Immediate loading of mandibular single implant by using surgical guide and modeless digital prosthesis: a case report (수술용 가이드와 modeless 디지털 보철물을 이용한 하악 구치부 단일 임플란트 즉시 하중 증례)

  • In this case report, immediate loading of an implant-supported single-tooth prosthesis through complete digital workflow. A patient presented for restoration of missing a single tooth in the mandibular first molar. The digital impression was made with intraoral scanner and implant was placed using surgical guide pre-fabricated with pre-operative computed tomography (CT) and scan data. After 1 week later, prefabricated customized abutment and provisional restoration were connected for immediate loading. After 8 weeks later, abutment level impression was taken by intraoral scanner. At 3 months later from implant installation, monolithic zirconia crown were fabricated. This clinical report presents satisfying result in accuracy and patient satisfaction. A completely modeless digital procedure met expectations regarding precision, esthetics, and functionality.

  • This report described a technique utilizing a computer-aided design (CAD) /computer-aided machining (CAM) - guided surgical implant placement and prefabricated fixed complete denture for an immediately loaded restoration. A patient with an edentulous maxilla and mandible received 6 implants in maxilla and 6 implants in the mandible using CAD/CAM surgical templates. Prefabricated provisional maxillary and mandibular implant supported fixed prostheses were connected immediately after implant installation. Provisional prostheses were evaluated for aesthetics, function during 6 months. Definitive prostheses were fabricated.

  • It is important to produce a provisional restoration reflecting the patient's jaw relation, occlusal plane, lip support, shape of teeth, and occlusion type for fully edentulous patients before making a definite prosthesis. The patient introduced in this study showed bad prognosis of remained tooth after severe periodontal diseases. Therefore, remaining teeth were extracted and replaced with dental implants. Provisional restorations were fabricated and the the patient's vertical and horizontal jaw relationship, occlusal plane, amount of overjet and overbite, size of teeth, and length of anterior tooth were recorded. Provisional restorations were scanned and CAD/CAM techniques were used to fabricate a monolithic zirconia bridge, which contour is identical with the provisional restorations. The patient was satisfied with the treatment results on functional, esthetic aspects and the prosthesis retained stable during the four-month clinical observation period.

  • Amelogenesis imperfecta (AI) is a hereditary disease that affects enamel formation. The patients with AI have esthetic and functional problems due to damage of multiple teeth. So most AI patients resolve these problem through the conservative and prosthodontic treatments. In our case, It was difficult to obtain good results in means of conservative and prosthodontic treatments, because the AI patient had skeletal Class III malocclusion. Moreover, because of vertical dimension loss due to severe dental caries and maxillofacial skeletal disharmony, the ordinary prosthodontic treatment was troublesome. So we planned orthognathic surgery to resolve these problems. After the endodontic treatment, temporary restoration was delivered for stable post-operative occlusion. Then orthognathic surgery was done, and final restoration was delivered in stable period. We obtained satisfactory results in esthetic and functional aspects through multidisciplinary management(conservative treatment, prosthodontics and orthognathic surgery).

  • In esthetic treatments, it is important to consider the arrangement of teeth and the relation between the teeth and soft tissues. A precise diagnosis and establishing an appropriate treatment plan is essential in an esthetic treatment of anterior maxillary teeth. For a fixed prosthesis to meet esthetic expectations, It is crucial to achieve symmetry and adequate proportions of the gingival contour around the crowns. To achieve an esthetic improvement and creating a favorable environment for gingival healing of a physiologic gingival contour, gingivectomy, crown lengthening and forced eruption can be applied to the appropriate site. All ceramics or porcelain laminate veneer can be selected for esthetic improvement of teeth contour and shade. In this case report, malposition of the remaining teeth made it hard to get an esthetic appearance. Gingivectomy, crown lengthening and provisional restoration insertion were performed before the final prosthesis fabrication to reform the gingival form. This case presents satisfying result esthetically and functionally.

  • When oral cancer develops in the maxilla, oro-nasal communication occurs after surgical treatment including removal of the primary site. Restoration through an obturator is necessary to prevent food from storing due to non-oral opening, and to ensure proper pronunciation and aesthetic restoration. In this case, the patient was treated with right hemi-maxillectomy due to oral cancer and has residual abutment and poor periodontal support due to the effect of head and neck radiotherapy. The obturator was treated with a hybrid telescopic double crown denture. Reporting a successful prognosis in 18 months of follow-up.

  • In the case of edentulous patients, the total amount of occlusal force is dispersed by the keratinized gingiva during mastication, in result, causing lower masticatory and chewing efficiency. In particular, the mandibular area has more side effects such as pain than the maxilla has. It gets worse when the patient has more absorption of alveolar bone, but the implant treatment is often interrupted due to the existence of the inferior alveolar nerve. In this case, a patient treated with the all-on-4 method by placing the implant in the anterior part of mandible and with the conventional complete denture for the maxilla has maintained without complications and was satisfied with the restoration both functionally and esthetically.

  • Decalcification of the buccal surface of the teeth often occurs during fixed orthodontic treatment. This case report describes two cases in which cervical decalcificated teeth that occurred during orthodontic treatment were treated with direct resin veneer restoration. Early lesions without caries can be remineralized through periodic fluoride application, diet control, and oral hygiene improvement. As it progresses, appropriate repair treatment is required, and it is more preferable to focus on prevention rather than treatment after the occurrence of the lesion.

  • As digital technology has advanced in the field of dentistry, the use of computer-aided design/computer-aided manufacturing (CAD/CAM) has brought changes to the stages of dental treatment. The use of CAD/CAM technology in dental restoration offers clinical efficiency and convenience by reducing production time and appointment intervals, while also simplifying the fabrication process to reduce errors. In this case, digital replication and printing of temporary teeth were used to aid a patient with complex medical histories and physical disabilities. The final impression obtained with silicone impression material included information on the vertical dimension, centric relation, and the angle and length of the anterior teeth, which shortened the production time and appointment intervals and increased patient satisfaction. The final restoration was fabricated using milling and monolithic disc techniques, demonstrating appropriate stability, retention, and support, resulting in functional and aesthetic satisfaction.

  • Complicated crown fractures of maxillary anterior teeth caused by trauma in adolescence can cause functional and aesthetic problems. For crown fractures with pulp exposure, various restorative methods can be considered depending on the amount of remaining tooth structure. Direct resin restorations are the most traditional and effective method, but they are likely to discolor and break over time. Fixed prosthesis have a high possibility of re-restoration due to marginal disharmony due to tooth movement during the growth period, and restorations using post which are mainly performed for extensive crown fractures increase the risk of root perforation and root fracture. However, endocrown is an integrated structure that gains retention force from the pulp space, enabling effective reconstruction from a biomechanical perspective and providing advantages in restoring function and aesthetics. Therefore, endocrown can be considered as a restoration method for complicated crown fractures caused by trauma in adolescence.
