A Study on the Mortality of Insureds (생명보험(生命保險) 가입자(加入者)의 사망(死亡)에 관한 고찰(考察))

  • We've reviewed 518 cases of claims by death that had occured for one year from April, 1983 to March, 1984. As a result, we came to following conclusions; First, the greater part of the death were due to Accidental death(19.1%), Heart disease(18.9%), Malignant neoplasm(17.4%), Liver cirrhosis(10.6%), and the Cerebrovascular disease(9.7%), which were occupied by 75.7% of the whole. Second, classifying them by medical examinations or non medical, death in case of non medical examination showed 89% of the whole. And for age, section ranging from 41 to 50, from 51 to 60 and from 31 to 40 took the overwhelming portion by 27.4%, 26.6%, and 17.4% respectively, those of which showed 71.4% of the whole. Third, for the period elapsed, death within 1 year from the entrance showed 31% and that from 1 year to under 2 years 20.5%. Thus the rate of early death under 2 years stood for 51.5% of the whole.

  • We've reviewed 704 cases of claims by death that had occured for one year from April, 1985 to March, 1986. As a result, we came to following conclusions; First, the greater part of the death were due to Accidental death(25.0%), Malignant neoplasm(21.2%), Heart disease(12.9%), Liver cirrhosis(9.1%), and the cerebrovascular disease(5.5%), which were occupied by 73.7% of the whole. Second, classifying them by medical examinations or non medical, death in case of non medical examinations showed 94.3% of whole. And for age, section ranging from 40 to 49, from 50 to 59, and from 60 to 69 took the overwhelming portion by 28.6%, 24.2%, and 16.6% respectively, those of wich showed 69.4% of the whole. Third, for the period elapsed, death within 1 year from the entrance showed 16.9% and that from 1 year to under 2 years 18.8%, Thus the rate of early death under 2 years stood for 35.7% of the whole.

  • Total anomalous pulmonary venous connection is relatively rare cyanotic congenital heart diaease, which represents 1-4% of all congenital cardiac defects. Generally in the majority cases, severe heart failure and cyanosis develops in the early infancy. Because of high mortality in the untreated infants and surgical risk, there are still many things to be improved. Two patients with total anomalous pulmonary venous connection are presented, which we recently experienced. The one was 10 year old female with supracardiac type drained through left innominate vein, and survived the operation and continuous to do well for 1 year. The other 5 year old female with mixed type (right pulmonary vein drained via coronary sinus and left pulmonary vein through left innominate vein) was operated successfully under hypothermia and extracorporeal circulation, and followed up for 6 months without problem. It was very rare case in the literature.

Long-term clinical results of the xenograft cardiac valves (이종조직판막의 장기임상성적)

