Imperforate Hymen Causing Hematocolpos and Acute Urinary Retention in a 14-Year-Old Adolescent

  • We report the case of a 14-year-old girl who visited the emergency room because of suprapubic discomfort and sudden acute urinary retention. She did not have any significant medical and surgical history, and her neurological examinations were all normal. Urinary catheterization led to the passage of 500 mL urine. Abdominal ultrasonography showed a hematocolpos that was compressing the urinary bladder. Gynecologic history taking revealed that the patient has not had menarche yet. Therefore, a cruciate incision was performed and her urination became normal. As the surgical outcome after adequate hymenotomy for imperforate hymen is usually good, the diagnosis of imperforate hymen is important. However, this condition is easily missed in the clinic because the first physician visited by the patient rarely takes a detailed gynecologic history or performs appropriate physical examinations. Although rare, imperforate hymen should be considered as a cause of acute urinary retention in the adolescence period. If an adolescent girl presents with abdominal pain and voiding dysfunction, a detailed gynecologic history and appropriate physical examinations of the genital introitus should be performed.

Two Pediatric Patients with Herbal Medicine-Induced Nephrotic Syndrome

  • Nephrotic syndrome (NS) is a common chronic disease in children; in 90 percent of cases, the condition is primary (idiopathic). Toxic nephropathy can be induced by herbal medicines, and is mainly manifested as tubulointerstitial nephritis and rarely, as glomerulopathy. Here in, we describe two cases of steroid-sensitive NS, which developed after the patients received herbal medicines. A 5-year-old boy and an 8-year-old girl were separately admitted within a short time period with acute onset of generalized edema, proteinuria, hypoalbuminemia, and hypercholesterolemia. Each patient had previously taken herbal medicine, which had been prescribed by different oriental medical clinics for different conditions. The patients were diagnosed with herbal medicine-induced NS and were treated empirically by a standard steroid therapy, with subsequent resolution of their NS. One patient relapsed, but her NS again responded to steroid therapy. We described two unusual cases of prototypical pediatric, steroid-sensitive NS, which was presumed to be minimal-change disease that developed after the administration of herbal medicines. We also reviewed the literature.

  • Burkholderia cepacia, a widespread gram-negative environmental bacillus associated with nosocomial infection, is considered to be of relatively low virulence and rarely to cause invasive disease in immunocompromised patients. Nosocomial infections resulting from the use of contaminted medication, antiseptics and instruments have also been reported in otherwise healthy hosts. We experienced two cases of B. cepacia sepsis in 10 year-old male who was medicated with the anticancer drugs for the treatment of acute lymphoblastic leukemia(ALL) and in 15 day-old newborn who was examined with voiding vesicourethrography(VCUG) for the evaluation of congenital hydronephrosis. The organism isolated from serial blood culture in ALL patient and from serial blood culture and urine culture in newborn examined with VCUG. The former ALL patient improved after antibacterial medication of imipenem and the latter newborn improved after treatment with imipenem and trimethoprim-sulfamethoxazole.

  • Pulmonary hamartoma is often incidental, asymptomatic finding on routine chest roentgenogram. It has been considered a congenital malformation. Since the original description by Albrecht in 1908, it has been classified into two types, a small, fibrocartilaginous mass in adults, and a cystic lobar mass in infants. We experienced two cases of pulmonary hamartoma which proved to be the adult form of hamartoma. One was located in left upper lobe of a 58 year old male patient, the other was located in the perihilar region of the right middle lobe of a 38 year old male patient. The former case was treated by wedge resection: the latter by right middle lobectomy and the postoperative courses of both cases were very good and without complication.

  • Solitary plasmacytoma is a rare malignant neoplasm that originate in immunoglobulin-producing plasma cell. Solitary bone plasmacytoma can be found at any site throughout the skeleton and in most cases eventually progresses to multiple myeloma, of which it is thought to be an unusual presentation. On the other hand, extramedullary plasmacytoma has a tendency to occur in the head and neck region, mainly in association with the upper air passages. Incisional biopsy is the primary approach to make a definitive diagnosis, and immunohistochemical staining can be very helpful in understanding the nature of these tumors. We report a 66 years old patient with solitary bone plasmacytoma and a 36 years old patient with solitary extramedullary plasmacytoma with literatures review.

