Cavoatrial bypass for Budd-Chiari Syndrome Associated with Obstruction of the Iinferior Vena Cava -Report of One Case- (하공정맥 폐색증에 의한 Budd-Chiari 증후군의 수술치험 -1례 보고-)

  • We report a case of a 45-year-old woman with Budd-Chiari syndrome caused by the obstruction of the inferior vena cava just below the diaphragm. Transatrial dilatation or membranotomy was not possible due to the severe fibrotic obliteration of the inferior vena cava. Instead, cavoatrial bypass with a Dacron graft[20 mm-Vascutek] was performed under the median sternotomy and median abdominal incision.The postoperative course was uneventful and generalized symptoms were much improved. During the following period[6 month] the graft patency was maintained with no recurrence of symptoms.

  • Bilateral traumatic rupture of diaphragm is very rare. One case due to car accident is reported. Preoperative chest X-ray revealed the diaphragm rupture in the left side and the hemothorax in the right side. During the completion of left diaphragm repair through left thoracoabdominal incision, right diaphragm rupture was found incidentally. Left diaphragm was repaired using pledgets which were anchored at the thoracic wall. Right diaphragm was also repaired by interrupted Halsted sutures through seperated right thoracotomy. Postoperative course was uneventful.

  • The bronchopleural fistula is the most common and serious complication of postpneumonectomy empyema. We experienced one case of postpneumonectomy empyema with bronchopleural fistula which treated with Abruzzini operation using residual long bronchial stump. Median sternotomy was used with extension about 3cm incision toward cephalic side. We ligated and divided the innominate vein. We did not open the pericardium with extrapericardial approach. Stapler was used to distal bronchial side and additional interupt sutures were used on proximal side.

Aortopulmonary Window -Report of A Case- (폐동맥 대동맥 중격결손증 1례 보고)

  • One case of surgical correction of a large aortopulmonary window in a 4 months old female is reported in detail. Clinical symptoms consisted of fatigue when feeding and recurrent upper respiratory tract infection. Under cardiopulmonary bypass with moderate hypothermia and cardioplegic arrest of the heart, the aortopulmonary window was approached through a vertical incision at the anterior wall of window itself. The defect was closed with pericardial patch using continuous suture posteriorly and sandwitch technique anteriorly. The patient was weaned from the cardiopulmoanry bypas without hemodynamic problem and postoperative course was uneventful. This surgical technique is recommandable as it provides good exposure of the defect and can be performed easily and safely.

  • The sternal turnover has a limited use in an asymmetrical funnel chest. However we tried `One-half sternal turnover` as a new operative approach for an asymmetrical funnel chest. Through the bilateral submammary skin incision, median sternotomy was made from xiphoid process to midsternum and extended horizontally. The segment of ribs were cut at the angle of depression. The en-bloc resected chest wall segment contained one-half sternum as well as a part of ribs and left half of rectus muscle. After turning over the en-bloc segment, reapproximation with wiring was done. Sternotomy wound was closed in layer after placing of substernal drainage tube. Postoperatively, the chest wall was stable and the recovery course was uneventful except left-sided minimal pneumothorax which was cured spontaneaously. The patient was discharged on postoperative 14th day.

  • The eyes, the saying goes, are the windows of the soul. It's the first thing you notice about a person. Therefore, many people want to possess beautiful eyelids. Surgical formation of a palpebral fold and sulcus divides the lid into two well-defined segments (palpebral and pretarsal), producing the double eyelid desired by many Oriental women as well as an increasing number of man recently. Upper lid blepharoplasty is the Oriental eye is one of the variations of standard upper lid blepharoplasty. In Oriental double eyelid surgery, there have been two approaches to form a superior palpebral fold: the buried suture(nonincision) method and the full external incision method. Conventionally, the nonincision technique has been shown to produce little postoperative edema. However, the probability of the fold disappearing is high, and this technique cannot be performed in patients with fatty eyelids. Conversely, the incision technique has contrary characteristics. Recently, partial incision(or semi-open) technique which is combination of mentioned methods is used, this technique is removal of pretarsal tissue, muscle, and/or orbital fat around 2 or 3 incision site to facilitate tarsus-dermal adhesion. Our method is on the basis of this technique, furthermore, compared with conventional semi-open method, Y(Yang's) needle assisted double eyelid operation is more easy, convenient, saving-time method and provide satisfactory results.

