  • Treacher Collins 증후군(TCS)은 두개안면 발육의 이상을 보이는 상염색체 우성 질환으로, 외이, 중이 및 이소골(auditory ossicle)의 형태이상과 상악골 발육 부전, 후퇴된 하악, 구개열 등의 특징을 보인다. TCS 환아의 경우, 청각 장애로 인한 환아의 불안으로 적절한 협조를 얻기 어렵고, 개구제한과 기도 확보의 어려움으로 인해 전신마취 하에 치과치료를 진행하는 것이 안전하다. 전신마취 시 하악 후퇴 등의 형태학적인 문제로 인하여 삽관의 난이도가 높을 수 있어 주의가 요구되며, 술 후에도 적절한 호흡의 유지를 위해 지속적인 감시가 필요하다. 마지막으로, 보호자로 하여금 구강 위생의 중요성이 강조되어야 한다.

서울지역 미세먼지가 호흡기계 질환으로 입원한 환자에 미치는 영향 (The Effects of PM10 on the Hospital Admission of Patients with Respiratory Disease in Seoul, Korea)

  • 서울지역의 호흡기질환으로 입원한 환자를 대상으로 미세먼지 노출에 대한 건강영향을 평가하였다. 건강보험공단의 2002-2010년 동안 표본코호트의 만성폐쇄성 폐 질환(COPD), 천식 및 폐렴과 같은 호흡기 질환으로 입원한 13,974명의 환자를 대상자로 하였다. 추적관찰 기간동안 미세먼지 농도가 50ug/m3 이상에서 10ug/m3 증가할 때 15세 미만의 연령층에서는 1.38%, 65세 이상의 연령층에서는 1.62%, 75세 이상 연령층에서는 2.87% 호흡기질환으로 입원이 증가하였고, 폐렴환자는 1.50%, COPD 환자는 1.51%, 폐렴 및 천식환자는 1.55% 입원이 증가하였다. 또한 미세먼지가 80ug/m3 이상에서는 65세 이상 연령층에서 3.71%, 75세 이상 연령층에서 4.25% 입원환자가 증가하였다. 높은 미세먼지농도와 호흡기 질환으로 입원한 환자들과, 특히 노인에서 관련성이 높게 나타났다. 이미 호흡기 질환이 있었던 사람들, 특히 나이가 많은 환자는 고농도의 미세먼지에 노출되지 않도록 주의해야 한다.

한국 노인의 넘어짐과 연계된 인체손상에 대한 이해와 예방: 체계적 문헌 고찰 (Understanding and Prevention of Fall-related Injuries in Older Adults in South Korea: A Systematic Review)

  • Background: Fall-related injuries in older adults are a major health problem, and the risks and mechanisms of these injuries should be affected by race, culture, living environment, and/or economic status. Objects: Research articles have been systematically reviewed to understand fall-related injuries in older adults in South Korea. Methods: 128 published research papers have been found through the Korea Citation Index and the Korean Studies Information Service System, and reviewed in various perspectives, including incidents, fall death rates, medical costs, causes, injury sites and types, locations where falls occurred, prevention strategies, scholarly fields interested in fall injuries, and the role of physical therapy. Results: Fall-related injuries were found to be more common in women than in men, and the number of incidents increased with age, with the highest rate found in individuals over 85 years old. Risk of fall injury was associated with education level, comorbidities, and fear of falling. Common places where falls occurred included the bathroom, living room, stairs, and hallway. Common types of injury included bruises, fractures, and sprains in the lower extremities. Intervention strategies included exercise programs, education, and protective clothing. Scholarly fields interested in fall-related injuries in older adults included medicine, nursing, physical therapy, occupational therapy, physical education, pharmacology, oriental medicine, biomedical engineering, design, clothing, and textiles. Physical therapy intervention using proprioceptive neuromuscular facilitation has been used to improve one's balance. Conclusion: Any movement during the activities of daily living can lead to a fall. Physical therapists are highly educated to analyze human movements and should be involved in more research and practices to solve fall-related injuries in older adults.