Epidural Abscess Following Epidural Catheterization -Two cases- (경막외 카테테르 거치후 발생한 척수경막외 농양 -증례 보고-)

  • Epidural abscess is associated with placement of epidural catheter is very rare. We experienced two cases of epidural abscess formation after placement of epidural catheter for pain management. A 63 years old female patient received thoracic epidural catheterization for management of pain due to herpes zoster on right T4 dermatome. Two weeks after catheterization, she complained of paraparesis and anesthesia below $T_4$ dermatome. Four weeks later magnetic resonance images was performed and revealed epidural abscess on $T_2-T_5$. Emergent decompressive laminectomy was performed but neurologic symptoms were not improved. In other case, a 75 years old male patient received lumbar epidural catheterization for management of Buerger's disease. About on month later, pus was aspirated from lumbar epidural space. But further evaluation could not be achieved because he wanted to discharge against advice. We emphasize that epidural abscess results sequele serious and prompt diagnosis and treatment is important.

A Case Report of Hwa-Byung with Menopausal Symptoms (갱년기 장애를 동반한 화병 환자 치험 1례)

  • Objectives : This study was performed to report the effect of Korean traditional treatments for Hwa-Byung with menopausal symptoms. Methods : In this case, a 48 years old female patient has been suffered by Hwa-Byung with menopausal symptoms for 3 years old. We treated the patient with Korean traditional treatments (acupuncture, herb-medication and aroma theraphy), mindfulness and Korean traditional psychotherapy. The effects of treatment were measured by Beck Depression Inventory(BDI), Zung Depression Rating Scale(SDS), Kupperman Index(KI), Menopause Rating Scale(MRS). Results : After treatment, the chief complaints and accompanying symptoms were subsided and improved. And BDI, SDS, KI and MRS score were decreased. Conclusions : According to this study, the Korean traditional treatment such as acupuncture, herb-medication is effective for the cure of Hwa-Byung with menopausal symptoms.

  • Advanced age is known to be a risk factor for early mortality after heart transplantation and is considered to be a relative contraindication. However, recent studies have shown that there are no significant differences in early and midterm survival rates between older and younger recipients. With rising life expectancy and improvements in medical support, the demand for heart transplantation in elderly patients continues to grow. We present a successful case of heart transplantation in a 78-year-old patient.

Recurrent hemoptysis in a 26-year-old woman with a ground-glass opacity lesion of the lung

  • Hemoptysis is a major reason for emergency department (ED) visits. Catamenial hemoptysis (CH), a rare condition of thoracic endometriosis, can cause recurrent hemoptysis but is difficult to diagnose in the ED due to the scarcity of cases and nonspecific clinical findings. We report a case of a 26-year-old woman who presented to the ED with recurrent hemoptysis since 2 years without a definite cause. Her vital signs and blood test findings were unremarkable. Chest computed tomography (CT) did not show any specific lesions other than a non-specific ground-glass opacity pattern in her right lung. She was on day 4 of her menstrual cycle and her hemoptysis frequently occurred during menstruation. Although there was no histological confirmation, based on her history of hemoptysis during menstruation and no other cause of the hemoptysis, the patient was tentatively diagnosed with CH and was administered gonadotropin-releasing hormone. She had no recurrence of hemoptysis for 3 months. While CH is difficult to diagnose in the ED, the patient's recurrent hemoptysis related to menstruation was a clue to the presence of CH. Therefore, physicians should determine the relationship between hemoptysis and menstruation for women of childbearing age presenting with repeated hemoptysis without a definite cause.

Wound Infiltration in Patients with Chronic Pain after Forehead Lift Surgery -A case report- (이마주름제거술 후 발생한 만성통증의 치료경험 -증례보고-)

  • Local infiltration in surgical wounds can reduce postoperative pain with very few side effects. We experienced two cases of chronic pain after forehead lift surgery. A 41-year-old male patient was suffering from a headache at the crown of the head and pain from the posterior neck. A 54-year-old female patient had pain around the forehead, worsened by psychological stress. They underwent forehead lift surgery 10 years and 8 months ago, respectively. Conservative treatments such as medication and physical therapy were not effective. After wound infiltration with 1% lidocaine more than ten times, pain significantly resolved in both patients.
