A Review of Health Promotion Program for the Elderly (국외의 노인 건강증진 프로그램 분석 연구)

  • The purpose of this study was to suggest directions for developing a Health Promotion Program for the elderly in Korea for the future. For this, twenty previously developed & implemented health promotion programs were reviewed and analyzed in terms of target population of the program, components of the program, measurement variables for effects of program, the effects of the program. The results were as follows. 1. Most of the target populations were older adults living independently in the community. 2. Components of the program were health education, health assessment and counseling and exercise program. - Health education was done in most of programs. The topics of health education that were often included in the programs were life style changes, medical knowledge, independent living, the concept of health promotion and changes related to aging. - In health assessment and counseling, health professionals discovered their health problems through health assessment or health risk appraisal. Then they developed health recommendations on each health problem and encouraged the elderly to implement the recommendations. 3. Variables measuring the direct effects of the program were health behavior, knowledge, attitude, skill, use of medical/health reference book. Variables measuring the indirect effects, biometric outcome, health status, functional status, medical service utilization, medical cost and wellbeing. 4. The analysis showed that health education was effective for changing health behaviors, improving knowledge, skill and attitude in the elderly. Those results were suggested to be used as guidelines for developing a health promotion program for the elderly in Korea for the future.

Analyses of Studies on Cardiac Rehabilitation for Patients with Cardiovascular Disease in Korea (심장 재활 프로그램에 대한 국내 연구 논문 분석)

  • Purpose: This study was to analyze research reports published in Korea on cardiac rehabilitation for patients with cardiovascular disease. Methods: Based on inclusion criteria, 19 research reports were included in this review. Published year of selected articles was between 1996 and 2008. Nineteen studies were analyzed by guidelines of the cardiac rehabilitation programs done by the American Heart Association(AHA) and Scottish Intercollegiate Guidelines Network(SIGN). Results: The characteristics of patients were mostly of ischemic heart disease, 50-59 yr old, and male. Educational sessions were administered twice, and each lesson lasted less than 30 min. Exercise was done 3 times per week for 6 weeks. Most educational content were about risk factors, but there was no information, such as coping with feelings. Most exercise was performed as ROM, treadmill, and cycle ergometer. A large percentage of outcome indicators were health behavior, hemodynamic changes, and exercise capacity. There was more 'no effect' than 'positive effect' in trait anxiety and depression, whereas similar in physiologic domain. Conclusion: Various types of cardiac rehabilitation in Korea were performed by researchers. Therefore, we need to develop the standard protocol, to add psychosocial intervention, and to study cost effectiveness of cardiac rehabilitation.

The Effects of Case Management for Medicaid on Healthcare Utilization by the Medicaid System (의료급여 사례관리가 본인부담제 및 선택병의원제 적용자의 의료이용에 미치는 영향)

  • Purpose: This study examined the effects of case management (CM) for Medicaid on healthcare utilization considering the Medicaid system. Methods: Data were extracted from survey data on "Healthcare utilization and health status of Medicaid beneficiaries" conducted in 2007 and 2008 by the Ministry for Health, Welfare and Family Affairs. This study was designed to compare the effects on healthcare utilization between the CM group and the non-CM group. The subjects were 535 Type I Medicaid beneficiaries who utilized healthcare more than 365 days during 2006. Results: The outpatient days and medication days of the CM group decreased significantly more than those of the non-CM group with the copayment system. There were no significant differences of healthcare utilization between the CM group and the non-CM group with the designated doctor system. Conclusion: CM worked effectively on Medicaid beneficiaries' outpatient healthcare utilization with the copayment system. However, its effects on hospitalization, which is a major cause increasing the total expense, were not observed. Therefore, future studies are needed to develop strategies to reduce hospitalization and Medicaid beneficiaries outpatient healthcare utilization with the designated doctor system.

Relations among Knowledge, Fear and Efficacy of Fall in the Community Dwelling Elderly (재가 노인의 낙상에 대한 지식, 두려움 및 효능감의 관계)

  • Purpose: This study was to find out knowledge, fear, and efficacy of fall in the community dwelling elderly and to investigate how the factors were connected to one another. Methods: Data were collected from June to August, 2009 from 101 aged people who aged 65 or older and agreed to participate in this study sampled among elders at 8 halls of the aged in local communities. The subjects were interviewed by a co-researcher and 3 trained assistants in a way of responding to the questionnaire after being read. The data were analyzed through t-test, ANOVA, Turkey's test and Pearson correlation coefficients. Results: Knowledge of fall was affected significantly by whether the living cost was provided or not, fear of fall was affected by sex, level of education, hearing impairment and regular exercise, and efficacy of fall was affected by age, sex, marital status, level of education, hearing impairment, vertigo and regular exercise. The results also showed a negative correlation between fear of fall and efficacy of fall. Conclusion: According to this study, regular exercise can build up knowledge and efficacy of fall and diminish fear of fall. Thus it is highly recommended to include regular exercise in an effective fall-prevention program.

Factors Influencing HPV Vaccination and Vaccination Intention among Korean Adult Women: A Systematic Review (한국 성인여성의 인유두종 바이러스(HPV)백신 접종 및 접종의도에 영향을 미치는 요인: 체계적 문헌고찰)

  • Purpose: This study was conducted to identify the factors influencing of HPV vaccination and vaccination intention among Korean adult women through a systematic review. Methods: A systematic review was conducted across international and domestic electronic databases and nine of 193 studies were identified for this review. Results: Meaningful variables influencing HPV vaccination and its intention included socioeconomic status (age, economic level, and education level), knowledge level, health beliefs, attitudes toward HPV vaccination, and previous experiences. Economic level and knowledge influenced HPV vaccination and vaccination intention, which was consistent with the results of previous studies. Eight studies measured knowledge level regarding cervical cancer or HPV vaccination; however some instruments lacked reliability and validity report. Conclusion: There is a need for governmental support to reduce the burden of HPV vaccination cost, to publicize the benefits and to provide general information regarding HPV vaccination. Finally, it is necessary to develop an instrument with confirmed reliability and validity to measure HPV knowledge.

