Oral Screen의 임상 적용에 대한 고찰 (APPLICATION OF AN ORAL SCREEN)

  • Oral screen은 가장 기본적인 기능성 장치로서, 근육의 변화를 도모하여 비정상적인 근육형태와 연관된 초기부정교합의 개선에 적합하다. 이것은 혀와 buccinator mechanism사이의 근평형을 향상시켜, 정상적인 성장이 가능하도록 한다. Oral screen은, (1) 손가락빨기 혀내밀기, 입술깨물기 등의 구강습관, (2) 기도가 열려있을 때의 구호흡, (3) 상악전돌경향을 동반한 경미한 원심교합, (4) 유치열과 혼합치열에서의 개방교합, (5) 무력순 등을 개선하는데 사용된다. Oral screen은 매일 밤, 그리고 가능하다면 낮에도 장착하여야 한다. 장치의 효과는 구순폐쇄훈련을 시행함으로써 발휘된다. 장치를 장착하는 동안, 입술은 장치의 효과를 높이고 구순폐쇄를 향상시키기 위해 항상 접촉한 상태로 유지되어야 한다. 본 증례는 구호흡과 무력순을 보이며 각각 상악절치의 순측경사와 치은비대, 개방교합 양상을 보이는 2명의 어린이환자에게 구순폐쇄훈련을 병행하면서 oral screen을 장착한 결과, 구호흡경향이 줄어들어 치은부종이 감소하였고 구순의 근력과 길이는 증가하여 구순폐쇄가 원활해졌다. 아울러 상악 절치의 경사도가 감소되어 개방교합의 치료에 효과적임을 알 수 있었다.

증감지와 필름에 따른 방사선상 변화에 대한 연구 (THE COMPARATIVE EVALUATION OF FILM-SCREEN COMBINATIONS)

  • This study was to compare the quality of image by different screen and film combinations. Using the sensitometer measured the speed and average gradient of blue sensitive films and orthochromatic films. The films was combined with rare earth screen LR, LM, LF and conventional screen OM, OH, XOR, OKa and exposed the step wedge to impulse 2, 3, 4, 6, 10, 15, 24,3 8, 60 and measured the density. The following results were obtained: 1. The density of film and film-screen combinations showed significant difference, then in film-screen combinations was significantly different by the screens than films. 2. The speed of blue sensitive films was little different, the TMG of orthochromatic films produced high speed, and the AX film was high average gradient. 3. The relative speed of film-screen combinations showed significant difference, and was high in the OKa of the conventional screens and in the LR of the rare earth screens, especially that of LR screen in the combination with blue sensitive films was high. 4. The average gradient of film-screen combinations showed no significant difference, and was high in the OKa screen and LR/OG combination, and that of OKa/AX combination was highest. 5. The latitude of film-screen combinations showed significant difference by screens, and was high in the LM screen in combination with blue sensitive films and in the OM screen in combination with orthochromatic films. 6. The subject contrast of film-screen combinations showed significant difference by screen, and was high in the LR screen in combination with blue sensitive AX film and orthochromatic TMG film.

Oral carcinoma cuniculatum, an unacquainted variant of oral squamous cell carcinoma: A systematic review

  • Purpose: Oral carcinoma cuniculatum is a rare well-differentiated variant of oral squamous cell carcinoma. The purpose was to systematically review its unique features to differentiate it from other variants as verrucous carcinoma, papillary squamous cell carcinoma and well-differentiated squamous cell carcinoma. Materials and Methods: A systematic review was performed using MEDLINE, Dentistry and Oral Sciences Source and PubMed databases and any existing articles related to the research subject missed in the search strategy to screen ones reporting cases occurring exclusively in the oral cavity in English literature. Variables analyzed included clinical, etiologic, imaging, histopatholgical features, treatment, follow-up and survival rates. Results: From 229 hits, 17 articles with 43 cases were included in the systematic review. Clinically it showed a female predilection with pain and/or ulceration of a relatively long duration and exudation being the most common symptoms. Histologically, it showed more endophytic features comprising well-differentiated squamous epithelium with absent or minimal cytological atypia and multiple keratin filled crypts or cuniculus. Inflammatory stromal reaction and discharging abscesses were reported in most of the cases. Bone destruction was predominant in most imaging features. Complete surgical resection with a safety margin was the treatment of choice in most of the cases with few recorded recurrence cases. Conclusion: Apprehensive knowledge of oral carcinoma cuniculatum unique features is essential to avoid its misdiagnosis and provide proper treatment especially for recurrent cases.

Identification of differentially expressed genes using an annealing control primer system in periodontitis

  • In the gingival tissues of patients with periodontitis, inflammatory responses are mediated by a wide variety of genes. In this study, we screened for differentially expressed genes (DEGs) in periodontitis compared with normal tissue using an annealing control primer (ACP) system. By ACP RT-PCR analysis, we obtained about 160 amplicons, 8 of which were found to be differentially expressed. DEGs in patients with periodontitis were thus successfully and reliably identified by the ACP-based RT PCR technique. The DEGs identified in the screen may also enhance our understanding of the pathogenesis of periodontitis.

