Purpose: This study was done to suggest the standard ICU nursing procedure and standard ICU nursing practice time carried out according to the standard ICU nursing procedure. Methods: Through the literature review, 35 standard ICU nursing activities were selected and classified under 6 categories. Standard nursing procedures were described by research team and 4 clinical practice experts. Content validity of descripted nursing procedure was performed by 6 clinical practice experts. After that standard ICU nursing procedures were identified, standard nursing practice times were checked through 1:1 observation. Results: 35 ICU nursing activities were identified and categorized into 6 domains of activities. Also, their standard nursing procedures were identified, and finally 33 standard nursing practice times were measured and analyzed. The result of the measurement, 1 ICU nursing activity was spent above 30 minutes, 3 activities done within 10-20 minutes, 4 activities done within 5-10 minutes, and 25 activities were spent below 5 minutes. Conclusion: Through this study, 35 standard ICU nursing procedures were described and 33 standard ICU nursing practice times which carried out according to standard ICU nursing procedures were checked. The result can be as fundamental data for the study of nursing activity time in the ICU.
Purpose: This study aimed to investigate possible ways to expand the services of home-visit nursing through a review of the progress, achievements, and obstacles of home-visit nursing; a pilot project of an integrated home-service; the application of the Omaha System; as well as a case analysis of providing home-visit nursing services. Method: An integrated review was conducted using various source materials, including laws, previous studies, and a case analysis. Results: In case analysis of providing visiting nursing service, rehabilitation nursing, end-of-life nursing, and dementia care showed high nursing needs. It was necessary that the various home visit nursing services in the intervention area of the Omaha System, administrative services, case management, and center operation activities were all included in the payment systems of long-term care insurance. Conclusion: In the future, home visit nursing services of long-term care insurance should be reborn in the form of a center for integrated case management in the community, which would set long-term goals until the time of a client's death and encompass the realm of human rights for health, quality of daily life, and a dignity of life.
The purpose of this study was to evaluate the self care focused home nursing' care outcomes of parturient women following Cesarean sections, The subjects included 56 parturient women following a cesarean section, and they were randomly assigned to an experimental group or a control group, The experimental group included women who were discharged early from the hospital. 4-5 days after having a cesarean section. Home nursing care for the experimental group consisted of stitch removal. wound care and education for parturient women. Home nursing care was provided for 2-3 days after early discharge. The outcomes of self care focused home nursing care were measured by self-care competency. Data collection was done by a self-reported questionnaire and by a telephone interview 2-3 days after discharge from the hospital(control) or after home care (experimental) from December, 20, 1998 to June, 10, 1999. The questionnaire consisted of 25 items on 9 self-care domains. Data was analyzed by a t-test. and as $x^2$-test. The study results were as follows: 1. The general characteristics of both groups were similar except for the total number of pregnancies, and whether or not they wanted the pregnancy at this time. 2. The self-care competency scores for the experimental group receiving self care focused home nursing care were higher than the scores for the control group(t=2.361. $p{\le}.05$). 3. There was no significant difference in the rate of OPD visiting, readmission, or emergency room use between the two groups. We concluded that self care focused home nursing care is effective in promoting the self-care competency of parturient women following Cesarean sections. It is suggested that further study is needed with a larger sample to be able to generalize these results.
Purpose: This study was conducted to identify factors related to developmental care performance among neonatal intensive care units (NICU) nurses. Methods: The participants were 139 nurses who had provided care to premature infants for more than 6 months and were recruited from the NICU of 8 hospitals. Data were collected from September 1 to December 1, 2017 through questionnaires that encompassed developmental care performance, developmental care perceptions, and the nursing work environment. Results: More than half (51.8%) of the participants responded that they had never received developmental care education, and for 89.6% of those who had received developmental care education, it was a one-time event. The average developmental care performance of NICU nurses was 0.81, with a range of 0.5~1 point. Multiple regression analysis, demonstrated that the nursing work environment (β=.27, p=.001) and developmental care perceptions (β=.23, p=.004) influenced developmental care performance, with a total explanatory power of 14%. Conclusion: Based on these results, developmental care education for the NICU nurses must be provided systematically. In addition, strategies to improve nurses' perceptions of developmental care and to provide appropriate support for the nursing work environment can promote developmental care performance.
