요통 여성 노인의 장애물과 이중 과제 보행 시 속도 및 입각기 시간 분석 (Analysis of Gait Velocity, Stance time on Obstacle and Dual Task Gait in Elderly Women with LBP)

  • Purpose : This study was to evaluate gait velocity and stance time on 5 conditions in elderly women and elderly women with LBP. The subjects were 70's generation. Methods : The subjects are 20 divided into 2 groups. They measured gait velocity, stance-time. The 5 conditions were normal gait, 10cm obstacle gait, 25cm obstacle gait, dual 10cm obstacle gait, dual 25cm obstacle gait. The experimental period was between 2008/12 and 2009/2. Statistical analysis was used Repeated measurement for difference between conditions, independent t-test for difference in two groups. Subjects were countdown from 50 during dual task gait. Results : The results were as follow: there were significantly difference 10cm obstacle velocity, dual 10cm obstacle velocity in two group. The others were not significantly differences. Velocity and stance-time were significantly difference in control group. In dual 25cm obstacle gait, velocity was difference of normal gait. Stance-time was difference in 25cm obstacle gait, and dual 25cm obstacle gait. In Experimental group, velocity and stance-time were not significantly difference. But measured value of velocity was gradually decreased and stance time was increased. Conclusion : These results indicate that elderly people with LBP women are reduced gait ability in dual task, and obstacle condition. So they need to prevent falling in dual task, and obstacle gait and to train obstacle/dual tak gait.

여성 노인의 장애물과 이중 과제 보행 시 속도 및 하지근육 활성도 변화 (Analysis of Gait Velocity, Lower Muscles Activity on Obstacle and Dual Task Gait in Elderly Women)

  • Purpose : This study was to evaluate gait velocity and muscle activity on 3 different gait conditions in elderly women. Methods : There were one group was 20's generation(n=12), and the other group was over 60's generation (n=12). The velocity of gait was analysed for using the Footscan system, EMG(MP150) for muscle activity. Subjects were measured 3 gait condition(normal, obstacle-10cm, dual obstacle-10cm. Measured values were used by the independent t-test for analysing between groups, and repeated measurement of ANOVA for analysing within group. Results : The results were as follow: velocity and acticities of lower 4 muscles were significantly differences in experimental group. In control group, there were not significantly differences. Contrast-groups were significantly differences of velocity(obstacle, dual obstacle), muscle activities(soleus-dual obstacle, hamstring-all). Conclusion : These results indicate that elderly people are reduced gait ability in dual task, and obstacle gait. So Elderly women need to training obstacle/dual gait for preventing falling.

장애물보행훈련이 뇌졸중 환자의 보행 및 균형에 미치는 효과 (The Effects of Obstacle Walking training on Gait and Balance of Stroke Patients)

  • Purpose : The purpose of the study was to find out the effects of obstacle walking training according on gait and balance in stroke patients. Methods : Twenty-four stroke patients are randomly assigned to experimental group 1 (n=8), experimental group 2 (n=8) and control group (n=8). Experimental group 1 performed unexpected obstacle walking training, experimental group 2 performed fixed obstacle walking training and control group performed non obstacle walking training for 12 minutes per session, 5 times a week for 4 weeks. The gait analyzer G-walk were evaluated using gait cadence, gait velocity, and stride length, balance was evaluated using FES-K and BBS. Results : In within-group comparison of gait cadence, gait velocity and stride length of change, the experimental 1,2 groups showed significant improvements post intervention (p<.05) but control group showed no significant improvement. In between-groups comparison there was significant difference in the change of gait cadence, gait velocity and stride length pre and post intervention. In within-group comparison of FES-K and BBS scores, the experimental 1,2 groups showed significant improvements post intervention (p<.05) but control group showed no significant improvement. In between-groups comparison there was significant difference in the change of FES-K and BBS scores pre and post intervention (p<.05). Conclusion : Obstacle walking training can improve the gait, function and balance of stroke patients and obstacle walking training is effective for improving gait and balance compared to non-obstacle walking training.

