Screening panoramic radiographs in a group of patients visiting a Health Promotion Center

  • Purpose : To report the incidence of radiological findings from screening panoramic radiographs and verify the validity of the panoramic radiography for screening purposes. Materials and Methods : Six thousand one hundred and sixty panoramic radiographs taken from the patients visiting the Health Promotion Center of CNUH were selected for this retrospective study. Panoramic radiographs were examined into the following pathologic conditions : the presence of periodontal bone loss, dental caries, periapical radiolucencies, retained roots, impacted supernumerary teeth, impacted third molars, odontoma, cystic lesions other than radicular cyst, sialoliths, and mixed radiolucent-radiopaque lesions. Number of pathologic conditions and Prevalence values were recorded. Results: The prevalences of pathologic conditions were $72.9\%$ of periodontal bone loss, $32.2\%$ of dental caries, $11.9\%$ of periapical radiolucencies, $10.8\%$ of retained roots, $0.4\%$ of root fracture, $1.0\%$ of impacted supernumerary teeth, $1.0\%$ of impacted third molars, $0.06\%$ of odontoma, $0.08\%$ of cystic lesion other than radicular cyst, $0.2\%$ of prolonged retention of deciduous tooth, $0.1\%$ of sialolith, and $0.04\%$ of mixed radiopaque and radiolucent lesion. Conclusion : Although the panoramic radiograph should not be used to replace intraoral radiographic and clinical examinations, this study showed that many dental pathologic conditions could be detected on panoramic radiographs. The panoramic radiograph might serve as a diagnostic aid in dental health evaluation programs.

악골에 발생한 석회화 치성낭의 임상 및 방사선학적 고찰 (THE CLINICAL AND RADIOLOGICAL CONSIDERATION OF CALCIFYING ODONTOGENIC CYST OF THE JAW)

  • Nine cases presented with a calcifying odontogenic cyst to the Dental Hospital of the College of Dentistry, Yonsei University. from January 1987 to September June, 1996, Clinical or histopathological findings were observed according to each radiologic criteria. The results obtained are as follows : 1. The male to female ratio was 2 : 1 with a mean age of 25 years. 2. The radiographic appearences of nine cases were well-defined radiolucent lesions in which eight cases were shown unilocular lesions and only one lesion was seen multilocular lesion. 3. Radiologically. three of nine cases were pure radiolucent lesions and others, six cases were radiolucent contained a variable amount of radiopaque material. 4. Histologically, two cases were classified simple cyst, five were cyst associated odonoma, two were neoplastic type. 5. Histological findings according to the radiological classification, two of three radiolucenct lesions were simple cysts; another was a cyst associated with odontoma. While, four of six radiologic mixed lesions turned out to be a cyst associated with odontoma, two was the neoplastic type.

복잡 치아종의 외과적 적출후 미맹출치의 자발적 맹출 유도 (SPONTANEOUS ERUPTION GUIDANCE OF UNERUPTED TOOTH AFTER SURGICAL REMOVAL OF COMPLEX ODONTOMA)

  • 치아종은 치성 종양 중 가장 흔한 형태로 대체로 병소의 크기나 위치에 따라 증상이 없는 경우가 대부분이며 성장이 제한적이다. 보통 10대 이전에 주로 진단되며 종종 영구치의 매복이나 맹출 지연을 유발한다. 형태에 따라 복합 치아종과 복잡 치아종으로 구별된다. 복잡 치아종은 치성 조직이 조직화되지 않은 덩어리로 형태학적으로 정상 치아 형태를 닳지 않으며 전체 치아종의 약 25%, 악골내 발생하는 치성 종양의 약 22%를 차지하고 남성에서 하악 구치부에 호발한다. 치아종의 원인은 아직 밝혀져 있지 않으나, 아마도 해당 부위의 국소적인 외상, 감염, 유전적 원인이 관여 할 것이라 추측된다. 치아종의 치료는 대부분 보존적인 외과적 적출술이 추천되며 재발은 거의 없다. 본 증례는 상악 제 1대구치의 매복을 일으킨 복잡 치아종으로 진단된 두 어린이에게서 병소의 외과적 적출술 및 개창술을 시행하고 관찰한 결과 약 2년후 제 1 대구치가 자발적으로 맹출함을 보여 이에 보고하는 바이다.

복잡 치아종으로 인한 매복치아의 교정적 견인 (THE ERUPTION GUIDANCE OF AN IMPACTED TOOTH ASSOCIATED WITH A COMPLEX ODONTOMA : CASE REPORT)

  • 치아종은 치성 종양 중 가장 흔한 형태로 증상이 없는 경우가 대부분이며 종종 영구치의 매복이나 맹출지연을 유발한다. 이는 치아를 구성하는 법랑질, 상아질, 백악질, 치수로 구성되며, 형태에 따라 복합 치아종과 복잡 치아종으로 구분된다. 복합 치아종은 다수의 작고 치아와 유사한 형태를 지니며 상악 전치부에 호발하고, 복잡 치아종은 무정형의 치성조직으로 구성되어 있으며 전체 치아종의 약 25%를 차지하고 하악 구치부에 호발한다. 치아종의 병인은 확실치는 않으나 국소적인 외상, 감염, 유전적 요인이 관여할 것이라 추측된다. 치료는 보존적인 외과적 적출술이 추천되며 재발은 거의 없다. 다음의 두 증례는 각각 하악 제 1대구치와 하악 측절치의 맹출 지연을 주소로 본원에 내원한 환아들로, 매복치 상방의 복잡 치아종을 외과적 적출술을 시행하고 매복치의 외과적 노출 및 교정적 견인을 시행하여 양호한 결과를 보였기에 보고하는 바이다.

