  • The behaviors associated with illness are different depend upon individuals even if Patients complaints same symptoms and have same disease. Understanding the patterns of those behaviors become one of the important elements in determining the diagnostic and treatment approaches and treatment compliance. The sick role plays a essencial part in abnormal illness behavior. The characteristics of abnormal illness behavior ran be applied to many parts in medicine. In case of the various kinds of functional disorders whose organic foundations are obscure and in applying the cognitive therapy, rehabilitation program and occupational therapy, the assessment and evaluation of the abnormal illness behvior is known to be beneficial. For improving the comprehensive psychiatric treatments which could be applied to the Koreans more effectively in patients with somatoform disorders and other various kinds of neurotic disorders further researches especially on the medico-historical and socio-cultural aspects of the illness behavior should be followed. And understanding the abnormal illness behavior would be helpful in enhancing the medical cost effectiveness.

Custom-Made ITE Type Hearing Protection Device Using a Small Acoustic Filter

  • Noise induced hearing loss (NIHS), the well-known occupational disease, is caused by continuous excessive noise. The prevention of NIHS is very important, because it is unrecoverable. There are some kinds of hearing protection device (HPD), and those are effective in preventing NIHS. But workers in noisy environment often resist to wearing them. Because they are ready - made products, so workers feel uncomfortable to wear. Also, they didn't maintain the conversation frequency range, so workers are hard to communicate in wearing them. To prevent hearing loss effectively, it is important that workers keep wearing HPD. Therefore, a HPD is needed to be comfortable to wear and be effective not only in hearing protection but also in preserving communication ability. So we proposed a custom - made hearing protection device in which a small acoustic filter is inserted. We designed several kinds of small acoustic filters and carried out some acoustic experiments for measuring characteristics of filters. We confirmed that acoustic transmission characteristic can be adjusted from experimental results using designed small acoustic filters. And we researched for the actual efficiency of a new developed custom - made hearing protection device using a small size acoustic filter. Also, we found out that workers are more satisfied with the new development than a former protection device from a workers' response.

  • Osteoarthritis(degenerative joint disease), the most common arthropathy affecting the aged people, is characterized by degeneration of articular cartilage with proliferation and remodeling of subchondral bone. Osteoarthritis of the knee, which probably has greater social cost and more associated disability than osteoarthritis of any other joint, prevalence is known to increase with age, and females have higher rates than males; radiographic abnormalities are present in more than 30% of persons more than 65 years old, with approximately 40% of these persons symptomatic. Though the etiology of osteoarthritis is not entirely understood, much information is available regarding risk factors for the development of knee osteoarthritis that permit some reasonable guideline for preventive strategies. Traumatic damage or occupational or recreational overuse of knee joint may result in osteoarthritis, and obesity also is related to osteoarthritis of the knee. A variety of other possible risk factors for the development of knee osteoarthritis have been proposed, including increased bone mass, smoking, diseases such as diabetes or elevated serum uric acid, and some metabolic factors, but the contribustions of these and other factors such as smoking or race and diseases such as diabetes are as yet undetermined. The usual clinical manifestations include pain, stiffnesss, crepitus and loss of function. In oriental medicine, osteoarthritis of the knee is very similar to diseases such as Bijung(痺症), Haksulpung(鶴膝風), Youkjeolpung(歷節風) in symptoms. The diseases such as Bijung(痺症), Haksuipung(鶴膝風), Youkjeolpung(歷節風) is related to the of function liver(肝) and kidney(腎) and risk factors are regarded as Pung(風), Han(寒), sub(濕). The diagnosis of osteoarthritis of the knee has often been based on radiographic appearance and clinical manifestations. The acupuncture therapy of osteoarthritis of the knee has often been based on Stomach meridian(ST), Spleen meridian(SP), Kidney meridian(KI), Liver meridian(LR).

