한센인의 삶: 소록도병원을 중심으로 (Life Experiences of Korean Patients with Hansen's Disease in Sorok Island Hospital)

  • Purpose: The purpose of this study was to identify life experiences of Korean patients with Hansen's disease (leprosy). Methods: For this study, 5 participants from Sorok Island Hospital were purposively chosen. Data were collected through in-depth individual interviews from June to July 2014. Data analysis method was based on Colaizzi's approach. Results: The study results showed that experiences of patients with Hansen's disease consisted of 14 themes and six theme clusters: 1) Bad disease approaching as fate; 2) Family breakup and far from the village; 3) New life in Sorok Island Hospital; 4) Treatment of Hansen's disease and disability; 5) Life in the disease community; 6) Comfort and hope of life. Conclusion: The findings of this study indicate that health care professionals should pay attention to patients with Hansen's disease not only to reduce their physical and psychological suffering, but also to help the community and public culture to reduce the social stigma surrounding this disease and causing suffering for the patients. The results of the present study can help us to have a better understanding of various aspects of patients' lived experiences.

Korean Immigrant Women's Taekyo Practices in the United States as a Traditional Prenatal Self-care

  • Purpose: The purpose of this study was to explore preserved belief system supporting Korean immigrant women's Taekyo practices and influencing factors while they observe the tradition within US sociocultural context. Methods: Leininger's exploratory focused ethnographic approach was used. Semi-structured in-depth interviews were conducted with purposive sample of sixteen Korean immigrant women who gave birth in the US within last 6 months. Researcher's observation and reflective field notes were also integrated into the interview data. Leininger and McFarland's four phases of ethnographic analysis guided data analysis process. Results: The perceived belief system supporting Taekyo practices included Taekyo as an enculturated Korean tradition, connecting parents with fetus, and positive impacts on fetal development. And Korean immigrant women's Taekyo practices were influenced by resources of information, woman's orientation toward Taekyo, pressure from local Korean community, and child order. Conclusion: The findings from this research would serve as an important knowledge base to expand US health care providers' understanding of Korean traditional Taekyo practices observed by Korean immigrant women's as important prenatal self-care practices. The findings could also aid in providing more patient-centered and culturally-tailored prenatal care plan to Korean immigrant by including Korean traditional belief system supporting Taekyo practices.

간호사의 투약대화의 구조와 내용에 대한 평가도구 개발 연구: 대화분석적 접근 (Evaluation of Nurses' Competency in Nurse-Patient Communication about Medications: Conversational Analysis Approach)

  • Purpose: The purpose of this study was to develop evaluation criteria for conversations about medication and to demonstrate conversational analysis with actual dialogues on medication as examples. Methods: This study was a secondary analysis of qualitative research using conversational analysis which showed functional phases and patterns of dialogue about medication (greeting, identifying the patient, medicating, finishing). Nurse-patient conversations were videotaped and transcribed and 75 conversations were used for analysis. Results: Not all functional phases were showed in the conversations about medication. Therefore, conversations about medication can be considered as incomplete dialogues. The evaluation-criteria were represented in terms of the structure and content of the dialogues. Structural evaluation-criteria were the same as the functional phases, as functional stage is the standard for evaluation. The criteria of evaluation for content suggested 3 domains, content, expression, and interaction with 20 items scored on a Likert-type scale of 5-points. Finally, analysis of actual conversations about medication according to the evaluative criteria were provided. Conclusion: The results provide the basic data to develop educational programs and strategies to improve nurses’ competency in conversation about medication.

