PURPOSE. The occlusal splint has been used for many years as an effective treatment of sleep bruxism. Several methods have been used to evaluate efficiency of the occlusal splints. However, the effect of the occlusal splints on occlusal force has not been clarified sufficiently. The purpose of this study was to evaluate the effect of occlusal splints on maximum occlusal force in patients with sleep bruxism and compare two type of splints that are Bruxogard-soft splint and canine protected hard stabilization splint. MATERIALS AND METHODS. Twelve students with sleep bruxism were participated in the present study. All participants used two different occlusal splints during sleep for 6 weeks. Maximum occlusal force was measured with two miniature strain-gage transducers before, 3 and 6 weeks after insertion of occlusal splints. Clinical examination of temporomandibular disorders was performed for all individuals according to the Craniomandibular Index (CMI) before and 6 weeks after the insertion of splints. The changes in mean occlusal force before, 3 and 6 weeks after insertion of both splints were analysed with paired sample t-test. The Wilcoxon test was used for the comparison of the CMI values before and 6 weeks after the insertion of splints. RESULTS. Participants using stabilization splints showed no statistically significant changes in occlusal force before, 3, and 6 weeks after insertion of splint (P>.05) and participants using Bruxogard-soft splint had statistically significant decreased occlusal force 6 weeks after insertion of splint (P<.05). There was statistically significant improvement in the CMI value of the participants in both of the splint groups (P<.05). CONCLUSION. Participants who used Bruxogard-soft splint showed decreases in occlusal force 6 weeks after insertion of splint. The use of both splints led to a significant reduction in the clinical symptoms.
There are evidences that occlusal splint therapy is critical to diagnose hidden akeleto-occlusal disharmonies in malocclusion patients and capable of enhancing stability after orthodontic treatment. In addition, evidences have implicated occlusal splint therapy in condylar positional changes during TMJ disorder treatment. In view of these evidences, this study was performed to investigate the effect of occlusal splint therapy on condylar positional changes in malocclusion patients and the possible clinical application of the occlusal splint as an additional orthodontic tool. For this study, 8 Angle's Class I malocclusion patients, who had centric occlusion-centric relation discrepancy within 1.0 mm and had no clinical symptoms of TMJ disorder, were selected as control group. And 22 malocclusion patients who had centric occlusion-centric relation discrepancy over 1.0 mm were selected and subdivided as Class I Malocclusion group, Class II div. 1 malocclusion group, Class II div. 2 malocclusion group, Open bite group, and Mandibular asymmetry group. For each subject the occlusal splint with mutually protected type of occlusal scheme was applied for 3 months. Condylar positions in centric relation and centric occlusion were measured using Panadent articulators and Panadent condylar position indicator (CPI) before and after occlusal splint therapy. On the basis of this study, the following conclusions might be drawn: 1, In control group, Class II div. 2 malocclusion group, and mandibular assymetry group, there were no significant differences in condylar positions before and after occlusal splint therapy. 2. In Class I malocclusion group, condyles were moved $0.27{\pm}0.45mm$ forward (p < 0.05) and $0.98{\pm}0.25mm$ upward (p < 0.01) after occlusal splint therapy. 3. In Class I malocclusion group, condyles were moved $0.24{\pm}0.21mm$ backward (p < 0.05) and $1.01{\pm}0.33mm$ upward (p < 0.01) after occlusal splint therapy. 4. In open bite group, condyles were moved $1.24{\pm}0.30mm$ upward (p < 0.01) after occlusal splint therapy. 5. In both control and experimental groups, there were no significant differences in lateral condylar positions before and after occlusal splint therapy.
Occlusal splints have been widely used in dentistry for treatment and diagnosis of signs and symptoms related to dental occlusion. The autor studyed the effect of occlusal splint on masticatory movement of young adult men (mean age 23.8 yrs.). Who had worn. occlusal splint for 1 week. The result were as follows: 1. Lateral movement of the mandible during mastication could be increased after wearing occlusal splint for 1 week. 2. Vertical movement of the mandible during mastication was increased after wearing occlusal splint for 1 week. 3. Maximum opening velocity of the mandible during mastication was increased after wearing occlusal splint for 1 week. 4. Mandibular movement during mastication was more affected by chewing of hard foods than of soft foods following occlusal changes. 5. Steepness of the anterior guidance affected several aspects of masticatory movement. 6. Masticatory movement of the mandible became more regular after wearing occlusal splint for 1 week.