입체시력 감소가 장애물 보행에 미치는 영향 (Effects of induced stereoacuity reduction on obstacle crossing)

  • 본 연구는 정상시를 가진 정상인을 대상으로 입체시 부족을 유발하여 장애물 보행 시 발생될 것으로 생각되는 하지관절의 운동 변화에 대한 운동학적 분석과 지면반력의 변화를 고찰하는 것이다. 본 연구의 대상자는 입체시 테스트를 거쳐 통과한 18명이 연구에 참여하였다(age: 22.1±2.7 years, height: 176.8±4.4 cm, weight: 67.6±5.8 kg). 3차원 동작분석 시스템과 지면반력기를 이용하여 분석한 결과는 다음과 같다. 보행속도는 장애물 보행 시 느리게 나타났다. 고관절 각변위는 대부분 보행구간에서 장애물 보행 시 굴곡이 크게 나타났다. 무릎관절 각변위는 모든 보행구간에서 장애물 보행 시 굴곡이 크게 일어났고, TO와 FC2에서 입체시 감소의 영향으로 굴곡이 크게 나타났다. 발목관절 각 변위는 FC2에서 장애물 보행 시 굴곡이 크게 나타났다. 몸통기울기는 MSt, TO, MSw에서 장애물 보행 시 신전이 크게 나타났다. 지면반력은 Fx 값(내외측힘)에서 차이가 나타나지 않았지만, Fy 값(전후힘)에서 좌우발 모두 장애물 보행 시 전방 최대힘(추진력)이 크게 나타났고, 후방 최대힘(제동력)은 오른발은 입체시부족 보행 시 크게 나타났으며, 왼발은 장애물 보행 시 크게 나타났다. Fz 값(수직힘)은 최대힘-1과 최대힘-2에서 좌우발 모두 장애물 보행 시 최대 힘이 크게 나타났고, 계곡힘에서 오른발은 입체시부족 보행이 정상시 보행보다 작은힘이 나타났다.

Correlation Between Executive Function and Walk While Crossing Over an Obstacle Under Different Gait Phases

  • Background and Purpose: Dual walking task such as crossing over an obstacle may serve as an excellent tool for predicting early cognitive decline. Thus, this study aimed to investigate correlation between walking while crossing over an obstacle and executive functions under different gait phases to validate the use of walking with an obstacle for predicting early cognitive decline. Methods: A cross-sectional study was conducted on 48 elderly individuals from 2 day-care centers and 3 welfare-centers in Seoul and Gyeonggi, Korea. Executive function tests (Trail Making Test, Stroop test) and dual walking tests (gait speed, cadence, stance time, gait cycle time) were performed and compared using partial correlation analysis. Results: There were significant correlations between executive function and most of the gait variables (stance time, cadence, and gait cycle time) (p<0.05) when crossing over an obstacle while walking. Especially, stance time exhibited significant correlations with most executive functions (p<0.05). Conclusions: When evaluating executive function during walking with an obstacle, post-obstacle-crossing phase and stance time need to be observed.

Effects of Ground Obstacle Walking Combined with Treadmill Training on Gait Ability in Chronic Stroke Patients -A Preliminary Study-

  • Purpose: Gait training for stroke patients focuses on adjusting to new environments to facilitate outdoor walking. Therefore, the purpose of this study was to identify the effects of various ground obstacle walking combined with treadmill walking on the gait parameters and functional gait ability of chronic stroke patients. Methods: Twenty-four chronic stroke patients were divided into two groups: an experimental group (n = 12) and a control group (n = 12). The experimental group received a combined gait training using various ground obstacle walking and treadmill walking (VGOW) five times/week for four weeks. The control group received traditional treadmill training (TW) five times/week for four weeks. Patients were evaluated using the figure-8 walk test (F8WT) and the Functional Gait Assessment (FGA) before and after each intervention. Results: The ANCOVA results showed that both treatments significantly influenced F8WT steps, F8WT time, and FGA score. The paired t-test results showed a significant improvement in F8WT steps, F8WT time, and FGA score in the experimental group compared to those in the control group. Conclusion: Combined gait training using various ground obstacle walking and treadmill walking can improve gait ability in chronic stroke patients.

Effects of Task-Specific Obstacle Crossing Training on Functional Gait Capability in Patients with Cerebellar Ataxia: Feasibility Study

  • Purpose: The purpose of this study was to examine the effects of a task-specific obstacle crossing rehabilitation program on functional gait ability in patients with cerebellar ataxia. Overall, we sought to provide ataxia-specific locomotor rehabilitation guidelines for use in clinical practice based on quantitative evidence using relevant analysis of gait kinematics including valid clinical tests. Methods: Patients with cerebellar disease (n=13) participated in obstacle crossing training focusing on maintenance of dynamic balance and posture, stable transferring of body weight, and production of coordinated limb movements for 8 weeks, 2 times per week, 90 minutes per session. Throughout the training of body weight transfer, the instructions emphasized conscious perception and control of the center of body stability, trunk and limb alignment, and stepping kinematics during the practice of each walking phase. Results: According to the results, compared with pre-training data, foot clearance, pre-&post-obstacle distance, delay time, and total obstacle crossing time were increased after intervention. In addition, body COM measures indicated that body sway and movement variability, therefore posture stability during obstacle crossing, showed improvement after training. Based on these results, body sway was reduced and stepping pattern became more consistent during obstacle crossing gait after participation in patients with cerebellar ataxia. Conclusion: Findings of this study suggest that task-relevant obstacle crossing training may have a beneficial effect on recovery of functional gait ability in patients with cerebellar disease.

