CHANGES IN HYOID BONE AND TONGUE POSITIONS, AND ORAL CAVITY VOLUME AFTER MANDIBULAR SETBACK BY SAGITTAL SPLIT RAMUS OSTEOTOMY

  • Purpose: This retrospective study evaluated the changes in hyoid bone and tongue positions as well as oral cavity volume after mandibular setback by BSSRO. Materials and Methods: 18 Koreans who underwent BSSRO to correct mandibular prognathism were studied. Lateral cephalograms were taken and traced preoperatively (T0), immediately (T1) and approximately 6 months postoperatively (T2). Submentovertex radiograghs were taken and traced before surgery (T0) and about 6 months after surgery (T2). The area and volume of oral cavity, the vertical and horizontal dimensions of the hyoid bone and tongue dorsum were measured. Results: Mandibular setback surgery resulted in a significant reduction of lower oral cavity volume. The hyoid bone displaced posteroinferiorly immediately after surgery, and it tended to return to its original vertical position at 6 month after mandibular setback by BSSRO. The retropalatal space around tongue was maintained and the retrolingual space around tongue was reduced immediately postoperatively. The readaptation of tongue was not evident for that the follow up period was not long enough. No significant statistical correlations between the amounts of mandibular setback and the changes of oral cavity volume were observed. Conclusion: Mandibular setback surgery resulted in a significant reduction of lower oral cavity volume, which was most likely attributable to the posterior movement of the mandible. More subjects and long-term observations should be performed to assess the changes of oropharyngeal configuration following mandibular setback surgery.

Reconstruction of cheek mucosal defect with a buccal fat pad flap in a squamous cell carcinoma patient: a case report and literature review

  • Background: Squamous cell carcinoma (SCC) is the most commonly occurring malignant tumor in the oral cavity. In South Korea, it occurs most frequently in the mandible, tongue, maxilla, buccal mucosa, other areas of the oral cavity, and lips. Radial forearm free flap (RFFF) is the most widely used reconstruction method for the buccal mucosal defect. The scar of the forearm donor, however, is highly visible and unsightly, and a secondary surgical site is needed when such technique is applied. For these reasons, buccal fat pad (BFP) flap has been commonly used for closing post-surgical excision sites since the recent decades because of its reliability, ease of harvest, and low complication rate. Case presentation: In the case reported herein, BFP flap was used to reconstruct a cheek mucosal defect after excision. The defect was completely covered by the BFP flap, without any complications. Conclusion: Discussed herein is the usefulness of BFP flap for the repair of the cheek mucosal defect. Also, further studies are needed to determine the possibility of using BFP flap when the defect is deep, and the maximum volume that can be harvested considering the changes in volume with age.

Comparison of changes in the nasal cavity, pharyngeal airway, and maxillary sinus volumes after expansion and maxillary protraction with two protocols: Rapid palatal expansion versus alternate rapid maxillary expansion and constriction

  • Objective: To evaluate and compare a series of volume changes in the nasal cavity (NC), nasopharynx, oropharynx, and maxillary sinuses (MS) in growing Class III patients after either rapid palatal expansion (RPE) or alternate rapid maxillary expansion and constriction (Alt-RAMEC) followed by facemask (FM) therapy, by using cone-beam computed tomography (CBCT). Methods: Forty growing Class III patients were retrospectively selected and divided into two matched groups: RPE/FM (14 females, 6 males; mean age, 9.66 ± 1.23 years) and Alt-RAMEC/FM groups (14 females, 6 males; mean age, 10.28 ± 1.45 years). The anteroposterior and vertical displacements of Point A, the volumes of the NC, nasopharyngeal, oropharyngeal, and MS were measured at different time points: pretreatment (T1), postexpansion (T2), and postprotraction (T3). Results: Both groups demonstrated significant maxilla advancement (by 1.3 mm) during expansion, with a statistically significant intergroup difference during protraction (RPE/FM, 1.1 mm; Alt-RAMEC/FM, 2.4 mm; p < 0.05) and throughout the treatment (RPE/FM, 2.4 mm; Alt-RAMEC/FM, 3.7 mm; p < 0.05). NC and nasopharyngeal airway volumes increased significantly in both groups after expansion, protraction, and treatment. The oropharyngeal and MS volumes increased in both groups after protraction and post-treatment. However, no volumetric differences were observed between the two groups. Conclusions: There was no significant difference in airway volume changes, including NC, nasopharyngeal, oropharyngeal airway, and MS, between RPE/FM and Alt-RAMEC/FM groups at different time points. Although there was significantly more forward movement after protraction in the Alt-RAMEC/FM group, the difference was deemed too small to be clinically relevant.