Many types of occlusal splints are used for treatment of craniomandibular disorders. Most widely used splint among them is flat-type centric relation splint. Insertion of splint into the mouth may cause increasing of vertical dimension, masticatory muscle realignment and rearrangement of maxillo-mandibular relationship, so as a result of splint treatment, occlusal relation may vary whether you like it or not. From this point of view, occlusal state of patient shold be frequently monitored to prevent undesired or harmful effect during occlusal splint therapy. The purpose of this study was to investigate the effect of occlusal splint, especially centric relation splint, on the occlusal contact state after 3 months treatment. 32 patients with craniomandibular disorders who had unilateral symptoms participated in this study. To observe and record occlusal contact state, the author used T-Scan system (Tekscan Co. U.S.A.) at both pretreatment and posttreatment. The recorded date were analyzed with regard to contact number, contact force and contact time, change of anterior tooth contact and coincidence of first contact point with affected side were observed, too. Aan last, the subjects were divided into 2 groups and compared, according to average value of VAS index, with respect to joint pain, sound and limitation of movement, respectively. The collected date were statistically processed with SPSS and the result as follows : 1. Total occlusal contact number and force were not changed by occlusal splint therapy but total occlusal contact time decreased slightly. 2. There was a tendency of increasing number of subjects with anterior tooth contact after treatment and change of first contact point side were observed in as many as 40.6% of subjects. 3. There were no difference between higher and lower group of VAS index, and between pretreatment and posttreatment in each group, either.
The purpose of this study was to evaluate the effects of stabilization occlusal splint by using a simplified condylar path recorder and a dental sound checker. For this study, 11 subjects (10 men and 1 woman) with TMJ disorder were selected from students at Chosun University, School of Dentistry. And they were treated with the stabilization occlusal splint. The condylar movement and the TMJ sound of each subject were recorded and analyzed by using a simplified condylar path recorder and a dental sound checker. The obtained results were as follows: 1. No statistically significant reduction of reduced PRI scores occurred before and immediately after wearing of stabilization occlusal splint. 2. The reduced PRI scores after wearing of stabilization occlusal splint showed statistically significant reduction with the lapse of time. 3. After stabilization occlusal splint therapy, Fisher angle and Bennett angle had almost no change. 4. TMJ sound disappeared in 4 out of 11 subjects.
Various conditions such as pain or effusion of temporomandibular joint, degenerative condylar resorption, and articular disc displacement can be a cause of malocclusion. However, the reasons of occlusal changes are ambiguous in some patients. Unexpected occlusal change in patients with or without temporomandibular disorder (TMD) symptom was mostly caused by masticatory muscular disorders. This article reports two cases of recovery of occlusal relationship in TMDs patients after stabilization splint therapy. Stabilization splint therapy could be useful in certain conditions of occlusal changes in TMD.
This study was performed to diagnose the existence and the degree of TMJ dysfunction using Pantronic-PRI and to investgate the degreeof dysfunction under treatment and to evaluate the period until dysfunction were disappeikared. For this study, 12 patients who had visited at the TMJ Clinic of Dankook University were investgated with Pan-PRI. 8 exper imental group patients were treated with occlusal splint therapy and 4 control group patients were not altered of occlusion during this study. The results were as follows. 1. Occlusal splint therapy was effective on treatment of TMJ dysfunction. 2. in the slight dysfunction group, dysfunction was disappeared after 4-5 weeks since occlusal splint therapy was started. 3. Pan-PRI is useful to evaluate the existence of TMJ dysfunction, the degree of dysfunction results of treatment with occlusal splint therapy and to decide other occlusal treatment modalies.
