CHANGES IN INTRAPULPAL NERVE ACTIVITY AND OCCLUDING ASPECTS OF DENTINAL TUBULES BY DENTIN DESENSITIZER CONTAINING GLUTARALDEHYDE (Glutaraldehyde계 상아질 과민증 탈감작제에 의한 치수신경 활동성 변화 및 상아세관 폐쇄양상)

  • The effects of application of dentin desensitizer containing glutaraldehyde (Gluma Desensitizer) and dentin adhesive system (All Bond 2) to the exposed dentin on the intradental nerve activity (INA) and the occluding aspects of dentinal tubules were investigated in cat canine teeth. Single pulp nerve units were dissected from the inferior alveolar nerve and indentified as $A{\delta}$-fiber units. The INAs elicited by 4M NaCl before and after the application of each experimental agent were compared. The morphological changes of exposed dentin surfaces and dentinal tubules in the dentin specimens used to record INAs were observed by SEM. The results obtained were as follows. 1. Eight $A{\delta}$-fiber units (conduction velocity: $8.0{\pm}4.0m$/sec) were identified. 4M NaCl evoked an irregular burst of action potentials which ceased immediately after washing. 2. In 4 $A{\delta}$-fiber units, the change of INA after the application of Gluma Desensitizer was $133.9{\pm}80.7%$ when it was compared with the INA before the application of the same agent. 3. In 4 $A{\delta}$-fiber units, application of All Bond 2 completely abolished the INA induced by 4M NaCl. 4. In specimens applied with Gluma Desensitizer, the formation of hybrid layer as well as the identification of resin penetration and reaction products with proteins in dentinal tubules were not clearly observed in interface between dentin and adhesive resin. In specimens applied with All Bond 2, the gap width of 2-$3{\mu}m$ was formed between exposed dentin and adhesive resin, and the hybrid layer and resin tags were not clearly formed either.

Comparative evaluation of NovaMin desensitizer and Gluma desensitizer on dentinal tubule occlusion: a scanning electron microscopic study

  • Purpose: In this study, the effect of calcium sodium phosphosilicate (NovaMin) desensitizing agent, which is a powder-based system, and hydroxyethyl methacrylate and glutaraldehyde (Gluma desensitizer), which is liquid-based system, on dentinal tubule occlusion was analyzed by scanning electron microscope. The effects of the above two along with one control group were compared to determine the more effective method of sealing the dentinal tubules after initial application. Methods: Twenty specimens were allocated to each of 3 groups: Control, Gluma desensitizer, and NovaMin. Two additional samples were also prepared and treated with Gluma and NovaMin; these samples were longitudinally fractured. The specimens were prepared from extracted sound human premolars and were stored in 10% formalin at room temperature. The teeth were cleaned of gross debris and then sectioned to provide one to two dentin specimens. The dentin specimens were etched with 6% citric acid for 2 minutes and rinsed in distilled water. Control discs were dried, and the test discs were treated with the desensitizing agents as per the manufacturer's instructions. The discs as well as longitudinal sections were later analyzed under the scanning electron microscope. The proportions of completely occluded, partially occluded, and open tubules within each group were calculated. The ratios of completely and partially occluded tubules to the total tubules for all the groups was determined, and the data was statistically analyzed using nonparametric tests and statistical significance was calculated. Results: NovaMin showed more completely occluded tubules ($0.545{\pm}0.051$) while Gluma desensitizer showed more partially occluded tubules ($0.532{\pm}0.075$). The differences among all the groups were statistically significant ($P{\leq}0.05$). Conclusion: Both materials were effective in occluding dentinal tubules but NovaMin appeared more promising in occluding tubules completely after initial application.

Uterine Arterial Embolization for the Treatment of Leiomyomas Accompanying with Adenomyosis (자궁선종을 동반한 자궁근종에서 자궁동맥 색전술을 이용한 치료의 효과)

