Purpose: This paper reports a study exploring factors related to patient satisfaction and dissatisfaction with inpatient care. Method: A cross-sectional study design was used, employing data from the National Health and Nutrition Survey conducted in 2001. Socio-demographic factors, utilization, self-rated health status, and disease characteristics were assessed by employing univariate comparisons and multivariate logistic regression analyses. Result: Out of 37,769 respondents, 1,043 aged 20 years and over had been admitted to a hospital or clinic at least once during the past year. About a quarter of the respondents were discharged from tertiary hospitals and $21\%$ from clinics. The majority of patients ($58\%$) were satisfied with inpatient care received, whereas $11\%$ were dissatisfied. Greater satisfaction was found in patients aged 45-64 years and those having formal education, discharge from tertiary hospitals, national health insurance as a payer, medical expenses not being burdensome, good self-rated health status, and neoplasm. Living in non-metropolitan urban areas, shorter length of stay, and musculoskeletal diseases were associated with greater dissatisfaction. Conclusion: Different factors were related to patient satisfaction and dissatisfaction with care. Those factors need to be taken into account when evaluating and comparing satisfaction levels between health care institutions.
Purpose: This study aimed to implement safe and suitable kangaroo mother care with healthy mothers and newborns immediately after childbirth. In addition, it aimed to assess its effects on maternal attachment, maternal role confidence, and maternal role satisfaction. Research design, data and methodology: A quasi-experimental design was used with pre and posttests. A total of 58 participants were selected by convenience sampling through public notifications at a women's hospital. The control group performed routine care without providing kangaroo contact, and a posttest was performed four weeks after the pretest. The experimental group comprised healthy mothers who had given birth by cesarean section. They performed kangaroo mother care twice a day (60 min each) for four weeks for their newborns after the protocol was validated by an expert. Results: The results showed significantly higher maternal attachment, maternal role confidence, and maternal role satisfaction in mothers who participated in kangaroo mother care than those who did not. Our findings suggested that kangaroo mother care has a positive effect on the enhancement of maternal attachment and facilitation of maternal role confidence and satisfaction. Conclusions: Thus, the expansion and implementation of kangaroo mother care are necessary to promote healthier mother-child relationships.
Purpose: The purpose of this study was to develop a lesson plan related to a simulation for nursing students by understanding what relationship learning satisfaction, self-efficacy and learning attitude have after applying simulation-based education on emergency nursing care. Methods: Simulation practice, seminar, class and group self-study about a patient having a myocardial infarction were applied to nursing students for 5 weeks. After applying the simulation-based education on emergency nursing care, students rated their learning attitude, learning satisfaction and a sense of self-efficacy with a self-administered questionnaire and academic achievement was divided into written and performance evaluation. SPSS/WIN 17.0 was used for descriptive analysis, independent t-test, one-way ANOVA and pearson correlation were also used. Results: In this study, the performance evaluation score was related to learning attitude (r=.210, p=.018).Learning satisfaction (r=.220, p=.013)and written evaluation score was related to learning attitude (r=.211, p=.017) and learning satisfaction (r=.190, p=.032). Conclusion: In this study, simulation-based learning was a useful method for practical ability and this is good to acquire both of knowledge and technique. Not only evaluation of theoretical knowledge but performance ability related to practice has to be done by developing lessons with various methods and content. In addition, a standard evaluation method needs to be developed.
Client(patient) satisfaction has been used for a variety of purposes in different settings, yet, those receiving home care services have been the focus of few satisfaction studies. Satisfaction is considered an indicator of quality of care and is also being used to assess the performance of home care providers and home health care delivery at multiple levels. The aim of this study was to develop the tool for evaluating client satisfaction in several aspects with home care service while maintaining or achieving acceptable reliability and validity. Firstly, Client Satisfaction Instrument made preliminary of 43 items composed basic 6 criteria and 7 indicators, based on a review of literature, quality improvement and client satisfaction findings in home care. Secondly, there were consisted in two expert panel groups who were made in professionals of research and practice in home care field. The tool was modified to 6 criteria and 7 indicators, 25 items after reviewing by two expert panel groups. The content validity index of the tool was above 0.8 and the results of internal consistency was more than 0.8 in all items. Finally. The Client Satisfaction Instrument in Home Care Service was made up 6 criteria such as client satisfaction of nurses's instruction of home care educational need. knowledge/technique/attitude of homecare nurses's performance. interpersonal relationship and emotional support of nurse. easiness of homecare accessibility, continuity of homecare service. overall efficacy of homecare use, 7 indicators. and 25 items. The results of evaluating client satisfaction will be able to contribute in quality improvement and service marketing in home care.