2009년부터 2011년까지 부산대학교병원 구강내과에 측두하악관절장애를 주소로 내원한 환자 중 임상검사와 방사선 검사 및 전산화 단층(Cone beam Computed tomographic view)촬영으로 측두하악관절의 골관절염으로 진단된 76명의 환자를 대상으로 초진 시와 교합안정장치 장착 전까지 물리치료 및 약물치료를 시행하고 교합안정장치 장착 시부터 최종 내원 시까지 월 1회 장치조정을 시행하여 시기별 치료결과를 비교하여 다음과 같은 결과를 얻었다. 1. 초진 시와 교합안정장치 장착 시와 최종 내원 사이에 모든 증상의 변화가 유의한 차이가 있게 개선되었다. 2. 교합안정장치 치료 전까지 치료에서는 급성군에서 통증과 관절잡음이 유의하게 개선되었고, 만성군에서 통증, 관절잡음, 개구제한 및 최대편이개구량이 모두 개선되었다. 3. 교합안정장치 장착 후에는 급성군에서 통증, 개구제한과 최대편이개구량이 유의하게 개선되었고, 만성군에서 통증, 관절잡음과 개구제한이 유의하게 개선되었다.
본 연구는 조선대학교 치과대학 부속치과병원 교정과에 내원한 환자 중 악태모형을 제작하고 CPI (condylar position indicator) 에 의한 하악과두의 위치를 평가하여 중심위 -중심교합 편위양이 전후방 및 수직적으로 1.0 mm 이내, 측방으로 0.3 mm 이내인 정상 범주를 벗어나는 성인 여자 환자 16명을 대상으로 교합안정장치를 3개월간 장착하고, 이들을 장착 기간에 따라 장착 직전, 장착 1개월 후, 장착 2개월 후, 장착 3개월 후로 분류하여 교합안정장치의 장착에 따른 교합력과 교합접촉점 수의 변화 여부를 평가하기 위해 T-scan system을 이용하여 다음과 같은 결론을 얻었다. 1. 교합력의 변화는 교합안정장치 장착 직전에서 장착 1개월 후 사이에 유의성 있게 감소되었다 (P<0.05). 2. 교합력의 변화는 교합안정장치 장착 1개월 후부터 장착 3개월 후 까지는 유의한 차이를 보이지 않았다 (P>0.05). 3. 전치부 교합접촉점 수의 변화는 장착기간에 관계없이 유의성있는 차이가 없었다 (P>0.05). 4. 구치부 교합접촉점 수의 변화는 교합안정장치 장착 직전에서 장착 1개월 후 사이에 유의성 있게 감소되었다 (P<0.05). 5. 구치부 교합접촉점 수의 변화는 교합안정장치 장착 1개월 후부터 장착 3개월 후 까지는 유의한 차이를 보이지 않았다 (P>0.05). 6. 교합접촉점 수의 변화는 전치부에서 보다는 구치부에서 절대적으로 영향을 미쳤다. 이상을 종합해볼 때, 교합안정장치는 장착 1개월 후면 어느정도 중심위로의 하악골 안정을 기대할 수 있을것으로 사료된다.
목적: 수면이갈이 환자에서 한달 간 수면 시 교합안정장치를 장착하였을 때 교합력과 교합 접촉 면적 및 동기능적교합분석의 변화량을 비교하고자 하였다. 연구 재료 및 방법: 2021년 10월부터 2022년 7월까지 연세대학교 치과대학병원 구강내과 외래에 방문한 수면이갈이 환자 30명 중 교합안정장치를 수면 중 착용하는 실험군(treatment; n = 15)과 교합안정장치를 착용하지 않는 대조군(control; n = 15)으로 구성하였다. 교합안정장치 장착 전, 장착 1개월 후에 교합력 검사와 동기능적교합분석(측방, 전후방 하악 운동 시 좌/우 힘의 균형, 평균 교합력, 최대 교합력, 최대 접촉 개수)을 진행하였다. 결과: 한달 간 수면 중 교합안정장치를 착용하는 실험군과 교합안정장치를 착용하지 않는 대조군에서 교합력과 교합 접촉 면적은 차이가 없었으나 측방 및 전후방 운동에서 평균 교합력과 최대 교합력, 전후방 운동에서 최대 접촉 개수가 유의한 차이가 있었음을 관찰하였다. 결론: 교합안정장치가 측방, 전후방 운동을 하는 이갈이 환자에게 도움이 될 것으로 사료되며, 향후 추가적으로 대단위 집단을 대상으로 하는 이중 맹검연구가 필요할 것으로 사료된다.
