폐동맥에서 발생한 악성 섬유성 조직구종 1례 보 (Malignant Fibrous Histiocytoma of the Pulmonary Artery A Case Report)

  • A case of malignant fibrous histiocytoma [MFH] of left pulmonary artery was reported in the 27 year-old male. He was admitted because of hemoptysis for several times. Chest x-ray revealed nodular lesions in left hilar area and left upper lobe. A left thoracotomy was performed followed left upper lobectomy. The main tumor was originated from the pulmonary arterial bifurcation and proximal portion of the left pulmonary artery. By light microscopy, there were many pleomorphic giant cells, which displayed the storiform pattern. And in those area, dilated or compressed vascular channels were involved by tumor cells, suggesting origin of This tumor. Pulmonary artery angiography and chest CT revealed this tumor was originated from pulmonary artery. It would be the first reported primary malignant fibrous histiocytoma of the pulmonary artery in our country. Postoperative prognosis was uneventful, but recently he was suffered from dyspnea, that was noticed by OVD follow-up for 1 month, probably the heart failure sign due to pulmonary arterial flow obstruction.

  • Confusion of a patent ductus arteriosus (PDA) for the descending thoracic aorta is a fatal error occurring occasionally in infants or neonates. As a result, the left pulmonary artery (LPA) may be misconceived as the PDA, and ligated. This surgical mishap of other hospital leads to serious congestive heart failure and loss of left lung function due to the underdevelopment in the peripheral vascular and alveolar structures in neonates and premature infants. In this report, 3 cases of LPA ligation and subsequent treatment are presented.

총폐정맥환류이상증의 외과적 치험 4례 (Total Anomalous Pulmonary Venous Return -Report of 4 Cases-)

  • This is case report of total anomalous pulmonary venous return with atrial septal defect which were corrected surgically by intracardiac procedure under total cardiopulmonary bypass.Two patients were supracardiac type,cardiac and mixed type was each one.The mixed type was three years old female patient.She was diagnosed as atrial septal defect with partial anomalous pulmonary venous return[right pulmonary vein drains into superior vena cava and right atrium] and corrected as usual.After operation,she underwent exertional dyspnea and frequent tachycardia.Chest x-ray film showed pulmonary congestion.Follow up cardiac cineangiogram revealed that left pulmonary vein also anomalously drained into left innominate vein through vertical vein.Through left thoracotomy,anastomosis was successfully carried between left atrium and vertical vein without cardiopulmonary bypass and there was no sign of pulmonary artery obstruction for two years follow up.The other three patient were corrected successfully without complication and got good result.

Bechet씨 병에 발생한 폐동맥류: 치험 1례 (Pulmonary Artery Aneurysm in Behcet`s Disease - Report of a Case -)

  • Pulmonary artery aneurysm is Behcet`s disease is rare and can be fatal due to rupture. We experienced a case of pulmonary artery aneurysm in Behcet`s disease. The patient was 21 year old woman who was adimitted with three month history of dyspnea, fever and cough. On examination, she had aphthous ulcer in the mouth and erythema nodosum on the left popletial fossa and forearm, but didn`t have any lesion at eyes and genitalia. The latex fixation test for rheumatoid factor, VDRL test for syphillis, antinuclear antibody and LE cell test were all negatives. The third and fourth components of complement in the serum, serum immunoglobulin concentrations[IgG, IgM, IgA] were within normal range. The chest radiography revealed a 5x6cm sized radiopaque mass density in the left hilar region. Two months later, the mass was enlarged to 6x7cm. The IV-DSA showed a single aneurysm at the proximal part of left lower lobe artery with lingular segment artery and no distal perfusion by thombotic obstruction. The steroid therapy was done for a month, but symptoms not improved. We performed resection of lingular segment and lower lobe including the aneurysm. The microscopic findings of the operative specimen were intimal hyperplasia and fragmentation of the internal elastic fibers. She was improved without remarkable event, except infection of the operative wound.

