관절염 노인의 자살생각에 관한 연구: 국민건강영양조사 자료를 이용하여 (Suicidal Ideation in Older Adults with Osteoarthritis: Data from the Korea National Health and Nutrition Examination Survey)

  • Purpose: The study was conducted to identify factors influence suicidal ideation among older adults with osteoarthritis. Methods: This study used a cross-sectional design with secondary analysis of the Korean National Health and Nutrition Examination Survey. Data for 174 men and 939 women who aged${\geq}65$, and were diagnosed osteoarthritis from the Korea National Health and Nutrition Examination Survey 2010~2012 were included. Suicidal idea, age, sex, education, marital status, living alone, income, body mass index, smoking and alcohol use, exercise, perceived health status, activity limitation, joint pain and stiffness, depressive mood, and perceived stress were measured. Influencing factors and odds ratios were analyzed using complex sample multivariate logistic regression. Results: In the results, 29.9% of the elderly with osteoporosis had suicidal idea. The adjusted odds ratios (ORs) of suicidal ideation were significantly higher among those over 80 years older (OR=2.88, 95% CI=1.49~5.58), those with depression (OR=4.01, 95% CI=2.66~6.04), and those with high levels of stress (OR=2.76, 95% CI=1.79~4.24). Conclusion: Therefore, older adults with osteoarthritis should be screened for depression and stress to decrease suicidal ideation and prevent suicide attempt.

Age at Smoking Initiation and Subsequent Smoking Among Korean Adolescent Smokers

  • Objectives: The aim of this study was to investigate the relationship between age at cigarette smoking initiation and smoking level among adolescent current smokers. Methods: In 2007, students from one or two classes of the 10th or 11th grade were randomly selected using the probability proportional to size sampling method. In total, 743 current smokers were included. Multiple logistic regression was used to determine the association between age at smoking initiation and smoking level as either frequent, daily, or heavy smokers. Results: The adjusted odds ratios for each smoking level were significantly higher for younger ages of smoking initiation than those for older ages of initiation were (p for trend <0.001). Compared with the students who started smoking in grades 10 to 11, the adjusted odds ratios for frequent, daily, and heavy smokers increased from 2.24 (95% confidence interval [CI], 1.30 to 3.87) to 3.90 (95% CI, 2.02 to 7.56), from 1.56 (95% CI, 0.92 to 2.62) to 3.17 (95% CI, 1.70 to 5.92), and from 2.56 (95% CI, 1.21 to 5.42) to 5.67 (95% CI, 2.61 to 12.30) with younger ages of smoking initiation. Conclusions: Smoking frequency and amount were closely associated with age at smoking initiation. Therefore, smoking prevention programs should be initiated from the young adolescent period.

사회자본 정도 및 건강행동이 한국인의 주관적 건강에 미치는 영향 분석 (The Influence of Social Capital and Health Behaviors on Self-rated Health in South Korea)

  • Objectives: This study intended to examine the influence of social capital and health behaviors on self-rated health in Korea. Methods: The data of the social statistics survey that the Korea National Statistical Office conducted in 2006 were chosen and 36,266 people from them, who were 30~59 years old were sampled. This paper made logistic regression analysis to examine the effect of social capital and health behaviors on self-rated health. Results: Odds ratios of social capital are family structure(1.321), marriage(1.214), satisfaction with family relationship(2.207), reliability of institutions(1.307), economic support(1.199), citizen's participation(1.531), and religious activity(1.138). Odds ratios of health behaviors are meal(1.431), exercise(1.356), and no drinking(0.648). Conclusion: Based on the results, this paper can suggest that the plan of keeping and building up social capital should be considered in the whole aspects of the society and the project of moderation in drink is required to consider social culture more.

