체위에 따른 균형 안정성 한계의 비교 (A Comparision of the Limits of Stability at Different Body Positioning)

  • The purposes of this study were to evaluate and compare the limits of stability(LOS) at different body positioning(standing and one leg standing) in normal 20 years of age. Fourty subjects participated in the study. Subjects comprised 20 males and 20 females who without neurologic, orthopaedic impairments and balance performance impaired. The LOS was measured at Two Feet Forceplate and One Leg Forceplate with BPM(Balance Performance Monitor) Dataprint Software Version 5.3. The subjects stood 4 inches between the feet at Two Feet Forceplate and stood one legged at One Leg Forceplate. In this study applied the paired t-test and independent t-test to determine the statistical significance of results at 0.01 and 0.05 level of significance. The results of this study were as follows: 1) The anteroposterior LOS significantly increased with one legged stance(p<0.05). 2) The mediolateral LOS significantly decreased with one legged stance(p<0.01). 3) There were significant difference posterior LOS in standing and anterior LOS in one legged stance according to sexual difference(p<0.05). 4) The mediolateral LOS was not significant difference between standing and one legged stance according to sexual difference(p>0.05).

하지 혈류제한 운동이 근육크기와 근력에 미치는 영향 (The Effects of Leg Blood Flow Restriction Exercise on Muscle Size and Muscle Strength)

  • Purpose : The purpose of this study was to investigate thigh muscle-bone CSA and leg strength during low-intensity exercise program with leg blood flow restriction by external compression to reduce muscle outflow. Methods : Eighteen health students gave informed written consent to participate in this investigation. An occlusion cuff was attached to the proximal end of the leg so that blood flow was reduced during the training. The training was conducted one times a day, three times a week, for 8 weeks using one sets of 30 minutes. The training program performed to squat with standing, lunge with standing and heel raise with one leg standing. Measurements of thigh muscle-bone CSA(cross-sectional area) and leg strength were evaluated pre and post-training. Statistical evaluation of these data was accomplished utilizing a paired t-test by SPSS 12.0 program for windows. Significance level was set at p <.05. Results : All data are reported as means and standard deviations(SD) for all variables. The result of the study is followed; After the training, muscle-bone CSA, gluteus maximus m, quadriceps m, hamstring m of both legs were significantly improved but not calf muscle(p<.05). There was no significant difference of change quantity between muscle-bone CSA and leg strength in Lt. and Rt. side. But the variation in leg muscle strength of Rt. leg(dominant) was much more increased than Lt. leg(non-dominant) after 8 weeks training. Conclusion : Low-intensity training with leg blood flow restriction offers a potentially useful method for improving leg muscle strength.

Association between one-leg standing ability and postural control in persons with chronic stroke

  • Objective: To investigate the association between one-leg standing ability and postural control for chronic hemiparetic stroke. Design: Cross-sectional study. Methods: Forty individuals who had a first diagnosis of stroke with hemiparesis before six months and over had participated in this study. To analyze the relationship between one-leg standing ability and postural control in the participants, six clinical measurement tools were used for assessment, including the Timed-Up-and-Go (TUG) test, Berg Balance Scale (BBS), Dynamic Gait Index (DGI), Fugl-Meyer Assessment (FMA), 5 times sit-to-stand (5TSTS) and one-leg standing (OLS). Results: After analyzation, the OLS scores in the more-affected side showed significant positive correlations with BBS scores (r=0.469, p<0.01), DGI scores (r=0.459, p<0.01).and FMA scores (r=0.425, p<0.01). The OLS scores in the more-affected side showed significant negative correlations with TUG score (r=-0.351, p<0.05). The OLS score in the less-affected side showed significant positive correlations with BBS scores (r=0.485, p<0.01), DGI scores (r=0.488, p<0.01) and FMA score (r=0.352, p<0.05). The OLS scores in the less-affected side showed significant negative correlation with TUG scores (r=-0.392, p<0.05) and 5TSTS (r= -0.430, p<0.01). The OLS scores in the more-affected side showed significant positive correlations with the OLS scores in less-affected side (r=0.712, p<0.01). Conclusions: The results of the study suggest that the OLS time may be moderately correlated with static and dynamic postural stabilities and motor recovery following stroke. This study also suggests that the OLS test is as a simple clinical tool for predicting postural control performance for individuals with chronic hemiparetic stroke.

