신경 이식과 정맥 포장을 이용한 연속성 신경종의 치료 - 1예 보고 - (The Treatment of Neuroma-in-Continuity with Interpositional Nerve Graft and Vein Wrapping - A Case Report -)

  • We report a case of 44 years old male patient with neuroma-in-continuity of ulna nerve. In the patient's past history, he had received operative treatment for the open supracondylar fracture of right distal humerus and ulnar nerve injury at 10 years ago, and neurolysis was tried 2 times due to severe neuropathic pain. Despite of these operations, the symptom was not improved. In operative field, we noticed neuroma-in-continuity and decided to resect the neuroma until normal nerve fascicle was noted. The nerve cable graft was done with auto sural nerve on the defect site and the nerve was wrapped with small saphenous vein. At post operative 7 months, pain was markedly decreased and sensory recovery was slightly improved and patient was satisfied with the result.

Severe Hypothermia Accompanying Acute Subdural Hemorrhage

  • Hypothermia is relatively a common condition and most cases involve mild hypothermia. But severe hypothermia below $30^{\circ}C$ is medical an emergency condition. We report the case of a 41-year-old man who had been left in a manhole for more than 9 hours on a freezing cold water. He was transported to our emergency room in semicomatose state with a body temperature $26.5^{\circ}C$. The patient was warmed with active rewarming. After initial stabilization, the patient was taken for a brain computed tomography and found to have large fronto-temporo-parietal[FTP] subdural hemorrhage. The patient underwent an emergent decompressive craniectomy and hematoma evacuation. After surgery, he recovered to drowsy mentation and vital signs were stable.

  • The recurrent intracerebral hemorrhage[ICH] in hypertensive patients is not an unusual event, but the simultaneous occurrence of multiple ICH is rare. A 70-year-old woman presented with bilateral simultaneous hypertensive intracerebral hemorrhages in both thalami. The complaints of the patient were unconsciousness [semicomatose mental state] and quadriparesis Grade II. The patient was managed conservatively. At discharge, the patient was awake [drowsy mental state], but Grade III according to the Glasgow Outcome Score. This paper reviews the clinical relevance, possible etiology, and treatment of bilateral thalamic ICH.

Kimura병 환자의 전신마취 사례 (Anesthetic Management of a Patient with Kimura's Disease)

  • Kimura's disease is an idiopathic chronic condition, associated with a high-titer of IgE and peripheral eosinophilia. It frequently presents as a solitary or multiple lesions in the head and neck area. During the perioperative period, anesthesiologists should understand the anatomical structures of the patient who has Kimura's disease involvement of the head and neck, especially the airway. It is important to pay attention to the occurrence of signs and symptoms of acute allergic reactions related to a high-titer of IgE and eosinophilia. We report our experience with anesthetic management in an 18-year-old patient with multiple neck masses due to Kimura's disease.

Rhinolith: An incidental radiographic finding

  • Rhinoliths are foreign bodies composed of mineralised deposits in the nasal cavity that are rarely encountered in routine dental practice. The current report presents a case of a 77-year-old female patient who visited a dental treatment centre for a routine dental check-up and prophylaxis. On the clinical examination, the patient did not report any breathing difficulty, nasal obstruction, or any form of discharge. Panoramic radiography showed a horizontally magnified radiopaque structure in the right nasal fossa region. Cone-beam computed tomography (CBCT) revealed a hyperdense entity with a laminated presentation in the right nasal fossa. The hyperdense entity was diagnosed as a rhinolith based on the radiographic findings. Although the patient was asymptomatic, a referral was made to an ear, nose, and throat surgeon for a further evaluation. This report highlights the importance of CBCT imaging in the diagnosis of soft-tissue calcifications.