Purpose: This study was performed to evaluate the outcomes of the home care nursing program conducted jointly by thirty two catholic churches and C hospital in Seoul. Method: The subjects included 173 patients who registered for the program during a 4 month-period from November 1, 2004 to February 28, 2005 and received home care services for more than 4 times and 32 professional nurses participating in the program. Using the concept of medical outcome study (MOS), the structure, process, outcome elements were analyzed. Result: 1) A Catholic homecare nursing center and nurses of the C hospital played a central role in providing nursing care, and each church operated its own vehicle from its own office. Home care nurse's job satisfaction was 2.8 out of total score of 4. The major illness was cerebrovascular disease including stroke followed by skeletomuscular disease including degenerative arthritis cancer, and diabetes. Among reasons for accessing the home care nursing program, hypertension management was most prevalent. More than half of the registration was done through catholic churches. Most people who referred the patient to the program was through the church. Most patients received home care nursing 1-2 times a week for 30 to 60 minutes in average and the most frequent type of service provided was basic nursing. 3) The most frequent reason for terminating home care services was death. The change in PPS(Palliative Performance Scale) level from the time of registration and after 4 visits was the same in 45%, decreased in 30%, and improved in 25%. Patient satisfaction was very high, showing 3.4 out of total score of 4. Conclusion: These results proved that the home care nursing program was highly appreciated by subjects and nurses were providing professional care. Thus the two parties involved in the program were actively supporting the program to fulfill their mission. However, several areas needed to be improved such as relating with local community, relating with family doctor, and issue of improving the working conditions for home care nurses.
Purpose: The purpose of this quasi-experimental research is to develop the CP for primary THR patients and verify its effectiveness. Method: The CP is designed for both patients and medical employees to meet the 14-day-long hospitalization with an expert and clinical validity test, and the standardized orders are also created. 21 eligible subjects for the control group(with common care plan) and 27 subjects for the experimental group(with CP service) were assigned. Data were collected from questionnaires to evaluate patients' satisfaction level of nursing care and CP, investigation of physical complications, inspection of LOS(length of hospital stay) and hospital charges from April 2000 to February 2001 at K-hospital in Daegu. The collected data were analyzed by using the SAS program. Results: After implementation of CP, there was a statistically significant reduction in mean length of stay. The hospital charges have no considerable difference between two groups. Frequency of physical complications is reduced considerably. The experimental group scored much higher than the control group on the satisfaction level toward nursing care. And in the analysis of CP satisfaction questionnaires, many subjects have high satisfaction in items of 'I see the treatment process and hospital life', 'I feel that I am participating in my treatment process with health care staff'. The analysis of variances which cause a prolonged LOS supports that it's necessary to strengthen an evaluation of pre-operative outpatients' condition and link the home nursing care system in discharge for more shortening of LOS. Conclusion: The above results show that CP can have a positive impact on satisfaction level of inpatients with primary THR and the hospital.
본 연구는 전국적으로 확대 시행하고 있는 간호 간병통합서비스병동 간호사와 일반병동 간호사의 직무만족, 직무스트레스, 직무소진 및 이직의도를 조사하고 비교하여 간호 간병통합서비스 병동의 효율적인 간호인력 관리를 위한 기초자료를 제공하기 위한 비교조사 연구이다. 대상자는 G광역시 소재 간호 간병통합서비스를 시행하고 있는 6개 병원에서 간호 간병통합서비스병동 간호사와 일반병동 간호사 181명으로, 자료수집은 구조화된 설문지를 이용하여 2017년 5월 16부터 6월 30일까지 실시하였고, t-test, one-way ANOVA, Pearson 상관관계를 이용하여 분석하였다. 연구결과, 간호 간병통합서비스병동 간호사와 일반병동 간호사의 직무만족, 직무스트레스, 직무소진 및 이직의도에서 집단 간의 차이는 없는 것으로 나타났으나, 간호 간병통합서비스병동 간호사가 직무만족 점수는 더 높았고, 직무스트레스, 직무소진 및 이직의도 점수는 더 낮았다. 또한 간호 간병통합서비스병동 간호사는 직무스트레스(r=.39, p=<.001)와 직무소진(r=.43, p=<.001)이 높을수록 이직의도가 높게 나타났고, 직무만족(r=-.66, p=<.001)이 높을수록 이직의도가 낮게 나타났다. 간호 간병통합서비스병동 간호사는 일반병동 간호사에 비해 직무 관련해서 더 긍정적으로 생각하고 있었다. 따라서 본 연구결과는 간호 간병통합서비스병동의 효율적인 간호인력 관리를 위한 기초자료로 활용될 수 있을 것이다.
