우리나라 성인 여성의 항산화비타민 섭취량과 비만의 연관성 : 2007 ~ 2016년 국민 건강영양조사 자료를 이용하여 (Association between antioxidant vitamin intake and obesity among Korean women: using the Korea National Health and Nutrition Examination Survey 2007 ~ 2016)

  • 본 연구에서는 2007 ~ 2016년 국민건강영양조사의 검진 조사와 식품섭취조사를 완료한 만 19세 이상의 성인 여성 30,425명을 대상으로 항산화비타민 섭취량과 비만 및 복부비만의 연관성을 분석하였다. 체질량지수가 $25kg/m^2$ 이상이거나 허리둘레가 85 cm 이상인 대상자를 각각 비만과 복부비만으로 판정하였고, 개인별 일일 항산화비타민 섭취량은 24시간 회상법을 이용한 식사자료와 한국인 상용식품의 항산화비타민 함량 데이터베이스를 연계하여 추정하였다. 항산화비타민에는 카로티노이드 6종, 레티놀, 비타민 A (레티놀 활성 당량), 비타민 C, 토코페롤 4종, 비타민 E (${\alpha}$-토코페롤 당량)이 포함되었다. 정상군과 비만군의 열량 1,000 kcal당 일일 항산화비타민 섭취량의 평균을 비교한 결과, 비만군은 ${\alpha}$-카로틴, 레티놀, 비타민 E, ${\alpha}$-토코페롤, ${\gamma}$-토코페롤을 정상군보다 적게 섭취한 것으로 나타났다. 항산화비타민 섭취수준에 따라 대상자를 삼분위로 구분했을 때, 라이코펜을 가장 많이 섭취하는 군은 가장 적게 섭취하는 군에 비해 비만 (OR = 0.89, 95% CI: 0.83-0.96)과 복부비만 (OR = 0.88, 95% CI: 0.81-0.95) 유병률이 유의하게 낮았다. 총 카로티노이드, ${\alpha}$-카로틴, 비타민 A, ${\gamma}$-토코페롤 섭취량 또한 복부비만의 오즈비와 음의 연관성을 나타냈다. 식품군별 항산화비타민 섭취량을 비교했을 때, 비만군은 정상군에 비해 난류, 우유류, 조미료류, 곡류를 통한 항산화비타민 섭취량이 적었다. 추후 전향적 설계를 이용한 후속연구를 수행하여 항산화비타민 섭취량과 비만 간의 인과성을 확인할 필요가 있을 것으로 사료된다.

External cross-validation of bioelectrical impedance analysis for the assessment of body composition in Korean adults

  • Bioelectrical impedance analysis (BIA) models must be validated against a reference method in a representative population sample before they can be accepted as accurate and applicable. The purpose of this study was to compare the eight-electrode BIA method with DEXA as a reference method in the assessment of body composition in Korean adults and to investigate the predictive accuracy and applicability of the eight-electrode BIA model. A total of 174 apparently healthy adults participated. The study was designed as a cross-sectional study. FM, %fat, and FFM were estimated by an eight-electrode BIA model and were measured by DEXA. Correlations between BIA_%fat and DEXA_%fat were 0.956 for men and 0.960 for women with a total error of 2.1%fat in men and 2.3%fat in women. The mean difference between BIA_%fat and DEXA_%fat was small but significant (P < 0.05), which resulted in an overestimation of $1.2{\pm}2.2$%fat (95% CI: -3.2-6.2%fat) in men and an underestimation of $-2.0{\pm}2.4$%fat (95% CI: -2.3-7.1%fat) in women. In the Bland-Altman analysis, the %fat of 86.3% of men was accurately estimated and the %fat of 66.0% of women was accurately estimated to within 3.5%fat. The BIA had good agreement for prediction of %fat in Korean adults. However, the eight-electrode BIA had small, but systemic, errors of %fat in the predictive accuracy for individual estimation. The total errors led to an overestimation of %fat in lean men and an underestimation of %fat in obese women.

