  • Cancer pathology reports play an important role in choice of patient care. They provide crucial information concerning diagnosis, therapy options, and prognosis. Professional pathology institutions, such as the College of American Pathologists (CAP), have developed checklists to ensure the presence of all the required elements in reports. In this study, 438 surgical pathology reports of patients with breast (148), colon (147), and stomach cancer (143) were evaluated with respect to the presence of mandated elements according to CAP checklists. The most common missing element in all the three types of cancer was 'staging' (73.6, 53.1, and 56.6% in breast, colon, and stomach cancer reports missed 'staging', respectively). The second most missing element was 'tumor site' in breast (64.2%) and stomach cancer (30.1%), and 'procedure' in colon cancer (29.3%). 'Perineural invasion' was the third most missing element in the three types of cancer (25.7, 17.0, and 22.4% in breast, colon, and stomach cancer, respectively). Only 11.4% of reports included all key elements required by CAP. The use of checklists was associated with higher rate of completeness. This study demonstrates that the key elements requiring the information on the requisition forms from the clinicians are commonly missed, leading to ambiguity.

  • As the sensors and communication technology get advance, the remote health diagnosis for patients and senior persons at home are possible now without visiting doctors in hospitals. A low-power ubiquitous health check device was developed adapting Real-Time Embedded Linux is developed. This ubiquitous device is consisted of three sensors. The wrist type health checking terminal acquires periodically the health data by using a blood pressure sensor, a pulse sensor and a body temperature sensor. It transmits the health data to the access point located at the home center through the ZigBee wireless communication modem. This health data collector or access point device sends the data again to the main server operated in a hospital or health care organization. The health server control continuously the input data and sends an alarm signal to the assigned. doctor and responsible persons using cellular SMS when any dangerous events occur. This wrist type health check device has an embedded linux OS using Intel PAX255 MPU. The developed U-Health system is applicable for checking patients health in remote at home. And their family or related persons in remote site can check the patients health status at any time. They can be assured by receiving SMS record and alarm of emergency case which is transmitted from the health server.

  • The author studied age and sex distribution, etiology, affected site and several radiographic features of osteomyelitis of the jaw. And radiologic classification of osteomyelitis was also done. The material consisted of 118 males and 96 females examined and/or treated under the diagnosis of osteomyelitis during past 11 years (1970-1980.6) in SNUDH. The obtained results were as followings. 1. The incidence is the highest in teenages(22.9%) and the lowest in seventies. (2.8%). 2. 199 cases were found in lower jaw, and 15 cases in upper jaw. 30.8% of all cases were located at the posterior portion of mandibular body comprising alveolar region. 3. Radiographic examination of osteolytic lesion revealed that 21. 5% of all patients had periapical and alveolar bone rarefaction combined with osteoporotic changes and that in most of patients two or more of the above described changes were present at the same time. 4. Sclerotic lesions were seen in 62.2 % of all patients and 21.5% of sclerotic lesion were diffuse or homogenuous type. 5. Based on the radiologic study, classification of the osteomyelitis of the jaw was made. Loclized osteolytic type was the highest in incidence (38.8%) and localized sclerotic type was the lowest (7.0%)

  • We had experienced very rare a case of renal cell carcinoma through ultrasonic diagnosis, september, 1985 at medical dept. Dae Han Kyoyuk Ins. Company. The conclusions that we gained, making a comparative-analysis of operation's view and ultrasonic view are as follows; 1. Echolucent area which was $0.6cm^2$ size in the center of tumor was shown by ultrasonography, we noticed it occured necrosis or cystic change and the extracts grossly after. operation accorded with ultrasonic view. 2. Tumor was 65mm in diameter on ultrasonography and made clear 55mm in diameter after operation 3. There was not fever, anemia, even typical triad of renal cell carcinoma, blood pressure was within normal limits. 4. The case was stage I by Robson's Modification method. 5. The case was clear cell type by classifying of histology. 6. The affected site was left side and origin was lower pole of the kidney. 7. After radical nephrectomy, until present prognosis was favorable and he exists.

