Effects of cardiopulmonary resuscitation reeducation on persistence of knowledge, performance and self-efficacy of nursing students (심폐소생술 교육 후 재교육이 간호대학생의 지식, 수행능력 및 자기효능감 지속에 미치는 효과)

  • Purpose: This study was undertaken to examine the effects regarding reeducation of cardiopulmonary resuscitation on persistence of nursing students' knowledge, performance, and self-efficacy. Methods: The research design for this study was a repeated experimental design featuring 35 female nursing students. Participants were educated using the standardized cardiopulmonary resuscitation protocol from the American Heart Association. Three months after the initial education, participants received reeducation about cardiopulmonary resuscitation. Knowledge and self-efficacy were measured before the initial education. Knowledge, performance, and self-efficacy were measured immediately after the initial education, 3 months later, and 6 months later. Collected data were analyzed using PASW Statistics 18.0. Results: Knowledge and self-efficacy significantly increased immediately after the initial education; knowledge, performance, and self-efficacy significantly decreased 3 months afterwards. No further decrease occurred until 3 months after reeducation. Conclusion: These results suggest that the duration between initial education and reeducation was insufficient, although the effects of cardiopulmonary resuscitation education were maintained 3 months after reeducation.

  • Purpose: This study aimed to present the management strategies necessary to improve the operational efficiency of visiting nurse centers and evaluate their effectiveness. Methods: The subjects of this study were visiting nurse centers registered as long-term care centers. Based on value chain analysis, cost information analysis, and data envelope analysis, the study was carried out according to the Magerison's management consulting procedure, for six months. This procedure comprised eight sub-steps of approach and application. Results: The following management strategies were agreed upon: establishment of a cooperative network with other visiting care centers, creation of high satisfaction of external customers by providing practical training to care workers, and making rehabilitation and exercise services as the core nursing activities to be focused on. Conclusion: The management consulting process and analysis method applied in this study can referred to as a useful methodological framework for revitalizing visiting nursing centers in the future.

Oral health of the elderly people receiving nursing care and home care serivces in Chungnam (충남 일부지역 요양기관과 재가 노인들의 구강보건에 관한 연구)

  • Objectives: The purpose of the study is to investigate the oral health of the elderly people receiving nursing care and home care services in Chungnam. Methods: The subjects were 350 elderly people receiving 21 nursing care and home care services in Chungnam. The direct interview with the elderly people and oral examination was carried out from July, 2012 to December, 2013 after explanation fo the purpose of the study. The subjects consisted of 178 elderly people receiving nursing care services and 172 elderly people receiving home care services. Except incomplete answers, 315 data were analyzed. The questionnaire consisted of general characteristics of the subjects, characteristics by facility, oral condition, oral care behavior, correlation by factors on oral health, influencing factor on dental caries, influencing factor on periodontal disease, and influencing factor on elasticity of gingival muscle. Data were analyzed by frequency analysis, chi-square test, and multiple regression analysis using SPSS 21.0 program. Results: Multivariate analysis of influencing factors on oral health revealed that the elderly people with low education level tended to have higher incidence rate of dental caries. The influencing factors on oral health were the elasticity of gingival muscle, periodontal disease, educational level, and economic level. (p<0.05). The explanation power was 26.2%. Conclusions: Oral Health Promotion should be obligatorily established as one of the medical system and medical fee system to promote oral health condition for the aged.

Determination of Nursing Price using Willingness to Pay (지불용의접근법을 이용한 간호서비스의 가격결정)

  • It will become more and more popular to use the long-term care facilities and home health care services with the chronic disease increasing. It depends on how much the consumers would pay and purchase the services. They might get more benefits from that kind of services than from ordinary hospitalization. So far, the study of determining the medical service price has focused most often on the efforts from the providers' view. But it must be reasonable to include the consumers' value for the service. This study was performed to assess WTP(Willingness to Pay) for home health care service in order to apply to the determination of nursing price in a reasonable manner. In this study, respondents were asked if they would pay for the service's intangible benefits under the four different types(open-ended minimum WTP, open-ended maximum WTP, bidding WTP, referendum WTP). The contingent valuation method is a potentially useful tool in understanding how people value the benefits of the service. As a result, average open-ended minimum WTP was W16,015 per day among 65 respondents. Average open-ended maximum WTP was W29,154 per day among 65 respondents. Average bidding WTP was W26,300 per day among 65 respondents. Average referendum WTP was W22,200 per day among 70 respondents. The results of regression analyses were also consistent with theoretical prediction, e.g., increasing WTP with consumers' value for the service, state of patients, and household income. This study demonstrated that it was more reasonable to consider the consumers' value in determining the services' price. In addition, a further study is needed to test the validity of this CV method and to determine a proper nursing price based on the consumers' view.

