Dynamic Motions of Model Fish Cage Systems under the Conditions of Waves and Current (파랑 및 흐름중 모형 가두리 시설의 운동 특성)

  • In order to analyze the dynamic motions of fish cage systems made of a frame and a netting under the conditions of waves and current, the hydraulic model experiment at towing tank and the numerical computation using boundary integral element method based on linear potential theory were carried out on a square and a circular type of fish cage, The computed and measured results for the dynamic motions of model fish cage systems showed that the heave and pitch motions were almost unaffected by the inclusion of nets, while the surge motions were very reduced by drag force acting on them. In addition, irregular wave-induced motions of fish cages included non-negligible 2nd order harmonic components at high frequency nearly twice the wave frequency. The reason why these motions were considered was due to resonance or structural components of frames being overflown and out of water during a wave cycle. It was found that circular type was more desirable structure in the open sea than square one only in the respect of dynamic motions due to waves and current. Further verifications were needed considering hydrodynamic forces, fatigue life, and structure analysis based on long term stochastic waves including frequency and time domain for the purpose of analyzing and designing fish cage systems.

Clinical and genetic characteristics of Korean patients with IARS2-related disorders

  • Purpose: Genetic defects in the nuclear-encoded mitochondrial aminoacyl-tRNA synthetases were first identified as causes of various disorders in 2007. Variants in IARS2, which encodes a mitochondrial isoleucyl-tRNA synthetase, were first reported in 2014. These variants are associated with diverse phenotypes ranging from CAGSSS (CAtaracts, Growth hormone deficiency, Sensory neuropathy, Sensorineural hearing loss, and Skeletal dysplasia) and Leigh syndrome to isolated nonsyndromic cataracts. Here, we describe the phenotypic and genetic spectrum of Korean patients with IARS2-related disorders. Materials and Methods: Using whole-exome sequencing followed by Sanger sequencing, we identified five patients with IARS2 mutations. Their medical records and brain magnetic resonance images were reviewed retrospectively. Results: All five patients presented with developmental delay or regression before 18 months of age. Three patients had bilateral cataracts, but none had hearing loss or sensory neuropathy. No evidence of skeletal dysplasia was noted, but two had short stature. One patient had cardiomyopathy and another exhibited renal tubulopathy and hypoparathyroidism. Their brain imaging findings were consistent with Leigh syndrome. Interestingly, we found the recurrent mutations p.R817H and p.V105Dfs*7 in IARS2. Conclusion: To our knowledge, this is the first report of Korean patients with IARS2-related disorders. Our findings broaden the phenotypic and genotypic spectrum of IARS2-related disorders in Korea and will help to increase clinical awareness of IARS2-related neurodegenerative diseases.

Working Memory Deficits in Patients with Schizophrenia:fMRI Investigation (정신분열병 환자의 작동기억 이상에 대한 기능적 자기공명영상 연구)

  • Objective:Impaired processing of working memory is one of the cognitive deficits seen in patients with schizophrenia. This aimed at corroborating the differences in the brain activities involved in the process of working memory between patients with schizophrenia and the control subjects. Method:Fourteen patients with schizophrenia and 12 healthy volunteers were recruited in this study. Functional magnetic resonance imaging(fMRI) was used to assess cortical activities during the performance of a 2-back visual working memory paradigm using the Korean alphabet as mnemonic content. Results:Group analysis revealed that left lateral prefrontal cortex and right parietal lobule showed decreased cortical activities in the patient group. On the other hand, an increased activation in left superior and middle frontal gyrus, left middle temporal gyrus, right cuneus, both occipital lobes, right fusiform gyrus and right cingulate gyrus. The activation in left anterior lobe and both declive of cerebellum was also increased. Conclusions:This study showed a decreased activation in left lateral prefrontal and right parietal neural networks from the patient group and confirmed the earlier findings on the impaired working memory of patients with schizophrenia using fMRI investigation. The regions implicated in our study suggest an abnormal functioning of the fronto-parietal cortical areas that are critical to the information processing stream, which might be correspondent to common pathophysiology rather than a common etiology in schizophrenia.

