Comparison of Korean Medicine Psychotherapy and Traditional Chinese Medicine Psychotherapy for Anxiety: Focusing on Clinical Studies (불안에 대한 한의정신요법과 중의정신요법의 비교고찰: 임상연구를 중심으로)

  • Objectives: To compare Korean medicine (KM) and traditional Chinese medicine (TCM) psychotherapy for anxiety. Methods: Databases including MEDLINE (via PubMed), EMBASE (via Elsevier), Cochrane Central Register of Controlled Trials, China National Knowledge Infrastructure, and Oriental Medicine Advanced Searching Integrated System were comprehensively searched. Prospective clinical studies on KM or TCM psychotherapy for patients with anxiety disorder or individuals with elevated anxiety levels published up to August 3, 2022 were reviewed. Psychotherapy was divided into counselling, art therapy, and meditation according to its characteristics. Results: A total of 12 clinical studies were reviewed, including nine randomized controlled trials. The most common disorder investigated was post-traumatic stress disorder. Ten studies used TCM psychotherapy and two used KM psychotherapy. As for differences between TCM psychotherapy and KM psychotherapy, TCM psychotherapy utilized pattern identification in the procedure more actively than KM psychotherapy. In addition, some TCM studies have attempted to directly converge Western psychotherapy (i.e., hypnosis) and Eastern psychotherapy (i.e., Taoin qigong therapy). In the case of KM psychotherapy, there was an attempt to incorporate psychotherapy with Sasang constitutional medicine. Reported effects of TCM psychotherapy and KM psychotherapy on anxiety were positive. Conclusions: Research status of KM psychotherapy and TCM psychotherapy for anxiety was investigated, revealing some of their characteristics, commonalities, and differences. Findings of this review have the potential to provide a clue to the development of conventional KM psychotherapy and new medical technology for KM psychotherapy.

Efficacy of Herbal Medicine on Sleep Disorders in Parkinson's Disease: A Review of Randomized Controlled Trials (파킨슨병에 동반된 수면장애의 한약 치료에 대한 임상 연구 동향 : 무작위 대조연구를 중심으로)

  • Objectives: This study reviewed randomized controlled trials (RCTs) investigating the efficacy of herbal medicine on sleep disorders associated with Parkinson's disease and suggests a better research process. Methods: We searched for RCTs for herbal medicine treatments for sleep disorders related to Parkinson's disease on July 31, 2023 using eight databases (PubMed, Embase, the Cochrane library, China National Knowledge Infrastructure [CNKI], the Research Information Service System [RISS], Science ON, the Oriental Medicine Advanced Searching Integrated System [OASIS], and the Korea Citation Index [KCI]). Cochrane's risk of bias tool was used to assess the quality of the RCTs. Results: A total of 16 RCTs met all the inclusion criteria, and in most reports, the treatment group showed a significant improvement in sleep disorders compared to the control group. Total effective rate (TER), Pittsburgh Sleep Quality Index (PSQI), Unified Parkinson's Disease Rating Scale (UPDRS), TCM Symptom Score (TSS), Parkinson's Disease Sleep Scale (PDSS), etc., were used as evaluation indicators. Conclusion: Herbal medicine is a potential treatment for sleep disorders associated with Parkinson's disease. However, the selected RCTs were of poor quality, and it is necessary to perform more systematic studies.

A Systematic Review of Acupuncture for Tennis Elbow

  • This review analyzed randomized controlled trials and case reports to identify the effectiveness, and level of evidence showing that acupuncture treatment aids the recovery of patients with tennis elbow. A literature search was performed between $1^{st}$ to $10^{th}$ September, 2019 which used PubMed, the Cochrane library, the Korean databases Oriental Medicine Advanced Searching Integrated System, Korean Studies Information Service System to retrieve Korean and international studies. Amongst the 243 articles retrieved there were 9 randomized controlled trials and 18 case reports. The level of evidence for the recovery of patients with tennis elbow, for efficacy and safety of acupuncture was low. It was observed that the studies had a high risk of bias, missing acupuncture details, multiple combinations of treatments rather than a single treatment, insufficient indicators of assessment, and lacked robustness. Compliance with international standards such as using consolidated standards of reporting trials, will improve the quality of evidence.