The purpose of this research was fourfold: (a) to identify the use rate of the indirect care interventions performed by nurses, (b) to estimate the time to perform each intervention, (c) to identify the indirect care interventions to be delegated to others, and (d) to determine the level of provider preparation needed to delegate indirect care interventions. The sample consisted of 199 nurses working in three hospitals. The Indirect Care Survey developed by the Iowa Intervention Project team was used for data collection. The instrument was translated to Korean and validated by nurse experts. Each of the 26 indirect care interventions were used several times a day. Four interventions (i.e.. Documentation, Shift Report, Specimen Management, and Transport) were performed several times a day by 50% or more of the nurses. The most frequently used intervention was Documentation, followed by the interventions Shift report. Environmental Management, Transport, and Examination Assistance. The least used intervention was Quality Monitoring, followed by the interventions Order Transcription, Referral, Health Care Information Exchange, Multidisciplinary Care Conference, and Product Evaluation. The intervention taking the most time to per-form was Technology Management (155.3 minutes), followed by the interventions Documentation, (122.2 minutes), Delegation (84.4 minutes), Supply management (83.4 minutes), and Preceptor: Student (79.9 minutes), Overall, the nurses reported that they would not delegate to others the majority of the interventions. More than 50% of the nurses would not delegate 21 interventions. Shift Report would not be delegated by 95% of the nurses and Documentation would not be delegated by 92% of the nurses. Caregiver Support would be delegated by 68% of the nurses to family. Three interventions (i.e.. Environmental Management, Examination Assistance, and Transport) would be delegated by more than 50% of the nurses to Nursing Assistant. This study will contributes to determining costs of nursing services and enhancing quality of nursing care. Replication study will be needed with large sample.
The purpose of this study was to 1) identify the current management status of the Visiting Health Care Services (VHCS) and 2) to analyze the workload of the staff in the VHCS located in the Public Health Centers (PHCs) in the urban and rural areas. Method: A descriptive research design and a prospective, time and motion research design were used. A total of 18 PHCs in Gangwon Province participated in this study. A questionnaire and semi-structured observational sheet were utilized. A total of 650 self report records of the work load from the VHCS personnel were collected for a 10 day period at each of the 18 PHCs. A descriptive analysis was then done. Results: The major results were as follows. 1) The VHCS staff (nurses and nurse aids) was being assigned additional work such as maternal health care, chronic disease care, mental health care and health promotion on top of their VHCS duties. 2) The average number of home visits per client during the past year was 5.8. More specifically, the clients in the severe dependent group received an average of 27.1 visits, those clients in the moderate dependent group received 14.0 visits those clients in the slightly dependent group received 5.0 visits and those clients in the self-care group received 1.6 visits. 3) The time required for the work duties of the VHCS staff totaled 488 minutes per day. The percentage of time for home visits was only 17.4%, and this didn't include travel time. Conclusion: The main problems of VHCS were identified as a lack of personnel and a lack of time for the home visits. Strategies that are directed at the construction of a better infrastructure for VHCS are needed.
Purpose: This study aimed to examine pediatric hospital nurses' perceptions and performance of family-centered care. Methods: A descriptive study design was used. This study surveyed 162 nurses who worked at a single tertiary children's hospital in South Korea. The modified Family-Centered Care Scale was used to assess nurses' perceptions and performance of family-centered care. Barriers to the implementation of family-centered care were described in an open-ended format. Results: Pediatric hospital nurses had a higher score for perceptions (mean score=4.07) than for performance (mean score=3.77). The collaboration subscale had the lowest scores for both perceptions and performance. The perceptions of family-centered care differed significantly according to the nurses' clinical career in the pediatric unit and familiarity with family-centered care, while performance differed according to clinical career only. Perceptions and performance were positively correlated (r=.594, p<.001). Barriers to implementation included a shortage of nursing personnel, a lack of time, and the absence of a family-centered care system. Conclusion: To improve the performance of family-centered care, nurses' perceptions of family-centered care should be improved by offering education programs and active support, including sufficient staffing, and establishing systems within hospitals.
