MMPI를 통해 관찰한 동통장애 환자 증례 (Clinical study on a case of Pain Disorder with MMPI)

  • Pain disorder is the chronic syndrome that onset and change of pain is associated with mood change. But the patient of pain disorder can't perceive their inner conflict. We experienced a 58 year-old woman who had general pain, and whose condition was improved through oriental medical treatment. In this case, we assessed the pain disorder patient at MMPI. So we learned out the patient's inner conflict and dynamic. The applicability of understanding with MMPI has positive effects on the patient with Panic disorder.

수술후 통증관리 환자에서 Scopolamine Patch에 의해 발생한 중추항콜린성증후군 -증례 보고- (Central Anticholinergic Syndrome by the Use of Scopolamine Patch -A case report-)

  • A 77-year-old male patient was admitted to the hospital with acute appendicitis and underwent emergency appendectomy under inhalation anesthesia without any complications. After the operation, the patient was placed on epidural analgesia with the mixture of fentanyl-bupivacaine and scopolamine patch for nausea and vomiting. The patient experienced central anticholinergic syndrome (CAS) with hallucination, confusion, somnolence, dysarthria and delirium at 20 hours after patch placement and recovered from all symptoms of CAS in 24 hours after the removal of the patch. The CAS occurs when central cholinergic sites are occupied by specific drugs and also as a result of an insufficient release of acetylcholine. The diagnosis of CAS is often determined by a process of exclusion and not actually made until a positive therapeutic response to physostigmine, a centrally active anticholiesterase agent, has taken place. Treatment of CAS includes prompt removal of the patch, cleansing of the area, and physostigmine administration. The scopolamine patch should be used with extreme caution in elderly patient.

Avoiding Venous Anastomotic Dehiscence of an Arteriovenous Graft in a Super-Obese Patient

  • Surgeons avoid creating arteriovenous fistulae in obese patients owing to deep vessels, cannulation complications, and inconsistent outcomes. We describe placing an arteriovenous polytetrafluoroethylene (PTFE) graft between the brachial artery and axillary vein to avoid these complications. A 39-year-old super-obese woman with end-stage renal disease had undergone several hemodialysis access procedures on both arms. We traced the course of the arteriovenous graft course with the patient sitting and lying down. The ideal course was more accurate with the patient sitting; thus, the patient sat when the course was drawn, before lying on the operating bed. The PTFE graft was placed between the right brachial artery and axillary vein, according to the course in the opposite arm. No anastomotic dehiscence or pseudoaneurysm has taken place during 2 years of follow-up. In super-obese patients, the ideal course for arteriovenous grafts should be drawn while they are sitting, avoiding skin folds. This tip could avoid anastomotic dehiscence and pseudoaneurysm between the axillary vein and a PTFE graft.

기훈 환자의 보허음 투여 후 호전 1례 (An Improved Case after Treating Chi-Vertigo Patient with Boheo-eum)

  • The subject was a 46-year-old woman who was suffering from severe vertigo. Her major symptoms were vertigo, headache, neck pain, feverishness, and her minor symptoms were intermittent nausea and tinnitus, mild chest discomfort, facial flushing and sweating. Her symptoms were diagnosed as Chi-Vertigo and the patient was treated with Boheo-eum(Buxu-yin). Next, the patient's vertigo was evaluated with Vestibular disorders activities of Daily Living scale and Dix-Hallpike maneuver per 3days. An improved state in vestibular disorders activities of daily living scale, a frequency and duration of Dix-Hallpike maneuver were observed. In this case Boheo-eum(Buxu-yin) has an effect on a patient with vertigo due to Chi-Vertigo.

  • Objectives The purpose of this study is to report the measurement results of Duchenne Muscular Dystrophy (DMD) in Patient Using Sensitiv $Imago^{TM}$ (SI) and Ryodoraku. Methods We conducted SI test and Ryodoraku test to a 7-year-old DMD patient who visited to Oriental pediatrics, Kyung Hee Medical Center. Results We obtained SI and Ryodoraku test results from a DMD patient. Conclusions Further study with more samples is necessary to establish accuracy of SI in clinical use.