Resection and Observation for Brain Metastasis without Prompt Postoperative Radiation Therapy

  • Objective : Total resection without consecutive postoperative whole brain radiation therapy is indicated for patients with a single or two sites of brain metastasis, with close follow-up by serial magnetic resonance imaging (MRI). In this study, we explored the effectiveness, usefulness, and safety of this follow-up regimen. Methods : From January 2006 to December 2015, a total of 109 patients (76 males, 33 females) underwent tumor resection as the first treatment for brain metastases (97 patients with single metastases, 12 with two metastases). The mean age was 59.8 years (range 27-80). The location of the 121 tumors in the 109 patients was supratentorial (n=98) and in the cerebellum (n=23). The origin of the primary cancers was lung (n=45), breast (n=17), gastrointestinal tract (n=18), hepatobiliary system (n=8), kidney (n=7), others (n=11), and unknown origin (n=3). The 121 tumors were totally resected. Follow-up involved regular clinical and MRI assessments. Recurrence-free survival (RFS) and overall survival (OS) after tumor resection were analyzed by Kaplan-Meier methods based on clinical prognostic factors. Results : During the follow-up, MRI scans were done for 85 patients (78%) with 97 tumors. Fifty-six of the 97 tumors showed no recurrence without adjuvant local treatment, representing a numerical tumor recurrence-free rate of 57.7%. Mean and median RFS was 13.6 and 5.3 months, respectively. Kaplan-Meier analysis revealed the cerebellar location of the tumor as the only statistically significant prognostic factor related to RFS (p=0.020). Mean and median OS was 15.2 and 8.1 months, respectively. There were no significant prognostic factors related to OS. The survival rate at one year was 8.2% (9 of 109). Conclusion : With close and regular clinical and image follow-up, initial postoperative observation without prompt postoperative radiation therapy can be applied in patients of brain metastasi(e)s when both the tumor(s) are completely resected.

Synthesis and Characterization of a Di-$\mu$-oxo-bridged Molybdeum(V) Complexes (두 개 산소가교형 몰리브덴(V) 착물의 합성과 그 성질에 관한 연구)

  • The Mo(V) $di-\mu-oxo$ type $(Mo_2O_4(H_2O)_2L)$ complexes $(L:\;C_3H_7CH(SCH_2COOH)_2,\;C_6H_5CH(SCH_2COOH)_2,\;CH_3OC_6H_4CH(SCH_2COOH)_2,\;C_5H_{10}C(SCH_2COOH)_2,\;C_3H_7C(CH_3)(SCH_2COOH)_2,\;C_3H_7CH(SCH_2CH_2COOH)_2,\;C_6H_5CH(SCH_2CH_2COOH)_2)$ have been prepared by the reaction of $[Mo_2O_4(H_2O)_6]^{2+}$ with a series of dithiodicarboxy ligands. These complexes are completed by two terminal oxygens arranged trans to one another and each ligand forms a chelate type between two molybdenum. In $Mo_2O_4(H_2O)_2L$, two $H_2O$ coordinated at trans site of terminal oxygens. The prepared complexes have been characterized by elemental analysis, infrared spectra, electronic spectra, and nuclear magnetic resonance spectra. In the potential range -0.00 V to -1.00 V at a scan rate of 20 $mVs^{-1}$, a cathodic peak at -0.50∼-0.58 V (vs. SCE) and an anodic peak at -0.40∼-0.43 V (vs. SCE) have been observed in aquous solution. The ratio of the cathodic to anodic current ($I_{pc}/I_{pa}$) is almost 1, we infer that redox is reversible reaction.

Comparative Study between MRI and pain, duration, physical examination in 20 patients with knee joint pain (20례 슬통환자의 통증, 이환기간, 물리적 검사법과 MRI소견과의 비교 고찰)

  • Background and Purpose : MRI(Magnetic Resonance Imaging) has become one of the most sensitive diagnostic tool no evaluate problems of the knee, because it enables us to identify not only osseous tissue but also soft tissues including muscle, fascia, tendon, ligament, meniscus and fat around the knee joint. Objective : To compare between MRI reading and pain, duration, physical examination in patients with pain of knee joint. Methods : 20 patients with pain of knee joint included in this study. This study researched pain-degree, duration, physical examination with pain of knee joint. Using MRI(Horizon Lx 1.0T-GE), we obtained the results. Results : 1. In the distribution of sex; Female rate was 80.0% and male rate was 20.0%. In the distribution of age, above sixty group was the largest group by each 55.0%. 2. In the distribution of MRI reading; 'Meniscus horm tear' was 70.0%, 'Osteoarthritis' was 55.5%, 'ACL partial tear' was 15.0%, 'Bone bruise at tibia, femur' was 15.0%. 3. Correlation between duration of pain and MRI reading showed that acute and subacute stage related various MRI reading, that chronic stage related 'Osteoarthritis' and 'Meniscus horn tear' by each 69.2%, 92.3%. 4. Correlation between pain and MRI reading showed that severe pain(GVI) related 'ACL partial tear, PCL partial tear' by each 100%, that mild pain(GII) related 'Osteoarthritis, Meniscus horn tear' by each 70.0%, 80.0%. 5. Correlation between physical examination and MRI reading showed that Drawer test related ACL partial tear by each 60.0%, that McMurray test related 'Meniscus horn tear' by each 75.0%. 6. Correlation between effect of treatment and MRI reading showed that 'Osteoarthritis, Meniscus horn tear' related good effect, that ACL partial tear related poor effect. Conclusions : These results suggest that acute and sever pain relate 'ACL partial tear, PCL partial tear', that chronic and mild pain related 'Osteoarthritis, Meniscus horn tear'. So it is responsibility to use MRI as a sensitive diagnostic tool in the knee problems.

