의료용 N95 마스크 착용방법에 대한 교육 전·후 밀착계수 비교 (Comparison of Fit Factor for Healthcare Workers Before and After Training with the N95 Mask)

  • Objectives: This study compares the differences of fit factors before and after training on the N95 mask. The results will be utilized to suggest the need of providing effective training on respirator use. Methods: A total of 49 study subjects were tested, comprised of nurses from a general hospital and undergraduate nursing students from a medical school. Anthropometric measurements of face length and face width were compared with the NIOSH(National Institute for Occupational Safety and Health) panel. Fit factors(FF) were measured with TSI Portacount Pro+8038 before and after on-site training regarding the proper use of respirators. The FF pass/fail criterion was set at 100. Results: Two subjects(4.1 %) passed the fit test before training on use of the N95. However, 36(73.5%) of the 49 passed the test after training. Overall the FF(GM(GSD)) was 13.4(3.2) before training, but improved to 106.6(2.1) after training, which was statistically significant. These findings suggest the efficacy of educational intervention, and the performance of the direct on-site training proved to be better than that of the traditional educational methods. Conclusions: This study showed the effect of on-site training of the N95 respirator among health care workers(HCW). Therefore, providing effective training on the use of N95 for HCWs before their work assignments will greatly reduce exposure to harmful agents. It is recommended that fit testing be mandated to check for adequate protection being provided by the given respirators.

냉요법 적용방법에 따른 냉요법 효과에 관한 연구-건강한 성인 여성에서 스폰지 목욕방법을 중심으로 (A Study on the Effect of Cold Application Using a Sponge Bath in Healthy Adults)

  • This study was a quasi-experimental research study to test the characteristics of temperature regulation according to sponge bath methods of cold application. Thirteen volunteers were selected from among nursing college students according to an established criteria using a purposive sampling technique. Four different cold application methods were used: $\circled1$ tepid water sponge bath at $28^{\circ}C$, $\circled2$ 20% alcohol sponge bath at $28^{\circ}C$, $\circled3$ 40% alcohol sponge bath at $28^{\circ}C$ and $\circled4$ tepid water sponge bath at 28$^{\circ}$C plus an ice bag to the head. Changes in rectal temperature, mean skin temperature, mean body temperature, heat content change and thermal discomfort during the cold application were measured at 5 minute intervals over a 120 minute period. The data collection period was from Dec. 20, 1988 to Feb. 3, 1989. The data were analyzed using descriptive statistics, simple regression, ANOVA, Duncan's multiple range test and Pearson correlation coefficient using the SPSS-X Program. The results of the study are summarized as follows. Five general hypothesis were tested. Hypothesis 1 that "Change in heat content will be decreased for each cold application method according to the cold application time" was rejected. (tepid water sponge bath: after 10 minutes of cold application, 20% alcohol sponge bath: after 25 minutes of cold application: 40% alcohol sponge bath: after 45 minutes of cold application, tepid water sponge bath plus an ice bag to the head: after 80 minutes of cold application) Hypothesis 2 that "Thermal discomfort will be changed for each cold application method according to the cold application time" was rejected after 5minutes of cold application. Hypothesis 3 that "Change in heat content will differ among the cold application methods" was accepted except 0~5, 0~10, 0~65, 0~105 and 0~120 minute. This difference showed significance only between sponge bath methods and tepid water sponge bath plus an ice bag to the head. Hypothesis 4 that "Thermal discomfort will differ among the cold application methods" was accepted at 15, 20, 35, 45, 75, 80, 90, 95, 100, 105, 110, 115 and 120 minute of cold application time. This difference showed significance only between sponge bath methods and tepid water sponge bath plus an ice bag to the head. Hypothesis 5 that "The higher the change in heat content, the higher the thermal discomfort during the cold application time" was accepted for between 10~60 and 75 minute of cold application. In conclusion, this study showed that in sponge bath at $28^{\circ}C$, 10~80 minute was a effective cold application time in the view of heat loss through the skin. Concerning the effects of evaporation and thermal discomfort, it was found that there was no difference with regard to the solutions; tepid water sponge bath; 20% alcohol sponge bath or 40% alcohol sponge bath at a $28^{\circ}C$ controlled solution temperature. So it was thought that the type of solution itself did not have a big influence on the heat loss through skin. The combined effect of sponge bath with an ice bag to the head showed a significant difference and also showed a slight increase in thermal discomfort. On the basis of this research it can be concluded that cold application, for example, an ice bag to the head during a tepid water sponge bath is a good method as it increase heat loss through conduction, although fit can also cause a slight increase in thermal discomfort. The correlation between changes in heat content and thermal discomfort were not high. So factors other than change in heat content are considered to have an effect on the cognition of thermal discomfort.

