Cognitive Impairments and Its Related Factors Among the Centenarians in Korea (우리나라 100세 이상 고령자의 인지기능장애 수준과 관련요인)

  • This study was conducted to determine the levels of cognitive impairment (MMSE-K) among centenarians, and to reveal their association with related variables. The study subjects were 268 centenarians who received medical check-ups from the National Health Insurance Corporation during the period of 2016-2017. The levels of cognitive impairment were significantly reduced among females as well as those living with spouses and children, lower monthly income, lower BMI and individuals who engaged in smoking and alcohol consumption without regular exercise relative to their respective counterparts. The risk of definitive cognitive impairment in study subjects was significantly higher among women (ORs=9.85, 95% CI=2.88-33.59) than men and was lower in obese group (ORs=0.25, 95% CI=0.07-0.86) than in low weight groups, as well as in non-smoking (ORs=0.30, 95% CI=0.10-0.83) than smoking groups. The factors influencing the cognitive impairment of study subjects were sex, living status, monthly income, BMI and smoking status. Taken together, these results suggest that the levels of cognitive impairments among centenarians differed significantly in response to socio-demographic characteristics and health related variables. Especially, the levels of cognitive impairments decreased among those with poor health related variables such as smoking, alcohol consumption and regular exercise.

Structural model updating of the Gageocho Ocean Research Station using mass reallocation method

  • To study oceanic and meteorological problems related to climate change, Korea has been operating several ocean research stations (ORSs). In 2011, the Gageocho ORS was attacked by Typhoon Muifa, and its structural members and several observation devices were severely damaged. After this event, the Gageocho ORS was rehabilitated with 5 m height to account for 100-yr extreme wave height, and the vibration measurement system was equipped to monitor the structural vibrational characteristics including natural frequencies and modal damping ratios. In this study, a mass reallocation method is presented for structural model updating of the Gageocho ORS based on the experimentally identified natural frequencies. A preliminary finite element (FE) model was constructed based on design drawings, and several of the candidate baseline FE models were manually built, taking into account the different structural conditions such as corroded thickness. Among these candidate baseline FE models, the most reasonable baseline FE model was selected by comparing the differences between the identified and calculated natural frequencies; the most suitable baseline FE model was updated based on the identified modal properties, and by using the pattern search method, which is one of direct search optimization methods. The mass reallocation method is newly proposed as a means to determine the equivalent mass quantities along the height and in a floor. It was found that the natural frequencies calculated based on the updated FE model was very close to the identified natural frequencies. In conclusion, it is expected that these results, which were obtained by updating a baseline FE model, can be useful for establishing the reference database for jacket-type offshore structures, and assessing the structural integrity of the Gageocho ORS.

Sasang Constitution may act as a Risk Factor for Hypertension and Pre-hypertension (고혈압 및 전기고혈압 위험요인으로서의 사상체질)

  • Objectives The purpose of this study was to reveal that Sasang constitution(SC) was associated with hypertension and pre-hypertension and could be a risk factor. Methods We introduced this study to educational personnel in D university in Daejeon, and 275 subjects joined this study. The SC classification was conducted with KS 15 questionnaire. The subjected measured the blood pressure with Jawon medical device automatically after 10 minute rest. The hypertension and pre-hypertension was classified by the guide of the Seventh Report of the Joint National Committee on Prevention, Detection, Evaluation, and Treatment of High Blood Pressure. The frequency analysis and T-test was used in general characteristics, and chi-square test was also used between SC and pre-hypertension and hypertension. Logistic regression was used to calculate the odds ratios (ORs) and 95% confidence interval (95% CI) for pre-hypertension and hypertension. Results The number of Taeeumin(TE), Soeumin(SE), and Soyangin(SY) was 142, 71, and 61 respectively. There was significantly different in systolic and diastolic blood pressure among SC types(p<.001). The distribution of the normal group, pre-hypertension and hypertension group by SC types was significantly different (p<.001). The ORs of TE was significantly increased (ORs 4.039, 95% CI=2.019-8.082 in pre-hypertension and ORs 4.235, 95% CI=1.581-11.348 in hypertension) compared with SE(p<.001), and after adjusting gender and smoking habit, it was still significantly different(p<.001). Conclusions It is possible that SC, especially TE could be a risk factor both pre-hypertension and hypertension.

