A Development Study of Common Clinical Document Forms for Traditional Korean Medicine Information Standardization (한의 정보 표준화를 위한 공통 임상 기록 서식 개발 연구)

  • Objectives: The clinical document forms, a format for collecting clinical data, is the most fundamental object of standardization. Doctors must have a mutual understanding of the clinical chart. Methods: Clinical document forms were developed by investigating existing conditions in hospitals and conducting demand surveys, doing literature research, and seeking expert advice for the improvement of version 1.0. In addition, an organization of a network of 19 Oriental medical doctors and nurses, 190 patients, and users of collected and assessed data was formed to come up with version 2.0. Results: The overall format was divided into different portions that the patient, nurse, and doctor must fill out, respectively. The patient's section consists of demographic data, lifestyle details, history, and symptoms. The data to be supplied by the nurse include the patient's vital signs and anthropometric parameters. As for the doctors, they are to supply data regarding the patient's palpitation, the detailed symptoms of the patient's head, ophthalmological and otorhinolaryngological symptoms (mouth), respiration, circulatory organ and chest conditions, digestive-organ conditions (thirst), neuropsychiatric conditions, reproductive system, musculoskeletal system, skin (depilation), etc. Conclusions: Common clinical chart development is the prior question to Traditional Korean Medicine standardization. A web-based clinical document format should be developed to support diagnosis and treatment, and furthermore EMR (electronic medical record system) and EHR (electronic health record) developed. Clinical information could be shared through a network of medical institutions and be useful Traditional Korean Medicine for evidence-based medicine.

Preliminary Study for Development of System for Oriental Nurse-specialist (한방전문간호사제도를 위한 예비조사)

  • This study was conducted to examine the nurses' role in Oriental medical hospital and the necessity of Oriental medical nursing system. Data were collected from 194 nurses in 14 Oriental medical hospitals located in Seoul, Busan, Daegu, Kwangju, Kangwon Province, Kyungbuk Province from July 1, 2000 to September 10, 2000. The results of this study are summarized as follows. 1. In nurse's level of knowledge of acupuncture point in current Oriental nursing practice, 28.4% of the respondents did not know acupuncture points at all, while 30.9% knew them a little. 40.7% of them did not almost know them. 2. According to nurses' role in Oriental medical hospital, they did a lot of duty for measurement of vitality, medical record, Dr. notifying, communication with medical team and related departments, and observation of patients' conditions. They did a little duty for handling Oriental medical practice. 3. As the subject of curriculum for Oriental medical nursing, 52.1% chose training course for Oriental medical nursing, while 25.8% chose inclusion of the curriculum in the school of nursing. 39.2% replied that they would take the training course for Oriental medical nursing, while 3.6% answered that they would not take it. 68.6% of them thought that Oriental medical nursing system should be introduced, while 4.1% objected to introducing the system. The nurses in Oriental medical hospitals are working with many problems because of their role confusion and low level of knowledge in Oriental medicine. Therefore, systematic curriculum and research related to Oriental medical nursing are necessary. Oriental medical nursing system must be introduced in order to make nurses have professional knowledge and skill in Oriental medicine. The nurses then can provide services of good quality for the patients.

  • Simpo(心包. Pericardium) was described early in "Hwangjenaegyeong(黃帝內經)". But there has been a lot of criticisms about this organ. Jinsatak(陳士鐸) is one of famous doctors in Cheong(淸) dynasty. He suggested a creative Oriental medical theory and he did a profound research on Simpo(心包). "Oegyeongmieon(外經微言)" is one of his books recording his oriental medical theory in detail so it is the good documentary record for observing his theory of the Simpo(心包). So we looked into his theory of Simpo(心包) in "Oegyeongmieon(外經微言)" and also referred to his other books. You might be able to get a viewpoint of utilizing Simpo(心包) in several ways through reading this paper.