Leaflet Fracture and Embolization of a CarboMedics Prosthetic Mitral Valve: Case Report

  • Fracture of prosthetic valve leaflets in the absence of traumatic injury is very rare. Leaflet fracture can cause acute pulmonary edema and cardiogenic shock and is potentially life-threatening, requiring emergency surgery. Thus, a leaflet fracture must be diagnosed quickly and accurately. We present the case of a 46-year-old man with CarboMedics prosthetic aortic and mitral valve replacements implanted 24 years previously. The patient presented at our emergency department with abrupt dyspnea and fever. We diagnosed severe mitral valve regurgitation with anterior leaflet fracture. The patient underwent venoarterial extracorporeal membrane oxygenation and delayed mitral valve replacement. The foreign body was removed step by step because the diagnosis was missed. Two pieces of broken leaflets were found in the left common iliac artery and left external iliac artery. The patient was treated successfully and remains asymptomatic 1 year following surgery.

Injury of Corticostriatal Tract between the Striatum and the Premotor Area in a Patient with Traumatic Brain Injury

  • Objectives: We investigated injury of corticostriatal (CStr) tract in patient with mild traumatic brain injury (mTBI), which was demonstrated by DTT. Method: A 44-year-old female with no previous history of neurological, physical, or psychiatric illness had suffered from head trauma resulting from a pedestrian car accident. She complained that could not quickly move the left hand with her intension. After three month's administration, her slowness movement of left hand recovered rapidly to the point that she was able to extend all fingers quickly. Results: On DTT configuration, the integrity of the left CStr tract was well-preserved, however the right CStr tract showed narrowing and partial tearing in the subcortical white matter on a DTT at 25 months after onset. Conclusion: Injury of the right CStr tract was demonstrated in a patient who developed mild motor control problems following mild TBI. We believe that the evaluation of the CStr tract from the secondary motor area for patients who showed unexplained motor control problem is necessary.

Functional abdominal pain syndrome treated with Korean medication

  • A 37-year-old female patient with chronic and stubborn abdominal pain had been hospitalized five times in three Western hospitals, but no effects were observed. No abnormalities were found in blood tests, gastrointestinal endoscopy, sonogram, and computed tomography of the abdomen, except mild paralytic ileus. The patient decided to rely on Korean medicine as an inpatient. She was diagnosed with functional abdominal pain syndrome, and her symptom differentiation was the "Yang deficiency of spleen and kidney." A herbal drug, Hwangikyeji-tang, along with moxibustion and acupuncture, was given to the patient. Abdominal pain and related symptoms were reduced radically within 16 days of treatment. This report shows a therapeutic potential of Korean medicine-based treatment for functional abdominal pain syndrome.

Malignant melanoma on a thermal burn scar

  • Chronic burn scars often cause various skin malignancies at rates of up to 2%. These lesions are usually squamous cell carcinomas, but rarely, malignant melanoma is reported. We report a 67-year-old male with a malignant melanoma on a burn scar with regional metastasis. This patient presented an ulcerative lesion only in 2 weeks. After histopathological diagnosis, we performed only palliative surgery on patient's demand, and followed up the subsequent deterioration course. Our case reemphasizes the need for rapid diagnosis and treatment when suspect lesions are present on chronic burn scar. Also, physician should be in mind and inform the patient about malignant melanoma and its aggressive course.

이물질 섭취 및 흡인 소아환자의 엑스선 영상 평가 (Evaluation of Radiography of Ingested and Aspirated Foreign Bodies in Pediatric Patients)

  • The purpose of this study was to introduce the radiography for the natural course and clinical diagnosis of foreign body ingestion and aspiration, to help diagnosis and treatment, to evaluate the accuracy of radiographic images of pediatric patients. A 2 to 7 year-old patient who ingested a foreign body was ingested and aspirated with foreign substances such as coin, cloth pin, earring, baduk stone, and hairpins, and chest and abdomen of the plain radiography. The pediatric patient who ingested and aspirated the foreign body of the coins, the clothespins, the earrings, the stones, and the hairpins were examined by chest and abdomen of the plain radiography and fluoroscopic images. The radiography examination can be combined to effectively cope with the treatment and the treatment of the foreign substance removal. It can be applied to the diagnosis of foreign body in pediatric patient's clinic and appropriate treatment and treatment direction.