노인들의 고관절골절 위험요인에 관한 환자-대조군 연구 (Risk Factors for Hip Fracture among the Elderly)

  • Objectives: This study was conducted to find out risk factors related to elderly hip fracture, so that the result could provide basic data to establish prevention programs for hip fracture among the elderly. Methods: The data were collected from 199 cases hospitalized in 4 university hospitals in Pusan from January, 1996 to December, 1998, 193 healthy controls who visited elderly facilities in Pusan. The data were obtained from medical records and by using questionnaires through telephone contact or direct personal interview with the subjects or their family, and were analyzed for 2-test and multiple logistic regression. The risk factors were shown with odds ratios and their 95% confidence intervals. Results: In univariate analysis, the odds ratio of hip fracture risk was estimated to be 1.9 for the elderly aged 75 years as compare with those aging less than 65 years; 42 for those with job as compared with those without job; 3.3 for those with more than 6 children as compared with those with one or two children. For the variables related to physical characteristics, small height (p=0.015), light weight (p=0.000), and low BMI (p=0.014) were risk factors for elderly hip fracture. Sane variables related to health, such as previous history of illness (OR=3.3.), abnormal blood pressure (OR=1.6), previous fracture history (OR=22), lower limbs weakness (OR=12.1) and gait disturbance (OR=42.6), were significantly associated with the risk of hip fracture. In multiple logistic regression, risk factors for hip fracture were age, having job, lower limb weakness and previous history of illness. The adjusted odds ratios of hip fracture risk among the elderly were age (OR=1.1), having jobs (OR=11.7), weak lower limb (OR=10.8) and previous history of illnesses (OR=3.3), respectively. Conclusion: This study suggests that the plan for improving the daily living environment for the elderly systematically should be implemented to avoid the chances of fall, and that programs encouraging to practice regular exercise for physical activity and to promote health of the elderly should be developed.

일부 60~64세 농촌 인구에서 사회조직망과 고혈압 (Social Networks and hypertension in Some rural residents Aged 60-64)

  • 농촌의 전통적인 사회조직망을 반영해주는 변수들과 고혈압과의 관련성을 규명하기 위해 달성군 60-64세 노인을 대상으로 1996년 4월부터 9월까지 일대일 면담방법으로 조사를 실시하였다. 사회조직망을 반영해주는 변수로 결혼, 규칙적인 종교행사 참석, 친한 친구 및 친척 수, 구성원으로 있는 모임 수, 자식 및 손주 수 그리고 형제 수 등이 선정되었다. 고혈압은 고혈압력을 가진 자, 수축기 혈압이 100 mmHg 이상 또는 이완기 혈압이 95 mmHg 이상의 기준 중 최소한 하나의 기준을 만족하는 자로 정의하였다. 최종 자료 분석에 이용된 대상자는 총 958명으로 남자 440명, 여자 518명이었다. 남자의 당일지수 분석에서 친구가 없는 자에 비해 1-4명 가진 자가 0.43배 (95% 신뢰구간 0.19-0.96) 더 고혈압일 확률이 더 낮았으며, 자식이 0-1명인 자에 비해 2-3명, 4명, 5명 이상에 따라 비차비가 각각 0.21 (95% 신뢰구간 0.06-0.72), 0.14 (95% 신뢰구간 0.04-0.49), 0.24(95% 신뢰구간 0.07-0.82)로 고혈압일 확률이 더 낮았다. 반면에 여자에서는 통계적으로 유의한 사회조직망 구성 변수는 없었으며 손주가 없는 자에 비해 5-9명 가진 자의 비차비가 0.42 (95% 신뢰구간 0.17-1.02)로 경계성 유의성을 나타내었다. 연령, 교육수준, 비만도, 흡연 및 음주를 다중지수회귀분석으로 통제를 한 후 비차비의 변화를 관찰하였다. 남자에서 단일변수 분석에서처럼 친구 수와 자식 수가 통계적으로 유의한 비차비를 나타내었으며 단일연수 분석의 결과에 비해 상관의 강도가 더 높아졌다. 특히 친척 수는 통계적인 유의성을 획득하였으며 친척이 없는 자에 비해 가진 자는 0.44-0.50의 비차비를 나타내었다. 여자에서 대체로 단일변수 분석에서와 같은 결과를 나타내었으나 손주 수는 없는 자에 비해 5-9명을 가진 자의 비차비자 0.42에서 0.37 (95% 신뢰구간 0.14-0.95)로 상관성이 더 높아졌으며 통계적인 유의성 역시 경계선에서 유의성을 획득하였다. 이러한 결과는 사회조직망 중 일부 특정한 구성요소가 고혈압과 관련이 있으며 남녀간에 관련 구성 요소가 서로 차이가 남을 시사한다.