  • Hepatitis C virus (HCV) infection is a major medical challenge affecting around 200 million people worldwide. The main site of HCV replication is the hepatocytes of the liver. HCV is a positive enveloped RNA virus from the flaviviridae family. Six major HCV genotypes are implicated in the human infection. In developed countries the children are infected mainly through vertical transmission during deliveries, while in developing countries it is still due to horizontal transmission from adults. Minimal nonspecific and brief symptoms are initially found in approximately 15% of children. Acute and chronic HCV infection is diagnosed through the recognition of HCV RNA. The main objective for treatment of chronic HCV is to convert detected HCV viremia to below the detection limit. Children with chronic HCV infection are usually asymptomatic and rarely develop severe liver damage. Therefore, the benefits from current therapies, pegylated-Interferon plus ribavirin, must be weighed against their adverse effects. This combined treatment offers a 50-90% chance of clearing HCV infection according to several studies and on different HCV genotype. Recent direct acting antiviral (DAA) drugs which are well established for adults have not yet been approved for children and young adults below 18 years. The most important field for the prevention of HCV infection in children would be the prevention of perinatal and parenteral transmission. There are areas of focus for new lines of research in pediatric HCV-related disease that can be addressed in the near future.

  • An inflammatory myofibroblastic tumor (IMT) is a rare disease and can occur in nearly every site of the body. The usual intrathoracic sites where an IMT has been recorded are the lung and bronchus. An IMT originating from the mediastinum has been rarely reported. A patient with a posterior mediastinal mass that was noticed incidentally on a chest X ray underwent mass extirpation. The pathologic diagnosis was an inflammatory myofibroblastic tumor. The tumor was positive for tuberculosis as determined by PCR. Tuberculosis can be thought to be the causative factor of the IMT in this case.

  • Metastatic tumors occur more frequently in the liver than in any other organ. Guided percutaneous fine-needle aspiration (FNA) of the liver is often recommended for confirmative diagnosis of the metastatic lesion, because of its simplicity, high yield, and reasonable safety. The authors studied retrospectively cytologic findings of 110 cases of metastatic tumors to the liver. The frequent primary sites were the stomach (23 cases), pancreas(19 cases), gallbladder(12 cases), and periampullary lesions(6 cases). Most of the metastases were carcinoma (106 cases). There were only 4 cases of sarcoma. The characteristic cytologic findings of FNA of meatastatic tumors were dirty background, abrupt change between hepatocytes and malignant cells, and desmoplasia. Some tumors displayed rather distinctive cytologic appearance that suggests primary sites. For example, the colonic adenocarcinoma showed tall columnar cells with a palisading arrangement, adenocarcinoma of gallbaldder showed focal squamous differentiation in some cases, and metastatic renal cell carcinoma and neuroblastoma showed also distinctive cytologic findings. Because the cytologic features of metastatic tumor are very similar to those of primary tumor, correct cytologic typing may be helpful in pursuit of an occult primary site of metastatic liver lesions, reducing extensive diagnostic investigation in poor prognostic patients.

  • Mucous membrane pemphigoid is uncommom disease in oral cavity and synonymous with cicatricial pemphigoid. This disease is caused by autoimmune reaction that autoantibody reacts antigen located in basement membrane and epithelium is separated from underlying connective tissue. It affects female over sixth decade, commonly. Oral mucosa, especially gingiva is common site but conjunctival, nasal, pharyngeal, laryngeal, esophageal, varginal mucosa and skin are involved. Intraoral findings show Nikolsky sign, irregular erythema, erosion, vesicle, and ulceration at mucous membrane. To differentiate from diseases of positive Nikolsky sign, should perform histologic, immunologic test. Histologic features show subbasilar cleft and direct immunologic features show IgG, C3 deposits at basement membrane in linear pattern. Mucous membrane pemphigoid is incurable disease because symptoms are repetitively improved or worsed for several years. Patiens are commonly managed with topical and systemic steroid. To avoid side effects of prolonged steroid therapy and to maintain immunosupressive effects, combination therapy of azathioprine with steroid is effective. This case reports that mucous membrane pemphigoid is diagnosed based on clinical and histologic features, is treated with topical, systemic steroid and azathioprine therapy.

  • During the electromyographic evaluation, the presence and site of lession may be predicted by a detection of denervation potentials such as fibrllation potentials or positive sharp waves in the group of muscles. Currently, clinicians diagnose the neuropathy by detecting fibrillation potentials during EMG tests, and sometimes it is not easy to distinguish between denervation potentials and endplate spikes. The purpose of this study was to find statistically significant parameters for the quantitative distinction between denervation potentials and endplate spikes. Endplate spikes and denervation potentials from the EDB muscle of 10 patients were extracted. Also, EMG signals were classified by experienced clinicians, and were collected using a 12 bit ADC with a sampling rate of 20KHz for the duration of 400msec. In order to find statistically significant parameters, positive and negative peaks were used for analysis. As a results, standard deviation of the endplate spikes and denervation potentials showed more significant difference than others specially for the positive sharp waves. It was concluded that the results of this study could be used to develope an automated system of a EMG analysis.
