Assesment of Bee-venom Acupuncture Effect on Herniated Disc Patients by Rating Scale (요추간판탈출증 환자 22례에 대한 봉독약침 치료의 요통 평가지수를 이용한 임상보고)

  • Herniation of intervertevral disc(HIVD) is the most common disease causing low back pain. Acupuncture, electroacupuncture and moxibustion has been used for treatment of HIVD. Bee venom (BV) also has been traditionally applied to relieve pain. Recently, it has been reported that BV is effective in treating HIVD. But more objective methods were required to assess the efficacy of BV on HlVD. So we assessed of BV-acupuncture effect on herniated disc patients by use of rating scale for low back pain. We researched 22 patients who were treated at Department of Acupuncture & Moxibustion, Kyung Hee Medical Center from January 1999 to June 1999. The patients were diagnosed by MRI as having a herniated disc. The results of treatment using rating scale for low back pain in HIVD showed that $43.77{\pm}14.39$ point of pre-treatment score and $84.5{\pm}3.97$ point post-treatment score. The improvement index showed $0.43{\pm}0.15$. Especially, clinical score of neurologic deficits showed most highest improvement index of $0.7{\pm}0.45$. Assessment of BV-acupuncture Effect by MRI finding were showed that protruded disc and extruded disc got more higher improvement index than bulging disc. Among 22 patients, satisfactory assessment of BV-acupuncture effect showed that 15(68.2%) were excellent, and 7(31.8%) were good. The result suggest that BV-acupuncture is good method for treatment of HIVD.

Initiation of Pharmaceutical Care Service in Medical Intensive Care Unit with Drug Interaction Monitoring Program (내과계 중환자실 약료 서비스 도입과 약물상호작용 모니터링)

  • Objective: It is to evaluate the drug interaction monitoring program as a pilot project to develop a pharmaceutical care model in a medical intensive care unit and to analyze the influencing factors of drug interactions. Method: Electronic medical records were retrospectively investigated for 116 patients who had been hospitalized in a medical intensive care unit from October to December in 2014. The prevalence of adverse reaction with risk rating higher than 'D' was investigated by Lexi-$Comp^{(R)}$ Online database. The factors related with potential drug interaction and with treatment outcomes were analyzed. Results: The number of patients with a potential interaction of drug combination was 92 (79.3%). Average ages, the length of stay in the intensive care unit and the numbers of prescription drugs showed significant differences between drug interaction group and non-drug interaction group. Opioids (14.4%), antibiotics (7.2%), and diuretics (7.2%) were most responsible drug classes for drug interactions and the individual medications included furosemide (6.4%), tramadol (4.9%), and remifentanil (4.5%). There were 950 cases with a risk rating of 'C' (84.6%), 142 cases with a risk rating of 'D' (12.6%), and 31 cases with a risk rating of 'X' (avoid combination) (2.8%). The factors affecting drug interactions were the number of drugs prescribed (p < 0.0001) and the length of stay at intensive care unit (p < 0.01). The patients in intensive care unit showed a high incidence of adverse reactions related to potential drug interaction. Therefore, drug interaction monitoring program as a one of pharmaceutical care services was successfully piloted and it showed to prevent adverse reaction and to improve therapeutic outcomes. Conclusion: Active participation of a pharmacist in the drug management at the intensive care unit should be considered.

Therapeutic Effect of Weizhong(BL40) Venepuncture on Low Back Pain (위중혈(委中穴) 자락(刺絡)의 요통(腰痛)에 대(對)한 치료효과(治療效果))

  • Objective : Since ancient times venepuncture has been widely used as a useful treatment for low back pain. This study is made in order to objectively evaluate the therapeutic effect of Weizhong(BL40) on low back pain. Methods : Total 46 cases of low back pain patients were divided into 2 groups; one group took only acupuncture therapy, and the other group took acupuncture and venepuncture therapy together. Each patient's pain was evaluated before the treatment and the 3 or 4 days later after the treatment. Pain rating score(PRS) was used to assess the pain for its intensity, frequency, duration, and aggravating factors. Results : Both acupuncture therapy group and acupuncture & venepuncture group showed good effect on low back pain. Though the latter group showed better pain release effect than the former, there was no statistical significance between pain rating score of two groups. Sciatica group showed no significant difference from non-sciatica group in pain rating score. Palpation on Weizhong(BL40) group showed significant difference only in pain duration from non-palpation group. Exposed vessel around Weizhong(BL40) group showed significant difference in pain rating score from non-exposed vessel group. Conclusion : Weizhong(BL40) venepuncture can be highly recommended as a useful therapy for low back pain patients especially who show exposed vessels on the popliteal area.