Assessment of Mild Cognitive Impairment in Elderly Subjects Using a Fully Automated Brain Segmentation Software

  • Purpose: Mild cognitive impairment (MCI) is a prodromal stage of Alzheimer's disease (AD). Brain atrophy in this disease spectrum begins in the medial temporal lobe structure, which can be recognized by magnetic resonance imaging. To overcome the unsatisfactory inter-observer reliability of visual evaluation, quantitative brain volumetry has been developed and widely investigated for the diagnosis of MCI and AD. The aim of this study was to assess the prediction accuracy of quantitative brain volumetry using a fully automated segmentation software package, NeuroQuant®, for the diagnosis of MCI. Materials and Methods: A total of 418 subjects from the Korean Brain Aging Study for Early Diagnosis and Prediction of Alzheimer's Disease cohort were included in our study. Each participant was allocated to either a cognitively normal old group (n = 285) or an MCI group (n = 133). Brain volumetric data were obtained from T1-weighted images using the NeuroQuant software package. Logistic regression and receiver operating characteristic (ROC) curve analyses were performed to investigate relevant brain regions and their prediction accuracies. Results: Multivariate logistic regression analysis revealed that normative percentiles of the hippocampus (P < 0.001), amygdala (P = 0.003), frontal lobe (P = 0.049), medial parietal lobe (P = 0.023), and third ventricle (P = 0.012) were independent predictive factors for MCI. In ROC analysis, normative percentiles of the hippocampus and amygdala showed fair accuracies in the diagnosis of MCI (area under the curve: 0.739 and 0.727, respectively). Conclusion: Normative percentiles of the hippocampus and amygdala provided by the fully automated segmentation software could be used for screening MCI with a reasonable post-processing time. This information might help us interpret structural MRI in patients with cognitive impairment.

우울증에 대한 구강건강 및 관리행태 위험요인 (Oral Health and Oral Health Behavior as Risk Factors for Depression)

  • 본 연구는 국민건강영양조사 7기(2016~2018) 자료를 이용하여 의사로부터 우울증 진단을 받았거나 우울증 선별도구(PHQ-9)를 바탕으로 우울증 분류된 대상자의 구강건강 상태와 구강 건강관리행태를 비교하고 관련 위험요인들을 분석하였다. 연구에 사용된 국민건강영양조사 7기 전체 대상자 수는 16,489명이며, 그중 성인(19세 이상)이며 구강검진을 시행한 13,199명을 대상으로 자료를 분석하였다. 일반적 특성 중 성별, 연령, 소득수준, 교육수준, 흡연과 음주 모두에서 우울증군과 비우울증군 간에 유의미한 차이를 확인하였다. 또한, 구강검진 및 구강 건강상태에서 비우울증군과 비교하여 우울증 군에서 말하기 불편감(OR=2.82), 저작불편감(OR=2.41), 치통경험(OR=1.49), 보철물(OR=1.33)이 높은 것으로 나타나 우울증 발생 위험도가 증가하는 것으로 관찰되었으나, 가공의치 필요 수는 우울증 발생관련 위험요인이 아닌 것으로 확인되었다. 심리적 요인에 있어서는 전신건강이 나쁘다고 인식할수록 우울증 발생 유병률이 증가하였으며(OR=9.10), 구강건강이 나쁘다고 느끼는 경우도 증가하여(OR=1.94) 우울증 발생 관련 위험요인으로 확인되었다. 구강 건강관리 행태에서는 일일 칫솔질 횟수가 감소할수록(OR=1.81), 치실을 사용하지 않고(OR=1.42) 구강검진을 받지 않는 사람 일수록(OR=1.31) 우울증 발생 위험도가 증가함이 확인되었다. 따라서 구강건강 상태 및 구강건강 행태는 성인 우울증의 잠재적 위험요인으로 인지되며, 이를 바탕으로 성인 우울증 관리에 있어서 일반적 정신건강뿐만 아니라 구강건강과 구강관리행태 고려한 성인건강 관련 프로그램이 필요할 것으로 생각된다.

Correlation analysis of muscle amino acid deposition and gut microbiota profile of broilers reared at different ambient temperatures