건강검진 수검자들의 대사증후군 위험인자별 유병률 및 비만지표와의 관련성 (Prevalence Rates of Risk Factors of Metabolic Syndrome, and Its Related with Obesity Indices Among the Health Checkup Examinees)

  • 본 연구는 일반 성인을 대상으로 대사증후군 위험인자의 유병률을 파악하고, 각 대사증후군 위험인자들과 비만지표들과의 관련성을 파악하며, 비만지표들의 대사증후군을 예측하기 위한 관련성과 타당도를 검토하고자 시도하였다. 조사대상은 2014년 2월부터 11월까지의 기간에 한국건강관리협회 D지부 건강검진센타에서 종합건강검진을 받았던 20세 이상 1,051명을 대상으로 하였다. 연구결과, 조사대상자의 대사증후군 유병률은 21.5%이었으며, 각 위험인자의 유병률은 허리둘레의 경우 남자 31.8%, 여자 41.6%이었으며, TG는 남자 35.6%, 여자 17.3%이었고, HDL-C는 남자 17.6%, 여자 34.2%이었다. 혈압은 남자 53.5%, 여자 35.9%이었으며, FBS는 남자 14.9%, 여자 6.8%이었다. 이 같은 유병률은 연령이 높은 군일수록, BMI가 높은 군일수록, 음주횟수가 많은 군일수록, 유의하게 높았다. 비만지표별 대사증후군 위험요인 개수별 예측도를 보면 허리둘레/신장비가 높게 나타났다. 이상과 같은 결과를 볼 때, 다른 비만지표에 비해 허리둘레/신장비가 대사증후군 위험요인을 선별하는데 좀 더 예측력이 있다고 나타났다. 허리둘레/신장비는 간단하고 실용적인 복부비만의 지표가 될 수 있으며 대사증후군에 대한 선별검사로서 체질량지수와 허리둘레보다 우수한 것으로 나타났다.

한국 성인의 요중 크레아티닌 농도 변화에 대한 성, 연령 그리고 체질량지수(BMI)의 관련성 연구 (Relevance of Gender, Age and the Body Mass Index to Changes in Urinary Creatinine Concentration in Korean Adults)

  • The purpose of this study was to investigate the relevance of gender, age, and BMI (Body Mass Index) to changes in the urinary creatinine concentration in Korean adults. We recruited and surveyed 2,156 persons $\geq$20 years of age from 98 districts across the country, and collected urine samples for analyzing the creatinine concentration. Participants were 41.6% men and 58.4% women. In terms of age, the percentage of the population in their 20's and $\geq$60 years of age was relatively similar, with 13.6% and 17.8%, respectively, while the percentages in their 30's, 40's and 50's were also relatively similar, with 22.0%, 24.0%, and 22.6%, respectively. The proportion of participants in their 20's and 30's who were underweight (according to the BMI) was 25% and 19.2%, respectively, but only 6.2~6.4% of those $\geq$40 years of age were underweight. This showed that the underweight proportion in elder generations was less than that in younger generations. Meanwhile, the proportion who were overweight was 13.7%, 18.8%, 28.6%, 27.9%, 32.0% when the participants were in their 20's, 30's, 40's, 50's and $\geq$60 years of age, respectively (p<0.000). This showed that the overweight proportion increased along with increasing age. The overall urinary creatinine concentration was 10 mg/dl (arithmetic mean: AM), and 92.2 mg/dl (geometric mean: GM). The urinary creatinine concentration in men (132.6 mg/dl, AM) was significantly higher than that in women (93.3 mg/dl, AM (p<0.000)). Showing a similar trend in men and women, urinary creatinine concentrations were highest when the participants were in their 20's (135.6 mg/dl, AM), and tended to decrease with increasing age. Urinary creatinine concentrations in overweight and obese subjects (AM of 117.9 mg/dl and 118.0 mg/dl, respectively) were significantly higher than in other groups, and this trend was similar in men and women. In conclusion, we found that urinary concentrations were significantly affected by gender, age, and BMI, and that care should therefore be exercised when correcting urinary metabolites according to the urinary creatinine concentration.