Simple Qualitative Sensory Assessment of Patients with Orofacial Sensory Dysfunction

  • Purpose: Oral and facial sensation is affected by various factors, including trauma and disease. This study assessed the clinical profile of patients diagnosed with sensory dysfunction and investigated their sensory perception using simple qualitative sensory tests. Methods: Based on a retrospective review of the medical records, we analyzed a total of 68 trigeminal nerve branches associated with sensory dysfunction in 52 subjects. We analyzed the frequency and etiology of sensory dysfunction, and the frequency of different types of sensory perception in response to qualitative sensory testing using tactile and pin-prick stimuli. Results: The inferior alveolar nerve branch was the most frequently involved in sensory dysfunction (88.5%). Third molar extraction (36.5%) and implant surgery (36.5%) were the most frequent etiological factors associated with sensory dysfunction. Hypoesthesia was the most frequent sensory response to tactile stimuli (60.3%). Pin-prick stimuli elicited hyperalgesia, hypoalgesia, and analgesia in 32.4%, 27.9%, and 36.8%, respectively. A significant association was found between the two kinds of stimuli (p=0.260). Conclusions: Sensory dysfunction frequently occurs in the branches of the trigeminal nerve, including the inferior alveolar nerve, mainly due to trauma associated with dental treatment. Simple qualitative sensory testing can be conveniently used to screen sensory dysfunction in patients with altered sensation involving oral and facial regions.

Screening of Differential Promoter Hypermethylated Genes in Primary Oral Squamous Cell Carcinoma

  • Background: Promoter hypermethylation leads to altered gene functions and may result in malignant cellular transformation. Thus, identification of biomarkers for hypermethylated genes could be useful for diagnosis, prognosis, and therapeutic treatment of oral squamous cell carcinoma (OSCC). Objectives: To screen hypermethylated genes with a microarray approach and to validate selected hypermethylated genes with the methylation-specific polymerase chain reaction (MSPCR). Materials and Methods: Genome-wide analysis of normal oral mucosa and OSCC tissues was conducted using the Illumina methylation microarray. The specified differential genes were selected and hypermethylation status was further verified with an independent cohort sample of OSCC samples. Candidate genes were screened using microarray assay and run by MSPCR analysis. Results: TP73, PIK3R5, and CELSR3 demonstrated high percentages of differential hypermethylation status. Conclusions: Our microarray screening and MSPCR approaches revealed that the signature candidates of differentially hypermethylated genes may possibly become potential biomarkers which would be useful for diagnostic, prognostic and therapeutic targets of OSCC in the near future.

Conservative Surgical Management of Fibrous Dysplasia with Osteomyelitis

  • Fibrous dysplasia (FD) accompanying osteomyelitis (OM) has been reported to result in recurrent, refractory pain and swelling. Although radical resection of the lesions has been suggested, effective surgical treatment strategies have not yet been established due to the limited number of studies on this pathological condition. In this report, we present the conservative surgical management of FD accompanying OM in two patients who exhibited recurrent signs and symptoms. The present report suggests that OM occurring in patients with FD can be successfully managed with conservative surgical treatment and following removal of the odontogenic origin although bone defect in which inflammatory fibro-osseous lesions is removed through decompression surgery may exhibit regeneration of dysplastic bone. In addition, for the prevention of OM in patients with FD, careful clinical examination and thorough management of dental-related pathologies are necessary with regular follow-up examinations to screen for the possibility of malignant changes.

Screening and Development of DNA Aptamers Specific to Several Oral Pathogens

  • Aptamers are composed of single-stranded oilgonucleotides that can selectively bind desired molecules. It has been reported that RNA or DNA could act as not only a genetic messenger but also a catalyst in metabolic pathways. RNA aptamers (average sizes 40-50 bp) are smaller than antibodies and have strong binding capacities to target molecules, similar to antigenantibody interactions. Once an aptamer was selected, it can be readily produced in large quantities at low cost. The objectives of this study are to screen and develop aptamers specific to oral pathogens such as Porphyromonas gingivalis, Treponema denticola, and Streptococcus mutans. The bacterial cell pellet was fixed with formaldehyde as a target molecule for the screening of aptamers. The SELEX method was used for the screening of aptamers and a modified western blot analysis was used to verify their specificities. Through SELEX, 40 kinds of aptamers were selected and the specificity of the aptamers to the bacterial cells was confirmed by modified western blot analysis. Through the SELEX method, 40 aptamers that specifically bind to oral pathogens were screened and isolated. The aptamers showed possibility as effective candidates for the detection agents of oral infections.

화학 발광 검사법을 이용한 구강 편평 태선의 진단 (CHEMILUMINESCENCE IN DIAGNOSIS OF ORAL LICHEN PLANUS)

  • Purpose: This study was conducted in order to evaluate how chemiluminescent lighting($ViziLite^{(R)}$) could increase the sharpness of margin and contrast to normal mucosa in the diagnosis of oral lichen planus(OLP), the most frequent oral premalignant lesion, compared with direct visual inspection under incandescent light. Methods: 41 consecutive patients, diagnosed to have OLP with visual inspection under incandescent light, were further examined with chemiluminescent light. The degrees of margin sharpness and lesion contrast were ranked on a scale from 1 to 3 for all patients under visual inspection and chemiluminescent light. The presence of additional lesion only detected by chemiluminescent light, complication, and discomfort were checked for each patient. After both screening tests, biopsy specimens were harvested from all patients with scalpels and histopathologic assessments were done. Results: All 41 patients were diagnosed to have OLP by both visual inspection and chemiluminescent light examination. This result was definitively diagnosed by histopathology. Degrees of margin sharpness and lesion contrast were increased by chemiluminescent light compared with visual inspection, but only the difference of lesion contrast was statistically significant. In 22.0% of patients, additional lesions were detected and 88.9% of them were diagnosed to have OLP histopathologically. 17.1% of patients noted discomfort and 9.8% of patients showed complications after chemiluminescent test. Conclusion: Chemiluminescent light may not be proper for the screen test of oral cancer or premalignant lesion but showed some possibility for additional diagnostic tool for definitively diagnosed patients in determination of lesion margin and scope.
