Full mouth rehabilitation using removable prosthesis of patient with unstable mandible movements: A case report (불안정한 하악운동을 보이는 환자에서 가철성 의치 수복 증례)

  • Patients with collapsed occlusal support and unilateral chewing show parafunctional mandibular movements, which can be complicated in older patients. Gothic arch tracing and split cast technique are used to confirm the deviation between centric relation and anterior habitual bite in patient who has oral dyskinesia and collapsed occlusion. Temporary denture as occlusal stabilization appliance was provided for stable occlusion and reproducible mandibular movement. Definitive denture was fabricated by lingualized occlusion concept.

Acute Occlusal Change Following Acute Anterior Disc Displacement without Reduction: A Case Report (급성 비정복성 관절원판 변위에 따른 급성 교합변화의 증례)

  • A 35 year-old female presented with the complaint of sudden occurrence of bite change and concurrent opening limitation, as well as pain in the right temporomandibular joint (TMJ) during mouth opening. From her history it was revealed that she had simple clicking of right TMJ for several years before onset of these symptoms, and that the clicking sound subsided recently after development of opening limitation. On clinical examination, anterior open bite, midline shift of the mandible to right, and premature contacts on left posterior teeth were observed. Maximum mouth opening and lateral movement to left were also restricted. On magnetic resonance images, the right TMJ showed anterior disc displacement without reduction and the posterior joint space is greatly collapsed by retrusion of the condyle. It was thought that the sudden occurrence of occlusal change would be resulted from abrupt displacement of the mandible associated with development of the anterior disc displacement without reduction. The stabilization appliance traction therapy was performed initially for first 3 months along with physical and pharmacologic therapy. However, the anterior open bite and opening limitation didn't resolve and the position of mandible still remained altered. So the stabilization appliance was changed to intermaxillary traction device. Then the mandible returned progressively to normal position and the occlusion became more stable and comfortable. After 5 months of intermaxillary traction therapy, the anterior open bite was dissolved completely and the occlusion became stabilized satisfactorily along with recovery of normal mouth opening range. On post-treatment magnetic resonance image, remodeling of condylar head was observed.

Labial and buccal surface contours of Korean normal occlusion in a three-dimensional digital model (한국인 정상교합자의 3차원 디지털 모형을 이용한 순,협측 치면 곡률)

  • Objective: This study was performed to investigate the labio/buccal clinical crown curvatures of Korean permanent teeth and to obtain the curve-ratio data in an attempt to fabricate bracket bases fit for each individual Korean permanent tooth. Methods: Three-dimensional digital models were made from 30 sets of dental casts with normal anatomic structures. According to the FA points, horizontal and vertical reference planes were established and lines were drawn on the tooth surfaces in reference to these planes. The curvature was expressed as the coefficient of a quadratic equation. Lines mesial, distal, gingival and occlusal to the horizontal, vertical reference planes and the FA point were drawn. Results: The curvature measured for each line revealed that there are no significant differences between male and female, except for maxillary canines and maxillary second bicuspids (p > 0.05). There were notable differences in the mesio-distal or gingivo-occlusal curvatures among the mandibular lateral incisors, maxillary canines, maxillary and mandibular first and second bicuspids and first molars (p < 0.05). Conclusions: The labial & buccal crown curvatures of teeth in Korean normal occlusion were measured on the mesial and distal, gingival and occlusal sides respectively in this study. Based on these data, a SWA can be developed to fit the individual features of Korean tooth crowns.

ECTOPIC ERUPTION OF MANDIBULAR FIRST PERMANENT MOLAR : A CASE REPORT (하악 제1대구치 이소맹출의 치험례)

  • Ectopic eruption is caused by an abnormal direction of eruptive path, most common in maxillary first molar, mandibular lateral incisor, and maxillary canine, and sometimes mandibular first molar. Ectopic eruption of first molar leads to abnormal root resorption of second deciduous molar, which, if left untreated, could cause premature loss of second deciduous molar; mesial tilting and rotation of first permanent molar; lack of space for eruption of second premolar; and occlusal problems. Therefore early treatment is advised when diagnosed as ectopic eruption. Treatment of ectopic eruption in the first permanent molar involves providing proper guidance for the direction of eruption using interproximal wedging and distal tipping methods while preserving second deciduous molar. This case report shows satisfactory results of the ectopic eruption of mandibular first molars in young patients who were treated with Humphrey appliance and Halterman appliance.