Clinical Application of the Korean Personality Rating Scale for Children in Attention-Deficit Hyperactivity Disorder (주의력결핍 과잉행동장애에서 한국아동 인성평정척도의 임상적 적용)

  • Objectives : The objective of this study was to compare the Korean Personality Rating Scale for Children (K-PRC) profile between children with attention-deficit hyperactivity disorder (ADHD) and typically developing children. We also aimed to investigate the association of K-PRC and ADHD symptoms. Methods : Ninety-nine youth (age $8.3{\pm}2.4$ years, 72 boys) with ADHD and 84 controls (age $9.2{\pm}2.5$ years, 43 boys) were recruited from the Department of Pediatric Psychiatry of the Asan Medical Center Children's Hospital. Diagnoses of ADHD and comorbid psychiatric disorders were confirmed with the Kiddie-Schedule for Affective Disorders and Schizophrenia-Present and Lifetime Version (K-SADS-PL). The parents of the subjects completed the ADHD rating scale, and K-PRC. Independent t-tests, analysis of covariance, partial correlation analyses, and Mc Nemar test were used for analysis. Results : Children and adolescents with ADHD showed higher K-PRC scores in verbal development, physical development, depression, delinquency, hyperactivity, family dysfunction and psychoticism. Delinquency and hyperactivity were significantly correlated with parent-rated ADHD rating scales and ADHD scores on K-SADS-PL. The hyperactive/impulsive and combined subtypes showed higher scores on hyperactivity and delinquency than the inattentive subtype, and the inattentive subtype showed higher scores on depression and social dysfunction of the K-PRC. Conclusion : Our results suggest that K-PRC could be used to comprehensively evaluate symptoms, combined psychopathologies, developmental delay and family dysfunction of children with ADHD.

Comparison between denture wearer's evaluation and clinician's rating for complete denture (총의치 사용에 대한 환자와 술자간 평가 비교)

  • Purpose: The aim of this study was to compare denture wearer's evaluation and clinician's technical rating for complete denture used on edentulous patients. Materials and methods: Total 43 edentulous patients who had complete denture fabricated more than one year ago were recalled. The questionnaire based on the various literatures was modified and applied to patients for subjective assessments. Functional aspects related to retention, stability, occlusion and denture condition were included in operator's evaluation. In addition, correlations were evaluated between patient's subjective and operator's objective assessments. Friedman test and Cohen's Kappa value were used for statistical analysis. Results: It was found that denture wearers' evaluations were slightly or fairly agree to clinician's rating for complete denture. More differences were found in maxillary denture than mandibular denture and moderate difference was found in esthetic, occlusion aspects. Conclusion: There were slightly or fairly agreement between subjective and objective evaluations.

Comparison of Treatment Outcome Assessment for Class I Malocclusion Patients: Peer Assessment Rating versus American Board of Orthodontics-Objective Grading System

