Factors Affecting Length of Stay and Death in Tuberculosis Patients(2008-2017): Focus on the Korean National Hospital Discharge In-depth Injury Survey (결핵 환자의 재원기간과 사망에 영향을 미치는 요인(2008-2017): 퇴원손상자료를 중심으로)

  • The purpose of this study is to identify factors affecting length of stay(LOS) and death in tuberculosis(TB) patients by disease type, patient characteristic, admission and disease characteristic, and hospital characteristic from 2008 to 2017. Survey data was using Korean national hospital discharge in-depth survey data produced by Korea Disease Control and Prevention Agency. Study subjects were 10,634 inpatients with TB(A15, A16, A17, A18, A19, U88.0, U88.1, U84.30, U84.31) and analyzed frequency, chi-square test, Fisher's exact test, and logistic regression by using STATA 13.0. As a study result, the type of TB(extrapulmonary TB, multidrug-resistant TB, extensively drug-resistant TB), sex(woman), age(35-49, 50-64, 65-74, 75 years old or older), admission type(outpatient department), CCI(1-2 point, 3 point over), hospital location(metropolitan city) and bed size(300-499, 500-999, over 1000) were significantly influence LOS. Also, the type of TB(extrapulmonary TB, extensively drug-resistant TB), sex(woman), age(50-64, 65-74, 75 years old or older), residence(small town/rural), admission type(outpatient department), CCI(1-2 point, 3 point over), hospital location(provincial) were significantly influence death. In conclusion, the existing tuberculosis management has been patient management with rapid diagnosis and treatment following early detection. But other studies should be carried out for the system that identifies and supports high-risk groups of the long-term length of stay in hospital or high mortality rates as a result of treatment.

Gender-Egalitarianism in Middle School Students (중학생의 남녀평등의식수준)

  • Purpose : In an attempt to analyse gender-egalitarianism related to the general characteristics, sexual consciousness and behavior in middle school students, this study was carried out. This study will provide data that can be used to develop sexuality education programs on the basis of gender-egalitarianism. Method : The subjects were 141 students (70 boys and 71 girls) in 4 classes that were randomly selected from among 8 classes of first graders in a coeducational middle school, in Seoul. For the evaluation of gender-egalitarianism, a Gender Egalitarianism Scale modified by the researcher was used. Results : Major findings were as follows: 1. Gender-egalitarianism was significantly higher in girls compared with boys. All of the boys and girls, had high gender-egalitarianism in the aspect of woman's ability in groups without sexual discrimination, but low gender-egalitarianism in woman's sexual conversation and style of dress. 2. Gender-egalitarianism in boys who had experienced intersexual physical contact, categorized as holding hands, embracing, kissing, and petting, was significantly higher when compared with inexperienced boys. 3. Gender-egalitarianism significantly increased as the father's education level increased as well as the father's open-mindedness. Factors influencing gender-egalitarianism was social acceptance, the mass media, the attitudes of the school teachers, and the attitudes of older and younger student in school. Conclusions : Based on these results, boys' gender-egalitarianism was lower than that of girls. Systematic education on gender-egalitarianism, combined with sexuality education, is necessary for middle school students, especially boys. In the long term, social acceptable the, mass media, school and home life should be changed for the achievement of the equality of the sexes.

Individual Diabetes Nutrition Education Can Help Management for Type II Diabetes (제2형 당뇨환자의 개인별 맞춤영양교육의 효과 평가)

  • This study was designed to evaluate the effect of the individual nutrition education for type 2 diabetes who participate the diabetes buffet. The subjects were 66 patients and divided into education (n=34) and control groups (n=32). The mean age of education and control groups were 59.8 and 56.6 years old, respectively. There were no differences in age and body mass index (BMI) between two groups. Initial glycosylated hemoglobin (HbA1c), post prandial plasma glucose (PP2), total plasma cholesterol, and blood pressure were not different between two groups. But fasting blood sugar (FBS) was higher in education group than in control group. On completion of the study, the education group showed significant decreases in body weight, BMI, FBS and PP2, however, the control group showed no changes in body weight, BMI and PP2, and showed a significant increase in FBS. Initial calorie and protein intakes of the education group did not meet the prescribed amount, however, mineral and vitamin intakes were higher than estimated average requirement (EAR). By the end of study, calorie and protein intakes were significantly increased to meet the prescription. In order to evaluate the effect of diet education, awareness of calorie requirement was used as an index of understanding diet prescription. The degree of awareness of calorie requirement was dependent on age: younger patients showed higher awareness than older subjects. The subject who showed better understanding of diet prescription showed lower levels of HbA1c, FBS, and PP2 at the end of the experiment period. The results of this study clearly show that individual diabetes diet education is effective to make the patient understand their diet prescription, and is effective to control body weight and blood sugar level. Awareness of calorie requirement could be used as an index of understanding of prescribed diet. Since age is an important variable to determine the awareness of calorie requirement, different strategies of nutrition education should be developed for different age groups, especially patients over 70 years old.

