  • Although various methods had been reported for reanimination of facial nerve palsy, interposition nerve graft remains superior to other methods if there is a wide gap to be bridged. Dott described a excllent facial nerve reconstruction by sural nerve graft bypassing petrous bone. But his method needs two surgical fields and is performed in two stages. Authors desribe a traumatic facial nerve palsy treated by one stage facial nerve reconstruction that is performed in one surgical field by using a great auricular nerve interposition graft and bypass the petous bone.

The Merits of Mannitol in the Repair of Orbital Blowout Fracture

  • Background One of the main concerns in orbital blowout fracture repair is a narrow operation field, due mainly to the innate complex three dimensions of the orbit; however, a deep location and extensive area of the fracture and soft tissue edema can also cause concern. Swelling of the orbital contents progresses as the operation continues. Mannitol has been used empirically in glaucoma, cerebral hemorrhage, and orbital compartment syndrome for decompression. The authors adopted mannitol for the control of intraorbital edema and pressure in orbital blowout fracture repair. Methods This prospective study included 108 consecutive patients who were treated for a pure blowout fracture from January 2007 to October 2012. For group I, mannitol was administered during the operation. Under general anesthesia, all patients underwent surgery by open reduction and insertion of an absorbable mesh implant. The authors compared postoperative complications, the reoperation rate, operation time, and surgical field improvement between the two groups. Results In patients who received intraoperative administration of mannitol, the reoperation rate and operation time were decreased; however, the difference was not statistically significant. The total postoperative complication rates did not differ. Panel assessment for the intraoperative surgical field video recordings showed significantly improved vision in group I. Conclusions For six years, mannitol proved itself an effective, reliable, and safe adjunctive drug in the repair of orbital blowout fractures. With its rapid onset and short duration of action, mannitol could be one of the best methods for obtaining a wider surgical field in blowout fracture defects.

  • Laryngeal web is formed from various causes, and it is a scar tissue covered with epithelium on both sides. The purpose of the laryngeal web surgery is to cover the vocal cords with healthy epithelium and to achieve the normal or near normal function of respiration and phonation. Until now, a lot of surgical procedures were introduced, but have some merits and disadvantages. Recently authors tried this new surgical technique on 6 patients using the suspension laryngomicroscope under the intubation general anesthesia, which was to suture the upper and lower edges of the severed web. This new approach has several advantages; 1) familiar instruments and visual field 2) no unnecessary surgical trauma beyond the operation field 3) one stage operation 4) shorter operation time 5) no serious complications 6) short hospitalization, etc.

Evaluation of Electron Boost Fields based on Surgical Clips and Operative Scars in Definitive Breast Irradiation (유방보존술 후 방사선치료에서 수술 흉터와 삽입된 클립을 이용한 전자설 추가 방사선 조사야 평가)

  • Purpose: To evaluate the role of surgical clips and scars in determining electron boost field for early stage breast cancer undergoing conserving surgery and postoperative radiotherapy and to provide an optimal method in drawing the boost field. Materials and Methods: Twenty patients who had $4{\sim}7$ surgical clips in the excision cavity were selected for this study. The depth informations were obtained to determine electron energy by measuring the distance from the skin to chest wall (SCD) and to the clip implanted in the most posterior area of tumor bed. Three different electron fields were outlined on a simulation film. The radiological tumor bed was determined by connecting all the clips implanted during surgery Clinical field (CF) was drawn by adding 3 cm margin around surgical scar. Surgical field (SF) was drawn by adding 2 cm margin around surgical clips and an Ideal field (IF) was outlined by adding 2 cm margin around both scar and clips. These fields were digitized into our planning system to measure the area of each separate field. The areas of the three different electron boost fields were compared. Finally, surgical clips were contoured on axial CT images and dose volume histogram was plotted to investigate 3-dimensional coverage of the clips. Results : The average depth difference between SCD and the maximal clip location was $0.7{\pm}0.55cm$. Greater difference of 5 mm or more was seen in 12 patients. The average shift between the borders of scar and clips were 1.7 1.2, 1.2, and 0.9 cm in superior, inferior, medial, and lateral directions, respectively. The area of the CF was larger than SF and IF in 6y20 patients. In 15/20 patients, the area difference between SF and if was less than 5%. One to three clips were seen outside the CF in 15/20 patients. In addition, dosimetrically inadequate coverage of clips (less than 80% of prescribed dose) were observed in 17/20 patients when CF was used as the boost field. Conclusion: The electron field determined from clinical scar underestimates the tumor bed in superior-inferior direction significantly and thereby underdosing the tissue at risk. The electron field obtained from surgical clips alone dose not cover the entire scar properly As a consequence, our technique, which combines the surgical clips and clinical scars in determining electron boost field, was proved to be effective in minimizing the geographical miss as well as normal tissue complications.

  • Congenital or acquired esophagotracheobronchial fistula are rare. The chief causes of the acquired form are malignancy developing on the esophagus or tracheobronchial system and infection, and trauma. The pathognomonic symptom is a paroxysmal cough occurring several seconds after ingestion of liquids. This report reviews a case of bronchoesophageal fistula of unknown origin accompanying bronchiectasis. The patient is 32 years old woman with excellent result by surgical intervention. But the fistula is accidently found in the operation field. The surgical procedures consissts of fistulectomy with Right lower lobectomy.