한국산 잡견에서의 단일 폐이식술후 조직학적 고찰 (Histologic Investigation on Canine Single Lung Transplantation)

  • We have performed 28 single lung transplantation in mongrel dogs transplanting the left lung exclusively from November 1989 to September 1991, in the department of thoracic surgery of Seoul National University Hospital. In the donor dogs, the main pulmonary artery was divided proximal to its bifurcation, and the left atrium was incised freeing the left veins with a generous atrial cuff. We used cold saline in the first 7 transplantations and Euro-Collins or modified Euro-Collins solution in the remaining 17 transplantations as a lung preservatives. The bronchus was divided at two cartilage rings proximal to the upper lobe bronchus take off. In the recipient procedure, we used a Fogarty catheter as a bronchus block. Left atrial anastomosis was performed first using 5-O prolene and the pulmonary artery was anastomosed using 6-O prolene. The bronchus was anastomosed next with 4-O vicryl interruptedly and covered with a greater omentum which had been prepared previously. All dogs received cyclosporin A and azathioprine as immunosuppressants and were divided into two group. In the 10 Group I dogs, they survived within 6 days, mean survival time was 66.8$\pm$53.4 hours. In remainder 14 Group lI dogs, they survived above 6 days, mean survival time was 9. 5$\pm$5.6 days. The cause of death were as follows: 2 cases of sacrifice, 2 cases of respiratory insufficiency during operation, 2 cases of arrhythmia immediate postoperatively, 2 cases of bleeding, others in Group I, and 6 cases of sacrifice, 4 cases of sepsis, 3 cases of bleeding, others in Group lI. Results of bronchoscopic findings were obstruction above 50% in 12 cases of 16 performance cases within 5th day. Early chest radiologic haziness were showed, and total lung perfusion defect was frequently showed in both group within 7th day. Main autopsy findings were left atrial and pulmonary arterial thrombi and bronchial obstruction The major histologic findings of Group I were pleural exudate, hemorrhagic infarct, pulmonary congestion, and interesting histologic findings of Group II were 3 cases of perivascular or peribronchial lymphocyte infiltration, 3 cases of hemorrhage infarct, 2 cases of interstitial pneumonitis. The structual change of bronchioles, suggesting bronchiolitis obliterans was not observed due to improper preparation of proximal pulmonary tissue and short term survival times.

심실중격결손과 폐동맥유출로 협착을 동반한 교정형 대혈관전위증의 해부학적 교정수술 (Modified Anatomic Repair of Corrected Transposition of the Great Arteries with Ventricular Septal Defect and Pulmonary Outflow Obstruction)

  • A modified procedure for the operative management of the corrected transposition of the great arteries with ventricular septal defect and pulmonary stenosis or atresia was performed in 4 patients. (age ; 18 months-9 years). The operation consists of a venous switch procedure (Senning), intraventricular baffling directing the blood flow from the morphologic left ventricle into the aorta via ventricular septal defect through the right ventriculotomy, and insertion of valved conduit between the morphologic right ventricle and the pulmonary artery. All the four patients had large non-restrictive ventriular septal defects and no evidence of significant mitral valve regurgitation. Both ventricles were well-developed. Three cases had pulmonary atresia, and the remainder had severe subpulmonic stenosis. Postperatively, all patients showed no findings of systemic or pulmonary venous obstruction withnormal sinus rhythm. One patient died of sepsis due to infection by Methicillin-resistant S. aureus on the thirteenth postoperative day. Follow-up is done on the survivors, and all of them are dong well with regular sinus rhythm, with their functional class I or II at 2 to 8 months after the operation.

Long-Term Follow-Up of the Half-Turned Truncal Switch Operation for Transposition of the Great Arteries with Ventricular Septal Defect and Pulmonary Stenosis

  • The half-turned truncal switch (HTTS) operation has been reported as an alternative to the Rastelli or $r{\acute{e}}paration$ $\grave{a}$ $l^{\prime}{\acute{e}}tage$ ventriculaire procedures. HTTS prevents left ventricular outflow tract (LVOT) obstruction in patients with complete transposition of the great arteries (TGA) with a ventricular septal defect (VSD) and pulmonary stenosis (PS), or in those with a Taussig-Bing anomaly with PS. The advantages of the HTTS procedure are avoidance of late LVOT or right ventricular outflow tract (RVOT) obstruction, and of overstretching of the pulmonary artery. We report the case of a patient who underwent HTTS for TGA with VSD and PS, in whom there was no LVOT obstruction and only mild aortic regurgitation and mild RVOT obstruction, including observations at 12-year follow-up. Our experience with long-term follow-up of HTTS supports a solution for late complications after the Rastelli procedure.