Comparision of Body Image between DM Patients who used Insulin Pump and didn't use Insulin Pump (인슐린 펌프 착용 유무에 따른 당뇨병환자의 신체상 비교)

  • The purpose of study was to compare body image between diabetes mellitus patients who used insulin pump therapy and didn't use insulin pump therapy. The study design was comparative survey study the subjects were 60 diabetes mellitus patients who used insulin pump therapy and 60 diabetes mellitus patients who didn't use insulin pump therapy at B hospital in Busan. The data were collected from 15th April to 20th August, 1998. The instrument used for this study were Osgood's body image scale. The collected data were analyzed frequency, percentage, $X^2$-test, mean, standard deviation, t-test, ANOVA, Scheffe test. The results were as follows 1. Demographical characteristics between diabetes mellitus patients who used insulin pump therapy and didn't use insulin pump therapy were no significant difference. 2. Characteristics related disease between diabetes mellitus patients who used insulin pump therapy and didn't use insulin pump therapy were significant difference in paticipation of D.M. meeting, no of paticipation of D.M. meeting. 3. Body inmage score of diabetes mellitus patients was $69.08{\pm}18.13$. In body image, diabetes mellitus patients who used insulin pump therapy were higher than that didn't use insulin pump therapy(t=1.964, P<.05) 4. In body image's each item, common-strange item, noble-humble item, competent-incompetent item, light-heavy item, diabetes mellitus patients who used insulin pump therapy were higher than diabetes mellitus patients who didn't use insulin pump therapy(P<.05). 5. In body image according to economic status, marital status, occupational status were significantly difference. 6. In body image according to causes of regular hospital visiting, paticipation of diabetes mellitus class were significantly difference. In conclusion, diabetes mellitus patients who used insulin pump therapy were more positive than diabetes mellitus patients who didn't use insulin pump therapy.

Effect of Sociodemographic Factors, Cancer, Psychiatric Disorder on Suicide: Gender and Age-specific Patterns (인구사회적 요인, 암, 일부 전신질환 등이 자살에 미치는 영향: 성별, 연령별 분석)

  • Objectives : We examined the effect of sociodemographic factors, cancer, and psychiatric disorders on suicide by gender and age-specific patterns in South Korea. Methods : The study is a case-control study. Claim data was obtained from the national health insurance database and national death registration database. The number of people who committed suicide was 11,523, which was matched with a control group consisting of ten times as many people at 115,230 selected from the national health insurance and medical aids beneficiaries. The medical utilization of the case group was one year before death and that of the control group was from July 1,2003 to June 30, 2004. Four variables-address, economic status, presence of a psychiatric disease, and cancer-were used in multiple logistic regression analyses. Results : Living in cities or in rural areas showed a greater risk for suicide than living in a metropolitan city. Low economic status, the presence of a psychiatric disorder, and cancer were also statistically meaningful risk factors for suicide. The three major psychiatric diseases, schizophrenia, alcohol abuse, and bipolar disorder, were meaningful in all age groups, but the scale of the odds ratio differed by the age group. Only the psychiatric disorder variable was meaningful in the adolescent group, whereas a psychiatric disorder and economic status were meaningful for the young adult group, and all variables were meaningful for the middle-aged group. A psychiatric disorder and cancer were meaningful in the elderly group, economic status was meaningful for male subjects, and address was meaningful for female subjects. Conclusions : Factors such as living in city or rural areas, low economic status, the presence of a psychiatric disorder, and cancer were statistically meaningful risk factors in suicide. These factors also differed by age group. Therefore, policymakers should establish policies for suicide prevention that are relevant for each age group.

