$Ureaplasma$ infections in pre-term infants: Recent information regarding the role of $Ureaplasma$ species as neonatal pathogens

  • Although numerous clinical observational studies have been conducted over a period of over 30 years, the clinical significance of $Ureaplasma$ infection is still under debate. The $Ureaplasma$ speices. is a commensal in the female genital tract and considered to have of low virulence; however, $Ureaplasma$ colonization has been associated with infertility, stillbirth, preterm delivery, histologic chorioamnionitis, and neonatal morbidities, including congenital pneumonia, meningitis, bronchopulmonary dysplasia, and perinatal death. Recently, $Ureaplasma$ was subdivided into 2 separate species and 14 serovars. $Ureaplasma$ $parvum$ is known as biovar 1 and contains serovars 1, 3, 6, and 14, and $Ureaplasma$ $urealyticum$ (biovar 2) contains the remaining serovars (2, 4, 5, and 7-13). The existence of differences in pathogenicities of these 14 serovars and 2 biovars is controversial. Although macrolides are the only antimicrobial agents currently available for use in neonatal ureaplasmal infections, in the current clinical field, it is difficult to make decisions regarding which antibiotics should be used. Future investigations involving large, multicenter, randomized, controlled studies are needed before proper recommendations can be made for clinical practice.

Reasons and Risk Factors for Readmission Following Hospitalization for Community-acquired Pneumonia in South Korea

  • Background: Limited studies have been performed to assess readmission following hospitalization for community-acquired pneumonia (CAP) in an Asian population. We evaluated the rates, reasons, and risk factors for 30-day readmission following hospitalization for CAP in the general adult population of Korea. Methods: We performed a retrospective observational study of 1,021 patients with CAP hospitalized at Yeungnam University from March 2012 to February 2014. The primary end point was all-cause hospital readmission within 30 days following discharge after the initial hospitalization. Hospital readmission was classified as pneumonia-related or pneumonia-unrelated readmission. Results: During the study period, 862 patients who survived to hospital discharge were eligible for inclusion and among them 72 (8.4%) were rehospitalized within 30 days. In the multivariable analysis, pneumonia-related readmission was associated with para/hemiplegia, malignancy, pneumonia severity index class ≥4 and clinical instability ≥1 at hospital discharge. Comorbidities such as chronic lung disease and chronic kidney disease, treatment failure, and decompensation of comorbidities were associated with the pneumonia-unrelated 30-day readmission rate. Conclusion: Rehospitalizations within 30 days following discharge were frequent among patients with CAP. The risk factors for pneumonia-related and -unrelated readmission were different. Aspiration prevention, discharge at the optimal time, and close monitoring of comorbidities may reduce the frequency of readmission among patients with CAP.

On the Role of Prefabricated Speech in L2 Acquisition Process: An Information Processing Approach

  • This study focused on the role of prefabricated speech (routines and patterns) in the L2 acquisition process. The data for this study consisted of spontaneous speech samples and various observational records of three Korean children learning English as L2 in a nursery school. The specific questions addressed here were: (1) What routines, patterns, and creative constructions did the children use? (2) What was the general trend in the three children's use of routines, patterns, and creative constructions over time? The data were collected over a period of one school year by observing the children in their school. The findings were discussed from the perspective of human information processing. This study found that prefabricated speech played a significant role in the three children's L2 acquisition. The automatic processing of prefabricated speech appeared to enable the children to reduce the burden on their information processing systems, which allowed the saved resources available for other language development activities. Also, the children's language development was evident in their increase in the use of patterns. The children were moving from heavy dependence on wholly unanalyzed routines to increased use of partly unanalyzed patterns. This increased control was the result of an increase in procedural knowledge.