북한이탈 청소년의 문제행동과 문화변용정체성에 관한 연구 (Problem behaviors, Cultural Identity and Acculturation of North Korean Refugee Youth)

  • Purpose: The purpose of this study was to investigate the characteristics of problem behaviors, cultural identity and acculturation of North Korean (NK) refugee youth and to suggest an appropriate approach for helping them to adjust to our society based on these findings. Methods: The subjects were 117 NK adolescent refugees in Hanawon. Data were collected by a demographic questionnaire, Youth Self Report (YSR), the South-North Korean cultural identity. Results: Problem behaviors of NK refugee youth were the highest in anxiety/depression and withdrawal behavior and were followed by internalized behavior, aggressiveness, overall problem behavior, externalized behavior, delinquency and positive behavior in that order. Acculturation was the highest in 'separation' followed by 'integration', 'assimilation' and 'marginalization' in that order. No significant relationship was found between acculturation types and problem behaviors except positive behavior. Conclusion: This study provides some important information that we have to develop specific strategies and interventions primarily focused on withdrawal, anxiety/depression, and internalization for the adjustment and mental health of NK refugee youth. It is necessary to explore further research related to cultural identity, acculturation processes, and the correlation between acculturation types and problem behaviors.

구성타당도 평가에 있어서 요인분석의 활용 (A Guide on the Use of Factor Analysis in the Assessment of Construct Validity)

  • Purpose: The purpose of this study is to provide researchers with a simplified approach to undertaking exploratory factor analysis for the assessment of construct validity. Methods: All articles published in 2010, 2011, and 2012 in Journal of Korean Academy of Nursing were reviewed and other relevant books and articles were chosen for the review. Results: In this paper, the following were discussed: preliminary analysis process of exploratory factor analysis to examine the sample size, distribution of measured variables, correlation coefficient, and results of KMO measure and Bartlett's test of sphericity. In addition, other areas to be considered in using factor analysis are discussed, including determination of the number of factors, the choice of rotation method or extraction method of the factor structure, and the interpretation of the factor loadings and explained variance. Conclusion: Content validity is the degree to which elements of an assessment instrument are relevant to and representative of the targeted construct for a particular assessment purpose. This measurement is difficult and challenging and takes a lot of time. Factor analysis is considered one of the strongest approaches to establishing construct validity and is the most commonly used method for establishing construct validity measured by an instrument.

Linking Clinical Events in Elderly to In-home Monitoring Sensor Data: A Brief Review and a Pilot Study on Predicting Pulse Pressure

  • Technology has had a tremendous impact on our daily lives. Recently, technology and its impact on aging has become an expanding field of inquiry. A major reason for this interest is that the use of technology can help older people who experience deteriorating health to live independently. In this paper we give a brief review of the in-home monitoring technologies for the elderly. In the pilot study, we analyze the possibility of employing the data generated by a continuous, unobtrusive nursing home monitoring system for predicting elevated(abnormal)pulse pressure(PP) in elderly(PP=systolic blood pressure-diastolic blood pressure). Our sensor data capture external information(behavioral) about the resident that is subsequently reflected in the predicted PP. By continuously predicting the possibility of elevated pulse pressure we may alert the nursing staff when some predefined threshold is exceeded. This approach may provide additional blood pressure monitoring for the elderly persons susceptible to blood pressure variations during the time between two nursing visits. We conducted a retrospective pilot study on two residents of the TigerPlace aging in place facility with age over 70, that had blood pressure measured between 100 and 300 times during a period of two years. The pilot study suggested that abnormal pulse pressure can be reasonably well estimated (an area under ROC curve of about 0.75) using apartment bed and motion sensors.

고혈압 노인에서 복식호흡 이완훈련이 혈압 및 스트레스 반응에 미치는 영향 (The Effects of Abdominal Relaxed Breathing Training on Stress response and Blood Pressure for Elderly Hypertensionve Patient)

  • In order to improve self-management of elderly hypertensives in this study, the researcher examined the effect of blood pressure regulation by an abdominal relaxed breathing training which regulates physical response to stress and lessens the activity of the sympathetic nerve. Method: A quasi-experimental pre-test and post-test design was used. Thirty elderly hypertensive patient ; Sixteen in the abdominal breathing group and fourteen in the control group participated in this study. The abdominal relaxed breathing training consisted of eight sessions twice a week for four weeks. There was no intervention for the control group. Result: 1. There was a significant decrease in systolic and diastolic blood pressure between the experimental group and control group over three different times, and interaction by groups and over time. 2. There was no significant difference in the level of total stress response, physical stress response, behavioral-cognitive stress response between the experimental group and control group over three different times, and interaction by groups and over time. Conclusion: Findings indicate that this study will contribute to develop nursing strategies for the regulation of blood pressure for the elderly, which is easy for the elderly to learn as a nonpharmacologic approach.