Literature Review for the Clinical Application of Dietary Supplements in Cellulite Treatment (셀룰라이트 치료 시 식이 보조제의 임상적 활용을 위한 문헌적 고찰)

  • Objectives: The purpose of this study is to investigate the efficacy and the mechanisms of dietary supplements in cellulite treatment, and then to provide the theoretical and clinical basis for the cellulite treatment in Korean Medicine. Methods: We searched for articles from Korea, China, and English electronic database (Koreanstudies Information Service System [KISS], National Digital Science Library [NDSL], KMbase, Research Information Sharing Service [RISS], Oriental Medicine Advanced Searching Integrated System [OASIS], National Assembly Library, Korean Traditional Knowledge Portal, Google scholar, PubMed, Scopus, China National Knowledge Infrastructure [CNKI]) until April 2018. We chose clinical trial studies by inclusion criteria through titles, abstracts and articles. Results: A total of 10 studies were selected through search. The experimental group had shown more effective cellulite improvement in 90% of studies. Also, improvement of symptoms related with cellulite like pain, edema, heaviness and increase of skin surface temperature were observed in experimental group. In addition, the density of connective tissues of the dermal layer was increased in experimental group. Conclusions: The use of dietary supplements in cellulite treatment is thought to be effective through mechanisms that antioxidant efficacy, microcirculation improvement, interstitial matrix improvement, diuretic effect, and skin metabolic activity effect.

Effects of Jakyakkamchobuja-tang on Rheumatoid Arthritis in Rat Model: Systemic Review and Meta-Analysis (류마티스 관절염 백서 모델에서 작약감초부자탕의 효과: 체계적 문헌고찰 및 메타분석)

  • Objectives This study was designed to review the effect of Jakyakkamchobuja-tang on rat model with rheumatoid arthritis. Methods We used seven databases (PubMed, EMBASE, Cochrane CENTRAL, China National Knowledge Infrastructure, Oriental Medicine Advanced Searching Integrated System, Korean studies Information Service System, National Digital Science Library) from their inception to May 2023 without language restrictions. Systematic Review Centre for Laboratory Animal Experimentation's tool was used to evaluate the risk of bias. RevMan software (V5.4) was used for the meta-analysis. Results Five studies were selected following our inclusion criteria. The arthritis index decreased significantly (standardized mean difference=-2.06; 95% confidence interval=-3.07 to -1.04; p<0.0001) in Jakyakkamchobuja-tang group. Also, serum cytokines in serum and paw swelling degree decreased in Jakyakkamchobuja-tang group. Conclusions Jakyakkamchobuja-tang may be effective in treating rheumatoid arthritis. Although there is a limitation that the design of drug dosage varies between papers, it can be expected to be applied as an alternative to Western medicine, and it is believed to contribute to the standardization of herbal treatment for rheumatoid arthritis.

An Analysis of Domestic Research Trends of Korean Medicine Treatments for Rotator Cuff Tear (회전근개 파열에 대한 한의치료의 국내 연구동향 분석)

  • Objectives The purpose of this study is to analyze trends of domestic research in Korean medicine treatment for rotator cuff tear. Methods Among the clinical papers published before March 2024, we used five databases for searching researches; Korean studies Information Service System, Oriental Medicine Advanced Searching Integrated System, Research Information Sharing Service, KMbase, ScienceON. Results 11 studies were finally selected. Acupuncture was used as a treatment in all 11 studies and pharmacopucture was used in 7 studies. Visual analogue scale and range of motion were frequently used as measurement tools. Most of the studies were case reports. Conclusions This study is a review about literatures on Korean medicine treatment for rotator cuff tear. In order to develop treatment and diagnosis of rotator cuff tear in the Korean medicine, more clinical studies with higher level of evidence should be conducted in the near future.

A Review on Clinical Studies of Abdominal Acupuncture Treatment for Endometriosis (자궁내막증 환자의 복부 침 치료 효과에 대한 임상 연구 고찰)

  • Objectives: The purpose of this study is to confirm the effectiveness and safety of acupuncture for Endometriosis. So providing a clinical basis for abdominal acupuncture treatment helping to develop clinical practice guideline. Methods: Clinical studies applying acupuncture on Endometriosis were searched through Pubmed, China Academic journal (CAJ), Koreanstudies Information Service System (KISS), Research Information Sharing Service (RISS), Oriental Medicine Advanced Searching Integrated System (OASIS). Interventions and results of the selected clinical studies were analyzed. Results: 8 Clinical studies were finally included according to inclusion and exclusion criteria. Those were randomized controlled studies. All of those studies were searched in China Academic journal (CAJ) in Chinese. Interventions of control group were made with simple acupuncture and oral administration of Danazol or herbal medicine. Outcome measurements included Treat effect, Relief in related symptoms, and CA-125. All studies reported statistically significant results on abdominal acupuncture and there were no serious adverse effects. Conclusions: This study suggested abdominal acupuncture has statistically significant treat effect. Thus this treat helps improve quality of life for patients. However the results should be taken cautiously as more clinical studies are needed.