Association between Socioeconomic Status and Hepatitis C Prevalence among Community-Dwelling Adults (전라남도 일지역 거주 성인의 사회경제적 지위와 C형 간염과의 관계)

  • Purpose: This study was conducted to evaluate the socioeconomic status as a risk factor for hepatitis C virus infection among community-dwelling adults in Korea. Methods: This study was a cross-sectional study including 2,617 adults. Data from hepatocellular carcinoma epidemiological investigation and information regarding three markers of socioeconomic status, education, employment, household income, were obtained. Multiple logistic regression was performed to calculate odds ratios (ORs) for hepatitis C virus infection for socioeconomic status markers, adjusting for the other soioeconomic status marker, behavioral and hepatitis-related factors. Results: Of the 2,617 study subjects, 104 subjects infected with hepatitis C virus and prevalence was 4%. When compared to subjects with a lowest education, ORs was 0.16 (95% confidence interval [CI]: 0.04-0.69) for highest education group. After adjusting behavioral and hepatitis-related factors, ORs was 0.20 (0.05-0.87) for highest education compared with lowest group. Conclusion: Low educational level was associated with an increased risk of hepatitis C virus infection.

Evaluation of Dietary and Life-Style Habits of Patients with Gastric Cancer: A Case-Control Study in Turkey

  • Objective: Gastric cancer is an important public health problem in the world and Turkey. In addition to Helicobacter pylori (H. pylori), smoking, alcohol consumption and family history, certain dietary factors have been associated with its occurrence. The impact of dietary habits and life-style factors on the risk of gastric cancer in Turkey were evaluated in this study. Design: A questionnaire was applied to 106 patients with gastric adenocarcinoma and 106 controls without cancer matched for age (range 28-85 years) and gender selected from a hospital based population. Adjusted odds ratios (ORs) and 95% confidence intervals (CI) were calculated with logistic regression analysis. Results: The incidence of H. pylori was 81.3% in patients. Frequent consumption of salty dishes, very salty foods like pickles, soup mixes, sausages, foods at hot temperature (ORs = 3.686, 7.784, 5.264, 3.148 and 3.273 respectively) and adding salt without tasting (OR = 4.198) were associated with increased gastric risk. Also heavy smoking and high amount of alcohol consumption (p = 0.000) were risk factors. Frequent consumption of green vegetables, onion, garlic and dried fruits (ORs = 0.569, 0.092, 0.795 and 0.041) was nonsignificantly associated with decreased risk. Conclusion: Improved dietary habits, reducing salt consumption and eradication of H. pylori infection may provide protection against gastric cancer in Turkey.

Factors Related to Anxiety and Depression of the Family Caregivers' with Dementia Patients: based on 2015 Community Health Survey (치매환자 가족부양자의 불안 및 우울 관련 요인: 2015년 지역사회 건강조사를 바탕으로)

  • Background & Objectives: This study aims to investigate the health status of family caregivers with dementia patients and identify the factors related to their anxiety and depression. Methods: Data from 2015 Community Health Survey(n=2,426) was used. A chi-square test was performed to investigate the health status of family caregivers, and a multiple logistic regression analysis was used to identify the factors associated with anxiety and depression. Results: Odds ratio(OR) of anxiety and depression was 1.29 times higher in female, 2.49 times higher in over 70 years versus under 39 years. ORs were lower 34.0%, 26.0%, 26.0% in the working group, the physical activity group, the alcohol drinking group respectively. ORs of anxiety and depression were 4.54 times, 1.57 times higher in the stress group, the chronic disease group respectively. And ORs were 61.0%, 28.0% lower respectively when social networks and social activities was present. Conclusions: The rate of experiencing anxiety and depression was high in family caregivers with dementia patients. It is necessary to provide diverse programs to reduce the burden of family support, anxiety and depression of family caregivers.

A Reversible Data Hiding Method for AMBTC Compressed Image without Expansion inside Stego Format

  • This paper proposes a new framework of reversible data hiding scheme for absolute moment truncation coding (AMBTC) compressed images. AMBTC-based RDH can be applied to optical remote sensing (ORS) image transmission, which achieves target region preservation and image compression simultaneously. Existing methods can be concluded as two types. In type I schemes, stego codes mimic the original AMBTC format where no file bloat occurs, yet the carried secret data is limited. Type II schemes utilize predication errors to recode quantity levels of AMBTC codes which achieves significant increase in embedding capacity. However, such recoding causes bloat inside stego format, which is not appropriate in mentioned ORS transmission. The proposed method is a novel type I RDH method which prevents bloat inside AMBTC stego codes with significant improvement in embedding capacity. The AMBTC compressed trios are grouped into two categories according to a given threshold. In smooth trio, the modified low quantity level is constructed by concatenating Huffman codes and secret bits. The reversible contrast mapping (RCM) is performed to complex trios for data embedment. Experiments show that the proposed scheme provides highest payload compared with existing type I methods. Meanwhile, no expansion inside stego codes is caused.