에어로빅 훈련이 만성정신분열병 환자에게 미치는 영향 (The Effects of Aerobic Exercise on Chronic Schizophrenia)

  • The purpose of this research was to determine whether or not a consistent program of aerobic exercise as a nursing intervention would have beneficial effect on schizophrenic patients. Nonequivalent control group pre-post test research was designed. Subjects for this study were selected from thirty-eight chronically schizophrenic patients who were hospitalized at S psychiatric hospital in Taejeon city. Of them seventeen were experimental group, twenty-one were control group. The data were collected during the period from November 1 to December 30, 1992. The instruments used in this study were Scale for Assessment of Negative Symptom developed by Andreason, Behavioral Observational Checklist (SASS) developed by Jorgensen, Harvard Step Up Test. The data were analyzed by descriptive statistics, t-test and paired t-test, using the SPSS program. The results of this study are summerized as follows : 1. The scores of SANS were a significantly difference from pre to posttesting, when the experimental group was contrasted to the control group (t=4.73, p=.00). 2. The scores of SASS were a significantly difference from pre to posttesting, when the experimental group was contrasted to the control group (t=3.42, p=.00). 3. The scores of physical fitness were a significantly difference from pre to posttesting, when experimental group was contrasted to the control group (t=-5.87, p=.00).

아동간호사의 상위의사소통 행위 (Qualitative Analysis of Nurses′ Metacommunicative Behaviors in a Pediatric Unit)

  • The purpose of this study was to identify metacommunicative behaviors between nurses and patients in a pediatric unit. The research method included observation using videotaping. Data were collected from December, 2001 to February, 2002. Total six nurses, and eight patients and their mothers in a pediatric unit participated in this study. The interactions were videotaped under the participants' consent. The participants were observed for total 8 hours over 2-day period. Special episodes which were identified as metacommunicative behaviors in the taped interactions were transcribed. Transcription included verbal and nonverbal interactions. Selected episodes were classified using Mitchell's definition. Each classified definitions were named, and categorized by its purpose. The results were as follows: Nineteen metacommunicative behaviors which used frequently by nurses-approaching, mediating eye level, eye contact, touching, encouraging, turnabout, mimic voice, giving choices, friendly demand, expansion, tagging, repeating and confirming, identification, reflection, baby talk, symbolization, description of acts, relaxed posture, turning away- were identified and organized into four categories. They were call for attention, facilitating response, empathy, and tension release. In conclusion, nurses in this study used metacommunicative behaviors frequently and these behaviors were effective in interacting with children. It is suggested that any educational programs to teach communication skills to nurses need to include techniques on metacommunicative behaviors. This will help nurses to be more sensitive to different characteristics of their patients.

폐경과 수면 (Sleep in Menopause)

  • Menopause, the cessation of menstruation caused by the decline in estrogen production, occurs in 95% of women between 40 and 60 years. Sleep disturbance is a frequent complaint during the perimenopause period. In contrast to premenopausla women, menopausal women experience more reduction in the total sleep hours and report more sleep disturbances, such as insomnia, noctiria and sleep disordered breathing. But the prevalence, etiology and treatment of sleep disturbances in menopause are still controversal. So further investigations are required to elucidate the factors that account for the differences in sleep disturbance between premenopausal and postmenopausal women. There are suggestive data that estrogen and progesterone deficiency may increase the susceptibility for sleep disorder in menopause. Furthermore, there are suggestive evidence from observational studies and a limited number of randomized, controlled trials that hormone replacement therapy after menopause improves sleep. However, the clinical relevance of hormone replacement therapy is unproved. So the overall benefit of hormonal replacement in postmenopausal women with sleep related disorders should be individualized to avoid potential side effects. Several studies evaluated the role of melatonin, because this hormone has effects on core body temperature & insomnia. But the exact dosage and the effects of long-term use of melatonin are unclear. So, caution is indicated in melatonin administration.