Changes in occlusal force and occlusal contact area after orthodontic treatment (교정 치료 후 교합력, 교합면적의 변화)

  • Objective: This study was performed to evaluate functional changes of occlusion after orthodontic treatment by measuring the occlusal force (OcFr) and occlusal contact area (OcAr), and to compare OcFr and OcAr change according to premolar extractions. Methods: Data were obtained from 74 patients who had finished orthodontic treatment using fixed appliance aged between 18 and 40 years. Subjects were divided into groups who had four premolars extractions or non-extraction (Male extraction-16, Male nonextraction-18, Female extraction-19, Female nonextraction-21). All subjects were asked to bite pressure-sensitive sheets into maximum intercuspation with maximum bite force, and OcFr and OcAr were evaluated by measuring the sheet with a CCD camera. Records were taken right after debonding, 1 week, 1 month, 3 months, 6 months and 1 year after debonding. Results: OcFr and OcAr increased gradually in all groups during the 1 year retention period (p < 0.05). Male groups showed higher OcFr and OcAr than female groups throughout the retention periods (p < 0.05). There were no statistically significant differences of OcFr and OcAr between extraction and non-extraction groups in both males and females (p > 0.05). Conclusions: Occlusion was improved functionally throughout the 1 year retention, and premolar extraction did not induce a decline in the functional aspect of occlusion.

The Relationship between Anterior Disc Displacement without Reduction and Development of Anterior Open Bite (비정복성 관절원판변위와 전치부 개교합 발생간의 관계)

  • The purpose of this paper is to evaluate if there is a relationship between anterior disc displacement without reduction and development of anterior open bite, and a relation between occurrence of open bite and occlusal appliance therapy. In general, the statistically significant differences were found between the Group 1 and 2 and normal mean group. The variables that represent mandibular size and form, showed a statistical significance in all 3 groups. Also 3 groups patients had a smaller ANB, a larger FMA than normal mean group. When we compared the 3 groups with respect to all cephalometric measurements by One-way analysis of variance (ANOVA), group 1 and 2 patients had a larger FMA, a larger SN to mandibular plane angle, a larger maxillomandibular plane angle, a larger occlusal plane to mandibular plane angle, a smaller total posterior facial height/total anterior facial height(%), and a larger gonial angle than group 3. The statistically significant differences were not found between the Group 1 and 2, and skeletal patterns were similar. Thus, morphologic features of patients with vertical discrepancies may represent a risk factor for the development of anterior open bite with or without occlusal appliance treatment. In case of patients with vertical discrepancy, we may have to be more careful when inducing a change of the vertical dimension.

A Case Report on the Treatment of A TMJ Osteoarthritis Patient with Anterior Open Bite Using An Intermaxillary Traction Device (전치부 개교합을 동반한 골관절염 환자에 대한 악간견인장치의 응용)

  • A patient with TMJ osteoarthritis and anterior open bite was treated with an intermaxillary traction device. Pretreatment examination revelaed a pain in both TMJ during mouth opening, moderate tendernesso f left sternocleidomastoid and right trapezius muscles. Anterior open Bite was aobserved with interincisal distance of 2mm. Tomograms and MRI showed anterior disc displacement withouit reductoin of both temporomandibular joints, and the condyles were flattened and slightly eroded. A pair of full-coverage occlusal appliances was made on both maxillary and mandibular dentition, with pivoting fulcrum on the site of the second moalr. Traction force was gained by the intermaxillary orthodontic elastics which were hooked by orthodontic brackets on the labial surfaces of the upper and lower anterior and premolar teeth. After 8 weeks of traction treatment, the joint pain was subsided completely and the anterior open bite was closed to get an edge to edge relationship of anterior teeth.

Conservative treatment modalities for patients with temporomandibular joint(TMJ) disorders (임상가를 위한 특집 1 - 턱관절장애의 보존적 치료)

  • In the management of temporomandibular joint(TMJ) disorders, dental practioner should conduct the reversible, conservative, and inexpensive treatment modalities prior to the irreversible and invasive treatment. That is to say, behavioral, pharmacologic, and physical therapy should be conducted firstly, and then the occlusal appliance therapy could be considered. If patients do not react to these conservative treatments, we have to consider surgical treatments. If the accurate diagnosis is confirmed by intimate history taking, clinical and imaging examinations, we can rehabilitate the normal TMJ function and relieve the clinical symptoms with only conservative treatments in most TMJ disorder cases.

  • Utilizing the repelling force of permanent magnet of find out weather it shows the findings of molar intrusion histologically and compares the result with the resin bite plane. As for the experimental animals, 10 cats of completion of permanent dentition with mean weight of 2.2kg which have flat occlusal plane of molar areas were used. I raised the cats under the condition of their being attached with manufactured appliance by direct bonding system and of their wearing vertical chin strap anesthetized for 12-14 hours per day through 5 days. Then, observing the root apex areas with lightmicroscope. The results were as follows; 1. In the group with resin bite plane, osteoblasts and osteoclasts could not find out but could find out periodontal ligment fiber obliquely angulated in the apical and lingual direction. 2. In the group with the permanent magnet, not only the obliquely angulated periodontal ligament fiber but also the osteoclasts at the apical areas were observed.