  • A total of 1,239 patients had cardiac valve replacement using 1,514 substitute valves at Seoul National University Hospital from 1968 to 1986. Of the total substitute vales, 84.9% were the glutaraldehyde-treated xenograft valves. Six hundred ninety-four patients who had 820 bioprosthetic tissue valves were studied for their clinical characteristics. They were a total and consecutive cases to the end of the study. Four hundred sixty-four patients had the lonescu-Shiley pericardial valves: MVR 291, AVR 66 and MVR+AVR 107; 163 had the Hancock porcine valves; 46 had the Angell-Shiley porcine valves; and 21 had the Carpentier-Edwards porcine valves. Five hundred forty patients underwent single valve replacement: MVR 460, AVR 76 and TVR 4; 154 had multiple valve replacement: MVR+AVR 141, MVR+TVR 12 and one triple valve replacement. Additional surgery was necessary in 22.3% of the cases. Operative mortality rate within 30 days of surgery was 6.77% for the total patients: 5.2% and 4.2% with MVR, 13.6% and 12.5% with AVR, and 7.5% and 7.4% with MVR+AVR using the lonescu and the Hancock valves respectively. A linealized annual late mortality rate was 2.56%/patient-year. Six hundred forty-three operative survivors were followed up for a total of 1482.7 patient-years [a mean 27.7 months], and the follow-up rate was 67.7%. The Idealized complication rates were: 2.02% emboli/patient-year, 0.94% bleeding/patient-year, 1.21% endocarditis/patient-year, and 3.84% overall valve failure/patient-year. A linealized rate of primary tissue failure was 0.87%/patient-year. Actuarial survival rates including the operative mortality were: 87.8*2.6%, 82.3*4.9% and 82.2*4.7% with MVR, AVR and MVR+AVR using the lonescu valves at 4 years after surgery respectively; and they were 88.0*4.1% with MVR at 8 years, 82.3*4.9% with AVR at 4 years and 84.9*7.0% with MVR+AVR at 6 years after surgery using the Hancock valves respectively. Probabilities of freedom from thromboembolism were 89.8*6.3% with MVR using the lonescu valves at postoperative 5 years and 89.2*3.8% with MVR using the Hancock valves at postoperative 7 years, and 93.3*3.9% with AVR using the lonescu valves at postoperative 5 years. None had embolic complication after AVR using the Hancock valves. Probabilities of freedom from valve failure [according to the Stanford criteria] were 81.0*7.1% with MVR using the lonescu valves at postoperative 4 years and 57.4*12.5% with MVR using the Hancock valves at postoperative 9 years. These clinical results prove the excellent antithrombogenicity of the glutaraldehyde-treated xenograft substitute valves and confirm the previously speculated rate of tissue failure. At the present situation, it may be concluded that there is a room for the further development of more durable bioprosthetic valves.

Clinical Analysis of Bjork-Shiley Mechanical Valve Replacement (Bjork-Shiley 기계판막의 임상적 연구)

  • Between Dec. 1984, and May, 1988,96 prostheses were implanted in 80 patients at Dept. of Thoracic k Cardiovascular Surgery of National Medical Center. 43 patients had mitral valve replacement, 21 underwent aortic valve replacement, and 15 had double valve replacement [Mitral k Aortic], and 1 had tricuspid valve replacement. Seventy-one cases [88.8 %] were in NYHA Class III or IV. The mean duration of follow up was 22.1 months and follow-up information was available for 74 [92.5 %] of the patients. The overall actuarial survival rate at 45 months was 93.05 % and overall hospital mortality was 10 %, late Mortality was 5 %. The linearlized incidence of thromboembolism [2.4%/pt-yr], thrombotic valve obstruction [1.6 %/pt-yr], anticoagulant related bleeding [0.8 %/pt-yr]. There were no fatal valve related complications. The blood was studied in 40 patients 1 year after valve operation. Hgb and reticulocyte count were within normal values and Serum LDH value was slightly elevated but it was not of clinical significance. In conclusion, Monostrut Bjork-Shiley valve prosthesis to be a reliable valve substitute with an acceptable incidence of complications.

  • Extracranial carotid artery disease is rare and closely related to cardiovascular morbidity and mortality.We experienced one case of surgical treatment of left common carotid artery stenosis using internal shunt.The patient was 54-year-old female presented as headache and dizziness for about 2 years.On Doppler ultrasound, an atheromatous plaque was noted along the posteromedial wall of left common carotid artery resulting in luminal narrowing in about 75s of cross section area.The atheroma was resected through carotid endarterectomy, measured about 5.2cm in length. Postoperative course was uneventful and she was discharged in good condition.

Spontaneous Rupture of the Esophagus (Boerhaave's Syndrome) -A case report- (식도 자연 천공 치험 1례)

  • Spontaneous esophageal perforation occurred rarely but often lead to a high mortality and morbidity. We had experienced one case of spontaneous rupture of esophagus. A 52 - year old male patient was admitted to our hospital because of the chest pain and massive hematemesis after emetic strain. Esophagogram that taken at private clinic revealed leakage of dye into the right pleural space. Under the diagnosis of the thoracic esophageal perforation, two stage operation was planned because the size of perforation was large and pyothorax was developed on the right side. At first, exclusion and diversion of the esophagus were carried out. After six months, the esophageal reconstruction with left colon was performed.