Analyzing the Problem of the Caregiver Education System through a Research of the Caregiving Service Activity (요양보호 서비스 활동 조사를 통한 요양보호사 교육과정의 문제점 분석)

  • Purpose: We evaluated caregivers' understanding of patients' diseases and disuse syndrome, the understanding of exercise and massage related to rehabilitation and the necessity of education about these, the difference in education and realities of the care-giving field, and the extra services needed in the field. Methods: The survey using questionnaires was performed from June 2008 to August 2008 with 220 people participated in caregive education programme in daegu city and area near dagu city. Among the 220 submitted questionnaires, 184 which were faithfully answered were selected and they were analyzed by i-STATistics statistical program. Results: The educational focus of the first and second level caregivers, as defined by the second clause of the 29th article of the Elderly Welfare law, is on basic knowledge of diseases such as dementia, stroke, and depression. However, other diseases are not covered and the information does not include information on decreased function, complications, functional rehabilitating exercises, or preventing disuse syndrome for long term patients. The most common diseases, in order of prevalence, are stroke, dementia, diabetes mellitus, Parkinson disease, arthritis, and geriatric inertness. The general level of awareness about disuse syndrome was low, and patients, while understanding the need for massage and rehabilitative exercise, receive little education about the proper methods and therefore cannot use them. Patients also did not understand how participating in these activities could reduce medical fees, indicating that further education on massage and rehabilitative exercise is needed. Caregivers desired to include positive rehabilitation, massage, and exercise-related services in their services. Finally, differences in caregiver education and reality resulted from a lack of diversity in education. Conclusion: We suggest providing education on disuse atrophy and improving the lack of diversity in the care-giving education system.

A study on environmental exposure levels of residents in an industrial complex area (일부 공단지역주민의 환경노출수준 평가 연구)

  • As a follow-up survey of A Study of Monitoring Method on Exposure Level and Biomarkers of Environmental Pollutants-Focused on Ulsan Industrial Comlpex Area, published in 2005, a close examination of the health status was conducted for selected Ulsan residents. Based on the previous study, a total number of 129 subjects were divided into two groups: the exposure group consisted of 39 residents whose blood levels of lead, mercury and/or cadmium were higher than international reference values, and 90 residents of control group with normal levels. Environmental exposure level and its association with health condition were examined by various methods such as questionnaire, analyses of lead, mercury and cadmium in blood and medical examinations. The geometric mean concentrations of lead, mercury and cadmium in blood were respectively $2.07{\mu}g/d{\ell}$ (exposure $2.54{\mu}g/d{\ell}$, control $1.90{\mu}g/d{\ell}$), $5.94{\mu}g/L$ (exposure $8.57{\mu}g/L$, control $5.07{\mu}g/L$), $1.32{\mu}g/L$ (exposure $1.30{\mu}g/L$, control $1.33{\mu}g/L$). The concentrations of the three heavy metals in blood showed lower levels than internationally recommended values except for blood mercury. Also, any abnormal or, peculiar disease, or target tissue damage related to the heavy metals was not observed among the all subjects. Therefore, it can be said that no significant difference of heavy metal concentrations and health conditions was found between the two groups.

A Study on Soldiers' Knowledge, Attitude and Health Belief about AIDS (일반 사병들의 에이즈에 대한 지식, 태도, 건강신념에 관한 연구)

  • Purpose: This study purposed to investigate the relationship between soldiers' general characteristic and their knowledge, attitude and health-belief about AIDS. Method: This study conducted a survey of 197 soldiers using a structured questionnaire. Data were collected from the 10th to 30th of May 2003. Result: The average age of the participants was 21.6 years, 78.2% of them were undergraduates of universities, and by religion the number of Christians was largest. In addition, 81.2% of them had lived with their parents and siblings before they joined the army and most of them were unmarried. Of the subjects, 75.1% finished education about AIDS, 64.5% experienced a sexual intercourse and 6.3% experienced a venereal disease. The participants' knowledge level about AIDS was 14.6 out of 20 points and their attitude about AIDS is 3.96 out of 5 points on the average. Their health-belief about AIDS was 4.0 out of 5 points in 'perceived benefits,' 2.9 in 'perceived barriers,' 2.6 in 'perceived seriousness' and 2.6 in 'perceived sensitivity. Among the subjects' general characteristics, religion was found to be a statistically significant variable for their knowledge level about AIDS. A variable that is statistically significant for the subjects' attitude toward AIDS was families they had lived together before joining the army. Statistically significant variables for the subjects' health-belief about AIDS were perceived sensitivity and experience in venereal diseases, perceived benefits and AIDS education and perceived barriers and marital status. The subjects' knowledge about AIDS was in a statistically significant correlation with their attitude toward AIDS, and their attitude toward AIDS with perceived benefits. Conclusion: According to the results of this study. those who had had AIDS education appeared to have high attitude and health-belief concerning AIDS. Thus it is necessary to execute AIDS education systematically and continuously in order to have right attitude and high health-belief concerning AIDS.