Occupational Status and Job Stress of Visiting Nurses for Public Health Services in Some Areas of the Capital During the COVID-19 Pandemic (코로나19 시기의 수도권 일부 지역 공공보건서비스 방문간호사의 직무실태 및 직무스트레스)

  • Purpose: This study investigated the occupational status and job stress of visiting nurses during the COVID-19 pandemic. Methods: Participants were 151 visiting nurses working in the public health services in Seoul and Gyeonggi-do. The study was conducted using a structured online questionnaire. Results: The average age of the visiting nurses was 49.5 years. A occupational period was 38.4% of the participants had worked for a period of 6-10 years and 34.5% of them had worked for a period of 11 years or more. In terms of employment type, 74.8% had permanent contracts and 13.9% had fixed terms. 93.4% of nurses were working on COVID-19-related work; and 57.6% of them were working more than 50% of their work. Each visiting nurse provided health services for a total of 436.4 cases on average and the per day count was 14.3 cases. The service was provided through home visits or phone consultations. The results of the job stress were relationship conflict (60.92±20.72), job demand (59.05±14.10), job autonomy (57.83±14.63), job instability (45.25±27.37), organizational system (44.97±17.21), workplace culture (42.71±18.00), and inadequate compensation (35.29±18.14). There was a significant difference in job stress according to the proportion of COVID-19 work was 'workplace culture' (p=.023), Job autonomy (p=.053) and Inappropriate compensation(p=.054). Conclusion: In order to provide a stable working environment for visiting nurses and effective public visiting health services during unexpected circumstances, such as the COVID-19 pandemic, it is necessary to promote progressive policies and systemic improvements.

Demand-supply of Advanced Practice Nurse (APN) and Alternative Benefit Strategies in the National Health Insurance (전문간호사의 수급 현황과 건강보험 급여화 방안)

  • Purpose: The purpose of this paper is to analyze the demand and supply of advanced practice nurses and suggest alternative benefit strategies in the Korean national health insurance. Methods: A revised demand & supply model was used to estimate the excess supply of APNs, and policy making process and key actors in the Korean health insurance were considered to develop a political approach to the APN issue. Results: The social demand for APNs is currently estimated to be less than 50% of its supply and the APN education program fell into difficulties in recruits. No reimbursement mechanism for APN's services in the national health insurance has given no economic incentive to hospital managers who have monopsony power in nursing labor market, which has caused the demand shortage of APNs in hospital industry. Payment for APN's services recognized as one of the most significant strategies to booster the social demand for APN's services should be carefully designed and implemented in the national health insurance. In line with this, key actors in health insurance policy decision-making include government, national assembly, labor unions, NGOs, civic groups, medical associations, and academia. Conclusion: The basic researches for APN's activities and cost-effectiveness analysis in clinical settings are required to support the strategies aforementioned. Constructing a policy network among key actors is able to make the payment strategy feasible, which will increase the socal demand for APNs.

Survey of ADL of Industrial Accident Disabled and the Caregiver Satisfaction Degree (간병요양중인 산재환자의 일상생활수행능력 및 간병인에 대한 만족도)

  • Purpose: The purpose of this study is to identify Activities of Daily Living(ADL) of industrial accident disabled with a caregiver and to evaluate the degree of satisfaction with services according to kinds of caregiver, professional or non-professional. Method: Data was collected from 178 patients in three workers' accident medical corporations of H, A and D cities from July to August, 2006. Five inspectors interviewed with the disabled by visiting the hospital. Collected data was analyzed for the frequency, percentages, t-test, etc. Results: The average score of ADL which ranged from 1 to 7 was 3.6. The 51.5 percent of caregivers for industrial accident disabled were non-professional and the 48.5 percent of caregivers were professional. 50.9 percent of the reason for the family caregivers was because of economical one. The caregiver satisfaction degree was 3.7 out of 4. The satisfaction degree with the professional caregivers was significantly higher than that with non-professional family caregivers for their excellent knowledge and techniques. Conclusion: A nursing expenses for the industrial accident disabled was intended to provide appropriate nursing services for the patient and so, it should not be a kind of income. So, the system for caring services should be investigated and the qualification of caregivers should be classified according to the health condition of the patient or ADL.