Factors associated with low water intake among South Korean adolescents - Korea National Health and Nutrition Examination Survey, 2007-2010

  • Water is essential for life and plain water instead of sugar-sweetened beverages is one approach for decreasing energy intake. Due to limited data on characteristics associated with water intake among Korean adolescents, this study examined associations of demographic and behavioral characteristics with plain water intake by using nationally representative sample of South Korean adolescents. The data (2007-2010 Korea National Health and Nutrition Examination Survey) for 1,288 high school-aged adolescents (15-18 years) were used. Multivariable logistic regression was used to calculate adjusted odds ratios (OR) for factors associated with low water intake (< 4 cups/day) and very low water intake (< 2.5 cups/day). Nationwide, 38.4% and 19.0% of adolescents reported drinking water < 4.0 cups/day and < 2.5 cups/day, respectively. The mean plain water intake was 5.7 cups/day for males and 4.1 cups/day for females. Females had significantly higher odds for drinking water < 2.5 cups/day (OR = 2.2) than males, whereas adolescents with low milk consumption had significantly lower odds for drinking water < 2.5 cups/day (OR = 0.7). Factors significantly associated with a greater odds for drinking water < 4 cups/daywere being female (OR = 2.8) and not meeting physical activity recommendations (${\geq}20$ min/day on < 3 days/week) (OR = 1.6). Being underweight, overweight, and obese were significantly associated with reduced odds for drinking water < 4 cups/day (OR = 0.7, 0.4 and 0.5, respectively). However, intake of soda, coffee drinks, fruits, vegetables, and sodium and eating out were not significantly associated with low or very low water intake. These findings may be used to target intervention efforts to increase plain water intake as part of a healty lifestyle.

Comparison of heavy metal concentrations in hairs of a small sample of Korean patients taking traditional herbal medicine

  • A concern of safety issue for traditional herbal medicine is the possibility of some herbs containing heavy metals responsible for several cases of metal poisoning. In our previous study, the physiological levels of heavy metals, such as Cd, Cu, Hg and Pb, were examined to evaluate a potential risk in patients taking herbal medicine. In this study, the levels of 12 heavy metals, including non-metallic element, Se, were analyzed in hair and compared to the results from the previous study. In the previous study, the levels of Cd, Cu, Hg and Pb in hair showed a significantly lower odds ratio than 1. In this study, however, all metals did not show any significant odds ratio higher or lower than 1 even if 5 of 12 metals showed lower odds ratio than 1. In addition, the levels of metal concentrations, especially for Cu, Fe, Pb and Zn, exceeding the WHO reference values were observed in hair. However, any evidence for metal accumulation in hair caused by taking herbal medicines for long duration was not observed in analysis of multiple regression and odds ratio from case-control study. This result would show another possibility for a role of herbs as a non-enzymatic chelator inhibiting the gastrointestinal absorption of heavy metals.

