Intraoperative periprosthetic humeral fractures are a rare but debilitating complication of reverse shoulder arthroplasty and can occur during multiple stages of the procedure. Prior biceps tenodesis has been found to reduce cortical humeral strength and predispose the patient to humeral fracture. We present a case of a 68-year-old female with a previous history of biceps tenodesis due to an irreparable rotator cuff tear. Months later, and after symptoms persisted, a reverse shoulder arthroplasty was performed. During the surgery and while performing final reduction, a fracture line was observed involving the hole used for the previous tenodesis procedure. The fracture was repaired, and the patient reported favorable outcomes. We report several factors that might have contributed to sustenance of this intraoperative fracture including prior biceps tenodesis, use of a press fit humeral stem, and the sex of the patient. Azygos vein injuries are rare consequences of blunt trauma. When there is high drainage output from a right-sided intercostal catheter, an azygos injury must be considered in the differential diagnosis. We report the case of a 38-year-old male patient involved in a fall from a height during a motorcycle accident. Computed tomography demonstrated a large right-sided hemothorax and left-sided pneumothorax. The patient was transferred to the operating theatre and underwent a clamshell thoracotomy. A laceration in the azygos vein at the confluence of the arch of the azygos and the right superior intercostal vein was identified. Bleeding was controlled at the trifurcation. The patient survived and was discharged home on postoperative day 15.

Fungal thrombophlebitis of the central vein is a rare, life-threatening disease associated with significant morbidity and mortality. It requires immediate central venous catheter removal and intravenous antifungal therapy, combined in some cases with either anticoagulation or aggressive surgical debridement. A 70-year-old male patient injured by a falling object weighing 1,000 kg was transferred to our hospital. A contained rupture of the abdominal aorta with retroperitoneal hematoma was treated with primary aortic repair, and a small bowel perforation with mesenteric laceration was treated with resection and anastomosis. After a computed tomography scan, the patient was diagnosed with thrombophlebitis of the left internal jugular vein and brachiocephalic vein. Despite antifungal treatment, fever and candidemia persisted. Therefore, emergency debridement and thrombectomy were performed. After the operation, the patient was treated with an oral antifungal agent and direct oral anticoagulants. During a 1-year follow-up, no signs of candidemia relapse were observed. There is no optimal timing of surgical treatment for relapsed fungal central thrombophlebitis. Surgical treatment should be considered for early recovery.

Objectives: The purpose of this study is to report the effect of Korean medicine on trigeminal neuralgia in a patient with systemic sclerosis. Methods: We treated a 49-year-old male patient who has systemic sclerosis and trigeminal neuralgia by Korean medicine. He was treated from October 29th, 2022 to November 12th, 2022. The treatment applied was herbal medicine, acupuncture, pharmacopuncture. The evaluation was conducted by Visual Analogue Scale (VAS). Results: After 14 days of treatment, the symptoms were alleviated. Facial pain and dullness of taste improved. VAS of discomfort was decreased from 10 to 2. Conclusions: These results showed that Korean medicine may have an effect on reducing symptoms of systemic sclerosis and trigeminal neuralgia.

The authors have observed a case of spontaneous closing of anterior diastema, after removal of impacted paired simple cornical mesiodenses, without active orthodontic treatement in a 8-years-old male patient. It is important that we should periodically check up and early detect them form the age of deciduous dentition.

We experienced a case of congenital tricuspid stenosis. The patient was 5 years old boy. The 2D-echo 4 cardiac catheterization with cineangiogram revealed as Tricuspid Stenosis, associated with functioning patent foramen ovale & mild R.V hypoplasia. The lesion was successfully corrected by TV commissurotomy & closure of PFO. Postoperatively, moderate degree of tricuspid regurgitation was remained.

We recently experienced a case of bronchogenic cyst associated with situs inversus and partial pericardial defect. The patient was 26-day-old-male who showed severe respiratory difficulty. Left upper lobectomy and direct suture of partial pericardial defect were undergone successfully, but his postoperative course was unfortunate because of respiratory insufficiency.

A Case of pemphigus bulgaris in buccal mucosa of 48 years old Korean female was reported. Final diagnosis was determined by evaluation of clinical and histopathological finding. The patient could be treated successfully by administration of prednisolone, injection of bethametasone, application of oramedy and irradiation of soft laser.