임상 간호사의 교대근무 기간이 circadian rhythm 변화에 미치는 영향 (Effect of Shift Interval for the Clinical Nurse on the Circadian Rhythm)

  • Circadian rhythm is entrained in the 24-hour time interval by periodic factors in the environment, known as zeitgeber. But most rotating work schedules are outside the range of the entrainment of the pacemaker timing the human circadian sleep - wake cycle. It has been postulated that physiological and emotional disturbances occur in most human functions when the circadian rhythm is disturbed. So application of circadian principles to the design of shift schedules can aid in maintaining the temporal integrity of the circadian system and thereby minimize for the shift worker any detrimental consequences of circadian disruption. This study was a quasi-experimental study to test the effect of shift intervals for the clinical nurse on the circadian rhythm. Twenty nurses newly employed in general units of two hospitals were selected as an experimental group and twelve college nursing students as a control group. Both groups were selected according to an established criteria using a purposive sampling technique. Ten subjects were assigned to a weekly shift group and another ten to a biweekly shift group engaged in a semi -continuous shift schedule(sunday off) with a backward direction, that is, morning -evening - night shift. The control group worked a morning shift for 42 days. Oral temperature rhythm, waking tim, sleep - wake cycle, fatigue, and mental performance were measured during the experimental period. The data collection period was from April 30, 1990 to June 10, 1990. MANOVA, paired t-test, ANOVA, and Student Newman Keuls method were used for statistical analysis. The results are summarized as follows. 1. Phase delay in the acrophase of temperature rhythm was shown according to the backward rotating shift. A complete adaptation to work on the night shift was achieved between the sixth and ninth day of the night shift. 2. There was no difference in either waking time or sleep- wake cycle according to the duration of the working day for every shift group. Significant difference was found in the waking time and the sleep -wake cycle for subjects on the morning, evening, and night shift in both of the shift groups(weekly shift group : λ=0.121, p<0.01, λ=0.112, p<0.01, biweekly shift group : λ=0.116, p<0.01, λ=0.084, p<0.01). 3. There was no difference in fatigue between the first working day and the last working day for the control group and for the biweekly shift group. In the weekly shift group, physical fatigue was significantly different for the first day and the sixth day of the night shift(t=-2.28, p<0.05). Physical fatigue and total fatigue on the first day of the night shift showed a significant difference among the control group, the weekly shift group, and the biweekly shift group(F=5.79, p<0.01, F=4.56, p<0.05). There was a significant difference between the shift groups and the control group(p<0.05), Physical fatigue, neurosensory fatigue and total fatigue on the last day of the night shift showed a significant difference among the control group, the weekly shift group, and the biweekly shift group(F=12.65, p<0.01, F=7.77, p<0.01, F=9.68, p<0.01). There was a significant difference between the shift groups and the control group(p<0.05). 4. No difference in mental performance was seen between the first day and the last day of work in each case. An arithmatic test on the first day of the night shift revealed a significant difference among the control group, the weekly shift group, and the biweekly shift group(F=3.79, p<0.05). There was a significant difference between the shift groups and the control group(p<0.05) . The digital symbol substitution test and the arithmetic test on the last day of the night shift showed a significant difference among the control group, the weekly shift group, and the biweekly shift group(F=3.68, p<0.05, F=5.55, p<0.01), and both showed a significant difference between the shift groups and the control group(p<0.05). Accordingly, this study showed that during night duty, the waking time, sleep- wake cycle, and fatigue increased and mental performance decreased compared with morning and evening duty. It was also found that the weekly shift group had a higher fatigue score on the sixth day of night duty as compared to the -first day, but the waking time, sleep- wake cycle, and mental performance revealed no difference for the duration of the night duty or between shift groups, and complete adaptation of temperature rhythm was achieved between the sixth and ninth day of night duty. It is possible to conclude from these results that for intermediate circadian type in a healthy young woman, a biweekly shift system is more compatible with the circadian timing system than weekly shift system.