Mediating Effect of Work-Family Conflict on the Relationship Between Long Commuting Time and Workers' Anxiety and Insomnia

  • Background: Our study aimed to investigate the mediating role of work-family conflict (WFC) on the relationship between long commutes and workers' anxiety and insomnia. Methods: Our study measured the two dimensions of WFC, time-related, and strain-related, which were considered multiple mediators. The mediating effect of WFC on anxiety and insomnia was investigated by decomposing the total effect into a direct effect (long commuting time → anxiety or insomnia) and an indirect effect (long commuting time → WFC → anxiety or insomnia). The combined indirect effect (joint indirect effect) of strain-related WFC and time-related WFC was estimated. The effects were presented as odds ratios and 95% confidence intervals (CIs). Results: The direct effect of 120 min or longer of commuting time was 1.39 (95% CI: 1.17-1.65) times increase in the odds of anxiety and 1.64 (95% CI: 1.41-1.90) times increase in the odds of insomnia than those whose commuting time was less than 60 min. In the case of indirect effects, those whose commuting time was 120 min or longer had 1.13 times higher odds of anxiety (95% CI: 1.07-1.18) and 1.12 times higher odds of insomnia (95% CI: 1.07-1.17) via WFC. The joint indirect effects accounted for 26.4% and 18.5% of the total effect on anxiety and insomnia, respectively. The longer the commuting time, the stronger both direct and indirect effects. Conclusions: Our findings highlight the mediating effect of WFC on the relationship between long commuting times and workers' anxiety and insomnia.

도시지역 주민의 관상동맥질환 위험인자에 관련한 비만지표의 유용성 검토 (The Usefulness of Obesity Indices for the Coronary Risk Factors in an Urban Inhabitants)

  • Objectives : To determine obesity for the screening of individuals at high risk of coronary heart disease in urban areas. Methods : Data were obtained from 4,137 adults between 19 and 85 years of age (2,372 males, 1,765 females), not recognized as taking medicines for cardiovascular diseases, who underwent a health check-up at the health promotion center of university hospitals in cities between Jan. 2003 and Dec. 2004. The variables studied were divided into two broad categories, and their relationships examined. obesity indices and risk factors for coronary heart disease. To reveal the relation between each of the obesity indices and the proportion of individuals at risk of coronary heart disease, the obesity indices were stratified and odds ratios obtained after age adjustment. Results : From a gender comparison of anthropometric measures, men were found to have significantly greater heights, weights, and waist and hip circumferences than women. From a gender comparison by the obesity indices, women were found to have significantly higher BMI, %Fat, waist to hip and waist to stature ratios than men. As obesity indices, the waist to stature ratio and the waist circumference were strongly correlated with coronary risk factors, both in men and women. The age-adjusted odds ratio of coronary risk factors increased significantly with increasing waist circumference, BMI, %fat, waist to hip and waist to stature ratios, and were highest specifically for the waist to stature ratio and the waist circumference. Conclusions : The study results showed that the waist to stature ratio and the waist circumference, as obesity indices, were most closely correlated with coronary risk factors. It is suggested that the waist to stature ratio and, specifically, the waist circumference can be effectively used in the field of health management for screening those with high levels of coronary risk factors.

Cross-sectional Study for Blood Metal Concentration in Patients with Herbal Medicine Intake

  • Evidences from various countries suggest that toxic heavy metals in herbal medicine may constitute a serious health problem. In order to evaluate whether the toxic heavy metals caused by herbal medicine intake, blood samples collected from 222 patients taking herbal medicine were analyzed. In average levels of analyzed metals, $0.4{\sim}33.9%$ of total samples for 8 metals such as Cd, Co, Cu, Hg, Mn, Ni, Pb and Zn except Cr and Fe exceeded the upper limit for WHO reference value. In analysis of regression coefficients indicating the levels of metals increased or decreased after taking herbal medicine for one month, however, there were different aspects by intake types for herbal medicine. For example, the metals increased by taking decoction in blood samples were as follows; Cd and Pb whether Mn, Ni and Pb as increased metals were identified in the group taking pill and decoction(combined intake group). The odds ratio showing values higher than 1 indicating that people who take herbal medicine would have possibility higher for metal accumulation in blood than that from people who do not take herbal medicine. The metals showing the odds ratio higher than 1 were Hg and Ni in decoction group, and Cd and Hg in combined intake group. However, eight of the total, 10 metals showed the odds ratios lower than 1 by taking herbal medicine. Thus, this may explain the possible role of herbal medicine as a chelator for heavy metals in body.