A Study on the Health Management of Dental Technicians and Their Awareness of the Same (치과기공사들의 건강관리 실태 및 인식수준에 관한 조사 연구)

  • The purpose of this study was to serve as a basis for the development of dental technology and for creating a condition that dental technicians could work with pride and right values, by examining what problems there were in their health care, what they thought about them, and how the problems could be solved, The findings of this study were as below: 1. The most serious and harmful element in dental technology workshop was a dust(57.5%) and a noise(33.3%). 2. Approximately 99.0% of the dental technicians investigated made a complain of air pollution caused by noise. Their opinion on a possible measure to remove noise air pollution was that the noise-generating machine should be replaced(64.1%) or that it should be isolated(28.8%). 3. 76.0% complained air pollution cause by dust deteriorates their working efficiency. As a way to eliminate it, they suggested a dust chamber(35.3%) or an air cleaner(27.5%) should be installde. 4. About 80% made a complain of gas air pollution. The most common related symptom was a headache(56.9%). They thought that gas-generating machine should be isolated(39.9%) or that an air purifier should be prepared(33.3%). 5. The largest impact of heat and light on their body was weakening their eyesight(56.9%). 47.1% got burn once though four times, and 34.3% did five times or more. The way to prevent them was to install an automatic casting machine(66.0%) or use protective glasses(28.1%). 6. Approximately 47.7% were considering a change of occupation, and the most common reason was heavy work(23.5%), followed by poor prospect(21.6%) and working environment(19.0%) in the order named. 7. 88.9% responded they were likely to have an occupational disease. Their idea about the proper frequency of regular health examination was once a year(53.6%), or once per every six months(41.8%). 8. The field they were most interested in was health care(39.2%), followed by academic research activities(31.4%). This fact indicated it's most urgently required to improve their working environment.

High Monocarboxylate Transporter 4 Protein Expression in Stromal Cells Predicts Adverse Survival in Gastric Cancer

  • Background: Increasing evidence suggests that stromal monocarboxylate transporter 4 (MCT4) and carbonic anhydrase IX (CA IX) may play key roles in tumor development. However, their clinical value remains largely unexplored in gastric cancer (GC). The present study aimed to determine clinicopathological significance and prognostic values of stromal MCT4 and CA IX in GC. Materials and Methods: Specimens from 143 GC patients were immunohistochemically stained using polyclonal anti-MCT4 and anti-CA IX antibodies. Expression was correlated with patient clinicopathologic characteristics and survival data. Results: High stromal MCT4 expression was detected in 72 of 143 (50.3%) GCs and high CA IX in 74 (51.7%). Both high stromal MCT4 and CA IX were correlated with advanced TNM stage (p=0.000; p=0.000). High CA IX expression was positively related to depth of invasion (p=0.022) and positive lymph nodes (p=0.002) as well. Survival analysis indicated high expression of stromal MCT4 to be an independent factor in predicting poor overall survival (OS) (HR and 95%CI=1.962, 1.032-3.729, p=0.040) and disease free survival (DFS) (HR and 95%CI=2.081, 1.158-3.741, p=0.014) of GC patients. However, high CA IX expression exhibited no significant predictive value. Conclusions: These findings suggest that high expression of stromal MCT4 and CA IX proteins is significantly correlated with GC progression. High stromal MCT4 heralds worse outcome of GC patient, suggesting a novel candidate prognostic marker and therapeutic target.