폐경색이 발생한 만성 폐색전증 -수술적 치험 1예- (Pulmonary Infarction due to Chronic Pulmonary Thromboembolism -Surgical Experience of One Case-)

  • 폐경색은 폐동맥의 폐색으로 인해 발생하며 기관지내 세균오염에 의해 폐감염이 흔하게 속발하며 이는 폐농양, 농흉 그리고 기관지 흉막루 등을 일으켜 결국 패혈증에 이르게 할 수 있다. 이러한 이유로 조기 진단이 중요하며 감염 조절을 위해 폐절제 등을 조기에 고려해야 한다. 67세 남자가 호흡곤란을 주소로 응급실에 내원하였다. 흉부 전산화 단층 촬영에서 좌하엽에 침윤성 병변과 소량의 흉수 그리고 늑막 비후가 관찰되었다. 폐색전은 하엽 폐동맥에서 상엽 폐동맥까지 자라나 있었다. 출혈성 경색이 완연한 좌하엽은 절제되었고 좌주폐동맥을 열어 나머지 폐색전을 제거하였다. 수술 후 15개월간 추적 관찰하고 있으며 폐색전이나 폐경색의 재발은 보이지 않았다.

흰쥐에서 폐정맥 폐쇄에 의해 유도된 폐동맥고혈압 발생 후의 폐장의 변화 (Changes in the Lung after Pulmonary Hypertension Induced by Obstruction of the Pulmonary Vein in Rats)

  • 배경: 저산소증, 공기색전증, monocrotaline 약물 주입 등에 의해 혈관 내막의 손상을 일으켜 폐동맥내세포외간질 단백질 합성을 활성화시키고 혈관의 중막비후나 신생내막의 형성을 일으키는 혈관개조(vascular remodeling) 실험이 활발하게 진행되고 있다. 본 연구는 폐정맥을 폐쇄시킨 후 변화된 혈류에 의해 폐장이 어떻게 반응하는지 확인하고자 동물실험을 실시하였다. 대상 및 방법: $352{\pm}18g$의 흰쥐(n=10)를 이용하여 ketamine 근육내 주사로 마취하여 정중흉골절개술을 시행하고 심장을 노출시킨 후 우측 폐정맥을 크립을 이용하여 폐쇄하였으며, 15일 후 폐동맥압, 좌심실과 심실중격 대비 우심실 무게비(RV/LV+Sweight ratio), 말초 폐동맥의 외측지름대비 벽두께비(percent wall thickness (%WT)) 등을 측정하여 대조군(n=5)과 비교하였다. 결과: 폐정맥 폐쇄군의 폐동맥압은 38{\pm}12 mmHg로 대조군의 $13{\pm}4mmHg$에 비해 의의 있게 증가되었다(p<0.05). 좌심실과 심실중격 대비 우심실 무게비는 대조군의 $0.35{\pm}0.04$에 비해 $0.52{\pm}0.07$로 통계적으로 유의한 우심실 비대 소견을 보였고(p<0.05), 말초 폐동맥의 외측지름대비 벽두께비는 폐쇄군이 $22.4{\pm}6.7%$로 대조군의 $6.7{\pm}3.4%$에 비해 증가되었다(p<0.05). 결론: 한쪽 폐의 폐정맥 폐쇄는 폐동맥고혈압, 우심실 비대, 말초 폐동맥의 중막 비후 소견을 유도하였다. 이는 폐동맥 혈관개조의 병리학적 특성을 나타내는 것으로 일측 폐정맥을 폐쇄하는 경우 반대측 폐장의 폐혈류량 증가뿐만 아니라 혈관개조를 유발하는 내막의 손상을 동반함을 알 수 있었다.

  • A 32-year-old woman complaining of cough, sputum, and chest discomfort for the past ten days was admitted to the hospital. The radiologic findings were transradiant left lung with reduced number and size of vessels, mediastinal shifting to the right at expiration, matched ventilation-perfusion defect on ventilation-perfusion scan, and diffuse hypoplasia of the left pulmonary artery and i1s branches on the pulmonary angiography. We describe a case of unilateral hyperlucent lung by main bronchus obstruction in a patient who presents a clinical picture suggestive of the Swyer-James syndrome.