Anatomy of the Sural Nerve in the Foot and Ankle and Its Surgical Implications (족부 및 족관절 부위에서 비복 신경의 해부학 및 수술시의 의미)

  • The course of the sural nerve in the calf has been well documented, but there is a general lack of information concerning the distal course of the nerve. The purpose of this study was to describe the distal course of the sural nerve and its surgical implications. Seven fresh amputated specimens were dissected to show the anatomy of the sural nerve in the foot and ankle. At the level of about 10cm proximal to the plantar surface, the sural nerve coursed anteriorly and inferiorly away from the Achilles tendon. 2 to 4 lateral calcaneal branches arose. The first branch of the lateral calcaneal branches coursed along the lateral border of the Achilles tendon, and it arose at 8cm proximal to the plantar surface in 2 specimens, 12cm proximal to the plantar surface in 4 specimens, and at 12cm proximal to the plantar surface in one specimen. The main nerve trunk continued distally plantar to the peroneal tendons and divided into two terminal branches and crossed peroneus longus tendon at the level of the inferior border of the calcaneo-cuboid joint, at about 3cm(range, $2.5\sim3.0$)cm from the plantar surface. In conclusion, a longitudinal incision lateral to the Achilles tendon would cross the path of the sural nerve at about 10cm proximal to the plantar surface. When the first branch of them arise more than 10cm above the plantar surface, a logitudinal incision lateral to the Achilles tendon may be made without damage. The other lateral calcaneal branches will be cut when we make transverse incision paralled to the plantar surface. The terminal branch also may be in danger by the same transverse incision.

The Investigation and Development of Astigmatism Correction Treatments by Finite Element Method and Animal Experiments (유한요소법과 동물실험을 통한 난시교정술의 고찰 및 개발)

  • The purpose of this study is to investigate the effects of various factors in keratotomy for astigmatism correction on surgical outcomes by finite element method as well as animal experiments. Three kinds of surgical techniques were mechanically investigated : arcuate, straight, and inverse arcuate keratotomy. Among the three techniques the arcuate keratotomy is the most popular one while the other two techniques are being investigated in this area. The arcuate keratotomy was found to be more controllable and effective in reducing the refractive power than the others. In arcuate keratotomy it was found most effective when the incision was located in the middle position between the apex and the edge of the cornea from the results of experiment as well as finite element study. Regarding to the range of the corneal incision in arcuate keratotomy, the incision angle of 90$^{\circ}$ was found th be most effective in reducing refractive power than other angles even it was incised up to 150$^{\circ}$. Therefore, it was concluded that 90$^{\circ}$ of incision angle results in the largest decrease in refractive power in arcuate keratotomy. However, other important findings were that the effect of the surgery decreased with time so the visco-effect of the cornea and auto-healing process. Therefore, these factors should be considered in future studies.

Surgical Correction of Intrahepatic Inferior Vena Cava Obstruction (간내 하공정맥 폐색증의 수술요법)

  • Seven patients [six women and one man] with obstruction of hepatic portion of inferior vena cava was operated on, from May, 1969 to January, 1985. Of seven patients, six were undergone corrective operation for IVC obstruction and another one was not operated because of far advanced liver cirrhosis. The occlusions were found at or close to the level of diaphragm and they were membranous or diffuse with or without thrombi. Most of their symptoms were referable to either inferior vena caval or hepatic venous obstruction and onset of the symptoms was usually gradual, beginning between the age of their thirties and forties. Most of the patients showed marked elevation of peripheral venous pressure of lower extremity [29-40 cm H2O] preoperatively, which decreased significantly after corrective operation [17-30 cm H2O]. Venous catheterization for pressure study and venography were essential for confirming the diagnosis. Of six cases, in which corrective operations were done, Transatrial membranotomy with or without IVC dilatation were performed in five cases [case 1, 2, 3, 5, 6], using cardiopulmonary bypass and in another one case, bypass operation between IVC, distal to obstruction, and RA was done using Dacron tube graft under the thoracoabdominal incision. All survived and their conditions were improved.

Three different operations made in cryptorchid dogs (개의 잠복고환증에 행해진 3가지 수술 방법)

  • The three cases of cryptorchid are reported here in order to demonstrate the different surgical techniques that were carried out to remove crytochid testes in dogs and to introduce clinical cases on crytorchidism. Three dogs were presented in the clinic of Ecole Veterinaire de Lyon in May, 1998 with two in the left-sided unilateral abdominal cryptorchid and with one in bilateral inguinal mobile cryptorchid. There were two poodle at the age of more than 4 years old and 8 years old respectively with one boxer at 4 years old. In two abdominal cryptotchids, the skin was incised in caudal paramedian approach. Then, the linea alba were incised and cryptorchidectomy was peformed accompaning one with castration and the other with vasectomy by the request of the owners. In inguinal testicles, prescrotal skin incision was made to remove both testicles. The surgical outcome was good without any Their behaviors became agreeable as the proprietors reported. In the dog with vasectomy, the behavior changes were not noticeable.