STUDY OF MAGNETIC HELICITY IN SOLAR ACTIVE REGIONS AND ITS RELATIONSHIP WITH SOLAR ERUPTIONS

  • It is generally believed that eruptive phenomena in the solar atmosphere such as solar flares and coronal mass ejections (CMEs) occur in the solar active regions with complex magnetic structures. Magnetic helicity has been recognized as a useful parameter to measure the complexity such as twists, kinks, and inter-linkages of magnetic field lines. The objective of this study is to understand a long-term (a few days) variation of magnetic helicity in active regions and its relationship with the energy buildup and instability leading to flares and CMEs. Statistical studies of flare productivity and magnetic helicity injection in about 400 active regions were carried out. The temporal variation of magnetic helicity injected through the photosphere of active regions was also examined related to 46 CMEs. The main findings in this study are as follows: (1) the study of magnetic helicity for active regions producing major flares and CMEs indicates that there is always a significant helicity injection through the active-region photosphere over a long period of 0.5 - a few days before the flares and CMEs; (2) for the 30 CMEs under investigation, it is found that there is a fairly good correlation (linear correlation coefficient of 0.71) between the average helicity injection in the CME-productive active regions and the CME speed. Beside the scientific contribution, a major impact of this study is the observational discovery of a characteristic variation pattern of magnetic helicity injection in flare/CME-productive active regions which can be used for the improvement of solar eruption forecasting.

심부전 환자 대상 국내 양적 간호연구 분석 및 중재연구의 질 평가 (Research Trends in Quantitative Nursing Studies and Quality Assessment of Intervention Studies in Patients with Heart Failure in South Korea)

  • Purpose: The purpose of this review was to identify the current status of nursing studies on heart failure (HF) patients in South Korea and to suggest future study direction. Methods: A literature review of databases such as KoreaMed, KERIS and nursing and allied health journal were searched with key terms 'heart failure' and 'nursing' for the period from January 2000 to February 2017. A total of 35 studies including 28 articles and 7 theses met the inclusion criteria. Results: Twenty-seven out of 35 studies were observational studies on outpatients and most of the studies did not mention the ejection fraction and New York Heart Association functional classification class (NYHA class) in the inclusion criteria. Self-care and health-related quality of life as psychological factors, and physical activity as a biological factor, were used as main variables. However, we found it difficult to understand how much score indicates better quality of life because of an inconsistent and wide score. In quality assessment, 8 intervention studies had no serious flaws. Conclusion: Further studies should consider more biological and social factors influencing HF. The quality assessment with respect to nursing intervention studies in HF showed that randomized and double-blind trials are needed.

창녕·함안보 방류량에 따른 하류지역 및 하구역의 수리환경특성에 관한 연구(1) (Study on Water Level and Salinity Characteristics of Nakdong River Estuary Area by Discharge Variations at Changnyeong-Haman Weir(1))

  • The present study analyzed the variation factors in inducing a salinity change using the existing observation network in the section between the Nakdong River Estuary Barrage and Changnyeong-Haman Weir, and also examined the seasonal changes in precipitation, salinity, and discharge. Furthermore, this study analyzed the causes of a salinity increase by collecting observational data during a period when abnormal salinity occurred, and further investigated the salinity transfer time in a section of approximately 5.3 km from the Nakdong River Estuary Barrage to Nakdong River Bridge to understand the behavioral characteristics of the salinity moving upstream. The study results would make it possible to control the increase in salinity and block salt water from moving upstream by understanding the salinity variation characteristics according to the discharge amount. This will provide stability in collecting water from various residential, agricultural, and industrial sources through water intake facilities scattered near the Nakdong River Estuary Barrage.

한의학의 치매관련 임상연구 동향분석과 제안 (Literature Review and suggestions : dementia clinical studies in Korean Oriental Medicine)

  • Objectives : This study was aimed to review the trend of dementia clinical studies in Korean Oriental Medicine and to suggest better clinical studies. Method : We collected 26 papers on dementia clinical studies in the internet site OASIS using the keyword 'Dementia'. Then we analyzed them. Results : There were 26 clinical study papers that were published between 1996 and 2010. Observational studies account for 61% of total clinical studies, whereas experimental studies account for only 31%. Effects of treatment in experimental studies was evaluated mainly by MMSE-K and K-DRS. Treatment period of experimental studies was longer than 100days. Conclusions : High quality experimental studies like randomized clinical trial(RCT) and intervention studies which use syndrome differentiation(辨證) have not yet been published. It is necessary to research and develop duration of treatment effect and tools for evaluating treatment effects.