소아에서 자가치아이식에의한 매복치의 처치 (MANAGEMENT OF IMPACTED TEETH BY AUTOTRANSPLANTATION IN CHILDREN)

  • 치아매복에 의한 미맹출시 치아 매복의 방향과 위치 치근단의 완성정도, 맹출공간의 존재여부, 매복치 주변에 과잉치, 치아종, 낭종 등의 존재여부 등에 따라 발치, 교정적 견인, 외과적 자가이식중 어떤 술식을 시행할 것인지 결정하게 된다. 자가 치아이식은 치아의 위치가 교정력을 가할 수 없는 위치에 존재하거나 치아이동이 제한을 받게 되어 통상적인 치료가 불가능할 경우에 발거에 앞서 고려할 수 있다. 자가치아이식의 예후는 치근의 완성도, 외과적 시술능력 환자의 나이, 근관치료, 고정기간과 형태, 치주인대의 보존, 저장 방법 등에 영향을 받는다. 특히 이식 수여부의 골의 결손이 너무 클 경우 치아의 동요도를 감소시키고 골치유를 빠르게 하기위해 골이식을 고려할 수 있다. 치아 미맹출을 주소로 본원에 내원한 증례들로 상악과 하악의 여러 부위에서 과잉치, 치아종, 이소맹출 등의 다양한 원인요소들이 발견되었다. 자가치아이식전 필요시 공간확보를 행하고, 과잉치 및 치아종의 발거로 인해 골결손이 클 때, 탈회냉동건조골과 자가골 이식을 시행하였다. 치아고정은 $2\sim3$주정도 시행하고 $3\sim4$주 후 방사선적 검사와 임상검사후 근과치료의 필요성을 결정하고 정기적인 검사를 시행한 결과 치근흡수소견 등의 비정상적인 소견은 보이지 않고 정상적인 치유소견을 관찰할 수 있었다.

악골에 발생한 양성 치성종양의 임상 및 방사선학적 연구 (CLINICAL AND RADIOGRAPHIC STUDY OF BENIGN ODONTOGENIC TUMORS IN THE JAWS)

  • The author observed and analyzed the age, sex, chief complaint and radiographic finding of sixty-one cases of benign odontogenic tumors seen in Yonsei Medical Center, for the period of Jan. 1979 to Aug. 1989. The results were as follows: 1. Benign odontogenic tumors of 61 cases included 52 cases (85.3%) of ameloblastoma and odontoma, and 9 cases of other lesions. Radiographically, the border of the lesions were well-defined. 2. Ameloblastoma constituting twenty-seven cases (44.3%) occurred the average age of 31.1 years and had a 3:1 male predominance. The most common complaint was swelling (20 cases, 74.0%) and followed by pain (13 cases, 48.2%). Radiographically, the most common site was mandibular body area (74.0%) and the lesions were mainly multiocular radiolucency; in 17 cases (63.0%) and unilocular radiolucent lesion were seen in 10 cases (37.0%). 16 cases (59.3%) showed the resorption of roots of adjacent teeth. 3. Odontoma constituting twenty-five cases (41.0%) discovered at the average age of 16.9 years and had a 3:2 male predominance. The most common complaint was delayed eruption of tooth (8 cases, 31.0%) and 7 cases (27.0%) detected on a routine radiograph of the area. Radiographically, 17 cases (68.0%) were of compound type and 8 were of complex variety and compound odontomas were common in the anterior maxilla, whereas complex odontomas occurred more frequently in the posterior mandible. 19 cases (76.0%) showed the impaction of adjacent teeth.

Complex dental anomalies in a belatedly diagnosed cleidocranial dysplasia patient

  • Cleidocranial dysplasia (CCD) is a rare congenital disorder, typically characterized by persistently open skull sutures, aplastic or hypoplastic clavicles, and supernumerary teeth. Mutations in the gene encoding the runt-related transcription factor 2 (RUNX2) protein are responsible for approximately two thirds of CCD patients. We report a 20-year-old CCD patient presenting not only with typical skeletal changes, but also complex dental anomalies. A previously undiagnosed odontoma, 14 supernumerary teeth, a cystic lesion, and previously unreported fused primary teeth were discovered on cone-beam computed tomography (CBCT) scans. Mutation analysis identified the causal c.578G>A (p.R193Q) mutation in the RUNX2 gene. At 20 years of age, the patient had already missed the optimal period for dental intervention. This report describes the complex dental anomalies in a belatedly diagnosed CCD patient, and emphasizes the significance of CBCT assessment for the detection of dental anomalies and the importance of early treatment to achieve good outcomes.

  • Permanent canine are very important teeth in point of function as well as esthetic. Therefore impaction of canines is a frequently encountered clinical problems. The incidence of impaction ranges between 1-3%. The cause of canine impaction can be result of ectopic position of tooth germ, defect of dental follicle, early loss or delayed exfoliation primary canine, space problem, supernumarary teeth, cyst, odontoma, Impaction frequently involves further complication such as root resorption of adjacent teeth, cyst formation and migration of the neighboring teeth. Various treatment modalities include extraction of primary canine, surgical opening and orthodontic traction, autotransplantation, surgical extraction of impacted canine.