Association Between VDR Polymorphisms and Breast Cancer: An Updated and Comparative Meta-analysis of Crude and Adjusted Odd Ratios

  • There is a lot of debate on the relationship between vitamin D receptor polymorphisms and risk of breast cancer. Herein, we quantitatively analyzed the published case-control studies on this relationship by meta-analysis, performing a bibliographic search from Pubmed and CNKI up to July 31, 2013. The included case-control studies for Fok1, Bsm1, Taq1, Apa1, Cdx2 and Poly-A were 16, 19, 20, 10, 4, 6, respectively. Crude and adjusted odd ratios and 95% confidence intervals were calculated to present and compare the strength of any associations. The results of combined analyses indicated that Fok1, Bsm1, Apa1, Cdx2 and Poly-A were not significantly associated with the risk of breast cancer. In contrast, the tt genotype of Taq1 was a modest risk factor for breast cancer development (tt vs. TT: OR = 1.21, 95% CI: 1.01-1.44). To further confirm the above results, adjusted effects for the six polymorphisms were pooled based on adjusted ORs reported in the original studies. Adjusted ORs of Fok1, Apa1, Cdx2 and Poly-A were similar to the crude ORs. However, Bsm1 and Taq1 showed inconsistent results. For Bsm1, OR for BB vs. bb was 0.85, 95% CI: 0.74-0.98; for Taq1, OR for tt vs. TT was 1.03, 95% CI: 0.92-1.15, and not associated with risk. Subgroup analyses for crude ORs showed some association between Bsm1, Taq1 and breast cancer in Caucasians only, but for adjusted ORs, no associations were found. This meta-analysis suggests that the roles that Fok1, Apa1, Cdx2 and Poly-A polymorphisms play in breast cancer risk are negligible, with Bsm1 and Taq1 as possible exceptions. To be conservative, we still assumed that they may play a modest role in determining breast cancer risk. Further studies are needed to validate our findings.

Preliminary Study on Usual Cold or Heat as a Risk Factor for Hypertension in an University Faculty (일개 대학 교직원 집단에서 고혈압의 위험요인으로서의 평소 한열 예비 연구)

  • This study was aimed to reveal that the usual cold or heat state was associated with hypertension and could be a risk. We emailed educational personnel in D university to join this study and 182 subjects participated in from March to December in 2016. The usual cold or heat diagnosis was conducted by two experts who had over 10 years expertise. The blood pressure was measured from the subjected after 10 minute rest with Jawon medical device. The hypertension was diagnosed by the guide of the Seventh Report of the Joint National Committee on Prevention, Detection, Evaluation, and Treatment of High Blood Pressure. The frequency analysis was used in general characteristics, Pearson's Correlation Coefficient analysis was conducted in among continuous variables, and chi-square test was also used between hypertension and cold or heat group. Logistic regression was analyzed to generate the odds ratios (ORs) and 95% confidence interval (CI) for hypertension. The cold score was suggested to have negative association with Body mass Index (BMI, -.374, p<.001), systolic blood pressure (-.333, p<.001), and diastolic pressure (-.261, p<.001). The heat score was analyzed to have positive association with Body mass Index (.413, p<.001), systolic blood pressure (.249, p<.001), and diastolic pressure (.156, p<.001). The distribution of the cold group (35.1%) and non-cold group (64.9%) in hypertension was significantly different (p=0.18). The distribution of the heat group (62.2%) and non-heat group (37.8%) was significantly different (p=0.27). The usual cold was associated with decreased ORs (ORs 0.405, 95% CI=0.191-0.857), and usual heat was associated with increased ORs (ORs 2.327, 95% CI=1.108-4.888). However, after adjusting body mass index, sex, and smoking, the association was not significantly different. It is possible that usual cold or heat associate with hypertension. Further study is needed to show that usual heat may be a independent risk factor for hypertension through follow up design.

A Study on the Adaptability of Oxygen Reduction System to Fire in Cold Storage through Fire Simulation Analysis (화재시뮬레이션 분석을 통한 냉장·냉동 창고 화재의 저산소 시스템 적응성에 관한 연구)

  • Purpose: The number of Cold Storages at home and abroad is on the rise, fires in large Cold Storages have recently occurred. As fires continue to occur and property damage is on the rise every year, the importance of preventing fires in large Cold Storage is growing. Method: Real Cold Storages were investigated on-site and fire cases were analyzed to derive and analyze fire risk, and the ORS, which is emerging as an adaptive fire prevention technology of Cold Storage, was investigated through FDS. Result: oxygen concentration 21, 15.7% and 17.7, 16.7% were analyzed through FDS, and flashover was reached within 3~4 minutes from 21, 17.7, 16.7%, but if oxygen concentration was lowered to 15.7%, it didn't ignite for 13 minutes. Conclusion: This study understood the concept and general part of the ORS, modeled the freezer through FDS, and analyzed the oxygen concentration to analyze the fire protection adaptability of the ORS. In the future, it is expected that large-scale empirical experiments and related regulations will be prepared to provide solutions for fire prevention in Cold Storages in blind spots of fire.