A Literature Review of the East-West Medical Combined Treatment (한양방 병용치료에 대한 문헌고찰)

  • Objectives : This study aims to analyze the papers of the east-west medical combined treatment and suggest the research direction. Methods : 183 research papers were collected by using 25 keywords including combination(in korean words, 병용), collaborative practice(in korean words, 협진) in Korean Medical Database(KMbase) and Oriental Medicine Advanced Searching Integrated System(OASIS). We analysed the type of studies, the kind of diseases, the treatment method, the kind of herbs and drugs in frequent use, and the result of research. Results : Among the analysed 183 papers, clinical studies are 89 cases and experimental studies are 94. 5 cases of clinical studies are Randomized Controlled Trial(RCT). The cancer(50 papers) and diseases of the circulatory system(25 papers) occupied mostly in treatment studies. Because Combined treatment was actively progressed in oriental medicine, treatment was mainly applied the combined oriental medical treatment with herbs and western drugs. In herbal medicine, Mahwangyounpae-tang(麻黃潤肺湯) and Eunkyo-San(銀翹散) were frequently used in the papers. In western drugs, Cyclophosphamide, Cisplatin, and Mitomycin C were frequently used in the papers. 154 papers introduced the treatment effect, 14 papers announced the safety, and 4 papers mentioned the side effect. Conclusions : We suggested several future research direction as follows. Clinical studies based on experiment studies must be more activated and many RCT shoud be shown. Experimental group in clinical studies should be clearly separated to confirm the treatment method is effective or isn't effective. Studies about the side effect must be expanded, and every study should be confirmed both the treatment effect and safety.

Review of Low Level Laser Therapy on The Growth of Epiphyseal Plate (성장판의 성장에 저단계 레이저가 미치는 영향에 대한 고찰)

  • Objectives We aimed to identify the effectiveness of photobiomodulation using low level laser therapy (LLLT), light emitting diode (LED) and others on the growth of the length of the growth plate by reviewing literatures. Methods We searched literatures using PubMed, Science Direct, CINAHL, Korea Traditional Knowledge Portal (KTKP), Oriental Medicine Advanced Searching Integrated System (OASIS), China Knowledge Resource Integrated Database (CNKI), Japan Science and Technology Information Aggregator, Electronic (J STAGE), and Japan National Institute of Informatics Scholarly and Academic Information Navigator (CiNii) using the keywords "Growth plate" "Epiphyseal growth" "Epiphyseal plate" and "Laser", "light emitting diode (LED)", "near-infrared light", and "photobiomodulation". Search range included only original article which provided English abstract were selected. The search strategy contained no language limitation. Results A total 556 studies were found. Then, 551 were excluded by scanning titles and abstracts and finally 5 articles were selected. Five articles were RCTs using rodents. Two of the 5 articles used InGaAlP Laser (630-685 nm), and the other 3 articles used GaAlAs Laser (780, 820, and 870 nm) to investigated the effects of LLLT on the growth of the length of the epiphyseal cartilage and the number of chondrocytes and thickness of each zone of the epiphyseal cartilage. Two articles concluded that LLLT had a beneficial effect on the longitudinal growth of the growth plate. In growth of the epiphyseal plate, there were no significant differences in others. Conclusions It is might that LLLT influenced on the growth of epiphyseal plate by positive affect. However, further rigorous RCTs are warranted.

A Systematic Review and Meta-Analysis on the Effect of Herbal Medicine for Endometrial Hyperplasia - Focusing on Chinese Randomised Controlled Trials (자궁내막증식증에서 한약 치료의 효과에 대한 체계적 문헌 고찰 및 메타분석 - 중국 임상 연구를 중심으로)

  • Objectives: The purpose of this study is to analyze the effectiveness of herbal medicine that are used for endometrial hyperplasia. Methods: We searched on three databases [PubMed, China national knowledge infrastructure (CNKI), and Oriental medicine advanced searching integrated system (OASIS)] with keywords for Endometrial hyperplasia(English, Korean, and Chinese) and evaluated the risk of bias. Meta-analysis was performed on the selected studies. Results: 15 RCTs were finally selected. According to the analysis, Most of the participants were endometrial hyperplasia without atypia. And the combined therapeutic effects of hormonal medicine and herbal medicine were statistically significant (RR 1.21 [95% CI 1.15, 1.28], P<0.00001), The endometrial thickness tended to be thinner when treated combined therapy. And they also showed improvement on levels of pictorial blood assessment chart (PBAC) and hemoglobin. Also, the therapeutic effects of herbal medicine can be identified as similar to Western medicine, and no superior effects have been confirmed (RR 1.05 [95% CI 0.87, 1.28], P=0.60). Conclusion: The combination Western and herbal medicine was more effective than Western medicine alone treated for endometrial hyperplasia without atypia. Moreover there were also significant improvements in endometrial thickness, PBAC and hemoglobin levels.