기능적 근력강화 운동이 뇌성마비 아동의 기립균형에 미치는 영향 (The Effect of Functional Strengthening Exercise of Standing Balance in a Child With Cerebral Palsy)

  • The purpose of this study was to investigate the effect of functional strengthening exercise on static and dynamic standing balance in a child with cerebral palsy. The subject was a 7 year old boy with diplegia whose Gross Motor Function Measure (GMFM) score was 80% along with G1 of the lower extremities in Modified Ashworth Scale. The subject was ambulatory with some degree of limitation and demonstrated muscle weakness and strength asymmetry in the lower extremities. A changing criterion design for a single-subject research was used for this study. The functional strengthening exercise consisted of lower extremity ergometer exercise and knee exercise with grading movement in standing position, each for 20 minutes, which lasted 18 sessions for 6 weeks. A knee extensor strength test on both extremities and standing balance test were conducted after each functional strengthening exercise. Two types of standing balance were tested: one leg stance test and functional reach test. One leg stance test was to evaluate static standing balance, and functional reach test was to evaluate dynamic standing balance. The results showed that the functional strengthening exercise had some positive effects on improvement of both static and dynamic standing balance, and there was a positive correlation between the knee strength and standing balance.

체간운동이 하지기능장애 한센노인의 균형능력에 미치는 영향 (The Effects of Trunk Exercises on the Balancing Ability of Elderly Hansen's Disease Patients with Lower Extremity Dysfunction)

  • Purpose : The purpose of this study was to investigate the effects of trunk exercises on the balancing ability of elderly Hansen's disease patients with lower extremity dysfunction. Method : A total of 24 elderly Hansen's disease patients were divided into two groups: 10 without lower extremity dysfunction and 14 with lower extremity dysfunction. The groups exercised for 60 minutes, two days a week, for a total 12 weeks; balancing ability was measured with the one leg standing test, tandem walking test, and timed up-and-go test. The patients were tested and their results were compared both before and after the completion of their exercise programs. Lower extremity dysfunction was assessed according to the following criteria: unilateral foot-drop, toe-loss, and below-knee amputation. Results : After the exercises, participants in both groups showed a positive, statistically significant difference in balance, compared with before the exercises (the one leg standing test, tandem walking test, and timed up-and-go test; p<.05). For comparison purposes, the group with dysfunction and the group without dysfunction were tested before and after the completion of their exercises. Before the exercises, there was a statistically significant difference in the one leg standing test, tandem walking test, and timed up-and-go test (p<.05). However, after the exercises, there was no significant difference in the one leg standing test, tandem walking test, and timed up-and-go test (p>.05). Conclusion : Ultimately, balancing ability was improved in both of the groups after trunk exercises were performed. Although balancing ability was improved, elements of lower extremity dysfunction remained, such as unilateral foot-drop, toe-loss, and below-knee amputation.

지지면에 따른 닫힌 사슬운동이 성인의 균형에 미치는 영향 (Effects of a Closed Chain Movement of the Support Surface on the Balance of Adults)

  • Purpose The purpose of this study was to investigate the change of balance ability by performing closed chain exercise on stable support surface and unstable support surface in twenties. Methods This study randomly selected 15 students in the closed chain exercise group on the stable support side and 15 on the closed chain exercise group on the unstable support side. Balance ability was measured before and after the start of exercise and static balance was measured by OLT(One Leg Standing Test) and FRT (Functional Reach Test). Result The changes of the function reach test of the closed chain movement according to the ground type were significant in the unstable and stable support surfaces and the change of function reach test after the intervention in the two groups was significantly improved compared with the closed chain movement respectively. The one leg standing test changes of the closed chain movement according to the ground type showed significant results on the unstable and stable support surfaces, but there was no significant difference in the one leg standing test changes after intervention between the two groups. Conclusion The effect of closed chain training on ground type is unstable. The change of function reach test and one leg standing test of the closed chain exercise group on the stable support surface resulted in significant changes after exercise, but there was a significant difference in the balance ability of function reach test change after intervention between the groups.