비만 중년여성의 댄스스포츠 참여가 신체조성과 혈중지질에 미치는 효과: 메타분석 (Effect of Dance Sports Participation in Obesity Middle-Women on Body Composition and Blood Lipids: Meta-Analysis)

  • 본 메타분석 연구의 목적은 비만 중년여성의 댄스스포츠 참여가 신체조성과 대사질환관련요인에 미치는 영향을 분석하는데 있다. 키워드를 '비만', '여성', '댄스스포츠'로 하여 검색된 2005년 1월 1일부터 2015년 11월 30일 사이의 논문 중, 총 9개의 연구를 최종적으로 선정하였다. 연구에 포함된 총 대상자의 수는 197명(실험군: 98명, 대조군: 99명)으로 댄스스포츠 참여기간은 평균 12.2주, 주당 3.13회, 회당 75분이 진행되었다. 본 운동에서 적용된 댄스스포츠 종목으로는 자이브와 차차차가 진행되었고, 그 중 2편의 연구에서는 룸바도 진행되었다. 메타분석 결과, 비만 중년여성에게 댄스스포츠는 체중, 체지방률, 중성지방, 저밀도 지단백 콜레스테롤, 렙틴을 감소시키는데 효과가 있었으며, 고밀도 지단백 콜레스테롤을 증가시키는데 효과가 있었다. 그러나 댄스스포츠가 체질량지수, 총콜레스테롤, 인슐린, 혈당에 미치는 효과에 대해서는 근거를 제시할 수 없었다. 또한, 효과의 크기로는 체지방량, 총콜레스테롤, 고밀도 지단백 콜레스테롤, 저밀로 지단백 콜레스테롤은 큰 효과크기를 나타내었고, 중성지방은 중간 효과크기를 나타냈으며, 체중, 렙틴, 인슐린, 혈당은 작은 효과크기를 나타냈다.

Effects of Obesity on Presentation of Breast Cancer, Lymph Node Metastasis and Patient Survival: A Retrospective Review

  • Background: As data on the relation between obesity and lymph node ratio are missing in the literature, we here aimed to assess the impact of obesity on this parameter and other clinicopathological features of breast cancer cases and patient survival. Materials and Methods: Medical data of 646 patients, all referred to two centers in Tehran, Iran, were reviewed. Factors that showed significant association on univariate analysis were entered in a regression model. Kaplan-Meier and Cox-regression were employed for survival analysis. Results: Obesity was correlated with the expression of estrogen and progesterone receptor (p=0.004 and p=0.039, respectively), metastasis to axillary lymph nodes (p=0.017), higher lymph node rate (p<0.001) and larger tumor size (p<0.001). The effect of obesity was stronger in premenopausal women. There was no association between obesity and expression of human epidermal growth factor receptor. Three factors showed independent association with BMI on multivariate analysis; tumor size, estrogen receptor and lymph node ratio. Obesity was predictive of shorter disease-free survival with a hazard ratio of 3.324 (95%CI: 1.225-9.017) after controlling for the above-mentioned variables. Conclusions: The findings of this study support the idea that obese women experience more advanced disease with higher axillary lymph node ratio, and therefore higher stage at the time of diagnosis. Furthermore, obesity was associated with poorer survival independent of lymph node rate.

슬림핏 청바지 패턴 개발을 위한 청바지 착용 실태 조사 (A Study on The Wearing Condition of Jeans for Development of Slim-fit Jeans Pattern)

  • This study provides information on how to improve the wearing sensation and the fit of slim-fit jeans through an analysis of actual wearing conditions and dissatisfaction. The study is based on a questionnaire survey. A survey was conducted on 296 women in their 20s living in Korea to analyze wearing dissatisfaction with slim-fit jeans. The study used descriptive statistics for analysis using SPSS Statistics Ver.23. Surveys on the actual situation of wearing jeans and level of dissatisfaction indicated that most women in their 20s mainly wore slim-fit jeans and were aware of the size of their jeans. Most tended to double-check the size of the jeans, indicating that the inaccurate sizing system of slim-fit jeans caused confusion for consumers. In addition, the results of the survey on the most considered parts of the body when consumers buy jeans are waist, thigh and leg length. They insisted that their waists were thin and their thighs were thicker than their waists; so their pants would not fit and they were obese. There is a problem between waist size and thigh size; therefore, it is necessary to develop new patterns for slim-fit jeans with improved fit around the waist area that can improve the negative results of surveys on the level of dissatisfaction, indicating discomfort in the waist, belly, and thigh areas while wearing slim-fit jeans.