비강(鼻腔)에서 분리(分離)한 황색포도구균(黃色葡萄球菌)의 생화학적(生化學的) 성상(性狀)에 관(關)한 연구(硏究) (Studies on the Pathogenic Staphylococci aureus from Nasal Cavity)

  • 비병원근무자(非病院勤務者)인 부산대학교(釜山大學校) 자연과학대학(自然科學大學) 생물학계열(生物學系列) 18명(名), 미생물학과(微生物學科) 2학년(學年) 31명(名), 미생물학과(微生物學科) 3학년(學年) 37명(名)과 병원근무자(病院勤務者)인 간호과학생(看護科學生) 30명(名), 간호원(看護員) 30명(名), 수술근무간호원(手術勤務看護員) 30명(名), 의사(醫師) 30명(名), 총(總) 256명(名)을 대상(對象)으로 하여 비강(鼻腔)에서 황색포도구균(黃色葡萄球菌)을 분리(分離)하여 얻은 성적(成績)은 다음과 같다. 1. 비병원근무자(非病院勤務者) 대학생(大學生)에서 황색포도구균분리율(黃色葡萄球菌分離率)은 49예(例)(29.7%), 병원근무자(病院勤務者)에서는 67예(例)(55.8%)이었다. 2. 비병원근무자(非病院勤務者) 대학생(大學生)의 비강(鼻腔)에서 분리(分離)한 황색포도구균(黃色葡萄球菌)의 40예(例)에서 coagulase 양성균주(陽性菌株) 29예(例)(72.5%), 음성균주(陰性菌株) 11예(例)(27.5%)이고, mannitol 분해(分解) 양성균주(陽性菌株) 29예(例)(72.5%), 음성균주(陰性菌株) 11예(例)(27.5%)이었다. 3. 병원근무자(病院勤務者)의 비강(鼻腔)에서 분리(分離)한 황색포도구균(黃色葡萄球菌)의 67예(例)에서 coagulase 양성균주(陽性菌株) 59예(例)(58.1%), 음성균주(陰性菌株) 8예(例)(11.9%)이고, mannitol 분해양성균주(分解陽性菌株) 49예(例)(73.1%), 음성균주(陰性菌株) 18예(例)(26.9%)이었다. 4. 분리균주중(分離菌株中)에서 coagulase양성(陽性) 및 mannitol분해양성(分解陽性) 49균주(菌株)에 대(對)한 용혈반응(溶血反應)의 감수성(感受性)의 순위(順位)는 가토(家兎) 40예(例)(81.6%), 모르못트 25예(例)(53.5%), 면양(緬羊) 13예(例)(26.5%)이고, 닭 및 사람의 적혈구(赤血球)에는 용혈반응음성(溶血反應陰性)이었다. 5. 분리균주중(分離菌株中)에서 coagulase양성(陽性) 및 mannitol분해양성(分解陽性) 49균주(菌株)에 대(對)한 용혈반응(溶血反應)은 전주(全株)에 대(對)해서 각각(各各) 음성(陰性)이었다. 6. 화학요법제(化學療法劑)에 대(對)한 분리균주(分離菌株)에 대(對)한 성적(成績)은 비병원근무자(非病院勤務者) 분리균주(分離菌株)는 대부분(大部分) 감수성(感愛性)이 있으나 병원근무자(病院勤務者) 분리균주(分離菌株)는 대부분(大部分) 저항성(抵抗性)을 나타냄을 관찰(觀察)할 수 있었다.