Frontal Sinus Mucocele with Massive Skull Destruction

  • A 63-year-old female complained of left frontal headache and swelling for several months. Physical examination revealed left supraorbital soft, nontender, nonpulsatile mass without bruit. The left eye was displaced downward with respect to the normal right globe. Based on the clinical and radiological findings, the patient was diagnosed as a mucocele arising from the left frontal sinus. The patient underwent a transcranial approach through coronal incision. In this patient, large portions of the anterior and posterior frontal sinus walls were destroyed in association with epidural spread, so we performed cranialization of the frontal sinus and removed the mucosal wall with the aid of a microscope. With a brief review we present a patient with mucocele of the frontal sinus extending into the intracranial and intraorbital region, which was successfully treated by a transcranial approach.

연구개암 환자에서의 상악신경차단 - 1예 보고 - (Maxillary Nerve Block for Patient with Soft Palate Cancer Pain - A case report -)

  • In early stages, cancer is not usually painful. However, many patients with recurrent or metastatic cancer eventually experience pain, which becomes progressively worse. Chemotherapy, sympathetic surgery, physical therapy and nerve block can be used to control cancer pain. A 60-year old patient had severe pain of the soft palate due to squamous cell carcinoma. We successfully treated this patient with maxillary nerve block using pure alcohol by a lateral approach. Four months after maxillay nerve block, the patient is still pain free.

Pneumocephalus in a Patient with No Cerebrospinal Fluid Leakage after Lumbar Epidural Block - A Case Report -

  • Several complications are possible after a lumbar epidural block. However pneumocephalus are rare. In this case, we report a case of pneumocephalus. A 68-year-old male patient received lumbar epidural block with the loss of resistance technique using air, and after 35 minutes, apnea, unconsciousness, hypotension, and bradycardia occurred. Immediately, brain CT was done, and we found pneumocephalus. The patient complained of severe occipital headache and itchiness due to pneumocehalus. After conservative treatment, the patient recovered without neurologic complications, and on the seventh day of his hospitalization, he was discharged from the hospital.

신장이식 환자에 있어서 Cyclosporin-A에 의한 치은비대의 치험례 (A Case Report on the Treatment of Cyclosporin-A Induced Gingival Enlargement in Renal Transplant Patient)

  • The authors treated a 58-year old female patient who had come to the Department of Oral Medicine, KNUH due to the chief complaints of gingival enlargement and bleeding on the upper and lower jaw. The lesions were diagnosed as cyclosporin-A induced gingival enlargement by patient's history and clinical examination. The patient was treated with gingivectomy using pulsed Nd:YAG laser. After gingivectomy the wound was compressed with 0.1% chlorhexidine-soaked gauze to prevent relapse of the lesion. Good healing process was observed and there were no recurrences until 3-month follow-up visit. From the results of this clinical trial it was suggested that a pulsed Nd:YAG laser gingivectomy would be helpful for the treatment of cyclosporin-A induced gingival enlargement in renal transplant patients.

Management of obstructive sleep apnea in a Treacher Collins syndrome patient using distraction osteogenesis of the mandible

  • In this study, we present the surgical treatment of obstructive sleep apnea in a child with Treacher Collins syndrome. A 10-year-old girl with a past history of Treacher Collins syndrome presented to our clinic with her parents for respiratory distress and insomnia. The patient was referred to a sleep laboratory where she was diagnosed with obstructive sleep apnea, which was a consequence of her Treacher Collins syndrome. The patient underwent mandibular distraction osteogenesis under general anesthesia. The mandible was expanded by 15 mm using internal bilateral distractors. After distraction osteogenesis, the patient's respiratory problems resolved, and she was able to sleep comfortably. Distraction osteogenesis was an effective method of advancing the mandible, increasing the upper airway space and ultimately preventing obstructive sleep apnea syndrome in patients with Treacher Collins syndrome.