  • Objective: Temperature could influence protein and amino acid deposition as well as gut microbiota profile and composition. However, the specific effects of ambient temperature on amino acids deposition and gut microbiota composition remain insufficiently understood. Methods: A total of 300 one-day-old Avian broilers were randomly divided into three groups and reared at high, medium, and low temperature (HT, MT, and LT), respectively. Breast muscle and fecal samples were collected for amino acid composition analysis and 16S rRNA gene sequence analysis. Results: Our data showed that compared to the MT group, there was a decrease of muscle leucine and tyrosine (p<0.05), as well as an increase of methionine in the HT group (p<0.05) and a decrease of serine in the LT group. Examination of microbiota shift revealed that at genus level, the relative abundance of Turicibacter and Parabacteroides was increased in the HT group (p<0.05) and that the relative abundances of Pandoraea, Achromobacter, Prevotella, Brevundimonas, and Stenotrophomonas in the LT group were higher than those in the MT group (p<0.05). In addition, there were substantial correlations between microbes and amino acids. In the HT group. Turicibacter was negatively correlated with aspartic acid and tyrosine, whereas Parabacteroides was positively correlated with methionine (p<0.05). In the LT group, there were multiple positive correlations between Achromobacter and arginine, isoleucine or tyrosine; between Prevotella and cysteine or phenylalanine; between Brevundimonas and cysteine; and between Stenotrophomonas and cysteine as well as a negative correlation between Stenotrophomonas and serine. Conclusion: Our findings demonstrated that amino acid content of breast muscle and intestinal microbiota profile was affected by different ambient temperatures. Under heat exposure, augmented abundance of Parabacteroides was correlated with elevated methionine. Low temperature treatment may affect muscle tyrosine content through the regulation of Achromobacter.

응급실 손상환자 심층조사 자료를 이용한 2017-2018년 중독 환자의 분석 (Analysis of Poisoning Patients Using 2017-18 ED Based Injury in-depth Surveillance Data)

  • Purpose: The annual statistics for poisoning are reported based on the data from poison control centers in many advanced countries. In 2016 a study was conducted to analyze the 2016 Korea Poisoning status. This study was conducted to make a better annual report for poisoning statistics in Korea from a 2017-2018 national representative database. Methods: This study was a retrospective analysis of poisoning patients based on the data from an emergency department (ED) based injury in-depth surveillance project by the Korea Centers for Disease Control and Prevention in 2017-2018. Bite or sting injuries were not included. Results: A total of 17714 patients presented to 23 EDs because of poisoning. Adults above 20 years old age accounted for 84.6% of the population, while the proportion of intentional poisoning was 60.8%. The poisoning substance presented in the ED were therapeutic drugs (51.2%), gas (20.3%), pesticides (16.4%), and artificial substances (11.4%). Overall, 35% of patients were admitted for further treatment. The mortality was 2.4% (422 cases), and the most common fatal substances in order were carbon monoxide, other herbicides, and paraquat. Conclusion: This study showed the 2017-2018 status of poisoning in Korea. The prognosis is different from the cause of poisoning and the initial mental state of the patient. Therefore, appropriate methods for preventing poisoning and therapeutic plans in specific situations are needed.

Evaluation of Near-infrared Fluorescence-conjugated Peptides for Visualization of Human Epidermal Receptor 2-overexpressed Gastric Cancer

  • Purpose: A near-infrared (NIR) fluorescence imaging is a promising tool for cancer-specific image guided surgery. Human epidermal receptor 2 (HER2) is one of the candidate markers for gastric cancer. In this study, we aimed to synthesize HER2-specific NIR fluorescence probes and evaluate their applicability in cancer-specific image-guided surgeries using an animal model. Materials and Methods: An NIR dye emitting light at 800 nm (IRDye800CW; Li-COR) was conjugated to trastuzumab and an HER2-specific affibody using a click mechanism. HER2 affinity was assessed using surface plasmon resonance. Gastric cancer cell lines (NCI-N87 and SNU-601) were subcutaneously implanted into female BALB/c nu (6-8 weeks old) mice. After intravenous injection of the probes, biodistribution and fluorescence signal intensity were measured using Lumina II (Perkin Elmer) and a laparoscopic NIR camera (InTheSmart). Results: Trastuzumab-IRDye800CW exhibited high affinity for HER2 (KD=2.093(3) pM). Fluorescence signals in the liver and spleen were the highest at 24 hours post injection, while the signal in HER2-positive tumor cells increased until 72 hours, as assessed using the Lumina II system. The signal corresponding to the tumor was visually identified and clearly differentiated from the liver after 72 hours using a laparoscopic NIR camera. Affibody-IRDye800CW also exhibited high affinity for HER2 (KD=4.71 nM); however, the signal was not identified in the tumor, probably owing to rapid renal clearance. Conclusions: Trastuzumab-IRDye800CW may be used as a potential NIR probe that can be injected 2-3 days before surgery to obtain high HER2-specific signal and contrast. Affibody-based NIR probes may require modifications to enhance mobilization to the tumor site.