Relationships between Family Support, Perceived Health Status, and Self-esteem in Korean Women with Breast Cancer (한국 유방암 환자의 가족지지, 지각한 건강상태, 자아존중감의 관계)

  • Purpose: The study was to identify relationships between the family support, perceived health status and self esteem in Korean women with breast cancer. Methods: Data were collected by questionnaires from 214 women with breast cancer in inpatient and outpatient settings at three different university hospitals and one cancer hospital in B city, Korea. The instruments included Family Support Scale, Perceived Health Status Scale, and Self Esteem Scale. The collected data were analyzed using frequency, percentage, t-test, ANOVA, Scheffe's test, and Pearson's correlation coefficients by SPSS WIN 15.0 program. Results: There were significant differences in the family support by age, education, and cost burden. There were significant differences in the perceived health status by education, occupation, economic status, pain, fatigue, and cancer insurance. There were significant differences in the self esteem by age, occupation, economic status, type of religion, pain, fatigue, and cancer insurance. Self-esteem was significantly correlated with family support and perceived health status. Conclusion: These results suggested that promoting perceived health status and enhancing family support would increase self-esteem effectively among Korean women with breast cancer.

Influencing Factors on Hope Among Patients with Breast Cancer (유방암 환자의 희망 영향요인)

  • Purpose: This study was aimed to identify the influencing factors on hope in the patients suffering with breast cancers. Methods: The subjects were 150 women who were diagnosed with breast cancer at three university hospitals and a general hospital. Data collection was conducted by hope scale, family support scale, self esteem scale, depression scale, pain scale, and fatigue scale. The collected data were analyzed using frequency, percentage, t-test, ANOVA, Pearson's correlation coefficients, and stepwise multiple regression. Results: Level of hope in the participants was high. There were a significant correlation among hope, family support, effects of religion, self esteem, depression, pain and fatigue. There were significant differences in hope by the age, education level, and cost burden. The most powerful predictor of hope was self esteem (38.2%). Altogether family support, effects of religion, depression, and fatigue explained 50.4% of hope of the participants. Conclusion: The findings of the study suggested that the concepts of self esteem, family support, effects of religion, depression and fatigue should be considered important factors in developing hope promoting program for breast cancer patients.

  • The purpose of this study is to identify the domestic research results related to the outcomes of home care services for 1981-1996. 12 studies were analized by the characteristics of the subjects, home care services provided, and outcome variables and tools during the period of Apirl-June, 1997. The results were as follows: the subjects of 8 studies conducted before 1993 were discharged patients comparing to early discharged patients of 2 studies conducted after 1994. The main home care service delivered to the clients and their families were education before 1993. The outcome variables were used the level of sat. isfaction of home care services, quality of care, cost-effectveness, health status, quality of life, and self -care ability. A variable, health status, were mainly measured by the global outcome measures such as quality of life, health hehelief, health perception, activity of daily living, health management pattern but also foused outcome measures that could be specified by the medical diagnoses.

A Model for Community Based Mother Infant Care Center - TMIC(transitional mother infant care center) using a Sanhujoriwon - (산후조리원의 모자건강관리 현황과 제도화방안 - 지역사회중심의 모자건강관리센터(TMIC) 개발을 위한 전략 -)

  • The purpose of this study was 1) analyze the current state of Sanhojoriwon; and 2) to suggest the new model for the community based mother infants health care delivery system: strategies of TMIC are related to Public Health policy, cost-effectiveness, mother infant care provision of medical professionalism, and so on. Method: Forty-seven workers from seventeen Sanhojoriwon participated to analyze several aspects of Sanhojoriwon. Using a questionnaire developed at Korean Sanhojori Research Forum (KSARF), such as the traditional and medical concept of the Sanhojori, postpartum care, Korean traditional postpartum care, job description on women and infant care at Sanhojoriwon, professional management, health care policy and the educational need. Results: Based on the descriptive study results, the TMIC, the community based transitional mother infants care center was suggested as a new model for the cyclic public health care system related on the reproductive health, using an already existing related center, Sanhojoriwon. Also, several strategies were presented on the TMIC.

Health Care Utilization and Costs for the Disabled Not Included in the Medical Aid Allowance (의료급여 장애인의 비 급여 의료비용 조사)

  • This study was conducted to identify the health care utilization, health care costs, and potential health care demands of the disabled in the Medicaid Aid beneficiaries. This study focused on the heath care costs not included in the medical aid allowance such as transportation, informal nursing costs, and ambulatory aids etc. Participants were the 864 subjects who were beneficiaries of the National Medical Aid program living in 10 district of Korea. A questionnaires were distributed to the disabled in the Medical Aid beneficiaries during August to September, 2001 through public offices. Data were collected through a home visiting by social workers working in public offices. Direct and indirect medical costs expended for one month by the participating disabled were examined. They expended 110.748 won $({\$}100)$ for heath care costs, which was not included in the medical aid allowance during the month. The disabled with cerebral diseases or who have level 4 disability expended more health care costs compare to those with other diseases. Gradual expansion of medical aid allowance for the disabled is recommended to alleviate economic burden of the disabled and their family.
