  • Purpose: To evaluate the clinical characteristics and the functional results of the osteochondral lesions of the talus (OLT) treated with arthroscopic osteochondroplasty. Materials and Methods: The study was based on 25 cases (25 feet) of osteochondral lesion of talus that were treated with arthroscopic osteochondroplasty from May 1997 to June 2005 with at least 1 year follow-up. Follow-up period was average 21.8 months. The average age was 37.2 years old. Male patients were 22 while female were 3. Postoperative evaluation was performed with American Orthopedic Foot and Ankle Society (AOFAS) Functional score, patient satisfaction and complications. Results: Ninety-two percent of patients had trauma history. Seventy-two percent of the lesions were located at the medial dome. As for the cartilage classification, 56% were fibrillation/fissure lesions while 44% were flap tears. Visual analogue scale (VAS) pain score improved from 6.8 points to 2.2 points, and AOFAS score improved from 71.3 points to 87.3 points. Eighty-eight percent of the patients were satisfied with the surgery and average period of returning to previous job was 6.5 weeks. Conclusion: We concluded that the arthroscopic osteochondroplasty is a viable surgical option for the osteochondral lesion of the talus.

  • In this paper, we proposed the MAC protocol of APON supporting multi-class traffic such as CBBUVBR, ABR, UBR, to guarantee the required QoS of each service. For this, we analyze the performance of variousrequest mechanisms and employee the different request mechanism for each traffic classes. Upstream anddownstream frame structures to minimize transmission overhead are proposed based on our request mechanism. The proposed MAC protocol applies the different priority to permit distribution process. CBBWBR traffic, withthe stringent requirements on CDV or delay, is allocated prior to any other class. ABR traffic, which hasnon-strict CDV or delay criteria, uses flexibly the available bandwidth but ensures a minimum cell rate (MCR).UBR traffic is allocated with lowest priority for the remaining capacity. The performance of proposed protocol isevaluated in terms of transfer delay and 1-point CDV with various offered load. The result of simulation showsthat the proposed protocol guarantees the required QoS of the corresponding category, while making use of theavailable resources in both an efficient and dynamical way.

  • Purpose The purpose of this study was to investigate the change of balance ability by performing closed chain exercise on stable support surface and unstable support surface in twenties. Methods This study randomly selected 15 students in the closed chain exercise group on the stable support side and 15 on the closed chain exercise group on the unstable support side. Balance ability was measured before and after the start of exercise and static balance was measured by OLT(One Leg Standing Test) and FRT (Functional Reach Test). Result The changes of the function reach test of the closed chain movement according to the ground type were significant in the unstable and stable support surfaces and the change of function reach test after the intervention in the two groups was significantly improved compared with the closed chain movement respectively. The one leg standing test changes of the closed chain movement according to the ground type showed significant results on the unstable and stable support surfaces, but there was no significant difference in the one leg standing test changes after intervention between the two groups. Conclusion The effect of closed chain training on ground type is unstable. The change of function reach test and one leg standing test of the closed chain exercise group on the stable support surface resulted in significant changes after exercise, but there was a significant difference in the balance ability of function reach test change after intervention between the groups.

  • The threshold level of a receiver is analyzed for the simplification of system and the improvement of upstream data ratios in optical subscriber network of which the upstream date rate and the optical transmitted power are changed to meet the requested BER (Bit Error Rate) defined per interactive multimedia services. In asynchronous optical subscriber network of which the upstream to downstream data ratios are 1:1/2, 1:1/4, 1:1/8 and 1:1/16 with manchester coded downstream and NRZ (Non Return to Zero) downstream re-modulation, the BER performance is theoretically analyzed and it is performed by simulation with MATLAB according to the four types of downstream data for four models. The results have shown that in the cases which the upstream to downstream data ratios are 1:1/4, 1:1/8 and 1:1/16 the conventional receiver with threshold level of 1/2 can be applied regardless of average received optical powers and the BER is not much deteriorated compared with using the optimal threshold level. In the case that the upstream to downstream data ratio is 1:1/2 the threshold level in an optical receiver could be fixed at 1/3 and the BER is not much deteriorated compared with using the optimal threshold level as the average received optical power increases.
