampness group(n = 8), blood stasis group(n = 3), phlegm group(n = 7), dual deficiency of Qi and blood group(n = 4), deficiency cold group(n = 2), liver-kidney deficiency group(n = 1) and meridian-collateral group(n = 12). Conclusions : On the basis of the classification of pattern-identifications, two groups of pattern-identifications for shoulder pain were suggested. The first group included the pattern-identification associated with a disease-cause, which included the wind-cold-dampness pattern(風寒濕型), blood stasis pattern(瘀血型), phlegm pattern(痰飮型), Qi-blood deficiency pattern(氣血兩虛型), deficiency cold pattern(虛寒型), and liver-kidney deficiency pattern(肝腎虧損型). The second included the pattern-identification associated with the meridian-collateral, which included the hand greater Yin meridian pattern(手太陰經型), hand Yang brightness meridian pattern(手陽明經型), hand lesser Yin meridian pattern(手少陰經型), hand greater Yang meridian pattern(手太陽經型), hand reverting Yin meridian pattern(手厥陰經型), hand lesser Yang meridian pattern(手少陽經型), and foot greater Yang meridian pattern(足太陽經型).

  • Objectives : This study aims to educate clinical doctors of the valuable practice of Manipulative Therapy(MT) as an opportunity for evidence-based medicine. Consequently it also serves to review the effectiveness and safety of MT. Methods : The literature studies of overseas were done by Cochrane Library and Medline website; those of domestic researches were completed by utilizing the sources which are gained from KERIS, KISS, DBpia, Kisti, and domestic institutes related with MT. Results : Eight papers related to the effectiveness of MT were published in South Korea, which concluded with the positive effects of MT. None of them are, however, well-designed randomized controlled trials(RCT). On the other hand, fifteen cases of nine articles indicate the adverse reaction of MT, and numbers of the researches in overseas revealed the side effects of MT in order of cervical, lumbar, and thoracic vertebrae. A vascular adverse reaction such as vertebral and carotid artery dissection was a highly reported cervical adverse reaction; in the thoracic and lumbar regions, neurological adverse reaction in terms of disc hemiation was frequently discovered. Conclusions : Henceforth, highly qualitative studies are required developing the effective outcomes and preventing any possible complications of MT. Therefore, systemic curriculums in institutions and sufficient clinical training in the filed are strongly recommended.