Purpose: This study was done to define nursing behavior in neonatal intensive care units so as to estimate resource-based relative value-. Method: Participating in this study were 292 nurses in neonatal intensive care units. The study surveyed physical and mental labor, stress and time involved in nursing work. Tool used in this study was a nursing labor per relative value tool. For analyzes, the relative value of each nursing behavior was calculated, where the mean value of the three components, labor intensity and component-by-component explanatory power were in percentage terms. Results: 1. Nursing behaviors in neonatal intensive care unit were classified and defined at three levels: 5 main domains, 17 mid-domains, and 42 small domains. 2. The per component explanatory power of intensity involved in nursing labor showed physical effort to be 32.45%, mental 32.86%, and stress 34.69%. 3. The reliability of nursing labor factors was very strong, Cronbach's alpha value of 0.96. Conclusion: In this research, which is a first in defining nursing behavior in neonatal intensive care units, individual nursing behavior were broken down using resource-based relative value for nursing cost, and each nursing behavior was successfully translated to a numerical value.
Purpose: This study identified the current status and perception of intensive care unit nurses' handover. Methods: A cross sectional descriptive survey was employed. The population included nurse managers and staff nurses who worked in intensive care units in hospitals with more than 500 beds and excluded nursing homes, psychiatric hospitals, and military hospitals. Results: Of the nurses, 61.7% were satisfied with the current handover method, 68.36% had no handover-related guidelines, and 83.2% of them perceived that the handover was important for patients' safety. The most frequent cause for errors related to handover was that the "nursing workload is heavy." The nurses perceived that their handover was informative ($5.62{\pm}0.79$) and efficient ($5.04{\pm}0.98$). The variables associated with their perception of the handover were the level of satisfaction with the current handover method, existence of handover guidelines, and importance of handover for patient safety. Conclusion: The development of standardized handover guidelines, especially for intensive care units, is necessary to reduce handover time and errors and to improve handover quality for patients'safety and high standards of nursing care.
These days, as the society has been in the trends of highly industrialized and the family has been downsized, there is remarkably increasing number of women who follow occupation. These changes have made it more difficult for the family to help post-partum mother, which had been performed in side of house. By the help of social believe that during at least 1 month after childbirth professional nursing program is indispensable for both maternity protection and physical-mental recuperation, now many post-partum care centers for post-partum mother have been in operation. Although these post-partum care center have in use for a long time, no study was performed before this study on the same subject. Data were analyzed using by SAS. The results of study are as follow : 1. The general features of the user of the post-partum care center. The predominant band of user's age is extended from 26 to 30. The users are mainly housewives and they are in higher level of incomes and educations. As for the feature of delivery methods, they performed the normal spontaneous vaginal delivery method by 58.0% and the Caesarean operation method by 42.0%. As for the sexuality for babies, 59.3% of infants are male, and 40.7% are female. The highest delivery order of users is first and admission after 1-3days delivery is highest. 2. The results for the investigation into the actual condition of the post-partum care center are as follow : About the main reason for entrance of the post-partum care center was found to be the needs for the better nursing programs for recuperation after childbirth. This demands are also supported by their husbands. The average length of stay in the post-partum care center is 17.6 days and the besides promised expense ; powdered milk, milk-suckers, disposable dippers, skin cares, body shape cares, entrophics, injections. The post-partum have private rooms for mother and infants. Over the half of average expense is 229 million won. They are paying accessory fees the post-partum care center have similar step organization : the nurses, the skin carers, the cleaners. Mostly both ways of feeding powdered milk and breast feeding are in use, and mother's milk is preserved in the night time to be given to infant by nurse in charge. 3. The results of the investigation on users' nursing demand to the post-partum care center and satisfaction are as follow : The ranking order of nursing demand of mothers who used the is that ; infant care demands, environmental demands, emotional and mental care demands, education and training demands. As much as 70% of the respondents have dissatisfaction in nursing program, The ranking order of service satisfaction of mothers who used appears to be higher following order ; satisfaction in infant care demand, satisfaction in physical nursing demand, satisfaction in emotional and mental nursing demand, satisfaction in education and training demand, satisfaction in environment nursing demand. The results of pearson correlation. between nursing demand and service satisfaction of mothers who used are found to be relative noticeable in the level of 0.05. only infant care. The 61.7% of the women who used the post-partum care center. are responding that they will reuse the same post-partum care center again.