  • Hippocampal sclerosis (HS) is one of the most common features of intractable temporal lobe epilepsy. Generally it can be identified through brain magnetic resonance imaging (MRI) with high degree of sensitivity and specificity. Typical brain MRI findings of HS are hippocampal atrophy with hyperintense signal confined to the lesion. On the other hand cortical dysplasia exhibits blurring of the gray-white matter junction and abnormal white matter signal intensity. We present a case where preoperative brain MRI strongly suggested the presence of diffuse cortical dysplasia in the left temporal lobe but postoperative pathology revealed the temporal lesion to be unremarkable except for hippocampal sclerosis.

  • Nuclear Medicine Images have comparatively poor spatial resolution, making it difficult to relate the functional information which they contain to precise anatomical structures. Anatomical structures useful in the interpretation of SPECT /PET Images were radiolabelled. PET/SPECT Images Provide functional information, whereas MRI mainly demonstrate morphology and anatomical. Fusion or Image Registration improves the information obtained by correlating images from various modalities. Brain Scan were studied on one or more occations using MRI and SPECT. The data were aligned using a point pair methods and surface matching. SPECT and MR Images was tested using a three dimensional water fillable Hoffman Brain Phantom with small marker and PET and MR Image was tested using a patient data. Registration of SPECT and MR Images is feasible and allows more accurate anatomic assessment of sites of abnormal uptake in radiolabeled studies. Point based registration was accurate and easily implemented three dimensional registration of multimodality data set for fusion of clinical anatomic and functional imaging modalities. Accuracy of a surface matching algorithm and homologous feature pair matching for three dimensional image registration of Single Photon Emission Computed Tomography Emission Computed Tomography (SPECT), Positron Emission Tomography (PET) and Magnetic Resonance Images(MRD was tested using a three dimensional water fill able brain phantom and Patients data. Transformation parameter for translation and scaling were determined by homologous feature point pair to match each SPECT and PET scan with MR images.

Comparison of Differences in Subcortical between Men and Women in their Seventies (70대의 성별에 따른 피질하부 차이 비교)

  • Magnetic resonance imaging(MRI) has become an important technique for examining changes in human brain structure with neurological disorders. Brain development is a very complex process, and is affected by neurogenesiss and genetic programs. As age increases, structures of the brain change, which can contribute to the formation of brain diseases. Among the various factors, Gender is one of the greatest influential factors that affect the development of a healthy brain. The images were analyzed through various programs found in FSL such as SIENAX, FIRST, and Vertex analysis. Our results show that significant gender-related differences in subcortical areas were observed at the particular age group. The magnitude of these differences between gender and volume varied depending on the area investigated. In this study, we used more advanced 3T MRI for the structural analysis of subcortical structures between each gender. In addition, Vertex Analysis was used to visualize the volumetric differences in subcortical structures between each gender. This study is limited to groups in their 70s, therefore, further studies are needed for wider age groups.

Arthroscopic Release of the Extensor Carpi Radialis Brevis Tendon for Chronic Recalcitrant Lateral Epicondylitis (만성 불응성 외 상과염에서 시행한 관절경적 단 요 수근 신건 유리술)

  • Purpose: The objective of this study was to assess the clinical outcome of arthroscopic release of the extensor carpi radialis brevis (ECRB) tendon in chronic recalcitrant lateral epicondylitis and tried to determine any prognostic factors. Materials and Methods: A retrospective review of 24 patients with lateral epicondylitis treated by arthroscopic ECRB release was performed. Outcome measures included a patient self rating and visual analog scale (VAS). Functional evaluation was made with Quick-disabilities of the arm, shoulder and hand (DASH) score system to identify preoperative factors that might be associated with the outcomes. Results: Twenty one (88%) elbows received benefit from the procedure. A mean preoperative VAS pain score and the mean Quick-DASH were significantly improved at final follow up. Age, sex, dominant arm and duration of symptom, presence of capsular tear or calcification did not correlate significantly with the clinical outcome. Three of four patients without T2-weighted high signal focus of the ECRB origin on preoperative magnetic resonance imaging (MRI) showed higher Quick-DASH score. Conclusion: Arthroscopic release of the ECRB is an effective option for chronic recalcitrant lateral epicondylitis. Lack of high signal focus on preoperative MRI is significantly associated with a poor surgical outcome and a considerable factor for the proper surgical indication.