General aspect of residents in Youngnam area and subjective evaluation about oral health and quality of life along with health activity (영남지역 일부 주민들의 일반적인 특성 및 건강생활에 따른 주관적인 구강보건과 삶의 질 평가)

  • This survey is to increase the quality of life by improved oral health. Total 572 Youngnam area residence joined the survey and answered for the subjects of general aspect & health activity, self-determined oral health status, oral health knowledge, oral health activity, food behavior, OHIP-14,THI, QOL. Survey was analyzed by SPSS (Ver. 12.0). Result of the study is summarized as follows; 1. Among total 572 residents, women were 58% and 20~29 aged were 35.5%, highest. In academic background, High school graduated were 59.6%. In average monthly income view, Lower than 1.5 million KRW was 43.5%, 49.3% replied drinks 1~3 times a month. No smoking group was 73.1%. No exercise group was 35.5% and every day exercise group was 11.4%. In self evaluating about oral health status question 40.4%(231) replied as bad, which was highest. 2. Women have higher score about oral health knowledge. Woman and Every day exercise group have better score about oral health practice. Man, 40~49 aged, more than 2.51 million KRW avg' income, every day exercise groups has good score about diet related question. 3. OHIP-14 has lower score as age is older, as more income. THI shows higher score from woman, 20~29, more income group and QOL has higher score from more than college educated, every day exercise groups. 4. Similar correlation indexes on QOL are OHIP-14, diet, oral health knowledge, oral health practice listed in order of effect size. Self-determined oral health index is important to improve quality of life along with health activity, especially oral health recognition index is different depends on age and monthly income that addresses it is necessary to deliver training with carefully designed oral health training program development.

Impact of Advanced Maternal and Paternal Age on Perinatal Outcome (분만여성과 배우자의 출산연령이 산모와 신생아 합병증에 미치는 영향)

  • Purpose: This study aimed to analyze the impact of advanced maternal and paternal age on perinatal outcome in Korea. Methods: We conducted a retrospective study involving 1,622 Korean women who delivered at M Woman Hospital from January to December 2010 and their spouses were included. We obtained obstetrics database which included demographic characteristics, medical and obstetrics history, course of the current pregnancy and advised perinatal outcome. Multivariable logistic regression was used to adjust for potential confounding variables. Results: Women giving birth age 35 or older were statistically significant in paternal age, gravidity, spontaneous abortion experience, method of conception, method of delivery, and multiple gestation compared to women aged <34 years. After adjusting for the confounding effects of maternal characteristics, women aged 35 or older were at increased risk for cesarean section delivery (adjusted OR 1.6, 95% CI 1.22-2.13) and preterm birth (adjusted OR 2.2, 95% CI 1.03-4.63). Conclusion: In this population of Korean women, advance maternal and paternal age is independently associated with specific adverse perinatal outcome, especially preterm birth and cesarean section delivery.

Screening for Breast Cancer in a Low Middle Income Country: Predictors in a Rural Area of Kerala, India

  • Background: In India, breast cancer is the leading malignancy among women in a majority of the cancer registries. Therefore it is important to understand screening practices and its predictors, including in rural areas with high female literacy and good health indices. Materials and Methods: A cross-sectional study with multistage sampling was conducted in Vypin Block, Ernakulam district, Kerala, India. Four Panchayats (self administration units) were randomly chosen and a woman in every second household was invited to participate from the tenth ward of each. Thus a total of 809 women were interviewed. Results: The majority of the repondents (82.1%) were not aware of risk factors and about a third (37.9%) were not aware of symptoms of breast cancer. About half of the population studied (46.6%) had undergone screening. Age (35-50 years), being married, health professionals as source of information and working were significant predictors of screening. Logistic regression showed that older women (35-50 yrs) were more likely to practice screening. Out of the never screened, about a third (35%) were desirous of doing it, but had not for various reasons and 53.5% were not willing to screen. The reasons identified for not screening among those desirous of doing it were grouped into knowledge 66 (43.4%), resources 23 (15.1%) and psychosocial 32(21.1%) factors. Unmarried women were significantly more likely to express factors related to all the three domains. Conclusions: This study showed that in spite of the absence of a population-based screening program, about half of the study population had undergone some type of screening. The older women (35-50 years) in particular were significantly more likely to practice screening. At this critical juncture, a high quality breast cancer awareness and screening initiative can help to consolidate the gains and tackle knowledge, resource and psychosocial barriers.

A late onset solitary mediastinal cystic lymphangioma in a 66-year-old woman who underwent kidney transplantation

  • Lymphangioma is a congenital abnormality of the lymphatic system detected primarily in early childhood. There are rare reports of mediastinal lymphangioma in older adults. We hereby report on a 66-year-old female patient who underwent kidney transplantation 20 years previously and who developed pathologically confirmed solitary mediastinal lymphangioma 1 year ago. Chest radiography showed a mediastinal nodule, which was not observed 2 year previously, therefore she was referred to the pulmonary division. She had no symptoms, and chest computed tomography demonstrated a 25-mm, well-defined, low-density nodule located at the anterior mediastinum. The size of the nodule had increased from 25 mm to 34 mm 1 year later, and it was completely resected via video-assisted thoracic surgery. The histological diagnosis was cystic lymphangioma. Therefore, we recommend that clinicians consider cystic lymphangioma as a possible diagnosis even in older patients with a mediastinal cystic mass that shows progressive enlargement.