Applying Theory Informed Global Trends in a Collaborative Model for Organizational Evidence-based Healthcare

  • Getting evidence in to practice tends to focus on strategies, theories and studies that aim to close the gap between research knowledge and clinical practice. The evidence to practice gap is more about systems than individual clinician decision making. The absence of evidence for administration and management in the organization of healthcare is persistent. Teaching nurses and providing evidence as the solution to evidence-based healthcare is no longer axiomatic. Previous studies have concluded that unit level strategies integrate multi-professional teams with organizational needs and priorities. This 'best fit' approach that characterizes how healthcare is structured and delivered. The published literature shows that increased readiness for change is aligned with integrated approaches informed by conceptual models. The Joanna Briggs Collaboration is the largest global collaboration to integrate evidence within a theory informed model that brings together academic centres, hospitals and health systems for evidence synthesis, transfer and implementation. The best approaches to implementation are tailored to local culture and context, benchmark against international evidence, combine a theory informed model and stakeholder perspectives to improve the structure and processes of health care policy and practice.

암환자의 기독교신앙성숙과 불안과의 관계 연구 (The Relationship Between Maturity of Christian Faith and Anxiety of Patients with Cancer)

  • The purpose of this study was to determine the relationship between the maturity of Christian faith and anxiety. Nurses are concerned with the spiritual dimension of clients' experience. The subjects of this study were patients with cancer in Y. University Hospital in Seoul Subjects were selected who knew their diagnosis, were aged 20 or older and agreed to the interview. A total of 35 patients were interviewed from August 1 to September 15, 1988. The instruments used in this study were the maturity of Christian faith scale developed by H.S. Choi and modified by the investigator, and an anxiety scale developed by the investigator. Data were analyzed by statistical methods including Frequency, ANOVA, and Pearson Correlation Coefficient. Results included the following : Hypothesis, “That as the degree of maturity of Christian faith of the patients increases, the degree of state anxiety will decreases”, was supported (${\gamma}$=-.8061, p<.001). According to this study, maturity of Christian faith is significantly related to on the reduction of anxiety of patients with cancer. In a holistic approach to their clients, nurses may have confidence that the maturity of Christian faith of their clients will contribute to the reduction of anxiety.

Effects of Interactive Pictorial Education on Community Dwelling Older Adult's Self Efficacy and Knowledge for Safe Medication

  • Purpose: The purpose of this study was to examine the effects of interactive pictorial education on community dwelling older Korean adults' self-efficacy and knowledge for safe medication. Methods: A quasi-experimental, three-group pre- and post-intervention design was used in this study. The interactive pictorial education was designed to suit the learning patterns and psychomotor skills of older adults. The education content, dealing with safe medication, was delivered over three sessions. A total of 136 older adults from local senior centers were assigned to one of the three groups: a) interactive pictorial education plus information booklet (experimental); b) education only with information booklet (conventional); or c) no intervention (control). Results: Participants receiving interactive pictorial education had significantly higher self-efficacy (F=24.32, p<.001) and knowledge (F=24.26, p<.001) scores than the information booklet or control group at post intervention. Post-hoc analyses indicated that both the interactive pictorial and the information booklet groups had significantly higher self-efficacy and knowledge scores than the control group at the post-test point (p<.05). Furthermore, the interactive pictorial group had higher self-efficacy and knowledge scores than the information booklet group at the post-test point (p<.05). Conclusion: These results suggest that the interactive pictorial education is an innovative approach that provides a means for older adults to learn appropriate medication use to improve their own health. It empowers older adults with different literacy levels to enhance their self-efficacy and knowledge for the safe use of medication.