Analysis of Changes in Hemodynamic Values after Modified Fontan Procedure (변형 Fontan 술후 혈류역학치 변화추이 분석)

  • Since 1978, We have experienced 87 cases of Fontan operations and the candidates of that increased in numbers recently with the improvement of the diagnostic and operative technique. We studied the prerequisite factors and hemodynamics of 22 cases of Fontan operations, done during the last one year period, which were 3 tricuspid atresia, 16 functional single ventricle and 3 anatomic single ventricle. The mean age was 68 months and the mortality rate 24%, and 9 patients of under 4 years of age were operated with 22.2% mortality rate, but the youngest, 16 months of age, patient survived well without problems. The preoperative pulmonary artery pressure[PAP], pulmonary vascular resistance[PVR] and postoperative right atrial pressure[RAP], left atrial pressure[LAP] value influenced the mortality, but age, preoperative Hb, preoperative PaO2 and pulmonary artery index[PAI] did not. There were favorable survival tendency in under 15mmHg of preop. PAP, 2a of preop. PVR and under 25cmHyO of postop. RAP, under 15cmHyO of postop LAP. The younger, the more pleural effusion and the longer postoperative admission days. The higher preop. Hb related to the higher postop. transpulmonary pressure gradient and the lower preop. PaO2 and PAI. The higher preop. PaO2, the less pleural effusion and postop. admission days. Preop. PAP closely related to preop. PVR and postop. LAP and high PVR increased the pleural effusion and postop. admission days. The larger PAI, the larger CI. We concluded that there were so many factors influencing the postoperative condition, but preop. PAP, PVR, Hb, postop. RAP and LAP were the most ones.

Repair of Total Anomalous Pulmonary Venous Return in Infant (1세이하의 총폐정맥 환류이상증의 완전교정)

  • Background: Total anomalous pulmonary venous return is a relatively rare disease which has a very high mortality(80% within a year) if not properly corrected surgically. Material and Method: Twenty-six infants with total anomalous pulmonary venous return underwent repair between May, 1991 and February, 1996. Result: There were 19 boys and 7 girls. The mean age at operation was 2.6 months(range: 5 day to 11 month) and the mean body weight was 4.3kg(range:2.8 to 6.7 kg). Preoperative stabilization included ventilator for 5 patients and inotropic support for 6 patients. There were 6 hospital mortalities. Significant risk factors of operative mortality were preoperative ventilator care(p<0.03) and preoperative inotropic support(p<0.05). Age, body weight at operation, pulmonary venous obstruction, high pulmonary arterial pressure, spurasystemic right ventricular pressure or emergency operation did not affected the operative outcome. Postperative pulmonary venous obstruction occurred in three patients 2 or 3 months later, among them one patient was reoperated. The actuarial survival was 76% at 40 months. Conclusion: Although early mortality was high, repair of total anomalous pulmonary venous return should be attempted in early life, but the patients receiving ventilator care or inotropic support need special attention.

Compositional and Contextual Factors Related to Area Differentials in Suicide (지역의 자살률 차이와 관련된 구성적 요인과 상황적 요인)

  • Objectives: Rural-urban differences in suicide have been observed in many settings. However, there has been little research addressing what factors can explain these differences. The purpose of this study was to analyze which compositional factors and contextual factors in local areas might be related to local suicide. Methods: The study design was cross-sectional. The data for 251 primary local governments on their age-standardized suicide mortality and their predefined indicators of compositional factors and contextual factors were obtained from Korean Statistical Information Service as of year 2010. Bivariate analysis including one-way ANOVA and chi-square test were used to identify the differences in local features by area type. Seven poisson regression models for each of total, males, and females were used to analyze which compositional and contextual factors were related to suicide. Results: There were differences in suicide between gu and goon in total, male, and female groups. For total, compositional factors including divorce and smoking rate, and contextual factors including financial independency, water and waterwaste coverage, and number of wastewater discharge factories were found to explain the urban-rural differences. Conclusions: This study provided some evidence that contextual factors at the local level as well as compositional factors are useful for predicting local suicide mortality.