Effects of dietary supplementation with Taiwanese tea byproducts and probiotics on growth performance, lipid metabolism, and the immune response in red feather native chickens

  • Objective: This study compared the catechin composition of different tea byproducts and investigated the effects of dietary supplementation with green tea byproducts on the accumulation of abdominal fat, the modulation of lipid metabolism, and the inflammatory response in red feather native chickens. Methods: Bioactive compounds were detected, and in vitro anti-obesity capacity analyzed via 3T3-L1 preadipocytes. In animal experiments, 320 one-day-old red feather native chickens were divided into 4 treatment groups: control, basal diet supplemented with 0.5% Jinxuan byproduct (JBP), basal diet supplemented with 1% JBP, or basal diet supplemented with 5×106 colony-forming unit (CFU)/kg Bacillus amyloliquefaciens+5×106 CFU/kg Saccharomyces cerevisiae (BA+SC). Growth performance, serum characteristics, carcass characteristics, and the mRNA expression of selected genes were measured. Results: This study compared several cultivars of tea, but Jinxuan showed the highest levels of the anti-obesity compound epigallocatechin gallate. 3T3-L1 preadipocytes treated with Jinxuan extract significantly reduced lipid accumulation. There were no significant differences in growth performance, serum characteristics, or carcass characteristics among the groups. However, in the 0.5% JBP group, mRNA expression of fatty acid synthase (FAS) and acetyl-CoA carboxylase (ACC) were significantly decreased. In the 1% JBP group, FAS, ACC and peroxisome proliferator-activated receptor γ levels were significantly decreased. Moreover, inflammation-related mRNA expression levels were decreased by the addition of JBP. Conclusion: JBP contained abundant catechins and related bioactive compounds, which reduced lipid accumulation in 3T3-L1 preadipocytes, however there was no significant reduction in abdominal fat. This may be due to a lack of active anti-obesity compounds or because the major changes in fat metabolism were not in the abdomen. Nonetheless, lipogenesis-related and inflammation-related mRNA expression were reduced in the 1% JBP group. In addition, dietary supplementation with tea byproducts could reduce the massive amount of byproducts created during tea production and modulate lipid metabolism and the inflammatory response in chickens.

독거노인의 외로움과 수면의 관계 (Association between Sleep and Loneliness in the Elderly Living Alone)

  • 연구목적 외로움과 수면의 질 저하는 노인에게서 흔히 나타나는 증상이다. 하지만 우리나라에서 노인의 외로움과 수면의 관계에 대해서는 연구된 바는 거의 없다. 본 연구는 독거노인의 외로움과 수면의 질의 연관성을 보고자 하였다. 방 법 본 연구는 천안 지역에 거주하는 독거노인 1,090명을 대상자로 시행되었다., 훈련된 간호사의 면담과 외로움 척도, 피츠버그 수면의 질 지수(Pittsburgh Sleep Quality Index, PSQI), 인구사회학적 정보, 신체 건강상태(Cumulative illness rating scale, CIRS), 인지기능, 단축형 노인우울척도 등의 자가설문지를 통해 조사하였다. 통계분석은 인구학적 변인과 외로움 척도 간의 관계를 탐색하기 위해 기술적 통계분석을 시행하였으며 외로움과 수면의 질의 연관성을 확인하기 위해 단변량 및 다중회귀분석을 사용하였다. 결 과 외로움군의 평균 PSQI 점수(9.2±4.2)는 정상군(7.3±3.7)에 비해 통계적으로 유의하게 높았다(student-t test, p<0.001). 성별, 기분장애 및 상관분석에서 유의성을 보인 변수를 통제한 다중회귀분석에서 외로움은 전체 PSQI 총점(standardized β=0.065, p=0.025), 수면방해(standardized β=0.086, p=0.005), 수면제 사용(standardized β=0.065, p=0.034)과 유의한 연관성을 보였다. 결 론 외로움은 수면문제와 연관성을 보였으며 이는 독거노인의 수면의 질 저하에 외로움이 위험요인이 될 수 있음을 의미한다. 독거노인의 외로움을 줄이기 위한 개입은 수면의 질을 호전시키고 신체 및 정신 질환을 줄일 수 있을 것이다. 따라서 독거노인의 외로움을 예방하고 줄일 수 있는 지역사회 기반의 보건복지 정책이 필요함을 시사한다.
