A Study of Evaluation System of NATM Tunnel using Delphi and AHP (델파이 및 AHP 기법을 활용한 NATM 터널의 평가체계 연구)

  • Since the 30-year-old facility is expected to surge from 10.5% to 23.9% in 10 years, the new evaluation system is needed to establish efficient maintenance system for securing the safety and extending the life span of existing facilities. In this study, Delphi and AHP(Analytic Hierarchy Process) was conducted to provide a systematic new evaluation system for the NATM tunnel, which is the most frequently constructed structure of the existing tunnel. Since the existing assessment systems are limited in scope of evlauatuon criteria, the survey was conducted in conjunction with closed questionnaires on existing items and open questionnaires for eliciting new items. In results, suitable evaluation factors were derived for the NATM tunnel through the validation of the survey results. Also after calculating weighted value of the derived assessment item using AHP technique, a new evaluation system is proposed to meet the characteristics of the NATM tunnel, so that they can be used as reference materials for revising and supplementing detatiled guidelines in the future.

  • Carpal tunnel syndrome is rare in children. When it does occur in children, the most common causes reported are mucopolysaccharidosis and mucolipidosis. The median artery is a transitory vessel that develops from the axillary artery in early embryonic life and does not normally survive until postfetal life. In a small percentage of individuals, however, it persists into adulthood and is frequently accompanied by a bifid median nerve. A persistent median artery can be a cause of carpal tunnel syndrome in adults, but it is extremely rare in children and adolescents. This paper reports a case of a carpal tunnel syndrome caused by a persistent median artery and bifid median nerve in a 13-year-old girl.

  • Ulnar nerve compression in the cubital tunnel is a common entrapment syndrome of the upper limb. Pulsed radiofrequency lesioning (PRFL) has been reported as a treatment method for relieving neuropathic pain. Since the placement of the electrode in close proximity to a targeted nerve is very important for the success of PRFL, ultrasound seems to be well suited for this technique. A 36-year-old woman presented with complaints of numbness and pain on the medial aspect of the elbow and the pain radiated down to the $4^{th}$ and $5^{th}$ fingers for 10 years after she suffered an elbow contusion, we then scheduled this woman for the ultrasound guided PRFL of the ulanr nerve. The initial ultrasound examination demonstrated a swollen nerve, loss of the fascicular pattern and an increased cross sectional area of the ulnar nerve. After confirmation of the most swollen site of the nerve via ultrasound, two sessions of PRFL were performed. The postprocedural 10 cm visual analog scale score decreased from 8 to 1 after the two sessions of PRFL.

  • Recurrent motor branch entrapment syndrome is a compressive mononeuropathy of recurrent motor branch of median nerve. It is a rare condition as a cause of thenar muscle wasting and may have different pathogenesis. If such an anatomical variation is the cause, there is a possibility that thenar muscle atrophy remains if only the transcarpal ligament release is performed. We report a 25-year-old male patient with carpal tunnel syndrome with thenar muscle wasting 1 month ago.

  • Carpal tunnel syndrome (CTS) caused by pseudogout is an uncommon disease. The authors report a 65-year-old female who complained of sudden pain and neurological symptoms on her left hand. Surgical decompression was performed. In the histologic study, a calcium pyrophosphate dihydrate crystal deposit was confirmed. Her pain and neurological symptoms were relieved. Because CTS caused by pseudogout is rare, it is difficult to differentiate it from other diseases. This paper reports an uncommon case of CTS caused by pseudogout.

  • Double compression of the ulnar nerve, including Guyon's canal syndrome associated with cubital tunnel syndrome caused by the anconeus epitrochlearis muscle, is a very rare condition. We present a case of double crush syndrome of the ulnar nerve at the wrist and elbow in a 55-year-old man, as well as a brief review of the literature. Although electrodiagnostic findings were consistent with an ulnar nerve lesion only at the elbow, ultrasonography revealed a ganglion compressing the ulnar nerve at the hypothenar area and the anconeus epitrochlearis muscle lying in the cubital tunnel. Careful physical examination and ultrasound assessment of the elbow and wrist confirmed the clinical diagnosis prior to surgery.

  • Carpal tunnel syndrome can be produced by abnormal mass effect due to trauma, ganglion cysts, various soft tissue tumors, musculotendinous variants, and aberrant vascular structures. Persistent median artery is one of the causes of the carpal tunnel syndrome. Thrombosed persistent median artery usually accompanies the anomaly of the median nerve and causes a sudden onset of severe pain and paresthesia. In contrast to previous literature, we report the rare case of gradual onset and mild symptom of a 53-year-old man with a thrombosed persistent median artery but without anomaly of the median nerve and abnormal finding of electrophysiologic study.

  • Purpose: Median nerve entrapment syndrome within carpal tunnel is usually called carpal tunnel syndrome and it is the most common form of peripheral nerve entrapment syndrome. Many factors such as diabetes mellitus, hypothyroidism, hormonal replacement theraphy, corticosteroid use, rhematoid arthritis and wrist fractures may cause carpal tunnel syndrome. To the best of our knowledge, this is the first case report of median nerve entrapment syndrome due to adhesion of laceration wound after suicidal attempt. Methods: A 28-year-old woman presented with a sensory change and thenar hypotrophy on her left hand. On her history, she attempted suicide by slashing her wrist. Initial electromyography (EMG) showed that the nerve conduction velocities of median nerve was delayed. Therefore, we performed surgical procedures. When exploration, Fibrous scar tissue observed around the median nerve but nerve had not been injured. Transcarpal ligament was completely released and adjacent fibrous tissue was removed to decompress the median nerve. Results: The postoperative course was uneventful until the first year. Opposition difficulty and thenar hypotrophy were improved progressively after the surgery. Sensory abnormality was slowly improved over one year. Conclusion: We report a case of median nerve entrapment syndrome that was caused by adhesion of laceration wound after suicidal attempt. This is an unusual cause of median nerve entrapment syndrome, the symptoms were relieved after transcarpal ligament release and fibrous scar tissue removal.

Effect of Tunnel Covering Date on Harvesting Time and Yield of Asparagus(Asparagus officinalis L.) in Jeju (제주에서 터널 피복시기가 아스파라거스의 수확기 및 수량에 미치는 영향)

  • Optimal date to cover plastic film tunnel for hastening the harvest and increasing the yield in asparagus was studied using 2-year-old 'Green tower' cultivar in Jeju. Plastic film tunnel was covered on January 10 and 21, and February 10 and 25 in 2004. The asparagus spear sprouted on March 3 in the control. They emerged earlier by about 2 to 3 weeks in the plots covered on January 10 and 25 than the control. First harvesting was hastened by 3 weeks in the plots covered on February 10 and two weeks in the plots covered on January 25 and February 10 compared to the control. Yield per plant was the highest in plastic film tunnel covered on January 10 and 25. Early harvesting in february in plastic film tunnel covered on January 10 and 25 were 325 and 333 kg/10a respectively with no significant difference between two treatments whereas yield was low significantly in plastic film tunnel covered on February 10 and 25.

  • Fiber reinforced concrete has been used for tunnel lining and rehabilitation of old structures. Recently, structural synthetic fiber was developed to overcome the corrosive properties of steel fibers. Fibers play a role to increase the tensile and cracking resistance of concrete structures. The Post cracking behavior must be clarified to predict cracking resistance of fiber reinforced concrete. The purpose of the present study is to develop a realistic analysis method for post cracking behavior of synthetic fiber reinforced concrete members.