The maternal prepregnancy body mass index and the risk of attention deficit hyperactivity disorder among children and adolescents: a systematic review and meta-analysis

  • Background: Attention deficit hyperactivity disorder (ADHD) symptoms have a major impact on individuals, families, and society. Therefore identification risk factors of ADHD are a public health priority. Purpose: This is meta-analysis evaluated the association between maternal prepregnancy body mass index and the risk of ADHD among the resulting offspring. Methods: The search identified studies published through December 2018 in the PubMed, Web of Science, and Scopus databases. The odds ratios (ORs) or hazard ratios (HRs) with 95% confidence intervals (CI) extracted from eligible studies were used as the common measure of association among studies. Results: A significant association was found between overweight women and the risk of ADHD among children with the pooled HR and OR estimates (HR, 1.27 and 95% CI, 1.17-1.37; OR, 1.28 and 95% CI, 1.15-1.40, respectively). This association was significant between obese women and the risk of ADHD among children and adolescents with the pooled estimates of HR and OR (HR, 1.65 and 95% CI, 1.55-1.76; OR, 1.42 and 95% CI, 1.23-1.61). Conclusion: The current epidemiological studies present sufficient evidence that prepregnancy overweight and obesity are significantly associated with an increased risk of ADHD among children and adolescents. These findings provide a new approach to preventing ADHD by controlling weight gain in the prenatal period, which should be considered by policymakers.

울산 지역 고혈압 노인의 비만 지표 및 혈중 총항산화도 (Obesity Indices and Plasma Total Antioxidant Status in Hypertensive Elderly Living in Ulsan Area)

  • The role of free radicals in the progression of many diseases and aging has been given a great attention and total antioxidant status (TAS) has shown to be reduced with aging. The incidence of hypertension has shown to be relatively high in the aged population, and it is known to be associated with increased obesity and oxidative stress. The aim of this study was to examine the obesity indices and the total antioxidant status in hypertensive elderly (64y$\alpha$-tocopherol, $\beta$-carotene and ascorbic acid did not show their significant correlations with TAS. These results showed that the hypertensive elderly are prone to be obese and to have increased TAS. Due to unknown factors affecting TAS values, measurement of plasma TAS as a sole indicator of total antioxidant capacity is limited to fully understanding changes in the body's free radical trapping power. However, the results from the current study may suggest that hypertension and/or obesity might increase oxidative stress, followed by increased the body's total antioxidant defense system.

20대 여대생의 체형 및 유방유형별 브래지어 착용실태조사 연구 (A Survey on Wearing of Brassieres according to Body and Breast Type of College Women)

  • We performed an assessment of wearing, purchasing, satisfaction, and the fit of brassieres according to body and breast type for college women to provide basic data on the manufacture of an ergonomic brassiere. The classifications of domed and conical breast type or projecting and conical breast type were unclear. Flat breast were dissatisfied with the shape of small and flat, and drooping breast were dissatisfied with the shape of big, projected, and drooping. In purchasing a brassiere, the majority of subjects were ignorant of their brassiere size, and over 80% of the subjects bought a brassiere without trying it on. Over 50% of those surveyed said they found it difficult to buy a well-fitted brassiere, particularly for the flat and drooping breast type. Over 70% of those surveyed wore a brassiere of 3/4 cup size, the slim body type wore a brassiere of 1/2 cup size; however, the obese body type wore a brassiere of full-cup size. The flat-breast or slim-body wore a brassiere of a thick pad type but projecting-breast or drooping breast wore brassiere of a thin pad type. Many of the subjects wore an unfitted brassiere in cup size and under bust size. In the classification of breast type, there were slight differences between the breast type that they recognized and the breast type that they were shown. There were distinctions in function, satisfaction, and fitness based on breast type. Therefore, the development of an ergonomic brassiere should considered the characteristics of breast type for fitness and comfort.