  • The importance of bronchial occlusion which occurs in the natural course of tuberculosis as an inconstant but very fortunate event became obscured with the popularity of resection therapy for pulmonary tuberculosis and the resectional surgery and thoracoplasty are the standard method of surgical procedure in the treatment of pulmonary tuberculosis. However in some cases of far advanced pulmonary tuberculosis, the need for another surgical methods arise when standard method is not indicated under the consideration of poor pulmonary function or operative and postoperative complications such as bronchial fistula. The ligation and division of bronchus draining the involved part of the lung is one of the applicable method among the another surgical procedures. The authors experienced one case of far advanced pulmonary tuberculosis who had a huge cavity in the right upper lobe and a small cavity in the superior segment accomanying with several nodular densities in the basal segment and contralateral left lung field, and treated with right upper lobectomy, ligation and division of the superior segmental bronchus and concomitant rib-resectional thoracoplasty in order to prevent postoperative bronchial fistula and to preserve maximal lung function. The postoperative course was smooth without complication regarding to bronchial ligation and division technique and the general condition has been excellent without symptoms. The postoperative sputum examination for AFB on smear and culture has been negative during the 11 month period of follow up, and X-ray of the chest including tomography demonstrated no evidence of residual cavity indicating succesful collapse of cavity.

Surgical Treatment of Congenital Cystic Lung Disease (선천성 낭성 폐질환의 수술적 치료)

  • Pulmonary sequestration, congenital cystic adenomatoid malformation[CCAM], infantile lobar emphysema[ILE], and bronchogenic cysts are four congenital lesions that show abnormal cystic areas within the lung field in early life. They share similar embryologic and clinical characteristics, Therefore they are sometimes difficult to make differential diagnosis each other, and all require surgical treatment. From 1984 to 1993, 20 patients underwent surgical corrections under these diagnostic categories[10 bronchogenic cyst, 4 pulmonary sequestration, 4 CCAM, and 2 ILE] in the department of thoracic & cardiovascular surgery, Inje University, Pusan Paik Hospital. There were 9 females and 11 males, Ages ranged from 26 days after birth to 69 years. Among them 5 cases of bronchogenic cyst were found out incidentally, but remained all 15 cases were noted as symptomatic cases. Recurrent pulmonary infections, respiratory distress and cough with cystic lesions in chest film were the main characteristics of them. Computed tomography and aortography were available for diagnostic conformation. For all the cases surgical resection were performed: 1 pneumonectomy, 2 bilobectomy, 9 lobectomy, 7 cyst resection and 1 mass[extralobar pulmonary sequestration] resection. All surgical treatments were well tolerated with no physical limitation. There was no operative mortality, and only one postoperative complication[empyema thoracis]. All patients were followed up ranging from 4 months to 9 years. A clinical awareness of these related lesions is important for prompt diagnosis and effective surgical treatment.

Surgical Treatment of Congenital Lung Cysts (선천성 폐낭성 질환의 외과적 치료)

  • A total of 69 patients were treated for congenital lung cysts at Seoul National University Hospital during the period between 1960 and Aug, 1989. They were 37 patients of bronchogenic cyst, 18 patients of pulmonary sequestration, 7 patients of congenital cystic adenomatoid malformation [C.C.A.M.], and 7 patients of congenital lobar emphysema. In cases of bronchogenic cyst, involved age was variable from 7 months to 53 years and most cases were asymptomatic. They were located 14 cases in the right lung, 6 cases in the left lung and 17 cases in the mediastinum. Their sizes were also variable from 2.5 to 12 cm and most of them except 3 cases were solitary cysts. There were 6 cases that had bronchial communication. In cases of pulmonary sequestration, aged from 3 months to 29years, all were intrapulmonary type. 15 cases of them were found in the left lower field and 3 cases in the right lower field. Anomalous vessels were confirmed in 16 cases by aortography and/or surgical exploration. In cases of congenital cystic adenomatoid malformation, aged from 7 days to 6 years, 3 cases of infants were admitted due to severe dyspnea and cyanosis but the remaining 4 cases were admitted due to large cyst or abscess on chest roentgenogram. 3 cases were located in the right lung and 4 cases in the left lung. In cases of congenital lobar emphysema, aged from 14 days to 11 years, 6 cases were located in the right lung and one case was located in the left lung. All of them were male and had shown some degree of dyspnea from birth There were 2 cases of mortality, one in C.C.A.M and one in congenital lobar emphysema. But the remaining cases were well treated and recovered with the surgical resections.

Surgical Treatment of Epiphrenic Esophageal Diverticulum -One case report- (횡격막상부 식도게실 수술 1예)

  • Recently we experienced one, case of epiphrenic esophageal diverticulum of a 50 - year - old female patient who had complained heaviness in her chest after meals and occasional dysphagia for one year. Preoperative barium study showed a large epiphrenic esophageal diverticulum at about 7cm above the diaphragm which protruded to the right side of the mid thorax. On the operation field, epiphrenic esophageal diverticulum, measuring 5x 6x3cm in size, was noted. Diverticulectomy and extended myotomy was performed and the postoperative course was uneventful.

Surgical Approach to the Oral Cavity and Oropharyngeal Cancer; Visor Flap Approach (구강 및 구인두암의 수술적 접근방법 ; 면갑피판(Visor Flap)의 임상적 평가)

  • Objectives: Visor flap is one of the useful surgical approach to the oral cavity cancer, but the report on its specific indications, advantages and disadvantages is lacking. Material and Methods: Seven patients treated with visor flap for oral cavity and oropharyngeal cancer were reviewed. Result: Visor flap provided excellent visual field to anterior oral cavity without splitting the lip and chin skin. Postoperative cosmesis was satisfactory. One complication associating with this flap was salivary leak through gingivo-labial and gingivo-buccal suture which was successfully repaired. Conclusion: Visor flap is an excellent approach on extirpation of the oral cavity and oropharyngeal cancer in case of combining with segmental mandibulectomy. Especially it was useful for the approach to the anterior floor of the mouth and oral tongue lesion. But, it has no actual advantage over the other approaches in cases without mandibulectomy.