장애물 보행훈련이 뇌졸중 환자의 균형, 보행 및 일상생활활동에 미치는 영향 - 예비연구 (The effect of obstacle gait training on balance, gait and ADL of the stroke patient - pilot study)

  • Background: The purpose of this study is intended to evaluate the effectiveness and analyse the change on balance, gait and ADL of the stroke patient by obstacle gait training. Design: Randomized controlled trial. Methods: Ten subjects with stroke patient are recruited from C rehablitation hospital located Chungbuck. The subjects which are divided to 2 group as experimental group (N=5) conducted the obstacle gait training and control group (N=5) conducted the general gait training. The gait (10mWT), balance (BBS) and ADL (FIM) were measured before and after intervention. Results: There were no significant differences (p>.05) in sex, age, height, weight and K-MMSE among subjects. There were significant differences (p<.05) in balance and gait between of the experimental group and control group. But there was no significant difference (p>.05) in the ADL between of the experimental group and control group. There were significant differences (p<.05) gait and balance in the experimental group. But there were no significant difference (p>.05) gait, balance and ADL in the control group. Conclusion: Obstacle gait training showed positive effects on the gait, balance of the stroke patient.

노인의 실행기능 평가와 보행 평가사이의 상관관계 분석 (Analysis of the Correlation Between Executive Function and Obstacle Gait Evaluation for the Elderly)

  • PURPOSE: This study aimed to evaluate the correlation between executive function and gait evaluations for the elderly, and validate the obstacle gait evaluation as a cognitive impairment test tool. METHODS: This study was a cross-sectional design. 79 people aged 65 years or older were selected as subjects. The Korean version of the Mini-Mental State Examination (MMSE-KC) to evaluate overall cognitive function and the Trail Making Test (TMT) A, B to measure executive function were performed. The 4-meter walking speed test and the walking speed test while crossing over an obstacle were carried out to evaluate gait. The Spearman's correlation was used to measure the correlation between cognition and gait speed. RESULTS: There was no significant correlation between the 4 m gait speed and executive function( TMT-A (p = .056), TMT-B (p = .115)). However, there was a significant correlation between the 4 m gait speed and MMSE-KC (r = .277, p < .05). There was also a significant correlation between walking speed while crossing over an obstacle and all tests (MMSE-KC (r = .382, p < .01). TMT-A (r = -.327, p < .01), TMT-B (r = -.283, p < .05)). CONCLUSION: It was found that the gait speed while crossing over an obstacle was correlated with all cognitive function tests. Therefore, we suggest the use of the gait speed test while crossing over an obstacle rather than the simple gait test to diagnose cognitive decline.

장애물 넘기 트레드밀 보행 훈련이 편마비 환자의 정적 및 동적 균형 능력에 미치는 영향 (Effects of Treadmill Gait Training with Obstacle-Crossing on Static and Dynamic Balance Ability in Patients with Post Stroke Hemiplegia)

  • PURPOSE: This study was conducted to determine the effects of treadmill gait training with obstacle-crossing on the static and dynamic balance ability of patients with post stroke hemiplegia. METHODS: Twenty-one patients with post stroke hemiplegia were divided into three groups as: treadmill gait training with obstacle-crossing (TOG, n=7), treadmill gait training without obstacle-crossing (TGG, n=7) and a control (CON, n=7). TOG and TGG performed exercise for 20 minutes, three times a week for 8 weeks. Static balance ability (stability typical, ST; weight distribution index, WDI; fourier harmony index, FHI; and fall index, FI) and dynamic balance ability (berg balance scale, BBS and timed up and go test, TUG) were measured before and after 8 -weeks in each exercise group. Statistical analyses were conducted using two-way ANOVA with repeated measures, a paired t-test, and multiple comparisons according to Tukey's HSD. RESULTS: FHI and BBS were significantly increased at TOG (p<.01) and TGG (p<.05) after 8-weeks compared to before treadmill gait training with obstacle-crossing. FHI and BBS were significantly increased at TOG compared with CON and TGG (p<.05). CONCLUSION: Treadmill gait training with obstacle-crossing was more effective than that without obstacle-crossing to improve posture control and independent daily life performance of hemiplegia patients.