A Study on Deconstructional Methods in Mordern Fashion Design and Consumer Acceptance (현대 복식 디자인에 나타난 해체주의적 표현과 그 수용에 관한 연구)

  • The purpose of this study was to examine what consumers virtyally thought about deconstructional method used in fashion and its cloths. For attaining the purpose, earlier studies were reviewed to see what kind of deconstructional methods were used in fashion and its cloth, and questionnaire survey was carried out based on the result. 10 kinds of deconstructional method were surveyed : indeterminate form, lingerie look, layering, unsymmetrical form, seethrough cloth, slash or making a hole, fray, padded cloth, patchworked cloth, and wrinkled cloth. There appeared 6 kinds of image for deconstructional fashion, esthetics, womanliness, youth, elegance, uniqueness, and fashionability. Consumers considered most deconstructional fashion to be unique but unique but lack elegance. However, they didn\`t feel indeterminate form, fray or padded cloth was unique, and one notable thing was that they had an impression wrinkled cloth was graceful. The design assessed to be generally esthetic was lingerie look, laying, seethrough cloth and wrinkled cloth. Those who were older set more esthetic value on layering, seethrough cloth, fray, padded cloth and wrinkled cloth. On the contrary, they didn\`t have a good impression from slash, making a hole or patchworked cloth. As a result of investigating the subjects\` willingness to wear deconstructional design, there was deconstructional design which they might possibly put on, and those who were older found more difficulties in wearing ir. Toward indeterminate and layering fashion, younger people whose age was ranged from 18 to 22 or from 18 to 22 or from 23 to 27 showed particularly better response. Married woman wanted to wear wrinkled cloth, and considered it possible to put on seethrough design more than unmarried women. Rather, younger people were more unwilling to wear seethrough design.

The Comparison of the Body Measurement of Chinese Adult Women by the Age -with the Focus on the Women Residing in Beijing and Shanghai- (중국 성인 여성의 연령별 신체 계측치 비교 -북경과 상해에 거주하는 여성을 중심으로-)

  • The purpose of this study is to look into the physical features for each residential area by the age, with the female subjects residing in Beijing and Shanghai. This would provide the specific information regarding the body types of Chinese adult women, and additionally it would assist the effective advancement into the Chinese local markets by serving as the basic data for the size development and fitting improvement of clothing products for Chinese women. The body measurements were made with 525 adult female subjects of 20 to 49 years of age, residing in Beijing and Shanghai, China. The number of measurements items was 79. The results are as follows: As they grew older, the women residing in Beijing and Shanghai all showed increases in the items of width, thickness, girth, and length, which are highly correlated with the physical obesity, while they did not show any differences by the age range in the item of height. Differences were also found in the tendency of the type distribution by the age range of Chinese adult women in each residential area. As they grew older, the women residing on Beijing showed increases in the distribution of Body type 2 and Body type 4, which clearly indicated the features of obesity. On the other hand, the women residing in Shanghai showed the almost similar distribution in their 20's and 30's, while those in their 40's showed a high frequency in Body type 2 than in Body type 4.

Comparison of old-old aged women's bodice pattern using 3D anthropometric data

  • The purpose of this study was to investigate the bodice prototype method suitable for the upper body shape of old-old aged women using the 3D anthropometric data. And it was to provide the basic data for the upper body garments of old-old aged women. In the overall appearance evaluation, the B pattern was rated as 4.00, and it was evaluated as the most suitable for the bodice prototype of the old-old aged woman. The E pattern was rated lower than normal, and the L pattern and the S pattern were found to be inadequate for older female bodice prototypes. As a result of the measurement of the waist and bust air gap of bodice prototype, the air gap of the bust was not significantly different between the patterns. But the waist air gap showed the largest difference between the L pattern and the S pattern. As a result of evaluating the appearance, the amount of space in the state of 3D simulation, and the air gap, the pattern B appeared to be the most appropriate prototype for the old-old aged women's body shape. However, there is a tendency that the shoulder end point is biased toward the back, so it is necessary to set the inclination of the back shoulder line to be more gentle. Conversely, the front shoulder should be more inclined. In the case of the 3D simulation, the B pattern showed that the other parts generally fit well. In the case of the 3D simulation program used in this study, it was evaluated that it is suitable only for the normal body shape because it is impossible to set the isometric angle which is one of the characteristics of the older female body shape. A study on the bodice prototype suitable for the bent body shape should be carried out through experiments on the actual body shape of various elderly women. In order to cope with the increase of elderly people who are familiar with digital, I think it is necessary to develop an avatar that reflects the old female body shape.