  • Purpose : This study was aimed at measuring the changes in the hyoid bone position, tongue position, and pharyngeal airway space in subjects with mandibular setback osteotomies. Methods : Twenty patients were evaluated retrospectively for their changes in pharyngeal airway space, tongue and hyoid bone positions. All patients underwent surgical mandibular setback using bilateral sagittal split osteotomies. The cephalometric analysis was performed preoperatively, and 1 week, 3-6 months, and 1 year postoperatively. Result : The hyoid bone moved inferiorly and posteriorly immediately after surgery, and it returned to the preoperative position during follow-up period. The nasopharyngeal airway space was not significantly changed after surgery. A considerable decrease in the oropharyngeal and hypopharyngeal airway spaces following mandibular setback surgery was found. The upper and lower tongue was posteriorly repositioned immediately after surgery. During follow-up period, the hypopharyngeal airway space and lower tongue posture returned to the preoperative positions, but the oropharyngeal airway space and upper tongue posture were not significantly changed. The position of pogonion remarkably changed to backward immediately after surgery, but slightly anterior advancement was found during follow-up period. Conclusion : Immediately after mandibular setback surgery, the oropharyngeal and hypopharyngeal airway spaces obviously decreased due to posterior and inferior repositions of the tongue and hyoid bone. During follow-up period, lower tongue and hyoid bone returned to the preoperative positions, it was related to advancement of the pogonion in this period. The narrowing of the oropharyngeal airway space and posterior movement of the upper tongue posture were relatively permanent after mandibular setback surgery. We suspected this phenomenon had an influence on maintaining the total volume of oral cavity against mandibular setback.

Does hyrax expansion therapy affect maxillary sinus volume? A cone-beam computed tomography report

  • Purpose : The aim of this study was to investigate the initial effects of maxillary expansion therapy with Hyrax appliance and to evaluate the related changes in maxillary sinus volume. Materials and Methods : Thirty patients (20 females, 10 males; 13.8 years) requiring maxillary expansion therapy, as part of their comprehensive orthodontic treatment, were examined. Each patient had cone-beam computed tomography (CBCT) images taken before (T1) and after (T2) maxillary expansion therapy with a banded Hyrax appliance. Multiplanar slices were used to measure linear dimensions and palatal vault angle. Volumetric analysis was used to measure maxillary sinus volumes. Student t tests were used to compare the pre- and post-treatment measurements. Additionally, differences between two age groups were compared with Mann-Whitney U test. The level of significance was set at p=0.05. Results : Comparison of pre-treatment to post-treatment variables revealed significant changes in the transverse dimension related to both maxillary skeletal and dental structures and palatal vault angle, resulting in a widened palatal vault (p<0.05). Hard palate showed no significant movement in the vertical and anteroposterior planes. Nasal cavity width increased on a mean value of 0.93mm(SD=0.23, p<0.05). Maxillary sinus volume remained virtually stable. No significant age differences were observed in the sample. Conclusion : Hyrax expansion therapy did not have a significant impact on maxillary sinus volume.

골격성 3급 부정교합 환자의 악교정 수술 후 설골의 위치와 기도변화에 관한 연구 (An Investigation of Hyoid Bone Position and Airway Space in Class III Malocclusion after Orthognathic Surgery)

  • Purpose: The purpose of this study was to investigate changes in the position of the hyoid bone and soft palate and the amount of airway space after bilateral sagittal split ramus osteotomy (B-SSRO). Methods: This study is a review of lateral cephalometric tracings of 30 patients who underwent B-SSRO with setbacks at Kyunghee Dental Hospital from 2005 to 2009. Lateral cephalograms were taken before (T0), within one month (T1), and more than six months after the surgery (T2). Results: The hyoid bone at T1 changed significantly towards the inferoposterior position. At T2, it had significantly moved superiorly, but not anteriorly. At T1, the nasopharyngeal space, extending from the posterior nasal spine to the posterior pharyngeal space, decreased significantly, but did not show a significant increase at T2. The nasopharyngeal space, extending from the middle of soft palate to the posterior pharyngeal space, decreased significantly at T1, but did not show a significant decrease at T2. The oropharyngeal airway space decreased significantly at T1 and did not return to its original position at T2. The hypopharyngeal space, extending from the anterior to the posterior pharyngeal space at the level of the most anterior point of the third cervical vertebrae, slightly decreased at T1, but the amount was insignificant; however, the amount of decrease at T2 was significant. The hypopharyngeal space extending from the anterior to the posterior pharyngeal space at the level of the lowest point of the third cervical vertebrae, decreased significantly at T1 but returned to its original position at T2. Conclusion: B-SSRO changes the position of the hyoid bone and muscles inferoposteriorly. These change allows enough space for the tongue and prevent airway obstruction. Airway changes may be related to post-operative edema, posterior movement of the soft palate, anteroposterior movement of the hyoid bone, or compensation for decreased oral cavity volume. The position of the pogonion which measures anterior relapse after surgery did not show significant differences during the follow-up period.