  • The information relating to the health of person has been increasing. The information is such as medical information and personal health record and the information collected by utilization and dissemination of mobile devices. Therefore, the interest and demand for systems that can integrate and manage the Personal Health Record(PHR) is increasing. Quantity and quality of information that is collected from the patient can have a major impact on the diagnosis and treatment of Korean Medicine(KM) in clinical practice. Because closely observe the usual clinical symptoms of patients to utilize the treatment. But if the interview when memories are not sure of the correct answer does not get much easier to find exactly the symptoms. So when recording original symptom(素證) and daily subjective symptom can be helpful for care. Therefore, the personal health care services that can record and manage and own is necessary based on KM. In this paper, we propose Korean Medicine Personal Health Record Platform(KM PHR Platform). We have selected the significant symptoms that mean to the personal records from symptom information required for diagnosis in KM. And classifying and scoring as the symptoms were used as personal health care indicators. And significant symptoms were easily configure a screen that can be recorded. simple operation is recorded as a symptom. It was designed to reflect these functions. So KM PHR Platform helps to Personal health care. Doctor may be able to help in the diagnosis and prognosis observation by reference to shared symptom. We look forward to a variety of health services based on KM using a symptom, a medical record, personal health device information.

A survey research to develope a personal health record application for atopic dermatitis in Korean medicine (아토피피부염 개인건강기록 어플리케이션 개발을 위한 전문가 집단 설문조사 연구)

  • Objectives : We conducted a survey aimed at developing a personal health record application for the treatment of atopic dermatitis in Korean medicine .Methods : We conducted a survey on Korean medicine doctors who attended the Korean Medicine Ophthalmology and Otolaryngology and Dermatology conference 2016. The questionnaire was based on priority of usage of the diagnostic indices and tools, and intention to use the personal health record application for treatment of atopic dermatitis in the clinic.Results : Data were collected from 50 Korean medicine doctors. Ninety-six per cent of respondents replied that they were willing to use the personal health record application for treating atopic dermatitis. Among the diagnostic indices related to atopic dermatitis, Korean medicine doctors regarded the following as important in the order of priority, i.e., condition of skin, lifestyle, risk factors, symptoms other than those of skin, past history, family history and medical history, results of tongue, pulse, and abdomen investigation, and constitution. These results did not vary with the purpose of diagnosis, and the results were consistent with those obtained with the intention to use diagnostic. Over 50% of respondents replied that they use immunoglobulin E, scoring atopic dermatitis, and visual analogue scale among the diagnostic tools.Conclusions : Our survey was conducted on clinicians who are the intended users of the personal health record application for the treatment of atopic dermatitis; hence, the results of this study can be helpful for developing a useful personal health record application for atopic dermatitis in the clinic.

  • In recent years, two major streams in medical information systems are:1) system integration among OCS(Order Communication System), EMR(Electronic Medical Record), PACS(Picture Archiving and Communication System), and ERP(Enterprise Resource Planning) and 2) system integration through medical collaboration between East and West medical service providers. One of the characteristics which differentiate the Korean medical industry from the western medical industry is the East-West medical collaboration. In many respects there are many differences between East and West medical treatment. Although East and West medical treatment have developed from different medical philosophies and standards, we assume that the better medical care can be provided by integrating their medical procedures effectively. The two possible approaches to the integration of East and West medical information systems are suggested in this paper:One is loosely coupled model and the other is tightly coupled model. EMR improves the quality of medical record which reflects the quality of clinical practice. It provides more efficient and convenient way of input, retrieval, storage, communication and management of medical data. We abstracted the standard medical procedures from the two medical procedures performed in Daejeon Oriental Hospital and Hehwa Clinic at Daejeon University and also abstracted database schema by analyzing the characteristics of information needed in East-West medical collaboration. Our EMR is composed of two types of data:one is structured data and the other is unstructured data, which are formalized by SOAP(Subjective, Objective, Assessment, Plan) format. Currently the integrated system is implemented and operated successfully for six months.