  • e-Health/u-Health has generally been considered as an expansion of current medical and medical relevant segments. However. as e-Health/u-Health has been known to have typical attributes and characteristics of services supporting a physically and mentally well-balanced life of its users, we can rationally assume that e-Health/u-Health can be not only an expansion of the existing medical field but also a result of the complex and sophisticated convergence among diverse industries such as the ICT industry. traditional care-relevant segments, etc. Thus, in this study, we carefully and cautiously consider e-Health/u-Health in accordance with both possible scenarios: 1) an expansion of a typical industry, and 2) a result of a convergence among various industries. The advent of new technologies, rapid development of current technologies, and convergence trends in various fields are creating dramatic innovations in the next generation health services market. Consumerism as a characteristic of c-Health/u-Health can be expected to find a solution of the existing healthcare service problems. In the initial phase. mainly due to the absence of a vanguard, as well as to various legalistic and regulative limitations, the role of the government would be immensely critical for the successful early settlement of the e-Health/u-Health industry. Both the government and private sector need to practice continuous and effective public education and publicity mainly to increase the overall recognition and usability of e-Health/u-Health services. Nursing as a unique professional discipline should be well aware of the new paradigm shift of the healthcare market, and make maximum use of the possibility of this trend to the advent of the professional nursing's new role.

Optimal Nursing Workforce and Financial Cost to Provide Comprehensive Nursing Service in the National Health Insurance System (국민건강보험 간호·간병통합서비스의 전면 도입을 위한 간호인력 및 재정비용 추계)

  • This study estimated the optimal nursing workforce and financial costs of providing comprehensive nursing services at hospitals under the national health insurance system. Data on registered nurses, nursing aids, medical institutions, and number of patients were obtained from the Health Insurance Review and Assessment Service. The optimal size of the nursing workforce was calculated using the workload model. A bottom-up approach was used to estimate the annual total financial cost of comprehensive nursing services. The number of registered nurses and nursing aids would need to be increased by 81.75% and 83.23%, respectively, in order to fully apply comprehensive nursing care on a national scale. The additional financial costs for comprehensive nursing services at all hospitals was estimated to be as much as 110.39% of the current cost. For the comprehensive nursing service, nurses with a career and newcomers need to be retained at their hospitals, and the validity of the nurse-patient ratio should be continuously checked. The financial shock to the national health insurance system could be minimized by gradually extending the system to all hospitals.

A study of the Consumer's Satisfaction and the Marketing Strategies of a General Hospital (일 종합병원의 소비자 만족과 마케팅 전략에 관한 연구)

  • As the medical market has gradually changed from supplier-centered market to consumer-centered one, it makes hospital managers seek the consumer's satisfaction and the various marketing strategies for survival. Under the these changes, this study was accomplished to measure the consumer's satisfaction about the medial services which a general hospital provided, to identify the marketing strategies that a general hospital could establish for survival. For these purposes, a questionnaire was developed and distributed to 200 patients in general units and 100 patients in oriental units of a general hospital in Seoul. Among them, 163 reponses in general units and 71 reponses in oriental units were turned out to be useful. they were used for final analysis. The results of the survey were as follows : First, among all items of medical services which the hospital provided, patients were most satisfied by nursing services. Second, to Improve the hospital's medical service quality, it was very important for hospital staffs to be courtesy and to explain about a process of treatment or care sufficiently. Third, to decrease cost of medical service, it was very important to develop the various items and premiums of medical insurance especially in field of oriental medicine. Fourth, to make people come to the hospital easy, it was very important of fully utilize the public transportation such as subway. Fifth, to make hospital's image good, it was very important to develop well-prepared public relationship and to continuously provide the diverse medical service in the community.