  • Purpose: The purpose of this retrospective study is to investigate the degree of coincidence between the peer assessment rating (PAR) index and American Board of Orthodontics objective grading system (ABO-OGS) in the assessment of orthodontic treatment outcomes of Class I malocclusion cases. Materials and Methods: The sample consisted of 26 Class I patients. The PAR index was used for evaluation of pre-(T0) and posttreatment (T1) casts, and the ABO-OGS for assessment of T1 casts. If there was a reduction in PAR scores from T0 to T1 of more than 30%, the label 'PAR+' was given to the case, and if not, it was labeled 'PAR-'. If the ABO-OGS was less than 27, the label 'OGS+' was given to the case and if not, it was labeled 'OGS-'. 'A PAR-only qualified group' (PAR+), 'ABO-OGS-only qualified group' (OGS+), 'both indices qualified group' (PAR+/OGS+), and 'both indices disqualified group' (PAR-/OGS-) were compared with a Wilcoxon rank-sum test, sensitivity/specifi city test and Spearman's correlation test. Result: PAR scores for T0, T1, and percentage reduction were 21.1, 6.4, and 65.9%, respectively, and 35.4 for ABOOGS. The distribution of the 'PAR+/OGS+', 'PAR+', and 'PAR-/OGS-' group was 19.3%, 76.9%, and 3.8%, respectively. The T0-PAR, T1-PAR and PAR point reductions for the 'PAR+' group were significantly higher than those of 'PAR+/OGS+' groups (23.1 vs. 15.6; 6.7 vs. 4.6; and 16.5 vs. 11.0; all P<0.05). However, the PAR-percentage reduction and treatment duration between the two groups were not statistically different (70.0% vs. 67.0%, P=0.4325; 24.1 months vs. 25.0 months, P=0.4057). The T1-ABO-OGS score for 'PAR+' group was significantly higher than that of the 'PAR+/OGS+' groups (38.2 vs. 24.0, P<0.001). Conclusion: Since the fraction of the 'PAR+/OGS+' group was less than 20% and there was no significant correlation between PAR-percentage reduction and T1-ABO-OGS, development of a new index system for the accurate evaluation of treatment outcome is needed.

The effects of Jajun-maum-dom-movement program on the attention and hyperactivity of middle school students (자전 동작이 중학생의 주의력 및 행동에 미치는 영향)

  • Objectives : The objective of this study is to investigate the effects of Jajun-maum-dom-movement program on the attention and hyperactivity of middle school students, Methods: Jajun-maum-dom-movement program was applied to 14 middle school students for 8 weeks, To measure the improvement of attention and decrease of hyperactivity, these tests were used - ADHD Diagnostic System(hereinafter ADS), Parent's Abbreviated Conners Rating Scale(hereinafter ACRS), Teacher's Korean ADHD Rating Scale(hereinafter K-ARS). For each variable in the 3 test methods, the results of before and after program were measured and then, the results were assessed by paired t-test, Interrelations between the resulted values of 10 variables were analyzed in these three groups: scores before program, scores after program, and change in scores before and after program, Among 10 variables, for variable showing significant difference by t-test, it was assessed by ANCOVA whether change in scores before and after program is affected by the grade and gender. Results: As for the changed score between before-the-program and after-the-program, in the ADS test, it was found that there is a significant effect in the "commission error" which is one of the indices showing the degree of hyperactivity, and "sensitivity(d')" which is one of the indices showing attention, and it was found that there is a significant effect in ACRS and K -ARSC total), Among the correlations of the 4 variables, "ACRS" and "K-ARSC total) ", which show the comprecessive criteria, have the positive correlation of significance in before-the-program, after-the-program and the changed score between before-the-program and after-the-program, However, there was no specific correlations among sub-variables about attention or hyperactivity, and in all of 4 variables, there was no significant degree of change according to grade or gender. Conclusions : As the results of assessment by ADHD Diagnostic System, Parent's Abbreviated Conners Rating Scale, Teacher's Korean ADHD Rating Scale to study the effects of Jajun-maum-dom-movement program on attention and hyperactivity, it is known that the program is helpful for improvement of attention and decrease of hyperactivity for the participated students.

The Evaluation of Permanent Impairment in Chronic Pain Patients (통증질환의 장애인정 여부에 대하여)

  • Pain is one of the most common causes of disability and work loss. However, patients with chronic pain have not been regarded as impaired persons in the Republic of Korea because of a lack of objective findings in those patients and difficulties in validating their genuine suffering and quantitative evaluation of pain. Here, the difficulties in the evaluation of impairment in chronic pain patients are discussed. In addition, guidelines for rating impairment in chronic pain patients, which are under development by the Korean Academy of Medical Sciences, are introduced.

  • The purpose of this paper is to present a model to design the layout of the user interface components that handles many numbers of qualitative factors. An alternate rating system is also proposed for the closeness relationship ratings between the various pairs of components evaluated by using GOMS (goals, operators, methods and selection rules) technique. The proposed model is applied to the design of the part of the user interface in order to obtain the best layout of the components. The results of the proposed model are compared with that of an existing model, which handles single qualitative factor applied to obtain the layouts of user interface components.