Validation of Age Estimation Methods Using Pulpal Volume Changes in Radiographs for Korean Adults

  • Purpose: It aims to verify the applicability of existing age estimation methods derived from data of foreign population groups to Korean population groups. Moreover it is to suggest a new way applicable to practical age estimation on the basis of newly calculated regression formulae from data of Korean population groups and develop a subsidiarily applicable method to the existing method. Methods: Ratio of pulp cavity to dental crown was calculated by measuring the height and width of dental crowns and pulp cavities at the cervical line from 4,034 first and second upper molars, first and second upper premolars, first and second lower molars and first and second lower premolars on both left and right sides of 400 patients who had been treated in Dental Hospital of Yonsei University College of Dentistry, and regression equations were derived from the values of the ratio. Results: The equation with correlation coefficients the highest among females was as follows: age=$107.96-75.684{\times}{\sharp}17$ TCHI-$53.741{\times}{\sharp}26$ TCVI-$40.664{\times}{\sharp}45$ TCVI-$56.307{\times}{\sharp}46$ TCVI. Randomized anohter Korean female subjects (n=20) are applicated to the new equation. Mean of error of estimate is 10.322 years, standard deviation is 12.852 years. Minimum of error of estimate is 1.018 years, maximum is 21.365 years Conclusions: The error range of age estimation was found to be slightly wider when the existing regression formulae of Drusini were applied to Korean population groups. Also age estimation in females using the ratio of pulp cavity to dental crown measured with the length and width of dental crowns and pulp cavities from maxillomandibular molars was observed to have the highest reliability in the research. However, we consider that advanced equations of regression are needed to apply to both molars and premolars of males and females in the future.

구강암 환자 치료시 치과용 기초상 왁스(Paraffin Wax)의 유용성 평가 (Radiotherapic Valuation of Paraffin Wax for Patients with Oral Cancer)

  • 목 적: 구강암 환자의 방사선 치료시 기존 강내 충전물인 Mouth piece, Putty 인상제와 자체 제작한 Paraffin Wax를 비교하여 그 유용성을 알아보고자 한다. 대상 및 방법: 재료별 충전물을 $10{\times}10{\times}1cm$의 크기로 제작하여 물(물 등가 팬텀)을 기준으로 선량 감쇠율을 측정하였으며, 구강암 환자 중 충전물별 각 2명씩을 대상으로 Aria system의 offline review 프로그램으로 DRR (Digitally Reconstructed Radiograph)과 각 5회 촬영한 Portal vision을 비교하여 고정기구로서의 재현성을 확인하였다. 원통형 팬텀(phantom)을 제작하여 치료 계획시 불가피 하게 충전물이 조사야 내에 포함되는 경우의 흡수선량 변화에 따른 MU (monitor unit) 차이를 비교하였다. 결 과: 선량감쇠율은 Mouth piece (Air) 6 MV +3.5%, 10 MV -0.7%, Paraffin 6 MV +0.34%, 10 MV +0.31%, Putty 6 MV -2.31%, 10 MV -1.76%로 측정되었다. 자세 재현성은 Mouth piece ${\pm}3mm$, Paraffin ${\pm}2mm$, Putty ${\pm}2mm$의 오차를 나타내었다. $5{\times}4{\times}2.5cm$ 크기의 충전물이 조사야 내에 포함되는 경우의 흡수선량 변화에 따른 선속의 MU 차이는 6 MV 계산시 Water 232 MU를 기준으로 백분율 환산시 Paraffin -0.9% (230 MU), Putty +7.8% (250 MU)의 증감을 나타내었다. 결 론: 기존 Mouth piece의 경우 상악과 하악을 이격시킴에는 문제가 없으나 구강내 구조물을 반영하지 못해 재현성을 확신하지 못하고 공동과 조직 사이의 계면선량이 감소하는 단점이 있었다. 그에 반해 Putty 인상제는 자세재현성이나 구강을 충전함에 있어 우수함을 알 수 있으나, 물에 비해 두께 1 cm당 약 -2.3%의 선량감쇠가 발생해 선량계획에 있어 불리하고 정상조직에 불필요한 과선량이 조사되었다. Paraffin의 경우 지방과 유사한 조직등가물질로 선량감쇠가 적고, 구강내 구조물을 반영한 맞춤형 제작이 가능해 재현성이 우수하며 강내 공동을 충분히 충전시켜 병변에 목적한 선량을 전달하는데 가장 이상적인 재료라 사료된다.