  • Objective: The purpose of this study was to make a guideline of uterine artery embolization for the treatment of uterine leiomyomas accompanying with adenomyosis in Korea. Materials and Methods : We performed the retrospective study for 37 women who had uterine leiomyomas accompanying with adenomyosis. Bilateral uterine artery embolization was performed in 37 patients (age range 25-65) during 17 months with pain, hypermenorrhea, urinary frequency etc due to leiomyomas. Ultrasound imaging was performed before the procedure and at mean 6.9 months after the procedure. Results: All procedures were technically successful. Mean clinical follow-up was 12.8 months. Minor complication occurred in 82% patients after the procedure. After imaging follow-up (mean, 6.9 months postprocedure), median uterine volume decreased 34.4%, and dominant myoma volume decreased 86%. There was no statistical difference in uterine volume reduction and dominant myoma size reduction whether occluding agents was polyvinyl alcohol, polyvinyl alcohol plus gelfoam, and gelfoam, and whether ultrasound measured Resistance Index value before the procedure was low or high. Conclusion: Primary candidates for uterine artery embolization include those with symptomatic uterine leiomyomas who no longer des ire fertility but wish to avoid surgery or are poor surgical risks. To our study, uterine volume reduction and dominant myoma size reduction in patients who had adenomyosis were similar to previous other studies in patients who had not adenomyosis. Therefore adenomyosis should not be considered as a contraindication for uterine artery embolization. Because there is little data about subsequent reproductive potential after this procedure, it should not be routinely advocated for infertile women. Further investigation is warranted for occluding agents and Resistance Index.

Milk Fat Globule-Epidermal Growth Factor VIII Ameliorates Brain Injury in the Subacute Phase of Cerebral Ischemia in an Animal Model

  • Objective : Milk fat globule-epidermal growth factor VIII (MFG-E8) may play a key role in inflammatory responses and has the potential to function as a neuroprotective agent for ameliorating brain injury in cerebral infarction. This study aimed to determine the role of MFG-E8 in brain injury in the subacute phase of cerebral ischemia in a rat model. Methods : Focal cerebral ischemia was induced in rats by occluding the middle cerebral artery with the modified intraluminal filament technique. Twenty-four hours after ischemia induction, rats were randomly assigned to two groups and treated with either recombinant human MFG-E8 or saline. Functional outcomes were assessed using the modified Neurological Severity Score (mNSS), and infarct volumes were evaluated using histology. Anti-inflammation, angiogenesis, and neurogenesis were assessed using immunohistochemistry with antibodies against ionized calcium-binding adapter molecule 1 (Iba-1), rat endothelial cell antigen-1 (RECA-1), and bromodeoxyuridine (BrdU)/doublecortin (DCX), respectively. Results : Our results showed that intravenous MFG-E8 treatment did not reduce the infarct volume; however, the mNSS test revealed that neurobehavioral deficits were significantly improved in the MFG-E8-treated group than in the vehicle group. Immunofluorescence staining revealed a significantly lower number of Iba-1-positive cells and higher number of RECA-1 in the periinfarcted brain region, and significantly higher numbers of BrdU- and DCX-positive cells in the subventricular zone in the MFG-E8-treated group than in the vehicle group. Conclusion : Our findings suggest that MFG-E8 improves neurological function by suppressing inflammation and enhancing angiogenesis and neuronal proliferation in the subacute phase of cerebral infarction.

THE EFFECT OF VARIOUS DESENSITIZING AGENTS ON THE PERMEABILITY OF HUMAN DENTIN (수종의 desensitizing agent가 상아질 투과도에 미치는 영향)

  • The hydrodynamic theory of dentin sensitivity states that movement of tubular contents or tubular fluid, in either direction of dentinal tubule, causes dentin sensitivity. A corollary of that theory is that anything that can decrease dentinal fluid movement or dentin permeability should decrease dentin sensitivity. A wide variety of physicochemical methods have been used to reduce the permeability and sensitivity of exposed dentin. The purpose of this study was to evaluate the ability of 4 kinds of clinical desensitizing agents(2% NaF, 30% Potassium oxalate, MS Coat$^{(R)}$, Tubulitec system$^{(R)}$) to reduce the rate of fluid flow through dentin in vitro. Sixty coronal dentin discs, 1mm in thickness, were prepared from extracted third molars, free from decay and wear. Dentin discs were treated with 3% EDTA(Tubulicid Plus$^{(R)}$(Dental Therapeutics AB, Sweden)) to remove the smear layer and debris occluding the tubular orifices. After placing the discs in a split chamber device, the rate at which physiologic saline solution could filter across dentin under 150cm $H_2O$ hydrostatic pressure was measured. The occlusal side of the discs were then treated with MS Coat$^{(R)}$, 2% NaF, Tubulitec system$^{(R)}$, and 30% Potassium oxalate, and the filter ratio of the saline solution was measured again. The following conclusions were drawn : 1. Hydraulic conductance which was measured after the application of desensitizing agents was decreased in all the groups(p<0.05). 2. % change of hydraulic conductance was compared but no significant difference was found among the four desensitizing agents(p>0.05). 2% NaF, 30% Potassium oxalate, MS Coat$^{(R)}$ and Tubulitec system$^{(R)}$ decreased the permeability of dentin. It is considered that above four agents can be used in treating the hypersensitive teeth.