  • Background : Much Opinions of inefficiency to put the standard term of oriental medicine in and around literature into medical records. Purpose : Suggest an alternative according to analysis of differences between 'Term System' from in and around literature and term of medical records. Method : Research on the difference according to analysis of difference between producing term from in and around literature using DB Program and term from medical records. Result : The best System is that reflecting 'Korean Expression' of term of oriental medicine and part of term of western medicine and general.

Study about Analysis of Current Case for Oriental Medical Disputes - With a Focus on the Oriental Medical Injury Relief Data from Korea Consumer Agency - (한방의료분쟁의 최신사례분석 연구 - 한국소비자원의 한방의료 피해구제 자료를 바탕으로 -)

  • Objectives: The purpose of this study was to analyze the current cases of oriental medical injury relief data from Korea Consumer Agency (KCA), and to report the current change of oriental medical disputes pattern. Methods: Oriental medical injury relief cases processed by KCA from January 2010 to February 2015 were collected and analyzed. Results: 149 Oriental medical injury relief cases from KCA were analyzed for the study. The highest number that had been relieved was 43 in 2014. In monthly status, 18 case in September was The highest. According to the record, the bigger city had the bigger amount of relief cases which was 53(Seoul), 43(Gyeng-gi), 11(Busan) and so on. In age categories, 30s had the highest number of injury relief cases. The injury relief cases of package program had been rapidly increasing since 2013. Cancellation was the dominant claims cause of package program and consumer required the refund of prepayment. Breast augmentation was the majority treatment type of injury relief cases of package program. Average prepayment of package program was \3,166,085. Conclusions: This study finded that the major cause of oriental medical dispute was changed from side effect of clinical treatment to patient's satisfaction of medical service.

  • The purpose of this study is to introduce the music therapy as one of the 17 kinds of oriental medicine music therapy and investigate the record in the literature. The spleen is an organ that has the function of digestion, absorption, and supply of nutrition. Damp, anxiety, and improper diet can cause deficiency syndrome of the spleen. The symptoms of deficiency of the spleen are poor appetite, abdominal distension, borborygmus, water-grain dysentery, dyspepsia, and lack of strength in limbs. CV12, CV6, S36, B20, B21, and Sp4 are can be used as acupuncture points that are a great help to the patients who suffer from deficiency of the spleen. This study describes the theory of invigorating the spleen music therapy that has a good possibility as a new method of oriental medical treatment and present the acupuncture points that can be used with invigorating the spleen music therapy. The tone of the spleen is Gung(宮) and the sound of the spleen is related to a song. The music therapy of invigorating the spleen includes beating Earth-Gi(地氣) rhythm with Cheonnyeonmanse, stimulating Spleen Meridian and Stomach Meridian with Gayaguem sanjo jungjungmori, and acupuncture with "Le Cygne" of Camille Saint-Saens.

A Clinical Report of the Patient with Limited-Stage Small Cell Lung Cancer (제한병기 소세포폐암 환자의 치험례 임상보고)

  • Objectives : This study was designed to evaluate the effects of oriental herb medicine therapy on a limited stage small ell lung cancer(SCLS) patient. Methods : The medical record of this case of SCLS was researched. The patient had been treated with oriental herb medicine for five months continuously in OPD. Clinical data was analyzed and the mass of small cell lung cancer before and after oriental herb medicine therapy was compared using X-ray and CT scan of the whole body in order to determine metastasis. Result : After 5 months of oriental herb medical treatment, most of symptoms disappeared or improved. The small cell lung cancer patially receeded and improvements were mainly seen in sputum, coughing, insomnia, diarrhea, and performance status. Conclusion : The study suggests that oriental herb medicine therapy effects the tumor size by causing remission and Improving health and relieving symptoms, and also positively effects the quality of life as a supportive or curative therapy for SCLC patients.