Er:YAG 레이저를 이용한 법랑질 및 상아질 와동 형성시의 치수내 온도변화 (Intrapulpal Temperature Change during Cavity Preparation on the Enamel and Dentin with an Er:YAG Laser)

  • 본 연구의 목적은 법랑질과 상아질에서 와동을 형성하는 동안 치수벽에서 발생하는 온도변화를 관찰하여 치수 손상 가능성을 평가하고자 하였고 조사반복율의 차이에 따른 영향도 함께 조사하고자 하였다. 발거된 건전 대구치에 access cavity를 형성하고 레진 block에 식립한 다음, 온도측정센서를 레이저가 조사될 치수벽에 위치시켰다. 레이저를 조사하는 동안 분사되는 물이 온도변화에 영향을 주지 않도록 하기 위해 레진을 이용하여 occlusal cap을 만들고 가운데 작은 구멍을 내어 온도측정센서가 들어갈 수 있도록 하였으며 치아의 내부는 생리식염수로 채웠다. 치아표본은 법랑질실험군과 상아질 실험군으로 나누고 각 군당 치아표본수는 5개로 하였다. 상아질군에서는 모든 표본의 상아질 두께를 일정하게 하기 위하여, 고속핸드피스를 이용하여 상아질 두께가 2 mm가 되도록 삭제하여 사용하였다. $3\;mm{\times}2\;mm$의 일정한 면적을 치면에 표시한 다음 와동을 형성하였는데, 법랑질군에서는 상아질 이 노출되는 순간까지, 상아질군에서는 상아질벽이 천공되는 순간까지 온도변화를 조사하였다. 조사방식은 접촉식(contact mode)으로 하였으며 레이저가 한번 조사되는 시간을 3초 이하로 유지하면서 와동을 형성하였다. 300 mJ의 펄스 에너지, 10, 15, 20 Hz의 조사반복율, 1.6 ml/min의 물분사량이 가해지는 조건에서 레이저 조사측의 온도변화를 측정하였다. 본 연구의 결과에 따르면, Er:YAG 레이저 조사시 적절한 양의 물분사가 이루어질 때, 즉 1.6 ml/min의 물분사량과 300 mJ의 펄스에너지의 조건에서 법랑질에서 와동형성시 10, 15, 20 Hz 의 조사반복율 모두에서 온도상승이 미미하여 치수손상을 야기할 만한 온도상승이 이루어 지지 않았고 세 군 간의 유의한 차이도 존재하지 않았다(p=0.358). 상아질에서의 와동형성시에는 조사반복율이 증가할수록 온도상승이 컸지만(p=0.001), 실제 온도상승은 여전히 치수손상을 야기 하지 않는 안전한 범위에 있었다. 결론적으로 적절한 양의 물이 레이저 조사면에 적절하게 분사되기만 하면 법랑질이나 상아질에 서의 와동형성은 안전하게 시행할 수 있을 것으로 생각된다.

구강암 세포주를 이종 이식한 누드마우스에서 apigenin의 경구투여에 따른 항암효능에 관한 연구 (Studies on the Anticancer Effect of Apigenin in KB Cell Xenograft Nude Mouse Model)

  • Apigenin은 과일과 야채에 들어있는 플라보노이드로 다양한 악성 세포에 항증식효과를 보여준다. 세포성장 저해효과를 확인하기 위하여 KB 구강암세포주를 96 well plate에 $6{\times}10^3$ cells/well로 분주하고 24시간 후에 apigenin을 24시간 동안 처치하여 MTT assay를 수행하였다. Apigenin은 배양 후 용량 의존적으로 세포사를 유도하였다. Apigenin 100 ${\mu}M$을 24시간 동안 처치하고 대조군과 세포성장을 비교하였을 때 유의적인 감소를 확인하였다. KB 구강암세포주에서의 apoptosis를 확인하기 위해 DAPI 염색을 수행하였다. Apigenin을 처치한 세포에서 핵의 응축이 존재함을 형광현미경으로 확인하였다. 우리는 누드마우스에 KB 구강암세포주를 이식하여 세포 성장 억제 효과를 알아보았다. Apigenin을 마우스에 25, 50 mg/kg을 0.2 ml의 PBS에 녹여 경구투여 하였다. 종양 사이즈는 대조군과 25, 50 mg/kg apigenin 투여군을 비교하였을 때 유의적으로 감소하였다. Apoptosis 분석을 위해 TUNEL염색을 수행하였다. 25 mg/kg apigenin 투여군과 대조군을 비교하였을 때 apoptosis의 유의적인 증가를 확인하였다. 육안적 소견을 위한 H&E 염색은 이상이 없었다. 본 연구는 apigenin이 구강암세포주 성장 억제를 apoptosis의 유도를 통하여 확인하였다.