The purpose of this study was to identify factors influencing patient satisfaction and to evaluate the utility of patient satisfaction as an outcome indicator. The study was conducted by mailed questionnaire. The subjects were 900 patients discharged from adult nursing units in a tertiary teaching hospital. On the discharge date, questionnaires were distributed by two trained research assistants. The questionnaire developed by the researchers was based on Larson(1996)'s study, and consisted of 71 items with the following components: overall satisfaction, domain-specific satisfaction(administration process, hospital facility and environment, nurses, and doctors), patients' loyalty(intention to use the health care service of the hospital in the future), recommendation to others, health benefits, and demographic characteristics. Each item was rated using a five point Likert scale ranging from '1=strongly disagree' to '5= strongly agree'. The response rate was 43%(387/900). The satisfaction level with the health care service was generally high. Perceived health status was the only significant factor influencing satisfaction level. Satisfaction with doctors contributed the most to explaining overall satisfaction. Overall satisfaction was significantly correlated with patient loyalty, recommendation, to other and perceived health benefit. It was found that the score of satisfaction was positively correlated with the score of loyalty, recommendation, and health benefit. Therefore, patient satisfaction seems to be a good outcome indicator.
This study was carried out to identify the difference between educational needs and levels of satisfaction of primigravida during the antepartum period. The goal of the study was to obtain data needed to develope educational programs and to improve the nursing quality for antepartal clients. The subjects were 106 primigravida who attend antenatal clinic at 3 general hospitals in the Seoul area. The data was gathered using a questionnaire which, consist of 71 items, was developed by the researcher from May 4 to June 3, 1998. Results found are as follows : 1. The characteristics of subjects : The majority of subjects were aged 25-29 years(73.6%), college graduates(61.3%), jobless(68.9%), had no religion(38.7%), attended antenatal clinic regularly(91.5%), duration of pregnancy was 38-39 weeks(49.06%), pregnancy was planned(67%), and 42.5% have had some sort of prenatal education. 2. Over all level of educational needs was relatively high(Mn. 3.97) but the level of satisfaction was of average level(Mn. 3.01). Therefore, differences between the level of educational needs and satisfaction was significant(P=.0001). 3. The educational needs by category, the highest need was on birth preparation(Mn. 4.18), self care of the mother and infant care(Mn 4.10), health maintenance and promotion(Mn. 3.79), the lowest was care of discomfort(Mn 3.66). The level of satisfaction was higher on postpartum self care(Mn 3.15), but the lowest was infant care(Mn 2.84). Differences between educational needs and satisfaction by categories was significant(P=.0001). 4. Relationship between educational needs and levels of satisfaction among primigravida of different characteristics were as follows : 1) Educational needs of minor discomforts area were significantly high among age of 25-29(P=.0108), and over 35 years of age, was satisfied on preparation of labor & delivery, postpartum self care (P=.036, .02). 2) With regard to different level of education, middle school graduates had higher educational needs on discomfort care and postpartum self care(P=.0014, .014). College graduates have had higher educational needs on health maintenance(P=.008) and were more satisfied on preparation of birth(P=.0025) 3) With regard to whether pregnancy was planned or not, no difference was found on educational needs. But the levels of satisfaction was significantly higher in the group of planned pregnancy(discomfort care P=.0454, birth preparation P=.0256, postpartum self care P=.0092). 4) with regard to antenatal education, those who have had some sort of antenatal education, educational needs on birth preparation(P=.0345) was significantly high. And also the levels of satisfaction were significantly higher on every category(P=.0004-.0001). 5) No difference was found on educational needs or level of satisfaction by Job, religions, regularity of antenatal care and complication of pregnancy.
Purpose. This study developed a web-based wound care course for undergraduate nursing students and evaluated the course's content, system, and student-satisfaction. Methods. This study was done in three stages, the development of the web-based wound care course, the implementation and evaluation of the course. The course was developed based on the ARCS model. 80 under-graduate nursing students to Y University in Korea used the web-based wound care program during four weeks. After that, they completed questionnaires, evaluating the contents, system, and their satisfaction. Results. Eighteen learning objectives were used to create the web-based wound care course and the course was developed with 7 chapters and 20 subsections. The analysis of the questionnaires showed a mean score for content and system-related items of 3.04 each, out of a possible 4 points. Student satisfaction items had a mean score of 2.89. Conclusions. The web-based course allowed students access to the course anytime and anywhere, and according to their own learning abilities. However this advantage would only be possible when nurse educators develop qualitative web-based course to meet the demand of a complex health care system as well as the needs of the students and the effectively incorporate it into traditional lectures.