연령별 자세변화에 따른 하지체중지지율에 대한 특성 (The Characteristics of Weight Bearing Ratio on Different Position of Ages Group)

  • Purpose : To know how much weight bearing on left and right leg on 14 different positions. The second is to know how about different the weight bearing ratio on same positions of ages group. Methods : The third is to know how about changes the weight bearing ration between exercise group and non exercise group. The group of age is divided 20s, 30s, 40s, 50s, over the 60 years. The subjects are 93 males(44), females(49). Results : The ordering of ratio of weight bearing on one side leg is as follows: The position of the highest weight bearing ratio is one leg standing with the other leg on chair(right 0.82, left 0.81) and the position of lowest weight bearing ratio is hooklying with natural leg position(both legs 0.08). There are statistically significant difference between right and left leg of weight bearing ratio on the 6 positions among the 14 positions. As for the ages, there are statistically significant difference on 5 different positions of 20 ages body weight bearing ratio between right and left leg. But as for the 50 ages there are no statistically significant difference on all of 14 positions between right and left leg body weight bearing. As for the exercise group there are statistically significant difference on only one position of one leg standing with the other leg on chair between right and left weight bearing ratio. But as for non exercise group there are statistically significant difference on 4 positions between right and left weight bearing ratio. Conclusion : When the therapist exercise with patient's always considerate of patient's position and weight bearing ratio.

Immediate Effects of Lumbar Rotational Mobilization on the One-Legged Standing Ability in Healthy Individuals: A Randomized Controlled Trial

  • The original focus of this study was to investigate the immediate effects of lumbar rotational mobilization on the one-legged standing ability. Fifteen subjects (6 men and 9 women, mean age = 22.77 (SD = 1.21), mean height = 165.46cm (SD = 11.65), mean weight = 61.46kg (SD = 8.29) volunteers from healthy individuals were recruited and randomized to a lumbar rotational mobilization (LRM) group and a trunk rotational exercise (TRE) group. Mobilization (grade 3 or 4) was applied to the LRM group on the lumbar spine (L1 to L5) in a side-lying, and trunk twist exercise (left and right side) was applied the to the TRE group with lunge position. Center of pressure (COP) and the velocity of the center of pressure (VCOP) of each participant were measured as a balance ability through one leg standing position. Results are as follows. In within-group difference, the COP of the LRM group reduced during standing with the right foot, but the VCOP change of the LRM was not statistically significant. In between-groups difference, COP of TRE group was decreased compared with LRM group only during left leg standing in the eyes (p <.05). The results of this study suggest that LRM is more effective than TRE in improving balance ability.

청소년기 특발성 척추측만증 환자의 임상적 균형 평가지수와 균형 수행 모니터 측정값의 상관성 연구 (The study of correlations between clinical balance scales and balance performance monitor parameters in patients with adolescent idiopathic scoliosis)

  • Purpose : The purpose of this study was to investigate the correlations between clinical balance scales and Balance Performance Monitor parameters in patient with adolescent idiopathic scoliosis (AIS). Methods : Twenty AIS subjects (age, $14.26{\pm}1.93yrs$; height, $160.56{\pm}7.98cm$; weight, $47.54{\pm}6.94kg$)were participated in this study. Postural sway(mean balance, sway angle, sway area, sway path, maximal sway velocity) were were evaluated by balance performance monitor. Measurements for clinical balance scales were Functional reach test (both side), the Lateral reach test (both side) and One leg standing test (both legs). Results : The results were as follows. There were positive strong correlation between major curve direction and left-right sway angle, sway path, maximal sway velocity. There were negative strong correlation between the functional reach and left-right sway angle, sway area, sway path, maximal sway velocity. And the lateral reach were also showed negative strong correlation parameters of balance performance monitor. One leg standing were negatively correlated with left-right sway angle, sway path, maximal sway velocity. Conclusion : The clinical balance scales will be useful tools for balance measurements, and basic tools for clinical setting for patient with AIS.

주 조절점 핸들링이 편마비 환자의 한발서기 자세적응에 미치는 영향 - 보바스의 신경발달치료 중심 - (The Effect of Key Point Control Handling for One-leg Standing Postural Adaptation in Hemiplegia)

  • This study is aimed at diagnosing transmutation aspect with the respective of hemiplegia patient's static adaptation of posture which is influenced by anti-gravity excercise, center of gravity shifting movement and segmental movement adaption of the legs and arms by key-point control in the process of Bobatli's treatment approach. The patients for the investigation of this study were selected as a total 17 patients who were doing the physical-therapy in Tae-gu Rehabilitation Center, and diagnosed as hemiplegia patients by Rehabilitation Medicine department in Kyung-buk University hospital. And also, there investigated into temporal transition that keep the attitude with one-leg standing on the ground concerning static attitude adaption on the basis of the before of anti-gravity movement and the after of 4-weeks movement. The findings of this study were as follows: With the respective the time to keep standing pose by one-leg at static attitude transmutation, affected side showed meaningful differences as a l.86/sec, 2.62/sec at 4 weeks later considering the before and after of this experiment.(p<0.01), non-affected side at the aspect of attitude keeping time, but there didn't showed meaningful differences statistically.
